01:48for the first patient my name is Adrian I am 50 51 now live in Bournemouth down on South Coast have a seat
01:58I'll let you know when you're ready for love we got a dog last year she's a one-year-old
02:04cockapoo which has been a big impact on my life lost 20 kilos and some in the past year just from

02:24hi how are you come on in and have a seat thank you I'm dr. Emma this is Donna hi yeah hi yeah so Adrian what can we do for you I've got a lump on the back of my neck I had it there for 20 odd years long time yeah yeah absolutely and as it's been growing the whole time yeah it's getting bigger and bigger
02:31started off really small and it grew would it be okay if I had a little look at it yeah yeah let's go for it get some gloves that's on the back of your neck yeah oh yeah just just poking out from your collar there
02:43all right okay it's quite mobile
02:46the skin over the top of it is not so it's a fair old size isn't it yeah yeah it was quite symmetrical which would put your mind of assist sometimes lipomas are a little bit more
03:16asymmetrical it was pretty huge in terms of cyst sizes particularly in in that region does it cause
03:25you any pain or no discomfort on a day-to-day no I don't tend to feel it it's just now I've lost a bit
03:32of weight recently and it's just yeah it's just stopping collars actually closing yeah it's a fit it's
03:39adding to your collar size and then your shirts too big yeah yeah exactly how much weight have you lost
03:44uh 20 odd kilos in the past 18 months or so what's that like three stone oh I don't know in old money
03:50yeah oh I only know old money I still haven't got into kilos that's a lot I'll try not to look in stone
03:55because it's more frightening than kilos all right well I think it's going to be tricky okay but I think
04:01we could give it a go that'd be great if we could yeah so what we have to do is we'll have to obviously
04:07make quite a sizable cut in the skin I think because it's going to be it feels really quite stuck in the
04:11whole sort of middle segment off it and and then we have to sort of shell it out and try and get
04:18it out whole that's a little perturbed saying that it may be a bit tricky as it wasn't moving under the
04:24skin very much but to be actually told yes they could hopefully take it out was absolutely great
04:30so this is a bit of a beast isn't it it's a big one in the hope that it's going to get a squirt
04:37usually there's like a little punctum in the middle which is the hair follicle that
04:43it's arisen from but I can't really determine that I wonder if it's that bit there never been
04:50any discharge from it yeah I might just remove a smaller lips like that because I like to remove a
04:57little bit of the extra skin I still did an ellipse you know if it's central hopefully that's going
05:02to be roughly where the punctum is and so if I remove the punctum then hopefully the cyst doesn't
05:07come back no scars about that big yeah okay yeah no worries I don't see it so yeah be hidden by your
05:15shirt collar hidden by your shirt collar that you'll be able to fasten absolutely right I'm gonna start
05:21this is a bit stingy okay I got Adrian to set up for this procedure because if he lay on his tummy
05:31that would have put pressure on the cyst which would make it more likely to pop on me but it also would
05:36have made all the skin wrinkle up and it would just have been a bit of an uneven surface to work on
05:40all right Adrian does this feel sharp anywhere no no no you think not yet not yet more drumroll
05:56well it definitely is a cyst not lipoma so almost as soon as I cut into the skin and I could see that
06:06sort of silvery shiny cyst wall there it is and that is a good thing because lipomas in that area
06:16often get like stuck down to the muscle underneath I've gone through the wall so this is probably going
06:22to give us the right treat all right shall we yeah Donna loves it when they burst so I just thought
06:31Donna's got to get first squeeze on this one a little bit of me inside thinking oh come on baby
06:39come to mama oh come back into the light we bet that's it whisk oh you beauty are you okay we're not
06:54pressing too hard no no not at all oh gosh it's like you know that mac and cheese yeah oh don't
07:01eat oh no it doesn't look like that right beautiful I didn't really know how it was going to come out
07:12whether it was just going to be one of those splurters or over it would come out in one solid ball
07:17coming up watching your face it is amazing just made a lovely home for it doesn't want to leave
07:27designed it wanted to stay we've literally been pulling stuff out of this for 10 minutes oh this
07:34is a gift that keeps on giving it's a packed day at the clinic in surgery room 2 dr emma is treating 51
07:55year old Adrian's large nexus just wonder if we do not lift the capsule up oh oh wow oh that did not
08:11disappoint it's nice for the pathologist if we can keep it a bit intact it is a really good size
08:24I was really surprised by the sheer volume of contents of Adrian's cyst I think the capsule wall is
08:40there it's like it's quite shocked that it got to this size and actually not caused him any symptoms
08:46oh gosh oh oh oh this is a gift that keeps on giving having four hands back there trying to extract it
08:57I was having to push back to counter out there pushing on my neck it was hard battle for for the
09:02pair of them yeah what's the best about that I knew this was going to be a corker and that's the best part
09:28of my job you know that bit alone would have been impressive yeah we thought we've like we've
09:36literally been pulling stuff out of this for 10 minutes and then there's another whole massive bit
09:44that yeah you know when a magician has the flags yes and just keep coming out yeah and yeah it's like
09:50that right Donna have a bit of a poke around in there wow holy moly right if you can sort of lift
10:00your head up straight for me yeah so we're going to put some stitches in underneath there's quite a
10:05deep space there it went down about three four centimeters which you wouldn't think you had that
10:10space on the back of your neck no no so when we have a big sort of void like that left under we will
10:16put stitches in underneath to sort of close that space down and bring everything together so if
10:21it bled into it it can't expand and form a hematoma so because this is the back of your neck you're gonna
10:27have to be really careful with what you do yeah for the next few weeks really okay all right yeah
10:33all right so you've got three stitches in there lovely together lovely yeah in fact shall I take a
10:40photograph of it oh yeah that'd be good and one from the side there's probably a tiny bit swollen
10:49because of the local anesthetic yeah yeah but oh wow yeah that's better isn't it a lot better
10:58totally flat yeah put my hand at the back of my neck and actually not feel it it's weird it's gonna
11:06take a little bit of getting used to but be good collars done up soon so are you ready for the the big
11:12reveal yeah okay oh there's bits of it that's a bit weird yeah oh it's like bits of fat on a bit of steak it was
11:26oh pop that in a pot so we can send it off and just get it double checked my pathologist maybe a
11:33slightly bigger pot yeah after 20 years living with that lump back there it's weird to be without

