09:35Get a 500ml saline bag, he needs to be rehydrated, and 4mg of Zofran.
09:39On it.
09:42Sorry, I'm late.
09:43Where were you?
09:44I got turned around.
09:45Ma'am, is that your husband?
09:46Yes.
09:47I need you to tell me everything that's happened right from the very beginning.
09:49She's already given a statement, it's in his chart.
09:51We were at the margarita mixer, the send-off, and Bert said he was feeling funny, and then he turned...
09:57Doing that.
09:58Okay, team, how are we going to tackle this?
10:00First things first, differential diagnosis.
10:02List the symptoms, name the associated conditions, narrow it down from there.
10:05Excuse me, sir.
10:07We have conjunctivitis, a rash, that would be symptomatic of allergic reaction.
10:12The vomiting would seem to suggest something ingested.
10:15Food allergy, maybe?
10:16He doesn't have food allergies.
10:17The margaritas at the send-off mixer tasted weird, like pennies, sort of.
10:22Interesting.
10:23Margaritas don't have any common allergens, but metallic taste can be associated with certain
10:26medications.
10:28Antibiotics like azithromycin.
10:31So what do we have?
10:32An allergen, something ingested metallic taste.
10:37Adverse reaction to an oral antibiotic is where I'd put my money.
10:40Nice.
10:41A reasonable deduction, if the patient were in fact taking any antibiotics.
10:45Which he is not.
10:47Mr. and Mrs. Rubens, before going to the mixer, did you by any chance stop by the seafood buffet?
10:52Oh, honey, you know I love a cocktail.
10:55A shrimp cocktail.
10:58This is the life.
10:59Have I died?
11:01Have I died and gone to hog heaven?
11:03Cheers!
11:04Cheers!
11:05We had some shrimp.
11:07But he's not allergic to shrimp.
11:09We love seafood.
11:10He's not having an allergic reaction.
11:12Not to the shrimp, nor to any antibiotics.
11:15This happens at least once a trip.
11:17Shrimp contain iodine.
11:18You eat too much shrimp too quickly, and it can lead to iodine poisoning.
11:22Iodine poisoning?
11:22You'd have to eat a hell of a lot.
11:24It happens to baby seals, too.
11:26Which is why we call this little ailment seal disease.
11:30Given two milligrams of Ativan?
11:31Mm-hmm.
11:32Oh, my poor little seal pup.
11:34So, what are you going to do?
11:37Well, Mr. Rubens, we are going to have to pump your stomach.
11:40But don't worry, we're going to have you feeling great by tomorrow morning.
11:42Oh.
11:45Quick deduction work back there.
11:47Well done.
11:47We see a lot of different things on cruises.
11:50We also see a lot of the same things.
11:52Your office has a library with a good section on maritime medicine.
11:55You may want to study up.
11:57Will there be a quiz?
11:59You're living it.
12:21You may want to study up on the various cruise ship maladies that afflicts cruise ship people.
12:28Cruise ship people?
12:30You mean, as in people having too much fun?
12:36Never in my life did I consider that it was possible for a human to eat enough shrimp to give
12:40themselves iodine poisoning.
12:43Now, doctor, you cannot judge our passengers.
12:48We're tending to their dreams.
12:50We have folks on here who've saved for years, sometimes our whole lives, just to pay for this experience at
12:57sea.
12:57And other than the birth of their child or, in some cases, their weddings, this will be the most exciting
13:03week of their entire lives.
13:04And it's our job to guarantee that.
13:09Understood.
13:11See, our mission is to preserve the dream.
13:14And you're here to keep these dreamers safe.
13:17You might think of yourself as a guardian angel.
13:22A guardian what now?
13:23Guardian angel.
13:25We make it look perfect so that resistance isn't even a possibility.
13:30And why should it be?
13:31There should be zero guilt on a pleasure cruise.
13:34Now, you may think that the staff we have up here in the white shorts running around are glorified extras
13:40from a country club.
13:42But make no mistake about it, they are certified the best.
