01:28start to the day I'm putting a note on to remind me to do that prescription tomorrow the team are
01:35feeling calm oh supper last night was really good I enjoyed that making a few adjustments oh no wonder it
01:45won't come on ready to welcome the first patient 78 year old Jim from Irvine in Scotland hello hello I'm
01:55here for my appointment Jim I was born in Glasgow but I've been in Irvine 36 years now I'm proud to be a

02:35lipoma I've got in the back of my head okay I've heard it for 14 years it's a long time it's called
02:42George George George yeah because I had an uncle that had a lump in the same place honestly and he
02:49was called George so I decided to call him a lump George I mean I didn't like my uncle George but
02:55he's gave me that I think yeah fair enough he's given me okay how is it bothering you I played
03:03bands all my life it's all right when I was on stage but see as soon as I left the stage you think
03:08people's look me do you know I mean standing in queues queueing at the bar so it's really affecting
03:13you yeah can I come and have a look and we'll see whether we can sort this out for you um and then
03:20I'll take some pictures if that's all right yes and then we'll decide what we can do oh yeah
03:26he's a beauty isn't he yeah are we able to shave a bit of your hair certainly it's just a horrible
03:33thing that shouldn't be there and it's it's growing I don't know why maybe it's my brain's

04:56some misfunctions of beds and things like that just settles the patient in
05:01when you're bringing together skin if they have long hair it sometimes flops over
05:10well there he is and get sutured into the scar and that will give you an infection
05:20sharp scratch coming up okay

07:36doing there fine thank you okay jim's lipoma was slightly odd in that i could see the membrane around
07:44it and it just wouldn't leave his scalp so i had to snip around the membrane to free it up at the base
07:52and allow the lipoma to come out how are we doing pain wise i'm fine doctor sure why
08:02okay come on george we see all the way through at the moment there's light there you can do it
08:10there we go there we go there we go okay george was really tricky um but dev and i were definitely
08:24going to have our way well it was very deep i have to say was it really yeah george was just so big
08:32that we had to put more sutures than i'd anticipated there we go how's that perfect well done it's
08:41looking good you've got a little bit of a dent in your head though so you've got to be careful
08:46here we are lump free it's gone george is gone how are we doing good okay that's what it looks like

11:14i have a lump on my back it's been there for about six years and it's impacting everything i do now
11:22because of the pain it just seems to shoot through my back when i do anything what is it stopping you
11:29from from doing i like to swim i can't do that because i can't lift my arm up without the pain
11:36and i also like to sing yeah um it affects me when i breathe deeply i see so i can't hold the
11:43notes that i used to hold it'd be good for me to take a look at it today and then i can tell you
11:49what could or could not be done is that okay do you want to maybe put your cardigan up onto the hooks
11:54there okay it's on my mind every single day it never it never goes away this area here yeah that's
12:04where it is is that hurting when i'm touching it a little bit just then okay so when i was uh examining
12:10paula it was actually a lot easier to to feel it um than it was to see it
12:15so what it feels like is a fatty growth under the surface of the skin called a lipoma so in terms of
12:24removing it it's a local anesthetic procedure and you can do it i can do it for you today yeah thank
12:30you yeah thank you um so that's there's no problem for me to do it i was over the moon it would open
12:38up the rest of my life for me so it's quite a long scar there yeah i'd sooner have that than the pain
12:43every time yeah probably most cases that will improve the pain but we don't know for sure that's
12:48just the thing yeah with a bit of luck yeah touch wood okay so the first thing i'd like to do is to
12:54mark the area i wasn't completely confident whether the pain that she was experiencing was caused by the
13:02lipoma but i thought there was a very good chance that it would help if you'd like to lie onto the bed
13:08onto your front now how's that it's good for me now the next thing that i need to do is i do need
13:16to numb the area okay just a bit sharp just you know like it when you're doing sewing and you prick
13:26yourself with a needle it's no different and i want you to tell me if it's sharp or you can feel it as a
13:35sharp touch on the skin okay how's that feeling can't feel anything
13:51so you're going to feel lots of stretching and pushing okay but you mustn't feel anything painful
13:57if you do you tell me okay okay how's that for you now yeah i don't know what you're doing that's good
14:12what kind of work did you do previously then i was a chef oh are you any of this uncomfortable for you
14:19no that's okay it's all kind of coming out very nicely
14:32oh so sorry a little bit more of being sitting in that area sorry yeah don't apologize just keep
14:38telling me whenever you feel anything okay it did feel as if something was being cut and it was quite
14:46quite painful we'll just go slowly yeah so we have put lots of local anesthetic in but even still
14:52sometimes there can be a tiny area where it's not completely numb for you yeah
14:59oh wow feeling that yeah sorry not to worry just let us know and we'll keep you just let us know
15:05we'll just put more local anesthetic whatever sorry about that please don't apologize just please
15:09keep telling me okay he said to me don't apologize well i can't help it it's just me that says oh i'm
15:16sorry it's hurting i can't help it it's obviously quite deep so it's quite common that you can feel a
15:21little bit snip snip snip yeah just kind of getting it all out for you
15:26so what are you working still or are you no no retired so what's it how do you keep yourself busy
15:39like i said i do my singing what would your song be if you were doing karaoke jolene
15:43jolene oh i love that as well i couldn't do a jolene i'll be honest what would you do

