00:01The Skin A&E Clinic is back.
00:05Morning.
00:06It's busier than ever.
00:08Hi, I'm here for my appointment.
00:10And everything's bigger.
00:12My God, that's huge.
00:14Wow.
00:15You want to get rid of that.
00:17Whoa.
00:18Our team of top dermatologists have moved into a new purpose-built clinic.
00:23Doors opening.
00:25The doctor's ready for you now.
00:29They're tackling more conditions.
00:31Gosh, you've got so many.
00:33Helping scores of patients.
00:35That's like toothpaste.
00:37Is it?
00:38It's coming.
00:39Put years of bad skin behind them.
00:42Wow, you are a magic man.
00:45Wow.
00:46It's amazing.
00:47I can't believe that was inside my head.
00:49Restoring confidence.
00:51He's gone.
00:52And transforming lives.
00:54Oh, thank you so much.
00:55Oh, it's so nice.
00:57This is Skin A&E.
01:06This week in the clinic...
01:08We can't really even count them.
01:09They've got so many.
01:10No.
01:11Everyone's seeing double.
01:12Do you have names for them?
01:14My horns, I got them.
01:15Your horns, yeah.
01:16Bit like two large baked beans, isn't it?
01:18Oh!
01:18I have two identical keloid scars.
01:21Oh, my God, they are identical.
01:23There's no stopping Dr Emma.
01:26How many was that then?
01:27That was it. 22.
01:2822, yeah.
01:29And Dr Adel encounters a worrying lump.
01:32I thought it could be cancer.
01:35But I think we'll have to take it out and see.
01:38It's a brand new day at Skin A&E.
01:42Right.
01:43Okay.
01:43Good morning.
01:44Good morning.
01:46Ready to roll.
01:47Are we all stocked up?
01:49All fully stocked, yeah?
01:50We are.
01:51Are we ready?
01:52I'm ready.
01:53Just wash my hands.
01:55And...
01:56Everyone's ready, including today's early bird patient,
02:0047-year-old Lucy, a nightclub door supervisor.
02:04In my spare time, I like to go into nature and watch birds.
02:08I do have quite a few tattoos of birds.
02:12Kingfisher.
02:14An owl.
02:16And my favourite, which is my Robin.
02:19Okay, Lucy, the doctor's ready for you down the hall on your left.
02:22Perfect.
02:23Thanks.
02:26Bird lover Lucy will be seen by Dr Magnus Lynch.
02:30Hello.
02:31Hi.
02:32Hi there.
02:33Is it Lucy?
02:35It is.
02:35Lovely to meet you.
02:36I'm Magnus.
02:37Have a seat for me.
02:37Nice to meet you.
02:38And how can I help you today?
02:41I've got two squatters on the top of my...
02:45Have you?
02:45My back.
02:46Towards my neck.
02:47What are they called?
02:49They're Barry and Barbara.
02:50Barry and Barbara.
02:52I thought it was Barry, but the ultrasound revealed in fact it's a couple.
02:57There's Barry and Barbara.
02:58Holding hands I think they are.
03:00This is Barry.
03:02And this is Barbara.
03:03Husband and wife.
03:06And how is it impacting on your life and the things that you like to do?
03:10I'm becoming more conscious of it as the more noticeable.
03:13People kept insisting it was actually part of my spine.
03:16And I was saying, no, there's a lump, there's a lump.
03:18Yeah.
03:18OK, let me just pop some gloves on.
03:20I'm going to have a careful look at the area and I might draw on you as well if that's
03:22OK.
03:23That's fine.
03:24Let's get your pen.
03:25Yeah.
03:25Your pen.
03:26They're just here.
03:31So there's one there and one there.
03:36They do feel like little lipomas.
03:39A lipoma is a harmless growth of the fat cells and it is unusual to have two right next to
03:46each other like that.
03:47What I can feel is two lumps, one here, one here.
03:51Whether they're connected or not, I'm not certain.
03:54We'll probably find out when we're doing the procedure.
03:57Yep.
03:57All right.
03:58Would you like me to remove them for you today?
04:01I would love you to evict Barry and Barbara right now if that's possible.
04:06Very good.
04:06OK.
04:07I have got some hang ups now about exposing my neck.
04:12So I think it would be lovely to just go back to being me in my body without extras.
04:21Right.
04:21So you're OK with local anaesthetic?
04:22Not allergic to local anaesthetic?
04:24No, not that well.
04:25Just going to start popping some local anaesthetic in there now.
04:27So I'll be as gentle as I can.
04:29There we are.
04:30So sorry.
04:30Oh, you nasty man.
04:32Sorry about that.
04:35I like to think that I am robust and rock hard.
04:40I felt like somebody was pinching me.
04:45There we go.
04:46Ooh!
04:49Yeah, that's a difficult bit done.
04:51So you can relax now.
04:52I must admit that was pretty wicked.
04:55Yeah, it's quite sharp, isn't it?
04:56But it's all done.
04:57I didn't mean to call you nasty man, I'm sorry.
05:00I've been called far, far worse, trust me.
05:03People, they just get a bit of a shock.
05:06And I have heard a lot worse than nasty man being blurted out.
05:10So I'm just starting the procedure.