13:46You don't get to be on my ship without hours and hours of intensive training in hospitality and scuba and
13:53CPR.
13:54The best of the best.
13:56Sometimes we even apply sunscreen.
13:59Because out here, there are no sunburns.
14:04It's about as far from hell as you can get.
14:08This ship.
14:11This ship is heaven.
14:14Usually we're about three hours from land.
14:17So medevac in the deep ocean is not even a possibility.
14:21So it'll be your job to keep everyone alive.
14:24That's a terrifying burden.
14:25It comes with enormous responsibility.
14:29So we have to be prepared for terrible things to happen.
14:32And you can't begrudge these mishaps and these accidents.
14:37Speaking of mishaps and accidents, I have to go.
14:40But, Captain, I hear you.
14:44And thank you.
15:03What happened?
15:04It broke.
15:07He's in shock.
15:08What broke, Mr. Arrington?
15:10Is it a bone?
15:11No.
15:13No.
15:14No.
15:14No.
15:16No.
15:16No.
15:16Penile fracture.
15:17You can't fracture your...
15:19It's not actually a bone.
15:20Broken fascia.
15:22The membrane beneath the skin that connects the erectile chamber through the pressure.
15:26The only treatment is surgery.
15:28It's a quick procedure.
15:2915 minutes.
15:30I'm in and out.
15:31Local anesthetic.
15:32We can't do surgery on board.
15:33It's against COVID.
15:34Policies we coordinate with the nearest...
15:35Except in cases of emergency.
15:37Every second that you lose blood is the second you lose the opportunity for a full recovery.
15:40We need to act now.
15:42Do what you need to do, doctor.
15:44Get in prep.
15:45We got this.
15:46The blood from his erection is pooled at the base of his penis.
15:49We can't do anything until we evacuate the hematoma.
15:56Scalpel.
15:59Generally speaking, penile fracture happens during intercourse when the receiving party is on top,
16:05vigorously moving up and down.
16:07And then they pull out too far.
16:09And when they sit back down, they miss.
16:13And you learn this in the Peace Corps.
16:16Absolutely not.
16:17Pre-med.
16:18Leandra.
16:19Suture?
16:20Who's Leandra Suture?
16:22Sutures.
16:22Stitches.
16:26Leandra is the woman who taught me about penile fracture.
16:29Sophomore year.
16:31Toga party.
16:32I'm sorry, wait.
16:33You broke your...
16:37Before you get too excited, I should probably tell you, it really only happens to the larger,
16:41firmer members.
16:45That's wholly inappropriate.
16:47Knocked off.
16:49Room service.
16:51Leftovers from the captain's center.
16:53How you feeling?
16:54Like I snapped my pee-pee in half, I had to stitch back together.
16:57Unfortunately, we are going to have to keep you in here overnight, and then monitor that
17:01throughout the rest of the week.
17:02Well, how long until he can, you know...
17:05Yeah, sorry.
17:06No more knocking boots or booties for at least four to six weeks.
17:10Man, I'm sorry, babe.
17:12I ruined our honeymoon cruise.
17:14You know, intimacy is infinitely nuanced.
17:18We're in Manzanita the day after tomorrow.
17:20I know a woman there, a tantric therapist.
17:23She works with couples, helps them explore deeper levels of connection through touch, breath
17:28work, pleasure centers.
17:30Pleasure centers?
17:32I mean, I'm not interested.
17:35Okay.
17:37Wonderful.
17:38Enjoy your dinner.
17:48Help!
17:48He's not breathing!
17:49He's not breathing!
17:50Cut that away back!
17:51You better give me some space.
17:55What happened?
17:56I told her to wait one of the times, but she jumped in right after.
17:58Oh, no!
17:59I wanted to do it together!
18:01I heard that.
18:06Is he going to be okay?
18:07Bird!
18:08Bird, can you hear me?
18:09Bird, breathe!
18:09Breathe, Bird!
18:10Bird!
18:11Bird, hang in there, buddy!
18:12Hang in, Birdie Bird!
18:14No, you didn't kill him, madam.
18:16When you knocked into your husband, his clavicle separated from his sternum.
