00:51I've got to eat my lunch after this is Skin A&E this week in the clinic I always thought that I'd
01:07watch everything a lumpy lipoma is a sight for sore eyes removing an ear mole is a sound idea for one
01:18patient his wigs like an ear like the scraping and with the usual selection of thrills yes I just love
01:25it I'm cyst free and spills funny looking vein here oh yeah it's gone this is how we want them
01:36ready for the day then Angela that's it the dermatology team have reported for duty let's set
01:46stuff up good morning good morning you've no hand towels have you hand towels these oh those ones oh
01:52take one of them yeah thank you very much time to give some skin conditions their marching orders
01:58my name is Tricia I live in Hertfordshire great have a seat for me I'll let you know when you're ready
02:04I teach primary school children I've taught a lot of dance over the years and I've taught art I'm at
02:12the moment making sort of ceramic animals the doctor's ready for you down the hall on your right

02:33you doing nice to meet you sit yourself down what brings you to the clinic well it's this large well
02:40I feel it's quite a large lump on my thigh okay and I it's actually been there for at least 23 years okay I
02:48did have it liposuctioned okay about 20 years ago they stuck a like a fat needle into it it was under a
02:57general anaesthetic in those days believe or not and they sucked the contents out but it just came
03:03back again after two or three months it's got so large I think that it's time to take it off before it
03:10gets any bigger okay how is it affecting you at the moment I like swimming okay swim several
03:17times a week and I personally feel embarrassed about it when I go into the pool yeah I shouldn't
03:24do but I do and what you do for a living I'm a primary school teacher are you and I am still
03:29teaching I'm again too good on you yeah because gosh we need as many teachers as we you know can
03:35get our hands on okay may I come and have a look when I've shown my friends my lump they've usually
03:41gone good grief crikey Trisha get it off so is that painful when I do that no there's a slight
03:52complication in that you've had liposuction so it may not come out as nicely as one would expect
03:58so imagine you've got a nice encapsulated band or balloon around a fatty lump liposuction
04:08puts a needle in and sucks out the lipoma and causes trauma and that band the the balloon around
04:19it sticks to the surrounding structures it just creates a much harder thing to remove because
04:25it's more sticky and it's not as spongy so the good news is I think we can do this for you today
04:32right okay yeah we could do it under local anesthetic yeah good but like I've said there
04:37may be a bit of pushing and pulling because of the liposuction that you've had so it's not just
04:42it may not just pop out no we hope it does but it may it may do if it's behaving I felt oh okay
04:52please but I thought oh well here we go it's gonna happen this is just him scribbling on you okay okay
05:00what age do you teach up to
05:04yes I know what you're doing yes I guess on the count of three well they're about five six seven
05:13gosh they're late so they're really sweet oh are they no no they really how many kids do you have
05:20yourself I have three children well they're grown-ups now how are you doing with these injections

06:47liposuction so I thought it would come out in a fragmented way but it came out as one solid mass
06:54it was just how lipoma should come out in my book if I could just have that there

07:13pinchy that's okay all right so let's just make sure you're nice and numb there putting a little
07:21bit more local in okay yeah there was a lot of pushing and pulling it honestly did feel pretty
07:28brutal yours is a squeezer apparently that's a good thing because it could come out encapsulated
07:36so if you can squeeze it all out in one it's good we're brilliant

08:17I'm going to buy myself a pair of white trousers because I will be able to wear a pair of white
08:24trousers without a big lump popping out and I will feel that I can go down to the pool into the swimming
08:31pool and I won't have a great big lump on my side that I feel everyone's looking at
08:36it's all done glad to say goodbye to it good yes I'm very pleased it's not there anymore
08:42do you want to have a look oh yeah yeah so can you see it's ridged yeah those ridges will go
08:50it'll all flatten out I'm feeling pleased that it's all over okay well done and let me get
08:58your trues there we go it's quite a surreal feeling really but it's all done and the lump's
09:05been taken out it's quite a relief ready to have a look okay there we go so we've taken that bit of skin
09:14off oh right yes okay it did look pretty foul I got to say a bit blobby and um yeah with that little
09:25bit of skin on the top it's like oh goodbye there we go all right and thanks for nothing yes yeah well