16:13well done okay so we've got all of the lipoma out now what i do need to do is i do need to just
16:22seal off the blood vessels now i'm so so pleased i might go to swim again now yeah good swimmer yeah
16:29not bad i used to swim for the school oh nice so in paula's case the discomfort that she was
16:35experiencing was a little bit unusual because she was describing a pain when she was moving her shoulder
16:41and i think that probably the reason for that is when she was constricting the muscles and moving
16:46that was pressing on the lipoma and distorting it well good news we're all done i can't wait to sit
16:53up yeah yeah you can sit up now in a moment considering what was going through gosh actually quite good fun
17:06that should heal well for you so it is just a straight scar yep like that okay lovely yeah
17:11now did you want to have a look at it before you go yeah so please it's been sitting in there long
17:17enough i want to see it wow i couldn't believe it oh my god it's like that there you go so have a
17:26little feel of that can you feel that right it's a little bit springy press on it it look like a bit
17:32a bit of a bit like a fish a little tail on it horrible thing bye bye then thank you thank you so

17:49minute of my life see you bye-bye bye-bye so with luck hopefully i'll be able to go back to being the

18:33how can i help you today i've got lots of skin tags under my arms both arms and when it gets
18:39somewhere it's just i set on fire if you will oh okay i had somebody moved about 10 15 years ago okay
18:46but this they've grown back we have engines how quickly do they grow back within five years all
18:52right it's probably about 15 each side okay that's a lot of skin tags okay so let's have a look at these
19:01see what's going on
19:05one arm both oh my goodness wowzers when you said 50 you weren't lying
19:10i was trying not to show my surprise like my 50 under each arm like it's definitely the most
19:17skin tags i've ever seen under someone's arms with him it was like a carpet of skin tags great news i
19:23can definitely remove these today that's brilliant when natalia said you could remove them it was just
19:28like a weight off my shoulders let's have you hop onto that couch we're going away to turkey with the
19:34heat and you sweat underneath it just it's like sticking a red hot poker under your armpits all
19:38the time when you're walking around how's that that's fine you look very relaxed all right so i'm
19:44going to just start with your anesthetic injections wow you don't feel it no because the underarm is
19:53really sensitive to needles rick seriously does that hurt you have to feel something here just little
20:00well that's it it stops i didn't even flinch because i've got a hard pain threshold i think
20:06it's rubbing tattoos so it helps all right that's been fully anesthetized i think so you have this
20:15interesting looking tattoo on your leg it's very pretty it's burning football club the middle parts
20:22it's a dress oh and then the rose at the bottom is for lancashire okay lancashire lancashire rose
20:28like shire i learned to speak english in america so this sure thing at the end is a bit confusing
20:33um yeah all these like lincoln lincoln sure okay so you love your football yeah you love it so much
20:40and you had them tattooed on your leg a fun thing for me is hearing a patient with a different accent
20:48and watching natalia trying to figure out where in the uk this person is from and the other where's
20:53where's burnley got me in manchester oh okay fine well you sound like a manchester person or is that an
21:00insult she is just not very good at recognizing accents all right tell me if you feel anything
21:08you're getting to the point of thinking i'm just gonna use a scalpel and just take them off how are
21:28you thinking of doing that were you having your wife do it i'm not just doing it myself in mirror
21:32well i've tried different methods in the past you can put string around them and tie it tight it's
21:38meant to stop them and they're meant to die and fall off but it never worked yeah i wouldn't recommend
21:43that generally i wouldn't say operate on yourself in any way or capacity at home just see a professional
21:54all right i'm done with all of them this side there's a couple little bleeding speckles because i
21:57might hyperkeep skin types can be really quick to remove if there's just one or two but if you have
22:02a lot of them like russell did uh then it can take a really long time to remove them all does this seem
22:09familiar from last time yeah the smell oh the smell yeah if you've ever burnt your skin on an iron or
22:16a flame or something it's a horrible smell but natalia just texted in a strand sorry all right half of it
22:24this is done one arm down just another 50 to go i definitely get in a pattern with the with kind of
22:41holding it with tweezers pinch and snip if you will and it's kind of like one of these meditative type
22:46processes where you're just like going through it systematically
22:49what can we call these what do you think they look like there's no sweet that's that size
22:58natalia is the only dermatologist i work with that compares everything to food i don't know why
23:05oh no but you know what you put in like oh no what you have to say like a pie
23:12the beads you put in pie you have to bake the base of a pie and you put those hard beads
23:19yeah i haven't really baked anything in my life ever but definitely not with bees i just
23:29all right i think we're done with this side okay are you ready to have a look it's a little bloody
23:35but you can see it'll be smooth once it heals i'm over the moon i'd have been happy if she'd only
23:41managed to take 50 of them off but to remove them all is just phenomenal okay so you can relax your arms now
23:49so if you want to have a seat at the edge just perch there for a moment
23:55um i quite like removing skin tags it's different from fighting with lipomas and cysts to get them out
24:00how are you feeling i'm fine thank you yes happy very happy would you like to see them yeah let's say i'm
24:06going to work let's have a look so there we go right and left just one or two yeah just one or two i
24:14think this is the most skin tags i've ever removed from someone in one go you did really well there's not
24:18too much blood see you're not a bleeder i'm stinging me my blood said you're the blood very good all
24:23right so we did it got them all done hopefully we won't see you again for about five five to five
24:27to ten years yep yep perfect thank you very much you're welcome thank you for coming in