18:19He punctured a hole in his limb pipe while he's having trouble breathing.
18:22With a hole in his limb?
18:24We're inserting a tracheal tube to make sure he gets all the oxygen he needs.
18:28He'll be breathing in no time.
18:35I know we were supposed to lay low after he caught the seal disease, but we were just
18:40so excited to be here just to have more fun.
18:43Don't worry, ma'am.
18:44We've had it from here.
18:45But how will you make him better?
18:47For good?
18:50He needs advanced thoracic surgery to put his clavicle back in place.
18:53We're not equipped to do that.
18:54This isn't stitching up a penis.
18:56Well, we're in Puerto Vallarta in six hours.
18:58I can contact EMS there.
18:59We can't just keep him here all night like this.
19:01Any delay in treatment could cause serious vascular damage.
19:04We have to do something.
19:06In certain cases, the sternoclavicular joint could be manually manipulated.
19:10That sounds extremely risky.
19:12If it doesn't work, it could severely exacerbate his condition.
19:15It would mean significantly longer recovery time.
19:20But it's your call, doctor.
19:24No, it's not.
19:26This procedure would take all of us.
19:27The decision has to be unanimous.
19:29If anybody has any reservations, speak now.
19:37Avery, will you make a rant, please?
19:39Tristan.
19:40I need you to grab him under the shoulders and brace his back with your knee.
19:50Now, on the count of three, I want you to rotate his shoulders counterclockwise.
19:55One, two...
19:56But then I'm pushing against you.
19:58That's the point, Tristan.
19:59Rotate him.
20:00One...
20:01It's going to snap his shoulder blade.
20:02Do it.
20:07Are you ready?
20:11One, two, three.
20:30You good?
20:32Cramp my pants, but I'm alive.
20:36You know, I just didn't do well being bossed around.
20:39Well, I am your boss, but...
20:42Yeah.
20:43And who has more experience on this shit, doing this?
20:46Guys, not in front of the patient.
20:48Please.
21:02Any signs of life in the great beyond?
21:04Oh, just a big swirling ball of hot air directly overhead.
21:13Oh, sorry.
21:18So, is it ever the same?
21:23Or is it always, like, a little bent?
21:29Your penis.
21:31After you broke it.
21:35The body is a miraculous healing machine.
21:39That was good work today.
21:41The tantric therapist in Manzanita.
21:44Lainey.
21:45She's amazing.
21:47She saved my life after a breakup a couple of years ago.
21:51Besides, it's not enough just to fix them up here.
21:53I have to also...
21:54Preserve the fantasy.
21:55Always.
21:56Always. All the time.
22:00Tristan.
22:01He's a good nurse.
22:03Don't get hung up on first impressions.
22:05He's been my right hand for three years.
22:07He knows a lot.
22:08He's a valuable tool.
22:10Oh, he's some kind of tool, all right.
22:11You respect him, he'll respect you.
22:15We're all here for the same reason.
22:17Work like a captain, play like a pirate.
22:20Yeah.
22:21Also that.
22:24So what about you?
22:26What are your first impressions of the new doctor?
22:29Well, admittedly, it was a little low after his misstep with the iodine poisoning.
22:35But good recognizes good.
22:38The penile fracture, the clavicular relocation.
22:41That was decent.
22:46Well, I try.
22:59Bedtime.
23:01You two.
23:02First port tomorrow.
23:04The passengers are out of sight and mind for five hours.
23:08Oh, thank God.
23:10All of which I will spend napping.
23:14No, you won't.
23:15If you thought today was a big day.
23:39Is this a don't tell captain kind of thing?
23:41Because I'd really rather not get fired on my second day.
23:44You're not going to get in trouble, dude.
23:45Be cool.
23:46Bonfire basically is captain's order.
23:48As long as we're back by dinner when the cruisers are done at court, we're good.
24:06Hey, wolf flower.
24:07You good?
24:09Just digesting the scenery.
24:11You need to loosen up, dog.
24:14And that means one thing.
24:17Shots.
