01:08smaller than what i thought size isn't everything d whiz way bigger than i thought it would be but some
01:15decisions are still massive i need to really think about you because i don't think they want you to
01:20ruin my tattoo at the same time yeah and the impact is huge they do affect my self-confidence
01:26they got pointed out to me q skin a&e wow yeah thank you so much way

01:48looks like dr emma and donna are ready for their first patient my name is jed i'm 34 i'm a project
01:58manager and i'm from nottingham so i live with my partner a four month old and twin boys aged seven

02:30thank you all right i'm good good what can we do for you i've got three lumps on my back they are
02:37literally in this section of my back how long have you had them tell me a bit about them what sort of
02:43i've had these since i was about 14. okay so quite a long time i don't think you look old
02:48i am 34 now 14's young to get a lipoma yeah yeah so you can remember them since i remember 14
02:54because they got pointed out to me because i didn't notice that's rough at 14. yeah
02:59i was in a football changing room and i was tying my shoelaces and that's when it's worse you can see
03:05them more and i got told what are them three lumps on your back and ever since it's just like
03:12yeah it's just had a massive impact on me because it's i don't want to be ridiculed for them
03:18well my mum took me to the gp at that point there was going to do something about them
03:24but the waiting list was x amount yeah went back and they basically said we don't do them anymore
03:30but the worst thing is i drive for a living so for work so you're resting on them all yeah yeah and
03:35because they're in this like lower part it just i have to keep getting out of the car standing up
03:40stretching because it's just like someone's just poking you in the back and you're like all the time
03:46on the car so it's not oh that's not good no so should we have a look at them and see if there's
03:51something we can do about these when the lipomas and cysts start to give symptoms like pain or
03:57discomfort or they're getting in the way of activities particularly things related to your job
04:02or you know lying down comfortably to sleep at night then you know these should be referred
04:06these should be removed because they're affecting that person's quality of life
04:10so there is oh yeah one there yeah and then one there and then one above it oh there's two here
04:17yeah there's two there yeah it can oh it's just that's a sizable one one and then this one
04:26i have never taken my kids to a public swimming bath i don't want to be the dad that sits there
04:33in a t-shirt i don't want to take my t-shirt off because i've got these lumps on my back
04:37so i want to feel the ability and the confidence to go and do that for my kids
04:42so what can we do about it we can remove them simply with under local anesthetic so we need to
04:50put quite a few injections in there make a little cut over the top of them and then have to sort of
04:55gradually tease them out or squeeze them out and see if we can get them all out in one piece
05:00ecstatic to be honest people can see the front but they will never see the back
05:05so this to be happening is the biggest relief of my life i'm just going to mark these up so
05:11all right you're going up in the world
05:20about there is that height good let me see so gonna do your local yeah
05:31no jad you can feel the liquid go in this should go numb really quickly okay it's like ice like a
05:42really cold sensation
05:44okay do let us know if there's anything you feel that's uncomfortable okay

06:08good feeling about this one but it's going to come in warm we'll see when i think something's going to
06:18be easy it always turns out to be difficult so with your job you don't do lifting do you is it just
06:24driving or i'm a project manager ah so yeah i wouldn't say i do the heavy lifting that's good
06:30then because we wouldn't want you doing that for a few weeks a few weeks all right oh
06:36come on come on come on oh it's going head down
06:45this is just number one i know i know jed's first lipoma was a little bit tricky to remove
06:55it might have had a little bit of trauma or it might have been a little bit tethered to some of
06:59his back muscles underneath
07:00and sporty are you sporty i am sporty i oh you'll have to be not be sporty i know
07:11is it football uh football i've always been into football since when i was five
07:17was that a beckham influence 100 the hairstyles over the years when i was a kid definitely
07:23definitely he had an influence on i think from the curtains
07:27i've just this one might be a bit calcified just might be a little bit scar fibrous tissue around it
07:32but there's a lot out but we're waiting for that now there's that pop yeah when that last bit
07:39comes out and it kind of flops out onto the skin it's um a bit like a
07:45yay i heard that you did you did you feel it i didn't feel it i heard it oh it's beautiful
07:58it is there was a lot of pushing and squeezing on it so that's the daddy right but as soon as that pop
08:07happens it was like right that's one gone right one down two to go
08:11hopefully the next two are going to be easier but i'm not going to say anything so i don't jinx it
08:17coming up we had a barbecue the other day and the smell reminded me of that