13:10um I've got a lump on my right shoulder okay so I first noticed it about seven years ago okay um but over the last couple of years it seems to have grown slowly growing yeah does it hurt is it
13:24it's uncomfortable more than anything yeah against the backs of chairs and you know in the car when I'm driving it sometimes bothers me okay and started off about
13:34walnut size and it's now about tangerine size all right yeah let's have let's have a look at this
13:40so can you see it through your clothing we will look at that yes I'm beginning to feel quite conscious of it
13:49particularly when I'm on holiday or you know during warmer months when you're wearing sort of thinner clothing or strappy tops
13:54okay so it's right where your bra strap kind of pushes on it yeah
13:59yeah this is probably a lipoma I think you might have realized that yeah yeah
14:03it's so unfortunate it's right above your top do you mean yeah okay um right we'll have a seat for a second
14:10so we can definitely remove this for you today great yay um do you have any questions no I'm
14:19looking forward to having it removed there's such a build up to this point and yeah to hear those words
14:26it was lovely so just rest your arms by your side
14:30it's really important to mark up the edges of lipoma because when you put anesthetic in the whole area can
14:37become swollen and then you won't be able to feel where the where the margins are
14:41so by marking it in the beginning I know roughly how far I have to go to get that whole lesion out
14:48all right so I'm just you're gonna feel a sharp scratch and a sting if you want to move your arms around
14:55move your head you just tell us okay you can move in whatever way you want
14:59and you're gonna feel a sharp scratch now ready and sharp good well done and now a sting
15:05it shouldn't last forever anything sharp now
15:11stingy a little a little bit I mean I don't like them does anybody like needles but um I've gone
15:21through childbirth so I think once you've experienced that anything else just pales into insignificance
15:27so I'm gonna get started now if you feel any pain you have to tell me okay okay
15:33how are you doing Sue yeah I'm good thank you
15:41just trying to stay relaxed
15:45are they quite common yeah well in our world yeah we see we see them all the time
16:03how's it looking looks good we're just unsticking it so it's sort of contained is it yeah yeah I think
16:14this is one ball I wanted to make sure you couldn't see the scar if she was wearing a strappy top or
16:20something in her back but I have to think about where I am on the body and what will give the best
16:25aesthetic outcome what do we think the size of it will be tangerine tangerine so maybe a tangerine
16:34or a clementine those are smaller right than tangerines are clementines smaller I have no idea
16:40clementines are smaller than tangerine I don't know I don't know because in the market you get a lot
16:48easy peelers now what are they are they tangerine or clementines easy peeling orange I won't be buying
16:56any easy peelers anytime soon I'm just amusing so what do you do for a living I'm a personal estate
17:07agent oh I watch all those real estate shows like um million dollar listing and things like that do you
17:15do you watch those I haven't watched that one but um most property programs I watch right now I'm
17:22watching um Meredith for Australia that's my favorite show of all time like it's such an epic
17:30show you get sucked in and then it's like you can't stop watching it you need to know what happens
17:36I'd say Natalia has an obsession with reality tv um she watches all of it all of the dating ones in
17:44particular I mean what's your everyone what's your favorite reality show do you have one I like
17:51documentaries well that's boring are we nearly there no no it's still really stuck down
18:01oh that sounds squishy yeah I could just feel her um pressure I suppose on on the um the lipoma
18:12and a little bit of tugging it's a couple little fascia bits sticking it down still
18:19so you just made a lovely home for it doesn't want to leave exactly design it wanted to stay
18:28I find that it's easier to remove lipomas in someone older who has more skin laxity
18:34the skin's very tight or the person's very skinny then there's not much room there for me to get in
18:40to that area you don't want to let go of it because if I let go of it then I'll lose the base of it
18:46oh it's hanging on for dear life this one
18:53just my luck yeah it's like I don't want to go stop
18:58we never leave anything behind
19:03there we go there it is congratulations