05:12Anything uncomfortable, you tell me, OK?
05:23So far, it's looking like a lipoma, which is good.
05:30Come on, Barry.
05:31The lights are on.
05:32The lights are over.
05:33Get out the club.
05:45It's interesting how much you're having to wrestle.
05:47I really...
05:48I'm so glad I'm not watching this.
05:50When you started ferreting about inside,
05:53you feel it, but you don't feel the pain.
05:56So it's really strange to be able to have the sensation of it happening,
05:59but be totally pain-free.
06:03They are connected.
06:05How bizarre.
06:07Yeah.
06:07So these are coming out via the first hole, both of them.
06:10Oh, excellent.
06:11That's handy.
06:11There's only one grave.
06:12They're in bed together.
06:14To be honest, I was suspicious.
06:15I was very suspicious,
06:16because it's a bit of a coincidence otherwise
06:19to get two right next to each other, isn't it?
06:21Yeah.
06:21When I was planning to remove the two lumps
06:23through two separate incisions,
06:25I realised that actually it was a single lipoma
06:28which was connected.
06:30So is it like one but a strand holding two parts together?
06:34Yeah.
06:35I'll show you afterwards.
06:36Oh, I can't wait.
06:37Yeah.
06:37It's a bit like two large baked beans, isn't it?
06:39Oh!
06:41Gosh.
06:43Right.
06:44So, yeah.
06:45Two little ones.
06:46They look like two little kidney beans joined together.
06:52Oh, I heard a right squelch then.
06:54What was...
06:54What was that?
06:56A little bit of just fluid under the surface there.
06:59After, I think, most of Barry had gone,
07:03Magnus dug round a bit and there was this almighty squelch.
07:07So is it Barbara that's sort of clinging on, refusing to go?
07:10I'm just trying to get her out of this small little hole.
07:13When I removed that big lump of lipoma,
07:16I felt that that could have been both Barry and Barbara,
07:19but I wasn't totally convinced.
07:25Coming up...
07:26Oh, I think you've just lost some weight there, sir.
07:29Very good.
07:31Oh!
07:32Oh, that's killing me!
07:33Oh!
07:34Oh!
07:34Wow!
07:35Wow!
07:46It's a hive of activity at the clinic this morning.
07:50We've got quite a few patients on the list today.
07:52You want me to check what we've got running order, don't you?
07:56Yeah.
07:56I'll just say that I've got a bit of a clue.
07:58We're already running late.
08:01Need to catch up.
08:02And next in line to be seen is 30-year-old Gary from Leeds.
08:07OK, Gary, the doctor's ready for you.
08:09It's the door straight ahead.
08:11No problem.
08:13Gary's booked in to see Dr Natalia Spearings.
08:17Hello.
08:18Hi.
08:19Hi.
08:20Come in, have a seat right there.
08:22You must be Gary.
08:23I am, yes.
08:24I'm Natalia.
08:25Hi.
08:25Hello.
08:25And this is Duta, she's assisting me today.
08:27Hi.
08:28Right, well how can I help you today?
08:29So I have two identical keloid scars on the backs of my earlobes.
08:34Identical?
08:35Pretty much.
08:36Oh, they are!
08:37This one may be a little bit bigger.
08:38It feels bigger.
08:39My God, they are identical.
08:40So they are a result of home piercing.
08:44And to be honest with you, they were completely fine for around a month.
08:47And then I took that jewellery out, replaced it with a ring.
08:49Mm.
08:50And that's when it started.
08:52Okay.
08:53And so how long ago was that?
08:55It was around two and a half years, three years ago.
08:57Okay.
08:58Erm.
08:58They started off quite small and the skin was just hard.
09:01And then over the past year, six months, they have just grown and grown and grown.
09:05And this one's getting really hard.
09:07And this one just seems to be getting bigger.
09:09Okay, okay.
09:10And I absolutely despise them.
09:12And I am so conscious of them.
09:15So much so that I wear a hood almost all the time when I'm out in public.
09:19I've been tempted just to chop them off myself, as silly as that sounds.
09:23So has this kind of changed the way you pierce your ears?
09:27Or what's it done?
09:27So it's...
09:28I won't pierce myself ever again.
09:30Good.
09:31I shouldn't have done it in the first place.
09:33Step one.
09:33But I won't be doing that again.
09:35Do people comment on this?
09:37I know people look.
09:38I know 100% people look.
09:41Okay.
09:41You're poor in the earlobe.
09:44Alright, let's have a look.
09:46A keloid scar is an overgrowth of scar tissue.
09:49And it's a reaction to trauma to the skin where your body over heals.
09:52So it creates more of a scar than it should.
09:55So what I look at is the base of it.
09:58So I want to see how thick the base is.
10:02What I'm looking for is to see how much of the keloid scar is stuck to the actual skin.
10:07Because that will tell me how big the open wound will be and the eventual scar that will form in
10:12place.
10:13I can definitely remove these for you.
10:15But I would advise you not to pierce this area again.
10:19Heard loud and clear.
10:20Good.
10:21Okay.
10:22I've got no one else to blame other than myself.
10:24So when Dr Natalia said, yeah, we can remove them today, it was just instant relief.
10:29So Gary, I'm going to do your anesthetic injection now.