10:53courses it's at least over a hundred i've done allergy awareness uh where was it it was like an electrical
11:02one cyber security i've done so many because you never know where you are in life you might have to
11:10know one of those skills come in hello hello come through take a seat thank you what are we doing
11:22for you today i was hoping for you to have a look at some um lumps that i have on my head
11:29that i've had now for a good three four years they started off small and now they are growing and
11:35i've got four of them currently on my head they just make me feel very uncomfortable because i'm
11:41constantly thinking are people looking at my head can people notice that i have lumps on my head
11:47are they going to think that i'm disgusting because i don't are they going to think i don't wash my hair
11:52are they thinking that i don't look after myself are they causing you any symptoms um so
11:59i i have to wear headsets a lot for work and one of them sits directly where my headset sits okay
12:05and so are the headset puts pressure on one of them so they do cause me quite a bit of bit of
12:11discomfort fine and what do you do for work um i'm a customer service advisor so you have that on a
12:16lot i have it on a lot yeah i have it on pretty much all day okay so let's have a look at them and
12:21then i will be able to tell you once i've seen them whether it's something we can do for you today
12:25all right okay okay so i want you to just pop yourself up on the couch you don't need to lie
12:29down just sit down get a pair of gloves i want these lumps to be taken off of my head so i don't
12:36have to worry anymore i would feel like a weight has just been lifted so so it's just there and yeah
12:45moves around thank you okay right let's just see the borders of this roughly there okay one where's the
12:55other ones one uh the second one is up here yeah okay oh yes yeah you can see a little bit of an
13:02opening at the top so they are entirely typical cysts okay yep number two there um the third one is at
13:12the back of my head here oh yeah okay okay yep and then the fourth one it's not unusual for patients
13:21who have multiple cysts on the scalp it's just the glands and the oil glands becoming blocked and then
13:27forming cysts under the skin which ones if we can do them today were you hoping to have done um it's
13:34mainly these ones in the back one because they're the main ones that will sit with my headset mainly
13:39so we could definitely remove um at least a couple of those today for you okay yeah all right local
13:45aesthetic can be painful so i just wanted to see how she would cope with the anesthetic and i didn't want
13:50to promise her that i would do four just in case i needed to give her more anesthetic in the initial
13:55ones that's it fine yeah i should be fine ready yeah it's a strange feeling it will sting it stings as
14:05it goes in as well well done you're doing really well oh dear i was trying so hard not to really really
14:13squeeze her hand i didn't want to like break her hand well done generally the needle tends not to
14:21hurt most patients it's the anesthetic itself can be quite stingy almost like a bee sting you've done
14:27very well thank you you're welcome that's okay well done okay so if you lie yourself down if i could rate
14:34the pain it would definitely be an eight out of ten definitely feel they've gone numb
14:45okay let's see a bit of pressure okay it's a beautiful cyst no
15:02okay so first one gone came out pretty easy oh good so let's see what the others do brilliant
15:20the first one just popped out but i couldn't feel anything good okay one down let's bring the next one
15:28okay okay sharp no
15:36so chloe which company do you work for um i work for st john ambulance i work in our events department
15:46i love my job i do
15:51except for the headphones yes except the headphones
15:58so the second one's out okay brilliant second one had the gunk first and then the cyst came out
16:07okay the second one i could feel it squirting out my head which was a very strange feeling to feel
16:17if we get you on your tummy to get the one on the back
16:21if you just hold that hair just like that that's it okay does that feel sharp there at all
16:33no it does not push up a bit of pressure yeah so this i think is your biggest one isn't it yes it is
16:40when my lumps were being pressed i could just hear a crack and then just a whole load of squelching
16:55almost there ah there we go
16:57okay okay so that's three down the ones that you originally came for brilliant and because you
17:12were so polite i'll do the fourth as well thank you it's by three get one free
17:18i'm just thankful that i was lucky enough to receive that offer from him
17:28any pain there no no pain uh let's see
17:39oh there we go amazing all the cysts came out without any real major issues
17:48it's really satisfying when a cyst comes out as it should and i'm able to stitch the skin back up
17:55knowing that hopefully it'll never come back again there's a mirror there you can just adjust your hair
18:02it feels really relieving i feel like a whole new person i went from having a lump in my head to it
18:10just being completely flat i feel really really good really good okay do you want to have a look yes
18:18okay so i'll lift this up four cysts first second that's the smallest smallest one and then that's
18:27the actual sack yeah the biggest one oh oh my god it's like hard yeah it is quite firm i was quite
18:36shocked because i thought it was going to feel quite squishy i'm so glad to have them out of my head
18:41have you ever gotten really really squeamish when doing poppin yeah no never the bigger the better

