01:52I've come to meet Gonzalo, who has provided refuge to over 7,000 migrants in Mexicali in the last few
01:58years.
01:59So, tell me a bit about this neighborhood. What sorts of issues have you been seeing?
02:22How many people have you personally seen or experienced that have had their kidneys extracted?

03:58In 2020, nearly 90,000 Americans were waiting for a kidney transplant,
04:04yet only one in five received a donor.
04:07On average, 20 U.S. citizens die every day waiting for an organ transplant.
04:14This leads some Americans to turn to the black market,
04:17but they have little idea about the human misery that lies behind the trade.
04:28After weeks of working connections in the criminal underworld,
04:32we've made contact with an organ trafficking gang who claim to work across international borders,
04:38bringing U.S. patients into Mexicali.
04:42They prey on migrants, offering them $5,000 in exchange for a kidney.
04:48I always feel nervous about meeting someone who operates within the black market.
04:54That's always something that I get the jitters before going in.

06:23The organ trafficker tells me that he pays off corrupt medical staff
06:27to get lists of kidney patients in U.S. hospitals.
06:30If they're interested in his service, they have to send him a blood sample.
06:35So, you're U.S. clients, going back to the U.S.,
06:38because that's what I'm really curious about.
06:39So, you have Arizona.

06:43Nevada.
06:49Oklahoma.
06:53Once the doctor and his gang has tested the sample, he scours the streets of Mexicali
06:58for homeless migrants whose blood is compatible with the American client.
07:04He lets us follow him as he preys on the most vulnerable.
07:09As soon as I start asking him how, how life is trading you in here, Mexicali,
07:17it starts blowing up, solving the problems that he had.
07:21So, I'm going to start to convince him that I've got his money.
07:43So, I'm going to start to convince him that he can live without a child.

09:49desperate would place their life in the hands of a black market surgeon.
09:58Organ trafficking, organ harvesting is a hideous crime. For the facilitators, this is just business.
10:06They see these people as money. And because they're vulnerable, because there's nobody
10:11to defend them there, many of these people are also tricked into it. They will even tell
10:18them, you're young, your kidney will grow back, which obviously is a medical impossibility.
10:25There are so many risks involved with these illegal surgeries that are performed in places
10:33that sometimes aren't even hospitals. In places, you know, where there's no hygiene.
10:39A lot of these people may become sick. It can also be deadly. A kidney can be sold with $100
10:47,000,
10:49and then the money that the migrant, the poor people get, are a tiny fraction of that. So
10:57most of the money is left with a facilitator and a surgeon. It takes a network to develop this type
11:05of
11:06organ trade. And it's always, almost always across borders. Because in order for a facilitator of this
11:15organ harvesting business to be successful, to make money, they need to operate between rich countries
11:25and poorer countries, or conflict areas and economies that are stable. Because this is where the money is.
11:43The trafficking gangs work with a network of corrupt doctors across Mexico.
11:50Surgeons here earn just $22,000 per year, 14 times less than doctors in the U.S. So some are
11:58easily
11:58lured into the trade. So my fixer just revealed some information to me about a doctor who's been
12:06operating in the black market business for six years. And we are about to go meet with him.
12:24So can you tell me a little bit about how you got into the clandestine part of your job?
12:39I've interviewed people who've recruited migrants off the street to sell their organs. What do you think
12:46about that? Who are you operating on? Are your clients American or what are their background?
12:58No, they're from all, from all the country. But there are also foreigners from the neighboring border,
13:07because obviously the cost varies a lot from there to here. That's why.
13:12Are the cartels involved?
13:13Well, I think that all the cartels are involved. Well, they were able to take me.
13:19There, yes, in a forced way, they had a person who was raised, a person who was arrested,
13:28because they had $60,000 and they wanted to pay for them. So, obviously, they paid for them.