10:32I'm just going to pinch the skin.
10:34You're going to feel sharp, scratch, sharp.
10:36And a bit of a sting now.
10:40Do you feel anything?
10:43Um, right now, no.
10:44Okay.
10:46The ear holds anesthetic really well, so it will be numb for a few hours.
10:51To remove Gary's keloid scars, I use a derma blade, which is a flexible blade that's sharp on one end.
10:57And it allows me to kind of wrap it around the keloid and evenly remove it.
11:09Okay, a bit of pressure, but you shouldn't feel anything painful.
11:11Mm-hmm.
11:24Are you okay?
11:25I'm fine, yeah.
11:28Are you squeamish at all?
11:29No.
11:30I'm quite excited to see the result.
11:36Good.
12:00You're going to smell a bit of barbecue.
12:05I couldn't smell no barbecue.
12:07Maybe that's just me though, so I don't know.
12:08Maybe my flesh is scentless.
12:12Okay.
12:13Other ear.
12:18It was actually my birthday a few days ago, and somebody bought me a voucher for my local
12:23tattooist.
12:24I'm thinking of putting it towards something to commemorate.
12:27Today?
12:27Today, yeah.
12:28So my name is spelled...
12:29No, just kidding.
12:31Maybe you might be the first name tattooed on it.
12:32The first name.
12:36Okay, well that one's done.
12:45There was no pain, couldn't really feel anything.
12:48All right, Gary, why don't you have a look, and you can use the big mirror to help you.
12:53Oh my God, that actually is amazing.
12:58They look amazing.
12:59I've not had my ears like this for the past five years or so.
13:03The sense of relief afterwards was just massive.
13:07All right.
13:08So here we go, right and left.
13:10They're slightly smaller than M&M's.
13:12Yeah.
13:12It's more like a Skittle.
13:13I'm gonna get some Skittles.
13:15Are you getting Skittles?
13:17Yes.
13:17Be right back.
13:18That's a new one.
13:24Oh my gosh.
13:27You could use a red Skittle.
13:30Other sugary snacks are available.
13:32Oh my God.
13:33They look like little lost baked beans.
13:37Oh my God.
13:38Gross.
13:40You better hope I picked the right one to put in the specimen pot.
13:43That's all I'm saying.
13:45The report.
13:46It has a chocolate consistency.
13:48Yes.
13:48There's a candy coating.
13:52Okay.
13:56Well thank you for coming.
13:57Thank you very much for today.
13:58Lovely to meet you.
13:59It is gonna change my life so much.
14:02So I have a holiday plan with my partner to celebrate our 30th birthday.
14:06And now I don't have to worry about hiding.
14:09But yeah, I'm excited for the future.
14:18Back in treatment room five, door supervisor Lucy is still under the knife.
14:24Dr Magnus thought he'd removed her twin lipomas, but can still feel another lump at the bottom of Lucy's neck.
14:31Come on.
14:32Where?
14:32Out the club you go love.
14:36It's the snipping noise.
14:38Yeah.
14:38It's just like...
14:40It's quite near you so you can really hear it.
14:42It's like it's cutting parts of me off.
14:44It was like cutting bacon basically.
14:46That's what it sounded like.
14:48No pain.
14:49Just a horrible noise.
14:52Ah, that.
14:52Feeling it?
14:53Coming keen, that one, yeah.
14:55Okay, sorry about that.
14:56Oh, don't worry.
14:57I just...
14:58Just keep telling me.
14:59I thought I'd best tell you.
15:00You must tell me, yeah.
15:00If you've got any worse.
15:01If you don't tell me, I don't know.
15:03I've been meowing like a cat.
15:04You have to tell me.
15:09Wow.
15:10Ah, that's killing me.
15:11Ah.
15:13Wow.
15:14There's some more local anaesthetic.
15:15Wah!
15:17Jesus.
15:17Okay, more local anaesthetic coming now.
15:19Sorry.
15:20Don't mean to blaspheme.
15:22Although there's no important nerves or vessels in that area,
15:25it could be that it was a little bit deeper
15:27than the local anaesthetic was injected.
15:30It's going slowly now.
15:34I did well though.
15:35I didn't say a bad word.
15:36You're doing really well.
15:37You're doing really, really well, honestly.
15:40It's just a bit weird there.
15:42Yeah, I'm just going to pop a bit more local anaesthetic there.
15:46You're nearly finished now.
15:51Okay, perfect.
15:52So we've removed everything that we need to remove.
15:53So that's fantastic.
15:55So that's it really.
15:56Now I just need to seal off the blood vessels.
15:58There were two or three occasions where it was really intense.
16:02But I think now that they're gone, I'm going to be less restricted in my neck.
16:09I'm just going to take a little photograph and then we're going to put the dressing on.
16:16We've got a big family wedding next year.
16:18It's going to be really lovely knowing that I can choose my clothing.
16:21I don't have to worry about, you know, whether they're going to be seen.
16:26That's like a straight line scar.
16:27The edges of the scar are standing up.
16:29We want that because if it's flat now, then it gets dipped when it heals.
16:32Yeah.
16:32So we want the edges looking like a ridge standing up.
16:35So that's all come together nicely.
16:36Would you like to have a look?