20:10it's ken isn't it yeah hi i'm dr emma this is donna hi come and have a seat ken
20:16so what can we do for you right i've got a lump here that i now know is a lipoma uh-huh
20:22how long has that one been there for about 10 years i teach music it's two mixed abilities and
20:29when they notice that especially the guys with autism who need everything explaining and spot on
20:35the lesson goes it's now not a music lesson it's a skin lesson yeah yeah my main instrument is guitar so
20:43when i'm teaching that arm is on show and if i'm teaching a guitar lesson for half an hour 20 minutes of
20:50it could be the young people saying to me oh what's that on your arm i've got a scan you had an
20:56i had a scan yeah and just have a little look at it there was a few concerns i think there's a
21:01big vein running over the top of that right and it looked quite vascular so it's one that might be
21:07a bit tricky and and better dealt with by somebody who specializes in sort of soft tissue problems
21:13but let's have a little feel of it
21:18yeah it feels very firm it doesn't feel as squishy as a normal lipoma it feels a bit fixed there as
21:25well so a normal lipoma is pretty much just fat cells but there are variants and one of them's an
21:31angiolipoma and that's a lipoma that's got lots of blood vessels running through it
21:36i think it'd probably be a good idea to take a biopsy off it in the at least and but when we're
21:42in there and if it feels easy to remove then we'll remove all of it if we can yeah so shall we do
21:48that whatever you think does that sound okay is it going to happen now yeah we'll do it now all right
21:52yeah we'll do it today we'll get you on the bed and get you comfy i've always wanted it removed but
21:59then i've always been worried is that going to have some effect on my left hand could they hit a nerve or
22:05something could it be that i would never be able to play an instrument again i'm just going to draw
22:13around the lipoma and then put your local anesthetic in
22:21right i'm just going to start doing your local okay
22:28take it you've played for years then do you play in a band or anything oh my band days have gone
22:36i'm just mainly teach now i'll go to the open mic night every now and again it's nothing like live
22:41music oh yeah brilliant wonderful they made me feel at ease i wasn't nervous i felt i was in the right
22:50hands um literally
22:52ken can you feel anything sharp here not a thing so let's have a look and see
23:05because that ultrasound scan report it did mention one large vessel running over the top of it
23:11i was quite worried about going through that coming up he did some wd-40 on it i think
23:19oh it's good it's weird because i can hear that the square can
23:25it's mid-afternoon at skin a and e and in treatment room two dr emma is assessing ken's lipoma
23:44so normal lipomas just look very sort of orangey yellow but sort of vascular ones often can be look
23:53a bit red purple that looks like a very pale yellow one are you all right there yeah i'm fine
24:01yeah wonderful i always thought that i'd watch everything that was going to happen and then the
24:08last minute i bottled it
24:10so when you were with a band what kind of music did you um a 70s rock yeah yeah but glam rock as
24:22well t-rex bowie zeppelin yeah sabbath from this area oh i wonder fabulous yeah
24:31how's it looking well there's one sort of funny looking vein here when i cut into ken's lipoma
24:39you could see lots of small vessels sort of running through it as i had to
24:43cauterize a few of them with the hyphricator it was generally quite vascular which may mean it's
24:48an angiolipoma start from it just because of the scan saying it had that big vein i'm being a bit
24:56more cautious than i normally am that's okay
24:58that looked like it was gonna i know be kind then and then it didn't did it a bit of patience with
25:15this one so yeah that i bit by bit i think it's a right awkward one go on give it some welly yeah
25:31once we'd started it was clear that even though it looked a bit odd it wasn't going to bleed loads
25:36and it didn't feel like a particularly difficult procedure to do therefore because it looked abnormal
25:40i thought let's get out as much as we can and then there's more tissue for the pathologist to review
25:45and and just make sure there's nothing nasty going on with this lesion
25:52it's quite fibrous and tough it looks like not as jelly like as oh yeah no it is tough
25:59so what i want to do actually in case it all comes out in tiny bits is just take a good big specimen this
26:04i haven't mashed up too much so that this pathologist has got a good chalk too yeah
26:12normally lipomas are just like they're almost like a jelly consistency so they're like kind of soft
26:17whereas the lesion felt really fibrous so if you can imagine like tendons it's sort of tough material
26:22there's no way that would have squeezed or popped out just because there's so much sort of dense tissue
26:27running through it so i had to use the scalpel to get bits of it out that little one there yeah
26:39oh that's a good chunk yeah can't see anything from this angle no i know you've got your beady eye
26:49emma asked me to keep an eye on the vein obviously she has to concentrate on where she's cutting
26:55so an extra pair of eyes is always very useful you just squeeze that i'll just what it is we'll
27:02loosen squeeze and then i'll just cut it with the scalpel
27:08let me know if your fingers get tired yeah that's that's coming out nicely
27:14oh sorry i nearly flung that yes i'm sorry oh that's brilliant
27:28i think we've got the majority of it
27:45if you look now
27:47are they allowed look so it's nice and flat okay so we've got most of it out but there's a few
27:53little stubborn bits i've been nice to get out that small
27:59and i started watching the procedure and wash i'd have watched it from the start because it was
28:04fascinating once i get this bit out i think we'll just leave it might have there
28:11yeah so i think you know we've left little bits in there
28:14because it's quite vascular yeah we're going to put some deep stitches in
28:18and by the end i think we managed to remove most of that lipoma i was expecting a lot of bleeding so
28:24it was a real relief when it came out so easily
28:30you're getting braver now you've got a good view there haven't you
28:34have some really nice work speech-wise yeah so look after it no drumming no no
28:41feels great it's something i've waited for for a long time and i won't miss it now it's gone if you
28:50want to sit up and have a look very juicy isn't it it was quite bitty yeah looks more like mince
29:00it looked like prime beef mince i'll be getting lamb mince in future i know that's tight but it's doing
29:08a job no all right yeah yeah it's been a pleasure would you believe totally delighted i'd just like

