02:23i was very impressed at pacific fair because on the form they ask you what tv show is your preferred

02:48so tell me what is it that you want you to achieve
02:55i am really into health these days because um i mean appearance is great you know obviously but
03:02i'm really into health and wellness and it's the chewing that's an issue for me and even when
03:09you're trying to chew the the greens and the um vegetable type things you know the insoluble fiber
03:14that you want to chew properly you know like a cow and i can't chew those all that well
03:19and i don't have pain it's just that i've got these i chew on the back and it's very flattened
03:27and in fact just this morning i bit my tongue yet again so i'm biting my tongue a bit um a little
03:34bit on the inside of my cheek and so yeah just just to be able to chew better and also to hopefully
03:40protect my teeth a little bit from you know i mean i probably might grind anyway but i think
03:46if they're aligned a bit better the load is better a little bit better yes i crack teeth and all that
03:52at the moment do you have you ever had any treatment before um not not for tea slating and
03:58like every issue it seems to only exacerbate the more birthdays you have because this did not look
04:06like this 10 years ago it didn't it really didn't all right and then from my understanding you
04:13haven't been ready to another clinic yes and you've been told that i've had a bit of a journey
04:18yeah and i was told that um i did go to smile outlet um because yeah i was hoping that i would be able
04:25to sort of be a little bit budget friendlier than the first place i wanted to and but after
04:29they only do mild to moderate all right so okay so i think i will do it with a little bit and see
04:36okay obviously you are helping friends train tracks train tracks well look whatever i'll go
04:47do you think i'll do it with a little bit more recommendation okay like it's fine

05:57your case is a very very much a case of expansion
06:01your arches are very narrow
06:04and so that narrowness is actually was probably causing you to bite in the tongue and other sort
06:10sort of thing so it's almost if you have no space in your lower jaw for even if your tongue is sick
06:17yeah it has no logic so yeah because it gets in the bio
06:20it gets in the bio yeah so i actually think that for you we need to do a really good extension
06:25extension so extension so extension means moving your back tip outwards to give you like a broader jaw
06:32yeah that would then give you better breathing the bite is better there's more room for your tongue
06:38and it will look better as well because it gives you like a wider smile and then by doing this extension we then have the space to straighten your teeth because at the moment there are a lot of the reason why you front tip the crowd is because your lower tip has to push them sort of forward and then there's no space for them
06:54there's no space for them so um but funny enough your case is actually not terribly functional like your canine is actually where it's supposed to be
07:04we usually classify by some three classes class one class three class three i don't know if you've been told that before
07:11class one is considered normal
07:13yep
07:14and that means that your canine is where it's supposed to be compared to your topology as well
07:20and then class two and class three are outside normal so you in terms of the position you take you actually are in a normal class
07:28oh right
07:30so for you you've got a bit of a cross bite on the right side but it's only one three um and then and then on the other side it's pretty much where it's supposed to be
07:39so your case is really your jaws haven't quite grown enough and wide enough for your to fit all your teeth
07:47so everything has been pushed forward um because of that
07:51and did that happen already in utero or did that help
07:55i think if it usually comes in from childhood um obviously it gets worse as we grow older as with everything has grown
08:03yeah um so yes it may have grown worse but i i i don't know because i haven't seen you in your childhood years but i would think that this is there for quite a while
08:13it's been there for quite a while and it's probably becoming more noticeable to you now because of the um
08:19fighting tongue or you know and also against you getting warm which which happens in time as well
08:27it escalated after i got three molars fixed at the back so i got three done at once
08:33yeah and it was after that repair work okay that things all of a sudden will change yeah yeah things can cause imbalances in the jaw as well
08:45um so so i think the case can be done with the line um it will be a little bit difficult with the extension
08:53because the line as you can imagine it's just like a plastic thing so the plastic will have to cause that extension
09:01but then the plastic itself has flexibility so it tends to go back as well so the more you push out it
09:07it tends to to deform and bend um so we may get some um challenges in trying to get this expansion but
09:15i definitely think we'll get some extension you may not be able to get as much as we would like but it will be enough i
09:23think for you to feel better in your mouth there is another option which is using extenders um
09:31so expanders like a little plastic but on the roof of your mouth um and then it's like a plastic thing on both sides
09:37that then causes you to move a bit better than you'd like because it's a rigid plastic instead of a flexible one
09:45um that would give you better results in terms of expansion but it's a bit harder to wear it
09:51because it's like a bit more bulky in your mouth and it sounds a bit yeah
09:57yeah i might have i'm not sure if i have one to show it
10:03uh the so the invisalign itself is just those ones yeah and and you can see how it's got some flexibility to it
10:09it's got some flexibility to it so even if we try oops even if we try to expand your top teeth like this
10:17because this is what we want to do we want to move your teeth this way okay
10:21yeah the plastic can do that but then because it's soft it doesn't always go there whereas the
10:27expansion i don't think i have one to show basically it's connected all the way between
10:33one side to the other and then so if you've got something here in the middle then that you can get
10:40a better extension but it's a little bit more uncomfortable the only the good thing though is
10:45it's not for a long time like you can probably return to an extension plate for let's say maybe three to six
10:52months cause that extension first and then once we've got the expansion we then come in with
10:57the aligner just to do some final um alignment so so we're probably looking like let's say six months
11:05of expansion and then probably another maybe six of this um and then you're good otherwise we can go
11:12straight to invisalign without the extension um which would probably be a probably about a nine month job
11:21maybe and then see how much extension you can achieve with this only but knowing that we may
11:26have a little bit of limitations there i mean it sounds sensible i have had other digital professionals