14:51In 2014, the Knights Templar were investigated on suspicion of kidnapping and murdering children
14:57to harvest their organs for sale. There have also been two horror stories about dead tourists' bodies
15:05being stripped for body parts in Mexican hospitals.
15:13But the scale of organ trafficking in Mexico is largely hidden, as most of the victims can't turn to the
15:19authorities for help.
15:22Migrants, because of their illegal status, in most cases, especially on the Mexico-US border,
15:31they have no rights. They need the money to cross the border. They need the money to survive.
15:37And they have nothing except their own bodies. There are so many people who are missing in Mexico,
15:45and nobody knows nothing about their fate. From time to time, they find these fields where lots of bodies
15:52are buried. And many of these people are migrants. Many of these, you know, bodies are burnt already,
16:00or chopped to pieces. They can't really go to the police because many of the police in these areas
16:07are corrupt, and they're working hand in hand with the traffickers. The migrants being the victims are not
16:14a priority at all for the Mexican law enforcement. There are so many other things that they're focused
16:20on, like the war on drugs. You know, the US is trying to stop this migration from Central America via
16:29Mexico into the United States. The migrants and their lives and their health is nowhere on the list of
16:39priorities, neither in Mexico nor in the United States. So this makes them the perfect victims.
16:47They're victims that will not leave a trace.
16:55We've discovered migrants are not the only victims.
17:00We've made contact with one trafficker who recruits directly from Mexico's prison system.
17:31The trafficker claims his money can help prisoners get back on their feet once they leave.

19:18isn't just confined to Mexico.
19:21A recent report found that organ trafficking
19:23generates an estimated $1.7 billion a year globally.
19:29there's one country where this trade happens on an industrial scale in china it isn't run by
19:37traffickers but by the state disturbing news just out from london a tribunal found that some of the
19:44victims of the forced organ harvesting are detainees from the falun gong movement former
19:49falun gong and uighur muslim inmates report they were subjected to ongoing medical checkups
19:55and blood testing in 2019 an international tribunal accused china of executing tens of
20:04thousands of political prisoners and selling their organs on the black market between 2000 and 2005
20:13the organs found a demand from wealthy foreigners from across the globe
20:17south korean police say they are investigating an organized crime group that sends patients to
20:24china for organ transplants the chinese regime banned organ transplant tourism in 2007 but the
20:31practice continues with people willing to pay tens of thousands of dollars to skip the long waiting
20:37list for organs in their own countries china claims it stopped harvesting prisoners organs in 2015
20:45but in 2019 the tribunal found that the constant supply of organs available in chinese hospitals could
20:53only be coming from executions makes you think of some grotesque restaurant where you go in and you pick
20:59your lobster and you pick your lobster in a tank but these are human beings we're talking about

25:08all organs going to foreign patients since then the pakistani government has made commercial organ
25:15transplants a criminal offense punishable by 10 years in jail but criminal networks have filled the void
25:21selling as many as a hundred organs every month i've come to pakistan because i hear it's a real
25:28hot spot for illegal organ trafficking and the third most popular country in the world for
25:34transplant tourism next to india and china and the cheapest where kidneys are sold for as little as
25:40a thousand dollars organ trafficking is so rife in pakistan that the government has set up an agency to
25:49combat it led by adnan bhati the punjab human organ transplantation authority or fota has so far put
25:5811 organ trafficking gangs behind bars adnan has just heard from an informant that an illegal transplant
26:09plan is about to take place
26:11to avoid detection the gangs commonly move patients immediately after surgery
26:29so adnan's team must intercept them as quickly as possible