09:28so i have lots of mold on my face which i'm very very um self-conscious of have been for ages
09:35okay i've had people in the park just you know point and say oh moly and spotty and
09:41they do affect my self-confidence you know over the years i have become aware that it it might
09:46be the first thing people notice when they look at me and what happens when you go in the sun
09:50i don't really go out much in the sun okay um and i cover myself in sun cream so do you have a hat
09:56as well yep good hat sunglasses yeah saying all the right things yeah well done saying all the right
10:01things which moles in particular bother you so there's that one right in the middle of my forehead
10:06the one on my chin there's this one on the side there and there's two there
10:12okay fine so we'll try and deal with as many as we feel possible yep okay let me just have a look
10:18at them do you want to remove the makeup on the ones that you want done okay i can't tell you how long
10:25i've wanted just clearer skin okay let's just have a little look here so we'll just have a look at this

10:32so this one looks absolutely fine nothing to worry about when you do have moles one always has to
10:43be more cautious in the sun because the sun can damage them that one is okay as well
10:50but the sun can create skin cancers on normal skin so you don't always have to get a skin cancer
10:57on a mole that one's okay right so they're all okay okay let's mark them i would suggest you only
11:04have one done here okay uh because they're too close yeah okay if you took two moles out and they're
11:11very close together you automatically give yourself a bigger scar than you need to it's better to let one
11:18heal and then come back and do the other moles so option one you can have it shaved which will give
11:26you a scar like that but the mole will still be present underneath the surface option two is to have
11:33the mole cut out which would give you a straight line scar that would probably catch one's eye as much
11:40as a mole would probably so i think what we should do is probably shave them okay are you happy to
11:47move forward yeah okay come and have a seat okay all right lovely and relaxed down
11:56sharp scratch coming up okay
12:00good so to shave off her moles i've decided to use one of these derma blades which you can bend to fit
12:14with the contour of her skin in order to remove enough of the mole but not to cause a dip of a scar
12:20okay just let me know if there's any pain all right anything sharp no
12:30there we go that's that one okay did you feel any of that then yeah
12:39mostly while the procedure was happening it was just a bit of pressure
12:54was that squeaking oh yeah it was squeaking i suppose wasn't it anything sharp now
13:06like this chest yeah because it's bending around the chin see yes yeah good idea that's a good
13:14idea very good idea you're very clever that's the comment i wanted you're amazing that's the
13:19comment yeah i have to go fishing for compliments nowadays i think you can tell they've known each
13:26other for a while they've got a really nice relationship not a hint of sarcasm either
13:31they're good at putting people at ease i think done all right so now just a little bit of buzzing
13:38okay okay this introduces a scar onto the skin and hopefully hopefully prevents it coming back

13:51we had a barbecue the other day and the smell from the quarterizer definitely reminded me of that
13:58swing yourself round come and have a little look when i looked in the mirror obviously i've still
14:06got marks there oh fun but it's just the lumpiness is completely gone there's nothing you know i don't
14:13feel any lumpiness anymore thank you all right so it's all gone very well would you like to have a
14:25look at the moles that we've removed yeah there we go there they are your left cheek right cheek
14:31chin and forehead seeing my moles on the trolley was just weird they just look really small and
14:37insignificant you want to say bye to you yes goodbye goodbye good riddance thank you nice to meet you
14:45and you look after yourself haven't you say you feel okay yes i'll definitely feel much more confident
14:51now these lumps have gone i can't wait to get the dressings off and and see what it looks like in a
14:56few weeks
14:57in surgery room two dr emma is in the process of removing three lipomas from 34 year old jeds lower
15:10back jeds first lipoma was a little bit tricky to remove hopefully the next two are going to be easier
15:16i was studying the mirror and my partner was like i wonder if it's going to affect your tattoo
15:33i was like i'm not really that bothered the best one i was ever assisting with on a tattoo was a
15:39tattoo of the topless lady oh yeah yes as he sewed it back up literally the lines came back together
15:47and she just had a boob job boob oh i thought you're gonna say she just had one
15:52it just it came back together unbelievably well
15:59they are an actual comedy duo yeah it's unbelievable oh presby yeah they could both end up in blackpool
16:09tower as peter k's warm-up act oh yeah yeah yeah sorry no no no i'm really pinching you no that's fine