11:34like this they'll talk about things as soon as you started talking about it yeah i thought oh yeah
11:40it's probably what what it is yeah to be honest with you talking about extension in terms of the health
11:45side of things yeah is the you tend to breathe better as well my breathing yeah i don't know if
11:51that's an issue no no well yeah i mean it can sometimes be like especially if i'm anxious and
11:57behind things like yeah yeah my knowledge tends to it it does it have an effect on my brain well
12:03sometimes it could be the other way around if you have a blocked nose then you can have issues in the
12:07mouth with breathing and then you tend to become a brown mouth breather which is no idea because
12:12i give you in your life you want to be breathing through the nose so do you have any issues with
12:17your mouth no but can can expansion help with the nose for you no it won't help with your nose it will
12:24help with space in the mouth for your tongue to make you kind of sit in a better place because like
12:31snoring for example i don't know if that's something you have but snoring is usually the tongue so the
12:37ceiling will be chewed back so sometimes when you move your jaw or the jaw forward like this you tend
12:44to breathe better so the mouth has a very direct link to breathing oh interesting so then it can be
12:54down there as well like i said so if you have issues with the nose that can also affect the mouth and
12:59then but then that can be addressed with any any ant especially but if it's not your case then you
13:08don't need to do it i think it's all right so what about braces for the expansion yeah braces is also one
13:15way to do it but braces also do have limitations um yeah because in your case if you were to embrace it
13:22we also have to cause this extension with the wire only but the wire also has flexibility so the
13:29wire will try to go back so for expansion to be to be successful we need something that actually
13:37has an anchor on the roof of your mouth to then cause the expansion so the only two things we have at
13:44the moment is a metal extension plate which actually gets bonded on your teeth and then there's like a
13:51hook that you turn so it does look like it's something like from a down a bit from the past but
13:58it's still available and then and then lately we've got these new ones made of plastic yeah that actually
14:04causes the extension as well so by having an anchor point on the roof of your mouth we then get a lot
14:09more stability to cause the extension but when you depend on something that only goes on your teeth
14:16whether it's this or wire we have meditations but we do we do achieve a very good amount so
14:23it could be that the amount of this extension we're going to achieve with this for example would maybe not
14:29be well look i might to be honest because at the moment it's a little bit of an intense time with my
14:34toddler and yeah he's still not sleeping through and so it's it's i wonder if it would be a good idea to
14:42start with those jet model that you're going to say one thousand yeah i was actually i was actually
14:46going to suggest that so this is what we do the most by the way yeah start with the misalign yeah see
14:50how much extension we get the good thing about a misalign is it's very flexible so let's say we
14:56finish the first round of it and you're like oh i want maybe i should try the extension plate you can
15:02actually go back on it so you go back on it and then do that and then come back here again so
15:07we can pretty much do whatever we want with the treatment once you lock into a package of
15:13the visalign it comes with unlimited revisions so like with the comprehensive package and the
15:21sorry the module and the comprehensive you get unlimited revisions so that means we can pretty
15:26much keep treating you for a period of i think it's two years the module and five years for um
15:32complexes and then like i said if we really find that we're getting it stuck uh with extension
15:39then okay maybe let's revisit the extension plate which can be done at any time yeah so that's a good
15:47good way to do it because it could be that you're going to have good results with this alone yeah and then
15:51you know how to go through the extension plate so that i'm so happy i think i'm here and get this
15:56done cool so the way we find out which level you're going to be is by doing a scan and you've probably
16:03had one before it's like a 3d i've had lots of scans so um the scans that were sent here you can't use
16:11yeah each account has and to be honest it's like two minutes so it's easier to just do a new one
16:17oh so it's the 3d scan the 3d scan is the when you want to take a photo of your mouth yeah and it's
16:23not too much reaction no this is zero yeah good yeah the other one is called uh cbct that's an
16:29actual action x-ray okay that one i'm guessing you had done as well oh yes i i do that one we can use
16:37from another practice but the one here is just like a photo yeah so if you like i can do that now
16:45there's no um cost for an assessment so i can run the assessment and get that to you okay so this is
16:51how long it's going to take um how many aligners how the cost um ethanol all that so i'm here i'm
17:00already solved because i have yes i would love thank you so much so you do the scan now and then um and
17:08then once i've got the email then the first step is what coming back and seeing yeah it's me so i
17:14i usually like to see you i feel in person but if you can kind of email you as well um but in person
17:20i'll be able to use it on the computer yeah and then from there you can ask more questions if you
17:26want um and then yeah pretty much proceed on that oh and at that coming back if you see you'll have
17:32something already to inform you if i trip the plan or is this no that is just discuss treatment and then
17:37get acceptance of treatment okay once you can accept treatment okay yes about it's about 10 days to get
17:43them and that's when you can start wearing them okay yeah so do you know a little bit about like you
17:48have to wear it all the time day night how do you clean your teeth you brush them um you can actually
17:55brush your tray and your teeth yeah you only remove them to brush and eat yeah oh you remove them to eat
18:00and then so you remove them to eat you remove them to brush your teeth and you remove them to drink very
18:07hot drinks okay at all other times it has to be in your mouth so because that's the way it works
18:13it is a sequence of alignments so each week you're going to go on with this one um so like let's say
18:19your case it's 40 alignments so it's going to be 40 things of these yeah for 40 weeks and they get
18:25tighter and tired i mean sorry they get each week there's like a small incremental force yeah and
18:31that's what they're called and do i come into the dental practice or do they get sent no you well i
18:37give it i usually give you enough for like probably six weeks at a time oh great okay and so you do
18:41that yourself come back and see me i then check if everything is moving okay okay then i give you
18:47another six and then that's how it goes oh perfect that doesn't sound too traumatic yeah okay all right