35:49Farooq was a bonded laborer.
35:51Landlords trapped families with high-interest loans,
35:54then forced them to work for a pittance.
35:57Many see selling their kidney as the only escape from this debt slavery.
36:04With 2010, the year when Pakistan banned this practice,
36:11there was probably hope that things will change.
36:15But what happened was the demand was still there.
36:21Medical doctors, surgeons were already part of this system inside Pakistan.
36:27The recruiters, the facilitators were still there,
36:31and the poor people were still in the villages.
36:33So while this trade had to go underground,
36:39what happened was when you make something illegal,
36:43you increase the price.
36:44When you just make it outright illegal,
36:47without taking care of the environment,
36:51without taking care of the root cause,
36:53without, you know, taking care of all the moving parts
36:57that make this trafficking work,
36:59you don't solve the problem.
37:02Although they outlaw the practice,
37:05it's still ongoing.
37:07I mean, you can see Pakistanis advertising via social networks
37:13that they're willing to sell kidneys.
37:17And in the end, what the facilitators leave behind them
37:22is a trail of dead bodies in many cases.
37:26Because when you're doing this illegal activity,
37:32and you're obviously not careful because you're after the money,
37:36you obviously don't have the proper means,
37:40the proper place, the proper surgeons.
37:43But this is a risk that many families are willing to take,
37:49and many desperate people are willing to take,
37:51and this is what the facilitators are counting on.
37:54They just want the money.
37:56And they, for them, it doesn't matter
37:58if the patient dies, if the donor dies.
38:07The gangs recruit their victims
38:09from the poorest parts of Pakistan.
38:13I'm on my way to Koht Momin,
38:15a town in central Punjab
38:17where agricultural workers get paid less than a dollar a day
38:20to pay off their huge debt to landowners.
38:26This town holds a morbid open secret.
38:29Almost all the people here have sold their kidneys.
38:38Zafar works as a social activist documenting the hundreds of villagers
38:42who have fallen victim to the trade in human flesh.
39:00Many of the villagers are now left with severely debilitating illnesses.
39:29Zafar has received a call from his friend.
39:33Having already sold his own,
39:35the man is now planning an unthinkable step.
39:39Selling his children's kidneys.
39:43Zulafkar, an agricultural worker,
39:45is in debt to his landlord.
39:58So, Zulafkar, what you're telling me is,
40:00you have three children,
40:02you're in $3,000 debt,
40:04and you're thinking about selling their kidneys.
40:07Is that correct?
40:08It's just like a medical product.

41:16Zulifkar had his own kidney removed from the Mukhtar Kidney Center in 2001 and was thinking of taking his children
41:23there.
41:24But Zafar has persuaded him not to go ahead.
41:27The Mukhtar Kidney Center is infamous for botched black market surgeries.
41:42It made the news in 2016 when 24 laborers were held against their will for forced kidney transplants.
42:07The recipients had travelled from all over Europe, America and the Middle East.
42:12Many of them returned home with complications.
42:16The lead surgeon, Mukhtar Shah, was put on a U.S. sanctions list of human rights abusers.
42:22His son, Dr. Zahid Mukhtar, and one other were also arrested.
42:26But all of them only served a short time in jail.
42:33Pakistan claims to be clamping down on the illegal kidney trade.
42:37But Dr. Zahid Mukhtar is still allowed to practice today.
42:43There are people who are working in the lower levels of the mafia who are being jailed and prosecuted.
42:50But it's the ones that are in the higher levels, the doctors, the surgeons, who are able to run away.
42:56Take Zahid Mukhtar, for example. He went to jail for a year, got out and is continuing at the kidney
43:03center.
43:08The victims in this vile international trade all have something in common.
43:14Whether they're undocumented migrants in Mexico, political prisoners in China, or bonded laborers in Pakistan, they are all from the
43:23most vulnerable parts of society.
43:27As wealth disparities widen across the world, the black market organ trade has become the grim new face of global
43:35inequality.
43:37If the wealthy nations feeding this demand don't take responsibility, it sets a dangerous precedent where poor people's bodies are
43:46allowed to become just another commodity for international organized crime.
43:53The illegal organ harvesting that happens all over the world is a global scourge.
44:00It's a hideous form of modern slavery because you don't just use other humans, you cripple them for life.
44:10It's really fueled by poverty and by conflict. Poverty and conflict create these masses of people who are displaced, who
44:21are disempowered, who are voiceless.
44:25Official estimates are that there are 12,000 illegal transplants each year.
44:33Now, because of the hidden nature of this crime is really hard to verify these numbers.
44:42But in my opinion, they are very conservative.
44:46The exchange of money for organs can never be ethical because that's already a level of coercion.
44:54This is one of those crimes where there's a need for kind of a global law enforcement unit who's able
45:06to understand how the traffickers work.