20:37hi right now i'm dr shiraz nice to meet you it's liam is it it is liam yeah all right liam this is
20:44vilma hi all right all right what are we doing for you so i've got a lump on the side of my leg
20:50that developed after having this tattoo you're more than lumps you've got a lot going on in your leg
20:55yeah just a bit let's go through what's on your leg so i'm a bit of a pink fan
21:00okay so yeah i love pink's music and i love pink so uh yeah i've got a bit of a shrine going on in
21:04my leg so i've got 21 pink tattoos but i've got about 30 tattoos in total it's getting pretty full
21:10now pink's not really my style of music and it's not really my choice color for a tattoo
21:16and you've got a lump what color is it it's pink isn't it it is it blends in it had to be pink
21:23all right yeah tell me about it how long has it been there for so i had this tattoo roughly about
21:28six years ago this one and it pretty much started straight away so as expected when you have a tattoo
21:34obviously you have to like you area shaved so you're used to getting the odd ingrowing hair
21:38yeah so i thought potentially might be an ingrowing hair so i kind of had a pick at it like
21:41usually do and couldn't see anything so i kind of just left it and yeah six years later it's
21:46still still there a few years and it's still there it's never really gone away it's grown
21:50slightly actually it's not grown in the last 12 months yeah but i'd say in the last sort of three
21:55years it's probably doubled i started to encroach into one of the tattoos so i'm scared of it ruining
22:00a tattoo but it's also taken up valuable space on the leg so normally with lumps and bumps in your
22:06skin if we can sort of confidently diagnose them then i can obviously tell you what it is and then
22:11you can decide whether we need to take it out or not or if i think it needs to come out i'll tell
22:15you it needs to come out yeah definitely then unfortunately you don't have a choice it just
22:18comes out then um but if it's something um harmless or something benign then so the choice is more with
22:24you whether it really bothers you and you want it out but let's look at it first and then we can
22:29decide okay so let's get you on the couch now
22:31when examining a growth or a lump of any sort essentially you want to first of all make sure
22:38it's nothing worrying and i'm looking at features of pigment any abnormal features that i may see
22:44before i make a decision as to what i can do with the lesion i'm just going to feel it first is it
22:49painful when i touch it there's not like pain or anything to it really so it is quite lumpy you can
22:53feel it yeah it's quite hard to do that gristly right let's have a look
22:56we had the alcohol gel didn't we yeah the gel is here yeah just pop a bit of the gel here
23:06okay so a bit of alcohol gel it'll feel a bit cold and then i'm going to have a look with this light
23:12yeah that's fine
23:13okay now what i'm going to do is connect it to this
23:24dr schras absolutely likes the gadgets the phone the ipad the matoscope yes
23:31i love being able just to show patients what the lesion is what i'm looking at what i'm seeing it's
23:38nice to be able to explain that that's your lump in hd yep zoomed up it's sort of pinkish around it
23:45yeah um and then the center is all white yeah yeah so this is entirely typical for something called a
23:54dermatofibroma okay derma footer footer footer fibrosis or something dermatofibroma is it dermatofibroma
24:03that's it put my teeth in okay dermatofibromas are completely harmless
24:10mm-hmm they're nothing to worry about they usually do occur at sites of trauma
24:16yeah and that trauma can be um an insect bite it can be an ingrowing hair follicle just as you
24:23mentioned so it may well have been something like that which you picked at
24:27and then it caused a traumatic correct me picking at it has possibly made it worse
24:31it will never go yeah it usually won't get much bigger than that so you have two options of what
24:39you can do either we leave it because it is definitely benign or we cut it out the only
24:46thing with cutting it out is it will leave you with a scar and it will encroach in that tattoo so
24:53we've got marker pen so if i draw it for you because you can see it's already going yeah it's
24:57already slightly in that scar so if i were to cut this out i would have to go a little bit beyond
25:04that because you want to make sure you get the whole thing out yeah so i would need to do something
25:09like so it's a lot bigger than you think so it would have to be sort of like this like an eye
25:15around it essentially so you can see it'll pull the wings together so that this wing will be
25:21touching with this wing but it will leave you with the scar from here to here
25:24i guess it depends on how much it's bothering you and whether you want it out or not
25:31the scar is often a lot bigger than what patients think it will be this is purely for cosmetic
25:37reasons a longer flat scar looks much better than just taking the lump out which will then
25:42leave a lumpy scar which doesn't look very good so i guess it's a decision for you to make
25:47whether you have it taken out or whether you leave it oh i need to really think about you
25:53because as much as it's not the nicest thing to look at i don't really want you to ruin my tattoo
25:57at the same time yeah it's the right color it's the right color yeah it blends in oh i don't know
26:04i'm gonna lost what to do it's a tricky one yeah it is i mean because i don't want to ruin my tattoo
26:09my suggestion would be leave it because it's benign it's definitely benign okay if there was any doubt
26:22yeah you know we would have to then take it out obviously if it does grow or change then you come
26:26back to the gp and have a look and they'll tell you and then look at it and they'll send you on if
26:30they're not sure and another thing is healing as well so there'll be a lot of stress and pressure on
26:35like if we cut that piece of skin out other sites heal differently legs when you cut out that type
26:41of skin it doesn't always heal the best yeah it's quite tough your skin doesn't it yeah a lose-lose
26:46situation in the sense of it now i know it's a dermatophiloma and it is benign it's nothing to
26:51worry about so i'm pretty pretty happy now all right liam are you okay yeah we've got all good