16:38Of course I want to have a look.
16:40I need to see.
16:42So if you want to just lift up that top one bit there, have a little look at them.
16:46So basically it's all one thing.
16:48This is just a little separate part.
16:50So basically like this is one of them.
16:52This is the other one.
16:52You can see the little neck between them.
16:55That's just another part that came out.
16:59No, no, no, no.
17:00So how does it feel?
17:01Horrible.
17:04It's two little things.
17:05I feel a bit, yeah, just bye.
17:09Shall we get rid of them now then?
17:11Can you take the gloves off for me now?
17:13It's odd.
17:14Really odd.
17:15Yeah.
17:16It was disgusting.
17:19Disgusting.
17:21Thank you, honestly.
17:23I really am.
17:23Lovely to meet you.
17:24I feel like the weight's literally lifted off my shoulders.
17:28I feel a bit lighter already, yeah.
17:29Dr Magnus, I am so grateful for what you did.
17:33You've been a gem.
17:34Thank you, love.
17:36It's just lovely going forward knowing that they're gone because I've had them for so long.
17:41They've gone, they've gone.
17:45Coming up.
17:46This is almost like hard cheese.
17:53It's a big one, that's why it felt so hard.
18:09At Skin A&E, the demands of dermatology can be very varied.
18:14What did you want me to do?
18:16Look beautiful, darling.
18:18Just look beautiful.
18:20But Dr Dev can't just sit pretty, as his next patient has arrived.
18:2536-year-old Alec from West Wales.
18:29I'm a carpet fitter.
18:31I've been doing it for 20 years.
18:34I did find a diamond ring one day, nailing some gripper down.
18:40Whether it was a real diamond, I don't know, it looked posh.
18:47Hello.
18:48Hiya.
18:48Come on in.
18:49Welcome.
18:49How are we?
18:50I'm very well.
18:50This is Dr Shar.
18:51Hello.
18:52How are you doing?
18:52Come and have a seat.
18:53All right there.
18:54So?
18:54So it's Alec.
18:55Is that correct?
18:56Yep.
18:57How can we help?
18:58I have two rather large lumps.
19:02This one's the biggest.
19:03Oh yeah.
19:04And a smaller one by there.
19:06Oh yeah.
19:06How long have you had them?
19:07I've had them for about 10, maybe 15 years.
19:10Long time?
19:10Yeah, long time.
19:11Started off, oh, tiny.
19:13I'm a carpet fitter, putting rolls on my shoulder.
19:16I keep knocking it, so I don't know whether that's why it's got bigger.
19:21Okay.
19:22But yeah, it's quite unsightly.
19:25How big are the rolls that you have to carry?
19:27Oh, great big rolls.
19:29Over 100 kilos.
19:31Keeps hitting it?
19:32Yeah.
19:32Can I have a little look?
19:33Yeah.
19:33Can I take some pictures?
19:35No problem.
19:35And then we'll go from there.
19:37Yeah.
19:38Do you have names for them?
19:40My horns, I call them.
19:41Your horns.
19:41Well, they are.
19:42They're really...
19:42Yeah, because they're both virtually in the same place.
19:45I mean, if you left them, you could make a career out of it.
19:48Do you know what I mean?
19:49Right, let's have a little look.
19:51Oh yeah.
19:52Okay.
19:53I would guess that that's a sis.
19:57So I'll take a picture now.
19:59If we spin you around so you face Cherry.
20:01That's it, lovely.
20:02Thanks.
20:02Nice.
20:04Now a cyst is where the skin goes into itself and forms a balloon and it's full of dead
20:10hair and skin cells.
20:12Until you actually cut something out, you can never be 100% sure that this is going to be
20:17a cyst.
20:18But I'm pretty confident based on my clinical examination.
20:21That's roughly where it is and that's roughly what your scar would look like.
20:25Oh.
20:25That red line.
20:26Yeah.
20:27Okay.
20:27And then similar for the other one.
20:29Yeah.
20:29Really.
20:30You all right with that?
20:31Yeah.
20:31I'll leave it to you.
20:32When he said he'd be able to remove them today, that made me feel, you know, great,
20:36fantastic and really thrilled to start looking normal again.
20:42If I get you to turn that way for me, let's just have a little look here.
20:49And we're just going to clip a little bit of hair in case that's the noise.
20:54Oh, you're going to want to get rid of that one.
20:56Yeah.
20:58It is a cracker.
21:00It's a good one.
21:04I grow my hair out just to try and cover it and I look quite rough sometimes.
21:12Maybe I could, you know, start looking a bit more tidy.
21:19Sharp scratch coming up.
21:21Yeah.
21:25How's that feeling?
21:26Can't feel it.
21:28Have you got children?
21:29Yes.
21:29I've got a daughter.
21:30She's six or seven in August.
21:32Yeah.
21:33Is she going to miss your lumps?
21:35Yes.
21:35Your horns.
21:36Yes.
21:36She was more worthy than I was.
21:38Oh, was she?
21:39Yeah.
21:43Are we ready?
21:46Okay.
21:47You all right?
21:48Yeah.
21:48Well done.
21:56We're going very gently and we're waiting to see what's underneath.
22:09What have we here?
22:16Not sure yet.