25:43hello hello hello hello is it you hello it is yeah you've got donna with me hello hello
25:50come in have a seat so yolanda what can we do for you well i've had eczema and psoriasis since i was
25:57seven years old okay and then i lost all the pigment on my face when i was nine wow and then it came back
26:04but because of my age the psoriasis is getting worse it's been particularly bad on my head especially
26:09and so when you were little whereabouts was the rash on your body can you remember yeah it started
26:15on my hands and my hands started peeling like this like this i've got there so it was all over my
26:19hands yeah the worst thing really is very self-conscious of the appearance of your skin
26:26it does impact the way you dress the way you feel and then it was the back of my knees okay so that's
26:33kind of in like an eczema area yeah yeah and then it started on my elbows this side of that side so
26:40that's more like psoriasis so because it's unusual for people to have both but it sounded like maybe
26:44you have had both of them so eczema and psoriasis are both conditions that cause red scaly bits on the
26:50skin they sort of tend to affect sort of opposite bits of the body so eczema attacks the flexures
26:57and psoriasis is on the extensor surfaces so if you think of either eczema's behind the knee psoriasis is
27:02on the top of the knee when i was nine i first got this and my hair fell out your hair fell out when
27:07you were nine when i was nine yeah when i started my cycle yeah first time yeah and then monthly ever
27:13since my hair always falls out time of the month it's been um i don't want to say a life sentence but
27:21it feels like that a little bit and then i lived in spain and it disappeared okay yeah in the lovely
27:26sunshine and a lovely climate yeah ultraviolet light from the sun actually suppresses the immune
27:32system in the skin and we actually use it in the form of uvb phototherapy to treat skin conditions
27:39may i have a little look at your skull yeah is that okay do you want me to hang your coat up for you
27:42be great thank you look at this i love it right away that is so cute i have a rescue cat she's my
27:50everything oh are you okay first yeah look yeah this looks oh gosh that's really really intense
27:56psoriasis it is isn't it very very thick yeah the itch is like something's eating you and biting you
28:02from inside it's so intense and do you have any active plaques anywhere else on your body at the moment
28:08on your elbows yeah my elbows can have a little look at an elbow and just clinch the diagnosis
28:13really yeah yeah okay an active plaque of psoriasis is one that is inflamed it's usually a little bit
28:20itchy whereas older plaques that have maybe started to settle just leave behind an area of discoloration
28:26they're not itchy they're not they're not scaly i can see where you know you've got richly pigmented
28:31skin and when you get inflammation it can either go pale but it can also go dark and you've got
28:36hyperpigmentation on this one but this one's a lot better oh look so you've got both you've got
28:41hyperpigmentation and hypopigmentation whereas you've lost your pigment if the dermatologist could
28:46give me some pieces of some nuggets that i don't know that would change this condition
28:52i had all of this with psoriasis all that's a common place so i've got a tiny little back there at the
28:58moment it could be a different half of my life without this condition now i've had something before