24:17Come on.
24:18Come on.
24:23All right.
24:29Okay.
24:30Okay.
24:31I see you.
24:32I see you, Dr. Odyssey, huh?
24:35Big kahuna.
24:36Woo!
24:36Playing nice, boys.
24:40Yes, Mama.
24:42Yes, we are.
24:50Oh, man.
24:51I am a secret.
24:54But I can't tell you ever.
24:56Sorry.
24:59Is it that you're in love with Avery?
25:02No.
25:04No, oh, my God, dude.
25:05No, no.
25:06Don't ever say that to anyone.
25:08You know I'm pretty good at keeping secrets.
25:10Democratic oath and all.
25:14Step into my office.
25:20Oh, boy.
25:24Talk to me.
25:26It is not good.
25:29I like her so much.
25:31You know?
25:33I've had a bad for, like, two years.
25:36It is rough.
25:38She always says to me, like, oh, you're like my little brother, you know?
25:42You know what, man?
25:44I'm never going to give up hope.
25:46I refuse to move on.
25:48Ever.
25:50Well, that seems healthy.
25:55Well.
25:58Till then.
25:59Must tend to my admirers.
26:03Hi.
26:04I think she's looking at me.
26:06Yeah.
26:07Hey.
26:10Hi.
26:11Are you looking at him?
26:13Or me?
26:16Sorry.
26:17I was looking at him.
26:19But you're cute, too.
26:22No.
26:23No.
26:24No.
26:26Uh-oh.
26:27Get up.
26:28Get up.
26:28Had to happen sooner or later.
26:29Let's go.
26:30What are you doing?
26:32Dance off, dog.
26:33Dance off, pants off.
26:35DJ!
26:45Come and move her in my direction.
26:48She thankful for her, such a blessing, yeah.
26:51Turn every situation into heaven, yeah.
26:54Oh, oh, you are.
26:56My sunrise on the darkest day.
26:59Got me feeling some kind of way.
27:01Make me want to savor every moment.
27:03Slowly, slowly.
27:06You fit me, tell her, my love, how you put it on.
27:09Got the only kid know how to turn it on.
27:12The way you'll never lie, my idiot, the only words I want to hear.
27:15Baby, take it slow so we can last long.
27:16Tú, tú eres el imán y yo soy el metal.
27:20Me voy acercando y voy armando el plan.
27:22Solo con pensarlo se acelera el pulso.
27:26Oh, yeah.
27:27Ya, ya me está gustando más de lo normal.
27:30Todo mi sedito va pidiendo más.
27:33Te voy a decir que tú me lo sigues.
27:35Te voy despacito.
27:39Quiero besarte a besos despacito.
27:55Tus lugares favoritos.
27:57Tus lugares favoritos.
27:58Déjame sobrepasar tus horas de peligro.
28:03Hasta provocar tus gritos.
28:05Y que olvides tu apellido.
28:09Pasito a pasito.
28:10Suave suavecito.
28:11Nos vamos pegando poquito.
28:13Te voy a decir que tú me lo sigues.
28:16Tus lugares favoritos.
28:17Te voy a decir que tú me lo sigues.
28:19Suave suavecito.
28:20Nos vamos pegando poquito a poquito.
28:22Hasta provocar tus gritos.
28:24Y que olvides tu apellido.
28:26Te voy despacito.
28:34This is wrong.
28:36But it feels so good.
28:40But it's wrong.
28:41We just met each other.
28:43And yet we feel as if we've known each other all our lives.
28:50But it's wrong.
28:53I know.
28:55I know.
28:56I know.
29:03I know.
29:08We need to be professional.
29:10This is the day drinking talking, not us.
29:12Yeah, definitely not us.
29:13Please don't tell the captain.
29:15Yeah, I was just about to text him.
29:19Guests are back soon.
29:21Here.
29:22Sit up.
29:24What are you doing?
29:27Nutrient drip.
29:30Electrolytes.
29:31B vitamins.
29:32This will do you a whole lot better than that nap you wanted.
29:45A doctor who's afraid of needles?