20:59you've got a small one there so which are the ones that bobby is that one there yeah and then this
21:05little cluster here even if they take a few off and just make it more bearable

21:35I started photography in 2006 at first I liked taking portraits but now I'm more into still life
21:44I sort of do quite macabre still life with first skulls and candles and things like that

22:13well since my 50s I've suddenly gotten these things under my breast area
22:17yeah
22:17they're like moles or skin tags I'm not quite sure what they are actually
22:21I have had about three or four of them taken frozen off by a doctor
22:27okay yeah
22:27but they are so annoying when I sweat
22:30yeah
22:30say I go on for a long walk they get so red and sore
22:34the itching is unbearable
22:37you can't scratch them because you know that you make them bleed or make them worse
22:41and it'll take longer to heal
22:42maybe if we get you to change into a gown would you be okay with that
22:45and we have a little look at the area so Angela will just give you a gown
22:48yeah
22:48wearing a bra is a nightmare when it gets like that
22:52even if they take a few off
22:54and just make it more bearable and you know more comfortable
22:57so which are the ones that bother you
22:59that one there

23:07what they are they're called seborrheic keratitis
23:09so they are completely harmless skin growths
23:11but as you found they can be quite annoying
23:14I can remove some of them for you
23:16yeah

23:17seborrheic keratitis are growths that arise from the surface layer of the skin
23:23slowly down here
23:24it's a tendency that you have
23:26and if you have that tendency then generally you will slowly develop more over time
23:30okay let's just start doing some injecting
23:34all right
23:34this is just a bit slightly uncomfortable
23:36so just be patient with me sorry
23:38the injection was a little bit stingy in certain areas but otherwise it was okay
23:45it's not as bad as a dentist honestly
23:47I used to be a dental nurse
23:48oh did you
23:48are you still working or what do you do now
23:50oh no I'm retired now
23:52it is pretty busy when you're retired
23:54have a weekend away with the girls
23:57and we love going on long walks
23:59I mean like 10k walks
24:01so it would be a relief not to have this problem while we're away with my friends
24:07so that should all be nice and nice
24:09you should not feel anything painful during these procedures

24:40and you can either scrape them off using a tool called a curette
24:44or you can lightly burn the surface and wipe them off
24:48and that's better for the flatter smaller ones

24:54quite satisfying to remove them as well
24:57because we just sort of almost like buzz the surface of them
25:01and just wipe them off and it's

29:56to operate on it

38:54if you're holding hands
38:56they're infectious
38:56you can spread them
38:58and you have probably
38:58already spread that
38:59from there

39:01yeah
39:01so that's

39:02them
39:03the best option
39:04would be
39:04to have something done
39:05and we should be able
39:06to do that for you