22:17Still haven't seen it.
22:26Is it nice and gooey?
22:28No.
22:29We're all right.
22:30We don't do gooey.
22:31We like to get it out.
22:33In one?
22:34Yeah.
22:34If we possibly can.
22:35Too much rice.
22:36Because with a cyst it's in a sack, is it?
22:38You're right.
22:39Absolutely.
22:40And this is looking like a cyst.
22:43Yeah.
22:45How are we doing there?
22:47Can't feel a thing.
22:48Well done.
22:51Oh, there she blows.
23:00Oh, I think you've just lost some weight there.
23:02There we go.
23:05Perfect.
23:06Perfect.
23:08One's out already.
23:09I thought, God, that was quick.
23:11Lovely.
23:12Okay.
23:13So you're going to hear a little bit of buzzing.
23:15Yeah.
23:15And that's just Dr. Shah just sealing some blood vessels.
23:24You could hear, like, snap, crackle, pop, Blake.
23:27It won't put me off my cereal.
23:30Okay.
23:31To the other side.
23:48Snip, snip, snip, horrid sounds.
24:09Okay.
24:16There.
24:17There.
24:18That little one's gone for you too.
24:21I hope so, lady.
24:22Yeah.
24:24Good.
24:25Done.
24:28Sit yourself up gently and just swivel yourself round and just sit there a moment.
24:32Yep.
24:33Who's that?
24:34Oh, look at him, man.
24:35Who's that handsome fella over there?
24:39Happy?
24:40Yeah.
24:40Good.
24:41Come and take a seat here, okay?
24:43Well done.
24:46How are you feeling?
24:48Fantastic.
24:49Okay.
24:49Yeah.
24:49So it's all come out really well.
24:50Yeah, good.
24:51I'll just show you the pictures.
24:52Yeah.
24:54So that's what you look like on the left.
24:56Yeah, yeah.
24:56That's what you look like on the right.
24:59It's definitely going to help me mentally because, you know, I can walk about now knowing
25:05that I'm looking half decent and people aren't going to be staring at me.
25:08So there's your hair.
25:09Yeah, yeah.
25:10That we've taken off.
25:11There's a bit of skin.
25:12Good God.
25:12Oh, they come out pretty clean, didn't they?
25:14They did.
25:14They've come out nicely.
25:15Yeah, don't they?
25:16Like fitting the carpet.
25:17Cracking job.
25:17That's it.
25:21Thank you very much.
25:22Lovely to see you.
25:23All right.
25:24So I can go back to work now and chuck the rolls on my shoulder now and not have to
25:29worry
25:29about knocking it.
25:30Thank you very much.
25:31Cheers.
25:32Lovely.
25:32Thanks a lot.
25:33Bye-bye.
25:33Bye.
25:35This came out nicely.
25:37Right.
25:38Welshman.
25:38Yes.
25:39I like him.
25:40Welshman.
25:41You like your Welshman.
25:42I like the Welsh.
25:44Free from his horns, Alec will be back to lifting carpet in no time.
25:48But until then, he's wondering which of our derms will be strong enough to help him out.
25:55In my job, I often lift heavy things up to as much as 100 kilos.
25:59What's the heaviest you can lift?
26:03100 kilos.
26:04That's like nearly two of me.
26:06I tried to lift a 20kg bag of soil the other day and didn't manage it.
26:12I've lived my life by one rule.
26:14You should always be able to lift your body weight and no more.
26:17So I can lift 65 kilos.
26:20I think probably 200kg in a squat and possibly also in a Romanian deadlift.
26:26Easy.
26:27No problem.
26:29I'm pretty strong.
26:36Someone who knows a thing or two about pulling their weight is the clinic's next patient,
26:4140-year-old nurse Emma.
26:42I'm a community nurse nurse which means I support children and their families.
26:48It's something which I absolutely love and wouldn't change for the world.
26:53OK Emma, the doctor's ready for you now.
26:55Down the hall on your left.
26:58Emma will be seen by her namesake, Dr Emma McMullen.
27:04Hello, is it Emma?
27:05Hi.
27:05Hi Donna.
27:06Come on in.
27:07Have a seat with Dr Emma there.
27:09Hi, thank you.
27:09Come and have a seat.
27:10How are you doing?
27:11I'm good, thank you very much.
27:13That's good.
27:13So Emma, what can we do for you today?
27:16I have some Seb K.
27:19Seborate keratosis, yeah?
27:20Yes.
27:21I have quite a lot.
27:23Have you?
27:23Quite a large one on my shoulder.
27:24I've had that from being a teenager.
27:26Never bothered me.
27:26That's really early to get them.
27:28Yeah.
27:29Never bothered me.
27:30But recently, I'd say over the last two years, I think that they've probably more than doubled.
27:36In number or in size or everything?
27:38Everything.
27:39Can I ask you, has anybody in your family got these?
27:42My dad has some.
27:43Has he got some?
27:44But nothing like what I have.
27:46Seb Ks are called seborate keratosis.
27:49They are little benign tumours of the base layer of your skin.
27:53And when Emma told me she got her first one in her teens, I was thinking, hmm.
27:57Because we normally don't start getting them until about our mid-30s.
28:01What kind of an impact are they having on you?