29:05with which is coconut base and i've found out that i'm intolerant to coconut that's what i was going
29:09to give you yeah so this is why i'm a phenomenon yeah we prescribe a lot of creams and ointments
29:17and things that you rub on the skin in dermatology they're not all very well tolerated some of them
29:22are really greasy some of them are really smelly and nasty to use and for most patients there's a good
29:27bit of trial and error until we get something that they like to use and that actually works for the skin
29:32i'm going to get you some of our special in-house made sec sal cap it's called so it doesn't have
29:38coconut oil in it but it has the coal tar yes and it has the salicylic acid in it in a really strong
29:45ointment wonderful that's amazing no problem amazing so tell us about your cat
29:53tell us about your cat she's called princess riley patience precious blessing
30:00yolanda's cat had a really long name but i can't quite remember it so this is my latest
30:05picture of her oh she's golden oh look at her green eyes she's a stunner it definitely had a title
30:14and a first name and a surname or something like that discovered this thing it must be a
30:19menopausal thing that i can draw i never knew i can draw so i've been drawing her and painting her
30:25that's very mindful i thought you were going to say that when women hit a certain age they start
30:31collecting casual dogs and it's called the many paws that's so funny get it many paws
30:43so i've written a little treatment plan in here can you prescribe like a maybe a summer holiday
30:48you know in germany they used to prescribe like two weeks on the beach for the
30:53right for the psoriasis patients because actually you're relaxed you're not stressed
30:57maybe you can suggest yeah unfortunately no okay but it would be a very cost effective
31:02thing i think we need to start a new political party called the let's go on holiday for skin
31:08conditions party led by dr emma thank you so much you're very welcome it's been great it's been great
31:15we'll let you get back to your cat i'm not going to try and say the name i just remember
31:20was there even a princess in there maybe there was i can't remember
31:24she'll answer to anything nice yeah yeah yeah if there's a treat involved
31:29so i've been giving some good advice and a good plan going forward take care bye bye

31:43it's really exciting and i can't wait to see the results

32:25actually i have no pets and i absolutely love animals so i did just get a puppy four tortoises

33:50barbers you know dealing with the skin issue is something that it doesn't make you feel great
33:54and think greater by yourself sure not nice is it it's not pleasant can i have a look yeah of course

34:15is a good size one isn't it yeah so one uh or i can feed another one here yeah there's another big one

34:36well yeah and then five they feel soft and squidgy as i would expect cysts to do they feel fluctuant
34:44like they're full of fluid and they are mobile as well i think we can probably do all of them for

34:55all five removed i was like great i want to wake up tomorrow and touch my head and nothing to be
35:00there and i'll be happy let's see what we can get okay so step one is the injection yep okay this bit
35:11will sting yeah but it will go numb and you're going to need obviously five because we're doing five so
35:16we'll have to do five lots of injection okay okay right you ready this is going to sting yeah i'm