30:48nice to meet you natalia hi this is dude she'll be assisting me today how can i help you today um
30:53basically i've got these lesions on my face in the last six months this one's got darker it's got bigger
31:00and there's other lesions just popping up willy-nilly over the last couple of months there's
31:04one here another one here and it's a bit worrying because i'm wondering where it's going to end how
31:10old are you i'm 63. okay so with the one on your under your eye you must have had that one for quite
31:16a while about six years six years okay and the one the bigger patch on this side the discoloration in
31:23the skin since i had my daughter so 24 years four years um has it changed no it's kind of been constant
31:31but one of these whatever this is one of them has started growing inside that and it's it feels
31:37rough itchy like crazy they all itch okay so let's have a look i've noticed that when i'm talking to
31:44people their eyes come down off from my eyes and they are literally looking at this and uh you know it's
31:54i can't do anything with it i can't hide it i would like it gone i'm just going to use this device
32:02a dermatoscope basically magnifies uh the lesion but also allows us to see almost like the
32:07microstructure of that lesion okay so i'm just going to point out to you which one's which okay
32:14this one is a seborrhe keratosis that's awardy growth you get as you age
32:20okay then you have the same kind of thing but flatter here here and up here and same on the other side of
32:27the face here here so a seborrhe keratosis doesn't spread or kill you it's on a skin cancer it never
32:33will be one you will never turn into anything thank goodness so they're all fine they're all fine
32:37thank you so everything is fine so which is good lovely you do you're welcome and you do have a
32:42fair amount of like freckling so this the flat bit of this is what's called an antigo so a soul antigo
32:48so it's a sun-related freckle okay not like a normal freckle that goes away when you're out in the sun that
32:52one stays her all the time to be told that it was all okay she gave me that confirmation absolutely
32:57fantastic with the main one that you're mainly concerned about which is this one i do think we
33:03need to cut this out it'll give you the nicest result you'll have one stitch in okay or two very
33:09fine ones um and then you have those out in seven days okay when she said that she could remove it
33:15perfect i wasn't keen on removing the other lesions on victoria's face because the mark
33:21left over from either shaving or excising would be cosmetically unacceptable at least to me and i
33:26think for victoria as well the lesion on her cheek is the one she was most concerned about but also
33:32most keen to remove just relax your face a little bit and you're going to feel a sharp scratch now ready
33:38and sharp your structure good well done and just deep breath you're going to feel a sting it's going to go
33:43down really quickly and that's it there are really only two ways to remove a lesion from the face you
33:52can either shave it off from the surface of the skin or you can remove it with an excision and just cut
33:58into the skin and put a suture there i always have to weigh which one will be better than the other for
34:04the aesthetic outcome the mid cheek is especially tricky and i do think that cutting this lesion out
34:11is going to be the best way to go okay so now i'm going to do the procedure you can close your eyes
34:16you don't have to watch me doing it
34:30okay just removing it now
34:33when removing victoria's lesion i decided to use a punch biopsy like this which is kind of
34:40like an apple core in order to take it as a perfect circle when you remove it as a perfect circle you
34:46can still close it without risking having little standing cones on either side so it gives a nice
34:50flat scar but only if lesion is small enough
35:04i'll drop it
35:09it's amazing you can't feel it a thing yeah it's amazing it's a weird sensation to hear it and not
35:14fade it i know it's weird it might look like a really simple procedure but everything that goes
35:20into planning that is actually quite complex and especially making sure the patient understands what
35:24the scar will look like you're very swollen already oh you can close your eyes i'm trying to make sure it
35:31looks even
35:40okay i'm really happy with this you're going to freak out a little bit when you see it
35:43but you have to believe that you know it's going to be fine okay it's just you're really swollen out
35:49and a bit poofy so can we get bigger or yes it might get more yes in the next 72 hours you can get
35:54increased volume sorry oh my god it sounds terrible doesn't it noisy bad needed some wd-40 on it i think
36:06oh yeah it's gone yeah it's gone brilliant sorry you can hold the mirror and look at yourself easier
36:11that's brilliant thank you very much stitches are so neat oh i'm so happy that it's not there
36:18i couldn't believe it i mean even now in its raw state it looks so much better than it did before
36:23wow so so you can pick it up if you want it kind of looks smaller doesn't it yeah it does it shrinks
36:30that they contract a bit of shrink when they get taken off so that's the fat underneath that's your
36:34skin right there and then you can see the furry thing on top do you see that wow yeah um all right
36:43perfect brilliant thank you very much you're welcome any questions i don't think so i think