28:03Have they stopped you doing anything?
28:04Or you've worn a bikini recently?
28:07No.
28:07No.
28:08The last time I wore a bikini, I didn't have half the amount that I've got now.
28:12And I had a lady sat staring at the top of my stomach.
28:16Has it impacted on any other areas of your life?
28:18I have to have clothes on to go to bed.
28:20I won't go to bed naked.
28:21Just to make me feel more comfortable.
28:23Yeah.
28:24And it's not because of my partner, because he's fully supportive.
28:29Don't bother him.
28:30No.
28:30It doesn't bother him at all, but it bothers me.
28:33It has a big impact on me and my own body confidence.
28:36All the time, every day, yeah.
28:39Right, well, we need to have a little look at them.
28:40Let me grab a pair of gloves.
28:43It's just something that I'm very conscious of, very aware of.
28:48Right.
28:49Let's have a little look, Emma.
28:52Oh, dear.
28:53Loads.
28:54Gosh, you're right.
28:55We can't really even count them.
28:56We've got so many, haven't we?
28:58No.
28:58These ones, particularly under here, they all get very, very, very itchy, especially after
29:02I've had a shower.
29:03Obviously, they catch quite a lot on my bottom of my brow as well.
29:07Can I have a little look at your back?
29:08I prefer to wear dark-coloured clothing, because the spots are so dark, you can see through
29:13the clothing.
29:14I've had people commenting on, have you got dirty on your top?
29:17And actually, it's been the spots underneath that you can see.
29:21Not so many on your back, thank goodness.
29:24Right, pop your T-shirt back down.
29:26So when Emma lifted her top and showed me her tummy, I was really shocked at the number
29:30of seborrheic keratosis she had.
29:32You wouldn't normally see that number until somebody was in their 60s or 70s.
29:37The good news about seborrheic keratosis are they're really simple to remove.
29:40We just gently scrape them off and cauterise them, and I think we can do a good number
29:45for you today.
29:47I don't think I can get rid of all of them.
29:49No.
29:49We'll focus on the big ones first, and then take it from there.
29:53Yep.
29:54When Dr Emma told me she was going to be able to remove some of them, it was music to
29:58me.
29:59Yes, I was so, so, so happy.
30:01So this one, that one is very, very, very itchy.
30:05So this one?
30:07The big one.
30:07This one?
30:08Yeah.
30:08All of these sort of slightly bigger ones?
30:11Yeah, that's fine.
30:11These ones are quite small, but that one's quite prominent on your necklace, so remove
30:15three around your neck.
30:17The reason we don't remove all of them is because there's a limit to the amount of local
30:21anaesthetic we can use, and actually they're really sore in the first day or two after
30:25removal, and it's a lot to cope with.
30:28Right, I'm going to start putting your local in.
30:38I'm going to just check.
30:40Does that feel?
30:41Can't feel anything.
30:43Great.
30:48Okay, just going to start with this one Emma, and just let me know if you feel anything
30:52sharp at all at any point.
30:57All good.
30:58All good.
30:59All good.
30:59Marvellous.
31:06Right, one down.
31:07A few to go.
31:08Yeah.
31:11This is a ring curette, and it has one sharp side and a blunt side, and we use the sharp
31:16side to just gently scrape the waxy material off the surface of the skin, and then we use
31:22a hyphricator to stop the bleeding, which is a little electric current.
31:29I didn't realise it would be this quick.
31:32Six.
31:32I've got to take a little bit.
31:33Seven.
31:34Yeah.
31:36I'll just make sure I've cauterised them all.
31:44Can I watch one?
31:46Yeah.
31:47So, do you want to watch us do a couple here?
31:49Oh, yeah.
31:50Can you see?
31:57Well, it's satisfying, isn't it?
31:59It is.
32:01I could see what Dr Emma was doing.
32:03I could feel the pressure, but I couldn't feel the actual scraping, the tugging of the
32:09skin, the removal.
32:12So, it was an odd experience, but one that I'm glad I watched, and I'm glad I did.
32:18So, just do those ones over there.
32:20I might switch to fresh curate.
32:30Removing seborrheic keratosis is a super satisfying procedure, because they come off really nicely,
32:36really easily, and you sort of get an instant result.
32:41Right, this is the big one.
32:42Are you watching this one?
32:43Yeah.
32:45I'm looking forward to this one going.
32:46Oh, it's tough as well.
32:47Look at that.
32:50That was the one that you could always see through a T-shirt, see through a jumper that
32:54people would point out the most.
32:56Watching it being removed, it was amazing.
32:59It's like scraping off little...
33:01I guess it's more like a...
33:02The consistency is almost like hard cheese.
33:06Wow.
33:09It was like an old, one of your kind of specialised, more fermented cheeses, I think.
33:15So, I won't be having any hard cheese anytime soon.
33:20How many was that then?
33:2122.
33:2222, yeah.
33:24It was so quick.
33:25She was removing two or three within seconds.
33:29Oh, it's like a cuckoo pop.
33:31But we've lost one.
33:33Oh, look, they're everywhere.
33:35They are.
33:36The very last one.
33:38We're calling it 32, then.
33:40Yep.
33:43Last but not least.
33:45Yes.