27:00i've had it for six years so i'll soon have that then live with a wonky tattoo for the rest of my

27:14song she's a really great artist she'd like her enough to have tattoos no i'm not a big fan of tattoos
27:21liam takes decorating his skin very seriously but what do the dermatology team think when it comes to ink
27:32i have three tattoos i think i would have a tattoo of a chameleon harry potter a batman sign a
27:42butterfly on my shoulder but i'm not allowed to get one they manchester bee on my foot i already have
27:50one tattoo and it is of the lovely continent of africa if i really had to uh it might be my
27:56kids names what about a tattoo of skin a and a no takers okay let's move on next my name's ronnie

28:56how can we help you today so i've had a couple of bumps on my head for several years now but this
29:02one particularly has been growing a little bit and so it protrudes yeah if i go to the hairdresser
29:07sort of bob as they scrape it yes and then it gets infected yeah is it sort of stopping you from
29:12doing things that you would you'd want to do so it doesn't really stop me um it's not particularly
29:16painful obvious sometimes um it's it's protruding so it's obvious so i have to have my hair a certain
29:21length things like that yeah people notice it particularly as they come up close or i'm lucky
29:25i'm relatively tall but if they look over you they definitely see it yeah i remember people would
29:30pass by me and if they're walking but by the back of my chair they said ronnie there's something
29:34looking at me from your head but i take it in good banter yeah but still i mean it's it's not nice
29:40yeah even though you just get on with it it's not yes yeah i'd just like to have a good look at it
29:44is that all right with you yes i'm gonna pop some gloves on yeah i'm not really sure exactly what
29:48they are they could be a cyst or a lipoma so there are two the largest one is exactly here okay
29:54so as they start to grow you start to wonder what's going on you worry are they more
29:59concerning so both of those lesions they do feel like cysts because they're mobile they're not
30:05attached to the deeper structures yeah so the options today we could just remove the larger one
30:10which is growing and leave the other one well alone yeah or we could remove both of them yeah
30:15my gut feeling is definitely the larger one because one way or another i feel like it's going to get
30:20knocked yeah so we can definitely do that for you today assist is a collection of dead skin cells with
30:25a lining under the surface of the skin great just let's just tuck that around you're okay like that
30:35yeah yeah absolutely do you want to shave her yeah should i just shave a little bit
30:40just shave over it and about five millimeters around it but you don't need to go beyond that
30:44dr magnus said that he's going to have to shave around the bum i was somewhat concerned
30:51this will grow back yeah we need to be very careful that hair doesn't get caught in the wound after
30:59it's stitched because of course the hair itself isn't sterile so it greatly increases the risk of
31:04infection i do an awful lot of head shaving around uh lesions it's not because i'm a good a hairdresser
31:12or anything so what kind of work do you do there i work in it actually in the bank yeah have a look
31:18at that and see if that's okay sure that's fine yeah yeah that's perfect yeah yeah how do you find
31:24that then no i've enjoyed it i've been there for for a while and um yeah technology changes at such a
