02:30thank you good right what are we doing for you today um well basically i've got lots of lumps and
02:35bumps okay on my arms but they've multiplied over the years so when did you notice your first one
02:43about 10 or 11 years ago i just find them a pain in the arms they irritate um they get in the way
02:54they stop me sleeping well and i just think that i would be better off without them
03:01how else do they bother you what do they prevent you from doing that you would otherwise um they
03:05prevent me from going around like this um i don't usually wear anything sleeveless and it's irritating
03:11the people ask me about it to be honest um i'd rather they just didn't notice
03:16i feel that i've changed since um i've had them that i'm less outward going i just like to
03:25keep within my own bubble my family and uh stay at home it makes life a bit easier
03:32and how many do you have have you counted um i did at one point i think probably about a dozen
03:39over the two arms okay so you would rather replace those lumps with scars oh definitely
03:45okay what we'll do is we'll get you on the couch i'll have a quick look they always say don't put
03:51off till tomorrow what you can do today so here we go let's see i'll have to take that out because
03:58i won't be able to
03:59right let's start marking these and see where they are so we've got one just there the first one was
04:10on my wrist which stopped me wearing a watch i tried wearing it on the other hand but that didn't
04:14work get this turn your hand over to quite a few here i think more than what you think
04:21dr shruss is an expert finding them let's have a look one two three four five six seven eight
04:30nine ten eleven twelve thirteen you were 13 on one arm before you had a dozen on both
04:36we haven't even started counting let's come on to the other side now let's just have a look
04:43when patients develop lipomas they can develop multiple particularly in the arms and the legs
04:48one two three four five six seven eight nine ten eleven twelve thirteen on this side as well
04:57well either way i lied to you when i said there is a limit to how much anaesthetic one can have
05:04too much anaesthetic which also contains adrenaline will have an impact on the heart my plan was
05:09essentially to try to get rid of the ones that were more noticeable or the ones that were causing her
05:14the most bother would you rather do one arm um i think it's both arm okay um so one two three four
05:23five six i'll say another one here seven when the doctor said that he was going to do seven
05:32i was well i was pleased he was going to do any we'll try to do as many as we can as long as we're within
05:38that safe limit of anaesthetic okay most of them were quite superficial this usually means that you
05:44don't need too much anaesthetic in each individual lipoma it will sting as the injection goes in that's
05:49probably the worst bit actually it is the worst bit yeah
05:52how's that okay not too bad no not too bad at all actually so we'll come up here now okay
06:07i decided that i'd like if i had the procedure to watch because i've always been interested in seeing it
06:16on other people and often wondered what it would be like to have it done to myself
06:21the interest to see what little nasties are in there at the end of the day it's part of my body
06:27even though it was a bit that had gone a bit wrong um i wanted to see it go right where you want to start
06:34uh the wrist that's always bugged me the most okay ready yep
06:39feel sharp no that's the right answer
06:51there it is can you see it trying to come out
06:57amazing there we go
06:59so that's just fat in there is it that's fat yeah
07:15i've often wondered how you do that
07:19because you sort of knot it don't you the sutures yeah yeah
07:22i see it's like when i do my hand sewing you do a double loop and then pass it through don't you
07:31that's exactly ah now i know
07:35louise was very experienced in stitching and sewing um so i felt a bit under pressure i had to make sure
07:41i gave her my best work hopefully you're not planning on doing one of these yourself
07:46why would you let me try on you no
07:51i'm quite good at sewing what about the cutting bit

08:00well they're quite forthcoming aren't they so far
08:03yeah not bad at all behaving that's what we want well behaved ones
08:10well not likely from me but now so far they've been good so far i wouldn't count the chickens
08:20we're definitely going to need um a couple more of these sutures okay fine a couple more
08:40i need to need a little bit of loosening
08:47what's it touch deeper down this is a little bit deeper yeah
08:51quite good at this aren't you i have done it once before
08:57the more messy ones where they had fragmented bits on them um i don't know it looks a bit like
09:03scrambled egg i suppose
09:05so we don't want to let go does it no i want to try and get the whole thing out in one rather than
09:14break it up because then you sort of know that you've got it all
09:17that's left quite a big dent in there isn't it yes where your skin should have been in the first
09:35place well it's interesting enough seeing it done i do find it strange when patients want to watch
09:43their procedures i've had some patients film their own procedures which is always a bit odd
09:48that's a blanket stitch you're doing there
09:51if i ever had a procedure there is no way i would want to watch it
09:55coming up you see that it's a sister it's really stuck in there you would have a lumpy right in there

