01:26an eye looks amazing thank you so much the Skin A&E team are on their marks for another busy day
01:36balls struggling there yeah it's not dangling in the patient's wounds and they're off
01:49my name is Sean I'm here for my appointment we live on our 40 acre farm in Lancashire I also run my own
02:02grounds maintenance business which keeps me pretty busy and fit that's why next month I'll be 60 but
02:08I don't look it do I down the hole on your right Sean has an appointment to see Dr. Natalia Spearings
02:17hello welcome hi hi you must be Sean I'm Natalia nice to meet you hello this is due to
02:26how she's at least listening to me today lovely to meet you so how can I help you hoping you can get
02:30rid of that little beauty oh yeah a little lump there so how long have you had that for
02:33six seven years okay it's been growing bigger bigger yeah slowly but surely surely is it painful
02:41no sometimes yeah you can roll over or with the fire machinery catch it as we do oh yeah as you do
02:49all right so let me have a look at this if you stand up just stand up here and if you can turn
02:56um so what impact has it's had on your life is that you notice on the farm around the own business
03:02so meeting new customers you talking to them you can see them going it's not that notice is it that
03:07notice okay no it's all right when I do that if I go oh if you go like that it's going to cost you
03:12this much okay oh it's quite you have a lot of skin laxity here when I was examining Sean's lesion
03:21I thought it would be a lipoma just from the squishiness of it how kind of it sat in the skin
03:27it was quite striking how much loose kidney had in that area it allowed me to really feel the entire
03:33lesion it's a pretty sizable lipoma so we'll have to cut through the skin um maybe I'll give you a
03:42little facelift on I'm just kidding I won't remove any of this cut the washing go like that like a men in
03:48black yes exactly all right have a seat have a seat yes we can definitely remove this for you
03:53today yes you're gonna have a little scar there though and it can come back but that's very
03:58unlikely the main thing is a scar like Clint Eastwood yeah you kind of look like Clint Eastwood
04:04no everybody says Schwarzenegger oh well maybe if Arnold Schwarzenegger was 100 pounds lighter
04:12maybe Clint Eastwood that's more of his vibe put a cowboy hat on him we're good to go okay perfect
04:21all right you can lie down happy oh yeah okay
04:26so I'm gonna inject anesthetic into the whole area hopefully not be more than one or two sharp
04:35scratches ready how's it feel like a bumblebees on your face all right good that's the way you
04:43should feel it's singing out now you don't want me to do it without it so I'm estimating four grams
04:50four oh for the weight of this yeah we have the scale I think four grams isn't a lot though
04:55anything sharp now nope all right do it again go on what kind of farm do you have you have chickens
05:06oh I have chickens and we got horses on self-livery oh I love horses I've never tried one I hear it's
05:13pretty lean oh disgusting do you feel anything sharp tell me you feel pressure but you shouldn't
05:22anything painful can you see the little darling yet I think I can yeah just try to make sure that
05:36yours is really squishy I'm a squishy kind of guy that's why it's so mobile pretty quickly when I cut
05:44into the skin I could tell that this was a lipoma I could see it so I was really happy that my diagnosis
05:49was correct oh do you feel anything no I just swallowed so this area the jawline can be really
05:58tricky to treat especially when you have a chatty patient because they keep moving their jawline the
06:03trickiness is about putting the scar in the right place because I want it to be tucked under the jawline
06:08and not above it you're gonna have to go dig it aren't you yeah I have to
06:12feels like you're trying to cut through a rope yeah it kind of feels like with the smallest scissors yeah
06:30do you feel it yep I can feel that a little tingle there girl sure remind me how much you thought it
06:39was gonna weigh four grams four grams Natalia what what would you say I don't know I don't know how
06:45where the bottom is higher lower Sean's lipoma was a lot bigger than I expected or that I think
06:52anyone expected oh if you're getting aggressive with it now sorry because I can feel it is that
06:58hurting you no is it hurting you no it really had kind of a root and base that went deeper into the
07:05area and that can make it a little bit trickier coming up I've looked at like home remedies putting
07:13a pin in it my GP was like do not do that I think you're gonna love it this is way bigger than you
07:19thought it was I think no one's ever said that to me before