31:30pace yeah so i'm just going to inject local anesthetic so i'll be as gentle as i can but
31:35you're going to feel the injections for a few seconds
31:37can you focus on your breathing
31:43it was a peculiar feeling once the anesthetic goes in it actually felt like i had an ice cube on my head
31:52can you just lift your head up for me sure back again thank you yep
31:56starting the procedure now
32:02was this one ever sort of infected or inflamed in the past yeah so um it's i used to play football
32:23and it brushed past my head yeah i went for a headache and this happened a ball skimmed over the top of my
32:30head and it cut the bump and of course then it got infected that looks good looks like it wants to
32:39come out my wife took great pleasure if i'm honest in squeezing the unpleasant pass out of that but it
32:48just came back later
32:54it's had enough time in there yeah time to go
32:58just try to go out
33:18perfect okay so that's that
33:31okay well it's out that's good i'm just going to be brilliant sealing off the tiny little blood vessels
33:36there yeah so you're going to have a buzzing noise so again it shouldn't be at all painful just
33:42some of you can control sure
33:46what sutures would you like you got a 4-0 molecule
33:50on a small yeah and then a 4-0 pirellium please
33:56the time flew by it felt like no longer than maybe 10 minutes i have no idea how long i was actually in
34:03there but that's how long it felt like we're all done oh great well the next thing you knew the
34:09procedure was done stitches were in all right so um do you want to have a little look at the area
34:14there so if you know can you see that so it's got a bit of gel out yeah i can see it now what do you
34:20think then well i think uh george clooney's got nothing to worry about yeah the shaving bit's fine
34:25but i'm grateful it's gone you want to have a look at the the cyst before you go yeah i would actually
34:30all right that is the little cyst wow that little lump there that little bit yeah it's a little
34:36bean it's like a little bean yeah yeah filled with a sort of a cheesy kind of material some of which
34:41has come out so that's it see you later thank you so much thanks very much you're very welcome all the

36:57going to walk away with the lump removed today but i've just got my fingers crossed the good news is
37:03we can remove this for you today your cyst is actually reasonably big oh wow underneath

37:17you suddenly realize the scale of what it was i thought i was coming for something very insignificant

37:39easy to diagnose you could feel that there was quite a ball underneath the skin with a central hole

41:40he did answer me by saying that his sewing skills weren't that good and i was concerned then because
41:48he was stitching me up there will be removable stitches on the surface how many of those are
41:55there are five great great okay so we've finished when he started to tell me what he was going to
42:04remove i imagined this enormous scar going down my back so to end up with only five stitches
42:10was pretty good come and take a seat thank you well done

43:22it's been so refreshing to be able to um not walk past people and have them say hey what's that bump
43:50sticking out of your head
43:56my moles have healed up really nicely i know it's not that big of a deal to other people
44:00but it's made a massive difference to me so thank you so much
44:03my scar is healing nicely i've just booked a holiday so i can now wear a strapless on top
44:21and i just want to say thank you to skin a and e for everything next time on skin a and e oh