11:13hi this is duda okay so how can i help you today so i'm here today because i've got a lump uh that
11:20came up a few years down the back of my head okay and um i'm done living with it is it growing bigger
11:26or changing uh it seemed pretty big instantly oh right i was actually recovering from something else
11:31at the time so right right it mine starts going uh all right let me have a look at this okay so have
11:38you ever had anything like this removed before no nothing sort of skin related i've had only bone
11:43related right okay oh yeah it's a squishy little guy me and my fiancee have been engaged for coming up
11:50nine years now things like weddings got put on hold knowing that there's a lot of people behind you
11:55having to worry about them looking at the back of my head rather than shannon who they should be
11:59looking at that day so this is probably a cyst it feels too mobile and fluid to be a lipoma
12:06you can get lipomas in the back of your head um but this feels more like a little cystic thing
12:12all right um great news i can definitely remove this today oh that'd be brilliant when dr natalia
12:17told me that she was able to remove the lump today uh i was just a great sense of relief and i was
12:25so glad that the lump was finally going to be gone all right let's do some local anesthetic now this is
12:30the bit you're not going to like you'll start stretching sharp how we doing yeah it's fine
12:39the injection side of things for me kind of got used to it over the years now
12:42so everything is numb so you shouldn't feel anything you might feel some pressure problem
12:58sam you mentioned you were recovering from something i had something called a
13:03atypical cartilaginous tumor oh to the top of my left femur
13:09um turned out to be a bone tumor um and it was within a two or three month period that i had to
13:19have major surgery and at the time the twins were only just turning four we went for a lot and the
13:25girls saw me in a place i probably wouldn't want to see me you know you feel vulnerable i actually
13:30delayed the surgery to after christmas uh just to have you know a normal christmas with the kids yeah
13:36yeah before going through that it was worrying my fiancee was great she managed to to get me through it
13:42so i was really appreciate her for that
13:51can you see that it's a cyst yet or i don't know because it's not showing up as a cyst
13:59when i was cutting into samuel's lesion on his scalp i was expecting to see kind of the shiny white
14:04surface of the cyst so i was going really slowly to make sure i didn't cut into it
14:09a cyst would have already appeared
14:14what i ended up cutting into was a ball of yellow tissue
14:19you had to have a lump without anything there it is a lipoma
14:25i was really surprised to see a lipoma under his scalp tissue when i was removing it
14:31i just didn't expect it to be there and it was really deep it's really stuck in there
14:38a little baby life home is it yeah wow congratulations
14:43not what i was expecting to hear today i was almost adamant from what i'd heard and what i'd felt
14:49that it was a cyst wow
14:51you got it yeah but thank god he has lots of hair so he'll never see that scar
15:05what are we doing now stitching sorry oh okay
15:08it's the thing stitching i thought it's like something different
15:12all right hard part is done everything's closed next project clean up sam
15:16just to know that once i'm healed up i'm just going to be able to feel the back of my head
15:23and not have to worry about feeling that people are looking at you
15:27and people do ask the questions so it'd be nice not to have to answer it again
15:31i can feel the difference completely good

