Medical biller R1 RCM announced in August, 2024 TowerBrook Capital Partners and Clayton, Dubilier & Rice (CD&R) would acquire the healthcare technology company in a deal that values it at $8.9 billion USD. TowerBrook and nonprofit hospital operator Ascension Health were the company’s largest shareholders. Ascension (19 states / 140 hospitals, 160000+ associates & 36000+ affiliated providers) is R1 RCM''s top client.

The Better Business Bureau website is a good place to look for inside information as to patient / customer experiences with companies like R1 RCM. Surprisingly, there are many reviews here by employees who report compensation issues within company:

- A complaint posted 11/17/2022 reads: "I was promised in writing a $1000 sign on bonus after three months of employment. My first day of work was on March 7th, 2022. I still have not received my $1000 bonus. I have asked multiple times for this payment and have not gotten any resolution. As of today it is November 17, 2022, and I am still currently with the company. I wish to be paid my $1000. At the end of March 2022 R1 RCM took over."

- Another employee complainant in 2023 writes they availed of a scheme to have them work through lunch breaks and do overtime and be compensated with credits that could be redeemed on online retailer Amazon. The credits were not automatically tallied and sent to the employees, who had to get them through their bosses and supervisors. After accumulating a significant number of such credits this employee says they were denied their credits after working overtime and skipping lunches.

- I read 2 other reviews which describe patients / customers (or releatives) of R1 RCM trying to recoup sums of around $2,500 USD and getting drawn into a process of phone tag that lasted 9 months. This, following being DOUBLE CHARGED for services.

In another video I shared a bill I received from R1 RCM Medical Financial Solutions PO BOX 42008 Phoenix AZ (Arizona) 85080-2008 USA for $2,185, stemming from a 4/22/19 ER visit at Saint Vincent East's Hospital (Ascension), Birmingham, Alabama. I would like the bill collector to see who they are collecting money for and for what reasons. I was misdiagnosed this visit, told I had no injury in the CT scans of my head and cervical neck regions (analysis was provided by Doctor Julian Patrick Druhan). I later obtained my records and found in the images that I had broken the underside of my skull, contrary to what I was told. I was later billed the above amount.

I may have been "patient dumped" in the course of my seeking treatment (a violation of EMTALA). When I told medical staff associated with Ascension about my discovery, I was personally attacked and my psychological state challenged, this is reflected in medical records I obtained later.