08:28help you today I've got these fatty deposits on the top of my eye and I think I've got another one
08:33just down here okay how long has it been going on for I first noticed it about 10 years ago yeah
08:39um and they've slowly got bigger over time is it affecting your confidence is it yeah massively
08:45affecting my confidence yeah could be having the best day ever and next thing you know I kind of look
08:50in the mirror and I'm like oh there we go like you can always fix like get a new haircut or whatever but
08:56you can't get rid of these on your eyes I've looked at like home remedies putting a pin in it see if I
09:01can squeeze it out my GP was like do not do that let's have a little look then kids are really brutal
09:11at school if you can think it they've said it the condition that you have is called xanthalasma it's just a
09:20collection of fatty sort of material under the surface of the skin it can be related to high
09:25cholesterol but often it we do see it in patients who've got completely normal cholesterol so it's
09:29it's really just bad luck it's not anything that you've done or should have done it is unusual for
09:33somebody of that age to have xanthalasma to that degree it's more common in people who are a little
09:40bit older it's something which we can treat for you amazing so it would involve cutting out where the lump
09:46is putting stitches in to bring things back together
09:51do you look up for me
09:55lie yourself up onto the bed
10:00I'm gonna tuck this in
10:04are you comfortable like that? yeah
10:06should be there for a little while you know
10:08I try not to go to sleep
10:11you wouldn't be the first
10:15as gentle as I can
10:16and then I'll go nice and numb for you
10:18if you need a break at any time obviously just let me know

10:22for injections around the eye you do need to be careful and you need to be aware that the patient may move
10:29however I am doing lots of surgery around the eyes so I'm very comfortable with working in that area safely
10:35and that's all the difficult bit

10:44so what kind of work do you do then?
10:45I'm a behaviour mentor in a school

10:48I work with like children with challenging behaviour

10:52or like special needs
10:53so it must be very rewarding

10:56because you can kind of really make a difference for this
10:58yeah you can see the like emotional development that they do
11:03but because of that they're really unfiltered

11:10because of the two blobs on my eyes
11:12they say they look like Tic Tacs
11:14how long have you done that for then?
11:16I've done that for four years now
11:18I was in the military before then
11:20I joined and I had my 17th birthday not long after
11:25I was in the army for five years
11:27loved it
11:28just travelling and making some real lifelong friends as well
11:38you feeling alright there?

11:46I'm going to pop a couple of stitches in
11:56and I'm just going to check there's no pull on your eye
12:01what happened after you'd finished your basic training?
12:04I went on tour
12:08I went abroad and stuff
12:10and then I got injured
12:12ruptured both ligaments in my knee
12:14and tore my melliscus cartridge
12:19seriously yeah?
12:20yeah
12:21injured both my hips
12:23and I had a really long recovery
12:27I spent a significant time in
12:29wheelchairs and hospitals and crutches
12:33the military try as much as they can
12:35to make you whole and fix you up
12:37but sometimes these things happen
12:39where you can't be fixed
12:41we've been through such a lot
12:44being in the military
12:45I'm sure that would have a
12:47a huge psychological impact on anyone
12:50and the fact he was injured and came back
12:52and now he's given his time to young people
12:55to help them come along
12:57I think it's great

13:20the eyelid skin is the thinnest skin on the body
13:35and it's often less than a millimetre thick
13:38so when you're placing the sutures
13:40you need to be very careful that they're at the right level
13:43and you would tend to use much finer sutures
13:46than you would in other parts of the body
13:48can you be nice and wide for me please?

13:53close your eyes as tight as you can

23:30the first cyst the wall was really friable and I just couldn't even get hold of it with the forceps
23:47so I had to sort of dig it out in like bits and pieces

23:53the reason I do a little elliptical excision over the top of a cyst is to try and remove the punctum where it's arisen from and remove the whole skin around that so that the cyst can't recur again

24:19I was really annoyed when I did the second one and I went straight through the sack and therefore I'm not gonna get it all out in one go again
24:27in one go again have you got celebrations coming up did you say my fifth year yeah it's just mainly
24:35holidays isn't i always want to go to the maldives my life has changed a lot in the past few years
24:41especially this year i've lost three and a half stone in three and a half months goodness i had
24:46my surgery in february is it like everything you expected so i can't eat fried food like oily stuff
24:53and i'm like because that's the reason why i had the operation not because i would always eat those
24:58things but it was easier for me to eat those things you know what's going on is this the same as the
25:07last one a little bit yeah it's just your type of cyst a very very fragile wall every now and again
25:16you come across just a big band of like fibrous tissue that's really into your subcutaneous fat
25:22i'll just cut that fat out gross it's a bit more off yeah that's pretty much it
25:33that's just
25:36so that's it number two they look like little explosions very satisfying the second one came out
25:44quicker than the first it's a shame they didn't come out in like whole balls but yeah i just couldn't wait
25:50to see it oh my god look at that like how weird is that color there's nothing what i expected it
25:57to be it's just feeling squidgy stuff

