42:36that they're suppressing.
42:38He's not a typical bulimic,
42:40though, is he?
42:40No.
42:41He wants to be fat.
42:42He wants meat on him.
42:44He doesn't want to throw up.
42:45He doesn't want to stick his fingers
42:47down his throat every day
42:48and do it.
42:49I admire him for being here
42:51and I don't want to make it sound like,
42:53oh, because he's a man,
42:54he's got bulimia,
42:55he's brave,
42:55because anybody with an eating disorder,
42:57but it is different
42:57to talk to a man with bulimia,
42:59isn't it?
43:00Let's get Andrew

43:20What does it feel like
43:21when you look at that picture, buddy?
43:24I feel sick.
43:26I want to be sick now.
43:28Why?
43:29I know.
43:31I know it started
43:32when you went out on the night
43:33and you got heavily drunk
43:34and like most people
43:35who get heavily drunk,
43:36you threw up the following day.
43:37That made you feel better
43:37and somewhere in your psyche
43:38you thought,
43:39I guess you thought,
43:41when I get to a point
43:42where I'm not feeling good
43:43and I can do this,
43:45it is a coping mechanism.
43:47Yeah.
43:47But it's destroying your health.
43:49It's destroying my life,
43:50not just my health.
43:53It's destroying everything.
43:54I don't go out,
43:55I don't see my friends,
43:58I don't do anything
43:59because I just don't think
44:00they'll approve no more,
44:01you know?
44:02I used to be so big and healthy
44:04and now I just feel like
44:07I don't even recognise who I am anymore.

44:15This is Aaron Goche,
44:16ladies and gentlemen.
44:18So I've just seen Andrew
44:19in the clinic today.
44:20Andrew's 30 years old
44:22and on the surface
44:23used to be very, very fit.
44:24But there's clearly
44:25deep psychological issues
44:27underlying with Andrew.
44:28Andrew suffers with bulimia
44:30and it's a very dangerous condition.
44:32The reason why bulimics vomit
44:34is because they get a sense of purging.
44:36They get rid of a poison.
44:38Many of us would have vomited
44:39when we feel unwell
44:40just to get rid of
44:41what was causing us
44:42to feel very, very sick.
44:44But Andrew's using this
44:46as a way of feeling normal
44:47and he's abusing that sense
44:48and it's going on and on
44:50to the point where
44:51he's actually depleting his body
44:52of essential nutrients.
44:55Why is that so dangerous?
44:57Because it can cause heart problems.
44:58It can cause malnutrition,
45:00conditions like osteoporosis,
45:02anemia.
45:02It can damage the esophagus,
45:04the tube from the mouth
45:05to the stomach.
45:06And clearly in Andrew's
45:07physical exam today,
45:08you can see the pain
45:09and discomfort he is in
45:10when we're examining his stomach.
45:13Andrew's eroded the lining
45:14of his mouth
45:15and his actual teeth.
45:17You can see them rotting away.
45:19They are more potent
45:21in acid,
45:21in your vomit acid,
45:23than it is
45:23in the most concentrated
45:24fizzy drinks.
45:26And when he's doing that
45:2720 to 30 times a day,
45:28he is literally going to have
45:30no teeth left.
45:31Again,
45:32why is that so worrying?
45:33Because he's opening himself up
45:34to huge heart
45:35and lung problems.
45:37This acid can fall back
45:38into the lungs
45:39and cause erosion on them.
45:42Added with this,
45:43Andrew's using cannabis
45:44and he's using it
45:45as a way of a coping mechanism.
45:47There is,
45:47without a doubt now,
45:49strong medical evidence
45:50to know that
45:51the long-term use of cannabis
45:52is going to make
45:53this condition worse.
45:55The long and short of it is,
45:57Andrew needs to stop
45:58if he wants to save his body.
46:00I'm just really worried
46:01it's not already too late.

46:27after 10 years
46:28to know that

46:34whether this purging
46:35has become
46:35an addiction in itself.
46:37It's become,
46:38as I said at the beginning
46:39of this part,
46:39almost like a habit.
46:41Maybe a coping mechanism,
46:42I don't know.
46:44There's no doubt,
46:45though,
46:45that the important message
46:47is Aaron's.
46:48What you're doing
46:49to your teeth,
46:51your body in general,
46:52the pain that we saw you in
46:53when you were having that VT.
46:56What do you want to say, Tim?