29:59A doctor who's afraid of needles?
30:02So where'd you learn to dance like that?
30:09You know, I always wanted to take dance classes as a kid.
30:12But it's not really something the boys did where I grew up.
30:16And then one day I got tapped on the shoulder and gently reminded that life is fleeting and unbelievably fragile.
30:27March 2nd, 2020.
30:29I was working at a hospital in New Haven, Connecticut.
30:31Oh, yeah, we know you went to Yale.
30:35Actually, I was the attending physician general internal medicine, but who's counting?
30:42One day, I got a cough.
30:43No big deal, but abundance of caution.
30:45I asked one of my residents to give me a strep and a flu test.
30:48And that night, cough gets a little worse, but both the strep and flu tests come back negative.
30:53And everybody gets worried that I've had a pulmonary embolism.
30:55So they give me a CT scan, check for a blood clot, but they also give me a new test,
31:00a coronavirus test.
31:03Now, at that point, there were barely any cases in the U.S., so I didn't even think about it
31:07as a realistic possibility.
31:08But as I'm sitting there waiting for the results of the CT scan, I found out on the news, actually.
31:15A hospital in New Haven reporting Connecticut's first ever case of the novel coronavirus.
31:20A physician who was admitted last night at J.D. Memorial ultimately testing positive for the virus.
31:26After that, everything moved very fast.
31:29They put me in a negative pressure isolation room, cut me off from all human contact.
31:33My blood oxygen dropped to 85% and then kept on dropping.
31:36I refused to sleep because I was afraid I wasn't going to wake up.
31:41And at that point, in China, they were giving patients with advanced cases second CT scans, right,
31:47so they could chart the progression of the virus, the only way to ensure proper treatment.
31:51But there was a problem.
31:53I was in isolation on the ninth floor, and the CT scanner was on the fourth floor.
31:58And they were afraid to move me.
32:00They want to risk exposing the rest of the hospital.
32:03We are following CDC and evidence-backed protocols.
32:06You are staying in this room.
32:07Yeah, and after that, I was just total darkness, just black.
32:13I thought I was dead, but dead people don't pray.
32:17Because somewhere, in the recesses of my consciousness, there was a voice.
32:22My voice, very faint.
32:24And I knew that I needed that second scan, so I just focused on that all day, every day.
32:30And I prayed.
32:31I prayed for them to give me the second scan.
32:34And they did.
32:35And the results from that scan got me approved for remdesivir for compassionate use.
32:40And that turned the tides.
32:41So against all odds, and despite being patient zero, I lived.
32:47And because I lived, and because I went through hell,
32:52I decided that I was going to pursue joy at all costs.
33:02Which I guess is a very, very long-winded way of saying that I finally enrolled in a dance class.
33:10And then you came here?
33:17Thank you for sharing that.
33:20I will caution you, joy at any cost is its own kind of hell.
33:32Show me if there's anyone wrong.
33:47Show me if there's anyone wrong.

38:54Avery, you and Trist can get him on low tidal volume.
38:56Prepare to assist on ventilating using ArdsNet protocol.
38:58Utilize the high-peat flow FiO2 table.
39:01And closely monitor his peak inspiratory pressures.
39:04Okay, Leo.
39:05It's going to be all right.
39:08Hey.
39:11It was good work out there.

39:25Look, uh...
39:27Tristan, I'm sorry about, you know, I shouldn't have done that.

40:46Maybe they get your leg chewed off by a shark.
40:50Exciting!
40:51Bye.
40:56I thought I picked up on a little something the other night.
40:59Some tension in the medical suite, Dr. Odyssey?
41:04Absolutely not.
41:06It's a love fest, sir.
41:08Good.
41:09I love hearing that.
41:11Love is in the air.
41:13Just not too thickly or quickly.
41:20A love fest, sir.
41:22Interesting.
41:23About the other day.
41:24I shouldn't have done that.

41:43It's Singles Week.
41:45Singles Week?
41:47Is every week a theme?
41:49I don't know.
42:17I'll be right now.
42:26You