27:38to meet you both right jackie what are we doing for you today i've got a large lipoma on my back
27:43yeah on the left side how long has it been there i'll say 13 13 years okay has it been getting bigger
27:50yes i was actually having a massage and the lady said oh you've got a lump on your back and then it
27:56grew and it grew and it grew i think currently it's at about four inches diameter does it bother
28:04you yes it does bother me um it's very very visible i'm embarrassed by it okay now we'll get
28:11you into a gown and then we'll have a proper examination thank you right you can see it whether
28:16i've got clothes over it or if it's showing obviously if it's a lower dress or something
28:22okay i'm just gonna have a feel is it painful at all if i touch it no okay you can see it quite
28:30visibly and it does there's a capsule you can feel it underneath the skin so that's all your normal
28:35skin tissue that i'm pinching now and the lipoma is actually underneath that okay so i'm just going to
28:40mark the area okay all right so lipoma is about roughly about that big underneath the skin um there's
28:51is you've got about that much thickness in terms of normal skin above it so we'll have to go through
28:56that before we actually get to the yeah lump there it's something that i can definitely do for you
29:01today oh thank you okay so we should be able to get it out any questions no i'm just ready to go okay
29:07good it's just a dream come true plus i'm going away on holiday so it would just be great not to have
29:14this um so visibly on show and i'll be absolutely over the moon sorry it's going to sting how are you
29:23doing there no i'm fine good at the local anesthetic i think there was quite a few of those i maybe felt
29:31one of them out of four or five
29:45how's that fine

30:01if you feel any pain you let me know okay okay okay it's a bit of stretching you'll feel yeah
30:20so you can see your lipoma moving around it's hiding is it now we can see it but we can see we just need
30:27to get it through this okay we need to loosen it a bit more

30:44just keep that pressure like you're doing that's up here

31:07there was a lot of pushing pulling and tugging and i think it was a bit of a workout for them

31:35jackie's lipoma was just very slippery because of the oil and the sebum however i was trying to get
31:42it through a relatively small hole so it was proving to be tricky we're getting there definitely
31:48work out this one it's sweating now
31:56what's the biggest lipoma you've ever taken out if you don't mind me asking
32:00yours isn't the biggest no no i'm not i'm not that sad about that though
32:12okay okay so it was just quite deep it wasn't actually not the biggest one we've seen it was
32:28deep deep yeah yeah it's not only a weight off my back it's a weight off my mind that it's now gone
32:37so much more confident about you know going over to portugal would the pool not be a good idea
32:44well depends on how healed it is yeah i mean you're just going to be aware of the risk of infection yeah
32:51i will be careful in the sun but at least i can wear a dress and um it not be visible
32:58so
33:01how was that for you it was good yeah it was fine there was no pain at all would you like to have
33:07a look at it yeah oh yeah i'm wondering where it is this is the bit of skin i took out oh yeah

35:27and i just really want to get rid of it how is it affecting you i've got hyperhydrosis and alopecia
35:33my hair's quite thin as it is and then if i'm sweating it kind of spreads my hair and then you
35:39can see it more clearly i feel like everyone notices it if i've got a wear helmet at work it feels like
35:46it's like pushing into my head okay it just feels really weird what do you do uh i'm an electronics
35:51engineer he does hurt you know and it just causes a lot of discomfort and i'm very paranoid about it

36:07just making sure it's not stuck so it's nice and movable it's quite hard isn't it it's hard
36:15with danny's lump i felt for how firm and how soft it is the harder it is the more chance it is of a
36:22bone growth which is something i can't operate on bone would feel like a stone this was uh like a rubber
36:30and it felt more like a cyst to me good news we can treat it for you if you like definitely i want to
36:37get rid of it come on to the couch there we go come and lie yourself down don't worry about your
36:43shoes they're all fine it's even just uh like when i brush my hand through my hair and i catch it it
36:48just hurts a little bit and i'm like i just want it gone who is that no you don't want to do that
36:56it seemed like there was some sort of problem with the bed this is a good game

43:00and then she can feel my hair too it was quite small but looked a lot bigger well and with thinning
43:07hair and being shorter yeah people do look down nothing wrong with being short there's absolutely nothing
43:14wrong i have no issues with it it's just the rest of them exactly