16:12back in surgery room two dr adele is powering through seven of louise's lipomas
16:19if i was going to rate dr adele's blanket stitching compared to mike i would say he's almost on a par
16:25it's what you call it it's called a running locked stitch his um gapping was slightly uneven so he
16:33could practice getting them a bit more even but other than that i'd say out of ten i'll give him a nine
16:40right now um for the other side now let's have a look i think i'm addressing something you find it
16:46easier working from one side or the other um no no just admit this feels sharp first does that feel sharp
16:54no that's fine sure are you right or left-handed left do you knock the arm quite a lot i'm always
17:02bumping into things and we live in a small cottage it's uh it's a lovely little place in its own little
17:10bit of garden do you like gardening my husband does yeah i i do the tomatoes and the greenhouse things he
17:18does the main veg potatoes and leeks and carrots and
17:26she was chatting to me and asking me about our lifestyle and everything and then i suddenly
17:31realized that she'd made me miss one of the procedures
17:36you've been chatting to me and i missed that one coming out i know this call a distracting technique yes
17:42that's that's not good enough you know but i wasn't gonna miss another one feel sharp no
17:50some predictable how people can feel having surgery on themselves
17:56if they can faint and then there's the blood
18:01so distracting is the main thing that we have to do to take their minds off
18:05it just comes second nature to just talk to them distract him
18:10now you didn't make me miss that one you said you didn't let me
18:13now the last one yes okay just tell me does that feel sharp
18:18no that's fine sure positive
18:24so this one doesn't seem
18:25has formed seems a bit more spread out is that going to be a pain no just it means it won't come
18:34as one uh that big lipoma near my elbow was brilliant to see come out the doctor thought
18:42he'd got the majority of it out and then poking around the corner there were a couple of little
18:48pockets so sorry even more come out which was brilliant i thought the more you can get out of that
18:53little so-and-so the better
18:59good you definitely lost skirmish yeah i was so pleased these lumps and bumps have sort of kept me
19:09a bit of a recluse indoors because i haven't been comfortable going out you've done so well i've just
19:17had the best time and got through it with much more ease than i anticipated is that me all done now
19:26yeah we're just gonna lower the bed on that side
19:31okay lovely thank you getting a bit too comfy there got get up now
19:36okay louise how was that oh brilliant absolutely thank you any pain no none whatsoever good so we've
19:47got rid of seven in total yeah you want to have a look yes please maybe even feel them let's get a
19:54pair of gloves i don't need gloves do i you don't have to but i'm well you can feel better can't you
19:59without if you want to but each bit is quite firm isn't it yeah there's firm bits and then there's softer
20:09bits that's you that is me i know that's your dna yeah you need to feel with your bare hands if you're
20:17going to experience um what exactly it was that was ailing you for so long and i i was glad to see them
20:24go so i thought say goodbye properly looks like you could fit a good few organs oh gosh no we all
20:33the only organ we treat is the skin that's it i'd feel probably a few pounds lighter with all the bumps
20:39gone um but generally happier myself you'll let me shake your hand now yes i will yeah absolutely
20:47i'll be able to wear tops like this which i don't usually and get a bit of sun on my skin rather than
20:54not wanting to go out in the sun because it's too hot with all the clothes i've got on thank you
21:03if he was in a culinary place i'd say he was a five-star doctor i feel a bit and he is seeing
21:10her touching with the hands it is part of her body so yes i know but still
21:15coming up i imagine i look like one of those uh flowers yes we can draw a flower on your head if

22:29angela now tell us how we can help you today when i got pregnant with my daughter i got these skin
22:36tags onto my armpits and and i've never been able to get rid of them okay and it's just happened
22:41during the pregnancy yeah and it's not been something that you're getting new ones coming
22:45up yeah no it hasn't got better or worse okay but obviously it's concerning you yeah and yeah it's not
22:52good for my confidence of course has it been stopping you from wearing certain clothes yeah
22:56like i won't wear strappy summer tops or dresses and i won't go swimming or to the gym okay um so if
23:04it's okay we'd like to have a look at that i feel like my life has been on hold it gets me down
23:09it's hard to shave and you know it's always there i see it and it's not nice maybe start on the right
23:14side i've got a number of skin tags there and they all look completely harmless i feel embarrassed
23:21and shy and insecure about anybody seeing them like family members especially little kids that point them
23:27out do you want to take a seat for me so it definitely is possible for for me to remove
23:35some or all of those skin tags for you i would like them all to be gone so i can get my confidence back
23:42yeah so we can certainly aim to do all of them for you yeah yeah we're planning our summer holiday
23:48it'd be so amazing to just be free to you know lift my arms up and play with the kids and it'll just be
23:56amazing so first of all a little bit of cleaning okay so really cold there we go easy
24:08there we are you okay there you're feeling all right yeah i'm just really nervous don't worry if
24:14you need a break at any time you just let me know okay let's clean the other side as well
24:18i felt a little bit anxious because i didn't know what to expect what i'll start to do if you're
24:25comfortable is i'll just start injecting a few of these and see how it feels for you is that okay

24:35and have your elbows yeah how's this feeling for you is it okay yes okay good so unfortunately it's
24:44very simple procedure the only bit is having so many injections and you've done really really well
24:52yeah okay so that's one side done in terms of injections
24:56it's dr magnus he's kind and i think that eases my anxiety
25:07so we'll just leave that for a minute so you've done the hard part you can relax
25:12let us know if and when you're ready to continue i'm ready when you are yeah