00:22Reportedly, Towerbrook Capital Partners and Clayton, Dublier, and Rice will acquire the
00:30healthcare technology company.
00:33The Utah-based company provides services for billing and revenue collection to hospitals,
00:38physician groups, and other healthcare organizations.
00:42Among them, Ascension deals expected to close by the end of this year.
00:47Ascension, a non-profit Catholic health system, is R1's largest client and biggest shareholder
00:54through an investment fund with private equity firm Towerbrook Capital Partners.
01:00I was a patient at Ascension's St. Vincent's East Hospital in Birmingham, Alabama.
01:05R1 handled the bill regarding my visit in 2019.
01:10I experienced medical fraud and negligence in my situation, unresolved outstanding as of
01:18September 2024.
01:21All right, I recently did a video showing a sampling of employee reviews of R1RCM posted to the
01:32Indeed.com website.
01:34And the people that work for this company report a lot of problems and a lot of evidence of
01:41corruption.
01:42Another good website to check to get an inside first-hand account at customer and patient
01:50dealings with this company is the Better Business Bureau website.
01:55And there you will find that right off the bat, R1RCM posts a D minus business rating here.
02:06And one thing that I've never seen that is employee grievances that make their way into BBB
02:14filings are that some of these are actually written by employees of R1RCM who have issues working
02:24for this company.
02:25So here's an example from November 2022.
02:30They write, I was promised in writing a $1,000 sign-on bonus after three months of employment.
02:38My first day of work was on March 7, 2022.
02:42I still have not received my $1,000 bonus.
02:47I've asked multiple times for this payment and have not gotten any resolution.
02:52All right, so eight months and they write, they're still employed with the company.
02:58They haven't gotten their promised sign-on bonus.
03:03And looking at the concerns about R1RCM, this is a thing you'll see over and over again when
03:11it comes to the issue of money leaving the hands of R1RCM and either returning to the
03:20customers, the patients, the payers, the employees, there seems to be a many, many month waiting
03:29period on the order of close to a year or more.
03:33And we're talking huge amounts of money to average people out there.
03:39Here's another employee review.
03:42Again, curiously on the BBB website.
03:45They write, I was let go a week before Christmas, 2022, and was told R1RCM was parting ways with
03:53me.
03:53The team I was on under Ascension went through a transition to a new EMR.
03:59During the transition and after, R1 offered incentives for overtime.
04:03This included working through lunches, that is lunch breaks, to receive a $20 Amazon credit
04:10or a free lunch.
04:13And every two hours working overtime, the employees would receive $20 Amazon credit.
04:19And this employee told their supervisor they were saving up their Amazon credits for Christmas.
04:27But here's the kicker.
04:29The Amazon credits were not sent immediately to the employee, so they couldn't cash them
04:35in themselves before they went through the chain of command.
04:39This employee gets ready to use their Amazon credits, goes to the supervisor.
04:44The supervisor comes back and says they will not honor their accrued credits.
04:49All right, so here are a couple of horror stories from patient families, customer families,
04:56who have waited close to a year to get refunds on several thousand dollars from R1RCM.
05:03This complaint, written in June 2022, reads,
05:09My father, a disabled senior citizen, had to be transported by ambulance.
05:14Initially, his declined paying for the $2,556 for transportation to the hospital.
05:22I processed an appeal on behalf of my father, and the decision was reversed to pay for the trip.
05:31This person called, and they confirmed getting double charged by their personal credit card
05:37and never contacting this individual to start the reimbursement.
05:43I've tried to get reimbursed from the ambulance company since the appeal was approved back in November 2021.
05:50Called six times.
05:51No one should have to wait eight to nine months for reimbursement of $2,556.
05:57Here's a similar review complaint from 2023.
06:02They write,
06:03I stupidly overpaid them on a doctor's issue, and it took them close to a year to refund $2,400.
06:12I had to jump through hoops to provide them proof that my insurance company paid the bill,
06:18and they were paid by me twice.
06:21Or when R1RCM is an offshore billing company,
06:23both entities do not hold themselves to a professional standard
06:27and make promises they just cannot keep.
06:30Then when they are caught in a vicious circle of deceit,
06:32they come clean as an offshore company with no power to escalate resolution on your behalf.
06:38After nine months of madness with the concentric circles,
06:41I had to hire a lawyer who navigated to Vituity's president and general counsel.
06:47And then, and only then, was I able to reach a promise of resolution and a check in hand.
06:54So here's a sample of some of the concerns posted to the Better Business Bureau regarding R1RCM.

07:04Please check this out before you do business or give your money to these people
07:08or any of their partners or team members that they work with.

07:31And I have a message that I would like to give to Ascension.
07:35Between 2018 and 2019, I was an emergency department patient at Ascension St. Vincent's East Hospital in Birmingham, Alabama.
07:44I believe I had broken my neck.
07:47I had an x-ray, two CT scans, and later an MRI looked at by Ascension St. Vincent's doctors.
07:54Each time they had told me I had no injuries in this area,
07:58I knew their diagnosis was an error as my condition worsened.
08:02The area became infected and infested with parasites by summer of 2019.
08:08I almost died.
08:09I obtained my medical records from your hospital along with the x-ray, CT, MRI imagery,
08:14and discovered the truth of my injury.
08:16I had broken bone in the underside of my skull in the area of the condylar canal and occipital condyle.
08:22How the doctors could admit such a large and serious injury over multiple hospital visits is inexcusable.
08:28I believe some element of organized corruption may exist in this hospital.
08:34I have compiled over 10 hours of video documentation on what happened to me on my YouTube channel.

08:44I believe I may have been a victim of patient dumping in the course of my seeking treatment,
08:50which is a violation of EMTALA, E-M-T-A-L-A.
08:53When I told medical staff associated with Ascension about my discovery that I had been misdiagnosed repeatedly,
09:01I was personally attacked and my psychological state was challenged.
09:06And this is reflected in medical records I obtained later.
09:10I could not obtain proper treatment or diagnosis for this injury
09:15because doctors and nurses that I would subsequently have encounters with
09:21would only entertain what the erroneous Ascension ER findings were.
09:26My mother, who was a nurse for 20 years,
09:29had the misfortune of working in an Ascension hospital
09:32when I questioned my MRR findings involving St. Vincent's Ambulatory Healthcare Network, LLC,
09:38and Dr. Michael Brandt Ruff, MD, radiologist, in March 2019.
09:45My mother, who was a nurse for 20 years and worked in the Ascension hospital,
09:48she was terminated from her job there around the same time.
09:52And so this phone call,
09:53I challenge the misdiagnosis of Ascension St. Vincent's East Hospital, Birmingham, Alabama,
10:00and I challenge the policy of this hospital.
10:04And Mr. BGK, I think you need to look into this, please.