37:55sort of rescues that were collected like you know when we lived in spain doctor's ready for you now

38:20back of my ear yeah it seems to be getting bigger over the last couple of years how long has it been
38:25there for in total would you say i think about probably about 15 years it's been one of those i'm always
38:29going to go and get something done and yeah never get an appointment or yeah fair enough well have
38:32a careful look at that for you thank you um do you want to take your coat off when i was in spain i
38:38tried to get an appointment with a dermatologist the dermatologist on another island it was one of
38:42those where i was always going to go always going to go and never got around to it okay let's have a
38:46little look at that then so i'm just having a look with the the dermatoscope which is like a skin
38:51microscope yep i mean it certainly looks like a like a harmless model so when i saw the lump behind
38:58stefan's ear i i thought that it was a mole called a benign intradermal nevus you can develop them
39:05anywhere but it was a little bit larger than average presumably it's having a bit of an impact on
39:10on you yeah in fact every time i go somewhere i think can anybody see it's there even though i
39:13perhaps know deep down they can't yeah it's just the fact it's there and then people noticing it
39:17and not yeah knowing what it is yeah yeah so it is that worry as well winter to get me air
39:22cut ones and he caught it and he made it bleed i don't think i've been back tomorrow since

41:39gone so i'm just going to seal off a few blood vessels and then i'll decide whether or not put one
41:44little stitch in there it's a little bit of buzzing and then we're yeah and i could feel him sealing the
41:49blood vessels and it was a really weird sensation because i could hear it but and i could feel it
41:54but not in a painful way it was a strange sensation so what i'm gonna do is put one dissolvable stitch
42:00in there because you just make it heal a little bit quicker yeah thank you um but it's all gone
42:04really smoothly yeah even without the stitches this would heal fine but i just think it will heal a
42:10little bit quicker i'm so glad that it's gone so just sit onto the side of the bed yep take a minute
42:17get all that stuff off you okay it feels better already when i looked in the mirror after the
42:24procedure it was just a massive relief it's perfect thank you very much but yeah it's brilliant now
42:28it's gone so the main thing is to keep it nice and dry yep for about 48 hours yep no problem i can't
42:37wait now to get to the barbers get my hair cut properly and give my wife a break from having to