25:29be good to get rid of these won't it it would be brilliant what are you going to do when they're
25:34all gone then what are you going to change you know get some new clothes or what are you going to do
25:37and go buy some new t-shirt like tops yeah vest tops yeah super duper that'll be good won't it
25:43and i have all new vest tops in all different colors exactly
25:52it's very easy to remove skin tags so we just put a little bit of local anesthetic at the base and then
25:56remove them flat with the skin surface and use the hyphuricator to seal off any any tiny blood vessels
26:02causing bleeding so very straightforward procedure is that okay yeah
26:06just finishing off this side now okay that's that side up you want a little bit of a break or are you
26:16okay no it's okay just keep going shall we yeah halfway there now all right then so you all right
26:25i can feel that one can you let's just put some liquid and set it into that one really little injection
26:33coming there we go nice and numb now
26:45okay now yeah good so did you try any sort of home remedies anything like that i've tried all sorts of
26:52stuff yeah what you tried dental floss yeah nail varnish i wouldn't recommend this just going to
26:59seal off the time of my vessels in those areas and then completely finished all right
27:04i wouldn't do diy surgery at home i think it's it's not a clean environment there's risks of infection
27:10so it'll be better to get it done in there in a clinic
27:14okay so that's the it's all done thank you is there anything you're worried about now i need
27:19about getting blood on my jumper on the way home okay i'm really excited that now i can lift my arms
27:25up and not have to worry about anyone seeing anything under my arms take a seat so do you want
27:33to see the ones that are removed no thank you don't blame you that's no problem at all all right

29:01years old i live on the south coast of england in a town called eastbourne and i'm a photographer and
29:06filmmaker about two years ago i was diagnosed with multiple myeloma which is the kind of cancer which
29:14got into my spine so i've lost four or five vertebrae after the recovery from the cancer i've decided
29:22i need to start enjoying life more and treating myself more okay anthony the doctor's ready for
29:27you down the hall on your right okay thank you no problem anthony's booked in with dr dev shah
29:37hello hi come on in hello hello how you doing okay thanks anthony nice to meet you yeah thank you
29:45everything okay a little bit nervous but uh this has been growing and growing over the past probably 10
29:50years maybe more a treatment for cancer part that makes me very photosensitive sometimes wear hats
29:55but obviously the hat comes down to where that is so it's very uncomfortable well like i sometimes
30:02people make sort of rude comments they say you've got a devil's horn gets a bit tedious after a while
30:07what do you do as a job photographer and filmmaker oh lovely you mentioned you were nervous
30:12um tell me what what's making you nervous i hate injections and sometimes they explode and
30:18because of its location i'm a bit worried it's going to go all over my face or something okay all
30:23right so i'm just going to have a feel i'll probably take a picture yeah and then i'll show you what we
30:27can do for you all right so it's pretty mobile yeah anthony's lump felt a bit like a rubber ball under the
30:36skin i felt that it was probably a cyst it wasn't hard enough to be bone and it wasn't soft enough to be
30:42the lipoma just stay as you are so having had a look good news is we can treat you today okay that's
30:50good yeah so if i come a bit closer to you my aim is not to burst it the main problem that happens here
30:57there are nerves that go from your eyebrow all the way up and supply sensation to your scalp right
31:04when you have cysts in this area sometimes they are below the muscle of your forehead if they're below
31:10the muscle then this nerve is likely to be cut right which means that you'll have a bit of numbness
31:16literally as a band on your scalp and that could be permanent okay if you cut one of those nerves
31:24doesn't cause any problems with appearance but it can just be annoying when you're combing your hair
31:28that's the main problem in this area raise your eyebrows yeah and relax again thank you so the reason
31:35i asked anthony to raise his eyebrows is to create creases on his forehead so that i know where to
31:42place the scar which would make it hide in the normal folds of his forehead drawn an eye on your
31:48head you look like a cyclops i have drawn a sharp scratch coming up okay yeah and breathe how are we
31:58doing how's that okay coping yeah i was very nervous going in but i hardly feel a thing it's really easy
32:05process deb's obviously a professional well done give you a little bit of a halo i imagine i look like one
32:12of those uh into school flowers we can draw we can draw a flower on your head if you like
32:19the petals i think anthony was talking about maybe the flower pot men from way way back you had a
32:28little person with a flower around them um who was called weed can i just scratch my eye no
32:41where do you want to scratch uh below my eye near my near my nose this one yeah she got some tissue or
32:47something and wipe my nose for me that's it thank you yeah let us know if we cause any pain at any
32:56point all right how are you doing under there my love yeah okay thank you good well done
33:17as a cyst grows and overlying skin the skin becomes much thinner and it was very obvious how i had to
33:26take a much wider margin of anthony's cyst to ensure that we don't cut the cyst wall and cause a big sort
33:35of eruption sounds like a sniffing it is what we are he's literally using some scissors just to free up this
33:45cyst ah you okay under there yeah you're fine yeah good the cyst forms from the upper layers of the
33:53skin and grows down so i need to make sure that we can peel it off the nerves at the base to avoid any nerve
34:02damage
34:16wonderful
34:20you're going to hear a little bit of buzzing and that's just us stopping a tiddly bit of bleeding
34:25can i just have a look through it oh yeah perfect yeah any pain in no
34:35i'll let you know if there is okay good okay
34:44so we're putting some stitches in now okay it didn't burst that's good okay yeah
34:49beforehand i was a bit concerned that might burst and uh i'll be covered in in the contents of it
34:57that was nice to avoid that you haven't got a lump on your head now that's good looking forward to
35:03showing my wife and see what she thinks of it swing yourself this way she's a nurse herself so uh she
35:08might even try and take the stitches out herself how are you feeling yeah good thank you it's a lot
35:13easier than i thought it was going to be same with us came out beautifully shelled out i'll let you
35:18have a look at it this is the um bit of skin that we've removed that's front yeah can you see that
35:26that's the top it's quite bizarre seeing the little fold of skin still attached to it do you want to see
35:33what would have happened had you squeezed it yeah okay yeah and now if you give it a little light
35:39squeeze give it a squeeze oh there we go nice yeah do you like that yeah it's pretty disgusting
35:47okay thank you i'll take your glove i'm feeling pretty good right now i'll be able to go without
35:52worrying about people making sarcastic comments be able to wear my hats now in the summer look after
35:57yourself yeah nice to see you i was quite nervous going in about what could possibly happen but they