33:50I thought today I would have a few removed.
33:54Three, four, five.
33:56I did not expect at all to have 32 removed.
34:00I'll have much clearer skin because of that.
34:03Thank you very much.
34:04No worries, it's been an absolute pleasure.
34:06Treating you, Emma.
34:07Good luck with everything.
34:08Thank you very much.
34:09Try and take it easy for a couple of days.
34:11All right, well, these all settle.
34:14Moving forward, it's going to definitely help me with my confidence.
34:17It's definitely going to help me with my self-esteem.
34:21I don't feel like I could have had a better experience.
34:23So, again, thank you.
34:25Oh, very satisfying.
34:30Coming up.
34:32Jodie was worried about cancer.
34:34Erm, and if I'm honest, that did cross my mind too.
34:49After a busy day at Skin A&E, the last patient through the doors is 38-year-old Jodie, a
34:56cleaner from Somerset.
34:58Jodie, let's check you in.
34:59My favourite part of my job is going into somewhere really, really gross and then getting it sparkling.
35:06I love it.
35:09OK, Jodie, the doctor's ready for you.
35:11Down the hall, on the left.
35:12No problem.
35:15Looking after Jodie today is Dr Adil Shiraz.
35:20Hi.
35:21Hello.
35:22Let's have a seat.
35:23I'm Dr Shiraz.
35:24Nice to meet you.
35:25And we have Jodie.
35:26Yes.
35:26Is that right?
35:27Right, Jodie.
35:27What can I do to you?
35:28I have a lump in my arm.
35:30Yeah.
35:36It's OK.
35:38Oh, yes.
35:40Have you ever done anything about it?
35:41I went to the GP.
35:43They've done an ultrasound scan on it.
35:46But that was the last of it.
35:49How does it affect you?
35:50It's painful.
35:51It...
35:52I can't lie on it.
35:53I knock it.
35:54It's embarrassing.
35:56It...
35:56It affects me all the time.
35:58I just...
35:58I hate it.
35:59I feel disgusting.
36:01Right.
36:02OK.
36:02It makes me feel horrible.
36:03It makes me feel dirty, I guess.
36:07Because it's...
36:09It's not...
36:09It's not supposed to be there.
36:12Oh, God, I'm going to start crying in a minute.
36:20OK.
36:22Erm...
36:22Fine.
36:22Right.
36:23Shall we have a look at it?
36:23Yes.
36:24Let's get you up here.
36:25What do you do for work, Jodie?
36:27I'm cleaner.
36:28Does it affect your work at all?
36:29Yes.
36:30When I'm...
36:31Reaching over, I'm cleaner.
36:32It's my dominant arm, so, like, I feel it quite often.
36:35I...
36:35I knock it quite often.
36:38I remember the first time one of my clients pointed it out,
36:41and I just stood there and cried.
36:45Is it OK if I feel it?
36:46Yeah, go for it.
36:48Yeah.
36:49So, it's quite...
36:49It's quite firm, isn't it?
36:50Yeah.
36:51It's quite hard.
36:52Yeah.
37:00Is it painful, Dr Schwarz is touching?
37:02Erm, sometimes.
37:03When you squeeze it, it hurts.
37:05It keeps on growing, and it's...
37:07It's making me more anxious.
37:10This would be a new milestone.
37:12It would make me a lot more confident.
37:14I think I'd be back to the old me, where I wore my summer tops,
37:18and I was, like, loving life.
37:20You said you had an ultrasound scan that showed that it was a cyst.
37:24It's...
37:25Yeah, they said it in the...
37:26In my doctor's notes.
37:27They never actually told me.
37:28They just said in the notes that it's an ovoid...
37:31A cyst, yeah.
37:32Yeah.
37:32It doesn't feel like a cyst.
37:34But I think we'll have to take it out and see,
37:36because it is not typical for a cyst.
37:39It's very sore, though.
37:40I'm so sorry.
37:40Oh!
37:41But, um, the cysts are usually not sore either.
37:45I was expecting it to be a typical cyst.
37:48Soft, fluctuant, maybe mobile.
37:51It was neither of these.
37:53Erm, it was very hard, very firm.
37:55Jodie's lump was also painful, which is not typical for a cyst.
38:00I'm worried that this could be something more sinister.
38:04So, I guess you prefer a scar to a lump?
38:07Yeah, definitely.
38:08Yeah.
38:09OK.
38:10All right.
38:10Easier to cover, isn't it?
38:11Yeah.
38:12Perfect.
38:13Good.
38:13Right.
38:14Do you want to take the cardigan off, if you're happy,
38:16and then lie yourself down?
38:18When he said that he didn't know what it was,
38:20I thought it could be cancer,
38:22because that's what I've been worried about,
38:25the whole time I've had it.
38:28Right.
38:29You OK?
38:30Yeah.
38:30Bit nervous?
38:31Yeah, a little bit.
38:32And I said it coming up now, all right?
38:33So...
38:34You OK?
38:34Yeah.
38:36OK.
38:37Ready?
38:37Yeah.
38:39I was really super nervous for the injections.
38:43I feel my, like, tapping on my leg,
38:45to take my mind off it, and that really helped.
38:47She was lovely.
38:50That's really good.
