Part 3

In a May 17, 2023 phone call I asked Christ Health CEO Dr. Robert Record MD about their clinic's billing practices regarding patient visits to Medical Doctors and Nurse Practitioners. In a 2019 visit with their MD Dr. Terry Daniel Smith (aka: T. Daniel Smith / Terry D. Smith) I was billed $109 at their Chalkville (Old Springville Road) location. I found it surprising when just a few days later I was billed $107 for a visit with Nurse Practioner Anna Musgraves at their Woodlawn (Birmingham, AL. branch). I believe there is a substantial skill level that separates doctors from nurses (average 4 years & $30,000 University tuition vs average 2.5 years $13,000 tuition in a community college). Dr. & CEO Record's answer (that "the medical codes and medical prices are the same") did not sit right with me, I have experienced that in the Birmingham, AL. market the average charge I encountered to see an NP was only $45.

I pointed out my experience visiting his Woodlawn clinic. During my 2019 visit I observed most their patients were foreign migrants to America, observably poor and I note hearing very few words of English spoken while waiting to see the nurse. I believe Christ Health is overcharging patients and customers for their services.

My 2018-2019 experiences as a patient being misdiagnosed and patient dumped (see: EMTALA) during visits to their ER is that they then referred me to community health clinic Christ Health Center who (I only found out much later) was not skilled or equipped to handle my injury. I observed and overheard other referred patients from Ascension Emergency department visits. I believe Christ Health is being used as a patient dumping destination. CEO Dr. Record confirms in this call they host Ascension's residency program. My own experience is that the doctors and nurses I had contact with at Christ Health were very young and not experienced. An MD I had a visit with who outright refused to give me a referral for an MRI scan (Dr. Terry Daniel Smith MD was later promoted and hired on by Andrews Sports Medicine / St. Vincent's Birmingham). I believe Ascension also uses Christ Health to train prospective future talent (my experience: Ascension does business in the wrong way) and those that show the qualities they are after at Christ Health get promoted.

Regarding visits I had at community health clinic The Christ Health Center (Birmingham, Alabama). I was referred here by Saint Vincent's East Hospital (Ascension) after a 2018 X-Ray (the injury was overlooked by reading radiologist Dr. Julian Patrick Druhan MD (also a member of Birmingham Radiological Group)) I had done was said to be "normal". Doctors Terry Daniel Smith ("T. Daniel Smith") did the encounter with me in May, 2019. Attempts to obtain a referral for an MRI to reveal the injury (I needed one that day, immediately) were not facilitated and he wanted to go a different route with the treatment.

00:00I'm speaking to Robert Record, CEO in State of Alabama, licensed medical doctor for Christ Health Center. Is that correct?
00:08Sir, yeah, this is Robert Record. How can I help you? It sounds like you're upset. I would love to
00:13be of great help to you. I know we've got you for you. What can I do for you? It's
00:17been four years.
00:18Oh, yes. It's been four years. I've been trying to get that issue resolved for four years, actually. I saw
00:25Dr. Terry Daniel Smith, MD, and he was at the Chalkville, Old Springville Road Christ Health Center.
00:34My issue with my Christ Health visit, 2019, I believe I received no measurable level of service in my visit,
00:43and I had two visits, okay?
00:46The one with the doctor, Terry Daniel Smith, and that visit was $109. I had a visit with nurse, NP,
00:56and Musgraves, $107.
00:58Why was I charged $107 for a visit with an NP and $109 for the visit with the MD? Because
01:07there's quite a gulf of experience.
01:11So you don't understand why you would charge the same bill for an NP as an NP? Okay.
01:16Yeah. So why?
01:20Because the medical codes are the same, and the medical price is the same on either one.
01:25I think that I should have been charged less for the visit with the NP than the MD.
01:31I honor your thought on that, sir.
01:34Yeah, and I mean, you know, I can go down to the Woodlawn Clinic today and take video.
01:40The people that go to your clinic there in Woodlawn, they don't have $100 to pay to see an NP.
01:45You obviously don't know the market prices, the going rate to see an NP is what I think.
01:50Well, you're welcome to accuse me and use language like that. That's your right.
01:55I can back it up with facts because I've seen other NPs. I can share what the market rate is
02:01in this area.
02:02Birmingham, Alabama, sir.
02:04You're welcome to say that, sir. You're welcome to say that.
02:07Our fees are set with the federal government, approved by our board, and reviewed every year by our board.
02:16And we charge on a sliding scale based on people bringing in documentation of their income, and then we reduce
02:23those fees based on that.
02:24And so I know what we collect on average on an uninsured patient, and it's all based on their salaries.
02:32They're done with the federal sliding scale, which is approved annually by the federal government.
02:37Yeah, again, I would like a better answer as to why I was charged $100 to see a nurse and
02:44a medical doctor.
02:46Because there's a gulf of discipline there, okay?
02:51Yeah.
02:52You know, a nurse is not a doctor, and you shouldn't be charged.
02:56She's not a nurse. She's a nurse practitioner.
02:59So she's certified registered nurse practitioner.
03:03And the charges are the same in both, which is pretty much standard practice across the industry.