37:22i call it i've had it 25 years the doctor first exercised it incised it i'm not quite sure what
37:29the wording was yeah um but it grew back so they probably just popped it and i think they popped
37:35okay so it sounds like a cyst then yeah yeah and then over the years i did the same thing oh you've
37:41popped it as well i have tried what came up gunge cheesy gun cheesy guns okay but then it grew
37:48grew back again all right do you mind if we have a little look at it with pleasure all right i'll just
37:53get some gloves i'm very conscious of it if someone puts their arm around me for instance as a
38:00gesture of friendship it's not comfortable so this time i want to get rid of it once and for all
38:05let's have a look yeah oh it's very mobile yeah it's got like a head to it here yeah it's kind of a
38:13weird head trying to escape out of this little bit here isn't it yeah okay fine lovely yeah yourself
38:21back down right philip so i do think we'll be able to remove this today wonderful okay so we'll try and
38:30get it out in one piece right right we'll pop a gown on you okay greek style with one shoulder out
38:38it does have an impact you know if you're sunbathing you're very conscious of it it does
38:45inhibit me wanting to go out and go for a swim it's quite big isn't it well it's very prominent

40:01obviously there's not much space between that and the cyst wall underneath so it's really easy when
40:06you make that first incision to go through the cyst as well you okay there i'm not feeling any of that

40:29i just love it when a cyst bursts and um and i get to give it a squeeze
40:34so it did pop a bit of the insides of it have come out but we're just getting all of the capsule
40:52right now and hopefully this won't come back is it quite deep no it's not too bad actually i think most of
41:01it was sitting proud stretching your skin up and making it look very prominent but it's actually
41:07very it's actually quite superficial very good so that's good yeah you won't have like a big divot
41:13underneath it while we've removed it all right it's all gone
41:20so we're just putting some stitches underneath the skin that will give it some
41:25strength whilst it heals over the next few months with this being on your shoulder you'll have to
41:33really watch your activity for the next four to six weeks yes do you think you've got the whole
41:38thing out yeah as well yeah brilliant did it have a name no we weren't on first name today
41:45well it's covered now so if you just want to feel how flat it is before we put this little
41:54pressure thing on you can just feel it because it's covered now oh yeah yeah yeah it's a lot better
42:00isn't it yeah yeah i went into the clinic not knowing if i was going to get it done and it's gone
42:08there was no pain it was in out and it was done so you can look at your shoulder in the mirror oh yeah
42:16so that's just the dressing yeah but there's no big lump yeah fantastic that's a bit of improvement
42:22isn't it yeah getting rid of my cyst will make a change i won't be conscious of it it'll just be