38:51Good.
38:51All right.
38:52Now, let's...
38:53I'm just going to test the area,
38:54so I want you to tell me if it still feels sharp, OK?
38:57Oh, no.
38:57I can just feel pressure.
38:58Pressure, good.
38:59Pressure is normal.
38:59OK.
39:00But if it feels sharp, then that's not normal.
39:02No, I don't feel anything sharp.
39:04Good.
39:04Right.
39:06Good.
39:06You OK?
39:07Yeah.
39:07Good?
39:07Yep.
39:08Good.
39:08I was concerned, um, going into this procedure.
39:12Jodie was worried about cancer.
39:13Um, and if I'm honest, that did cross my mind, too.
39:18OK.
39:19Do you feel that?
39:20No.
39:29So, what do you think it would look like?
39:31Yeah.
39:33Um...
39:34Um...
39:34We know.
39:35I don't know.
39:36I don't want to see.
39:37Did it just spit out?
39:39It did, yeah.
39:39It did.
39:39What is it?
39:40It is a cyst.
39:41Oh, it's a cyst?
39:42Just a really hard cyst.
39:44I was panicking that it was something worse.
39:47I can't even put into words how I feel right now.
39:49It got me.
39:50Oh, I'm sorry.
39:52That's all right.
39:53Is that painful at all?
39:54No.
39:57OK, I'm going to give a bit of a squeeze.
40:00As I made the incision, the lump exploded, um, which was actually a big relief.
40:15I think it's a big one, that's why it felt so hard.
40:20OK, that's a lot of...
40:22Wow, OK.
40:31How are you doing? Are you OK?
40:32Yeah, I'm OK.
40:37The pressure, while he was digging around, well, I could feel him pushing and squeezing on my arm,
40:43but obviously I didn't want to look.
40:45Oh, it keeps coming.
40:46Oh, my God.
40:48Was that the noise?
40:49Yeah.
40:51OK, I can see the sack.
41:05Let's see.
41:18OK.
41:18How are you doing? Are you OK?
41:19Yeah, I'm OK, thank you.
41:20Good.
41:21It wasn't as bad as I thought.
41:27So, it's good that it's a cyst, because actually, when I originally felt it, it felt so hard.
41:35And I couldn't really see, because most cysts will have a tiny opening on the surface.
41:39And you didn't have the opening, either.
41:41No.
41:42And I was a little bit concerned that it might be something...
41:46More sinister.
41:46...a little bit dodgy, yeah, a little bit more sinister.
41:48Yeah, that's why I was so worried about it.
41:50But as soon as it popped, and I wasn't expecting it to pop, otherwise I would have stepped out of
41:54the way.
41:55More some goggles.
41:56As soon as it popped, it was good news for you.
41:58Yeah.
41:59Bad for me, because I got caught, but good news for you.
42:01I'm really sorry.
42:02That's all right.
42:05Knowing it was just a cyst, I'm so relieved.
42:10For me, it was a big, big deal.
42:13How was that?
42:14It wasn't anywhere near as bad as I thought it was going to be.
42:17Good. And it was pretty straightforward in the end, and luckily for you, it was nothing else of more concern.
42:23It was a straightforward cyst.
42:24I'm so relieved.
42:26Do you want to see it?
42:26Yes, please.
42:27Well, you can see it here first.
42:29Yeah, I can see it.
42:29You can see bits of it on me all round out.
42:32I'm going to have to change these in a bit.
42:33I'm really sorry.
42:37Lift that up.
42:39Oh, my God.
42:40That's so strange.
42:42So that's what was inside all of this material.
42:46That's inside it.
42:47And that, this is the sack.
42:49But that's a different texture.
42:51You can have a feel with your fingers or with that if you want to.
42:54Is it pus?
42:56It's not.
42:56Well, it's a mixture of things.
42:58It's oil and dead skin cells and debris.
43:01It's like off-coloured mashed potato or semolina or something.
43:05It was really gross.
43:06It was really gross.
43:08Good.
43:08All right.
43:09Thank you so much for coming.
43:10And I'm glad it's nothing to be worried about.
43:13Me too.
43:13Thank you so much.
43:14Oh, my God, it's so cool.
43:16Bye.
43:17I feel like I'm going to get my confidence back.
43:20I can wear those summer tops, the summer dresses.
43:23I'm not going to have people pointing at me and saying,
43:25what's that?
43:27Bye.
43:28Bye.
43:28Take care.
43:31I feel like I can be me again.
43:36Right, I'm going to have to go and change these now.
43:39Yeah.
43:47I'm hoping to wear a bikini with pride later in the year when I go away on holiday,
43:52which I'm really, really happy about.
43:59And I do promise never to self-pace.
44:02So Dr. Natalia, if you're watching, thank you for everything.
44:18I'm a lot more comfortable.
44:19I'm not worried about knocking my arm.
44:21I'm not worried about people seeing it because it's gone.
44:25So thank you very much.
44:28Next time on Skin A&E.
44:32I just got squirted.
44:35You're a braver woman than I am.
44:38You okay?
44:38Oh, no.
44:39No.
44:41Honestly, it goes straight into your chest when that patient makes a reaction like that.
44:49He's never been one to bite his tongue.
