01:18piercing you could feel a little prick yeah I've been called worse oh I could call him a lot worse than
01:25that it's always a juggle for the team with admin and prep first thing in the morning only me do my
01:40emails doing your emails that's my euphemism okay word word word oh you can't make this change
01:50because selection is locked what let's hope the treatments go better than the tech my name is

02:38to meet you welcome to the clinic take a seat how can we help Richard well I have a fatty lump okay on my
02:48side it's been with me for actually like 20 plus years I think when I started off I first noticed it
02:54it was the size of a probably a pea yeah now it's more like a satsuma it'd be great if something could
03:00be done before it gets to the size of a grapefruit okay fair enough fair enough how does it affect you
03:06any sort of activity where you're having to bend or twist you know like digging in the garden or part of
03:11my hobby which is running around firing a musket sometimes I will I will twist and it almost feels
03:19like it's moved and almost gone between the ribs or something I do historical reenactment okay so I do
03:27Napoleonic reenactment we did we do reenactments all over the UK and and quite a lot in Europe as
03:31well during my hobby with a heavy wool uniform and quite a lot of leather straps and belts can
03:40get in the way so let's have a little look we'll take your top off as well there we go perfect
03:48so there it is can I just have a feel of course yes yeah and if you want to raise your arm up
03:58it's more pronounced when yeah when you do that yeah lipomas in general are big spongy bits of fat
04:09that have a very clear membrane like a balloon around them my aim would be probably to make a cut
04:16about that size yeah and hopefully get it out that way if it doesn't come out nicely we may have to
04:23make the the cut slightly longer so you're looking at something like that yeah okay so that's roughly
04:32what we're going to do you happy with that yeah that sounds great excellent coming back after a long
04:38sort of hot sweaty battle when the first thing you want to do is get that jacket off get your shirt off
04:44to not be conscious of the bumps sticking out that would be brilliant you'll fill a sharp scratch okay
04:50on the count of three one two three how's that I was bracing myself for something a lot worse than
04:57the reality the anticipation of a needle puncture is is worse than the reality of it

05:22actually I did feel a little you don't have to be polite

05:53he's just releasing it yeah it always felt it always felt like it was sort of movable to a degree but
06:00it's obviously within its own nice and superficial it's good I was sort of aware of devs finger sort
06:08of running around the inside quite bizarre but not not unpleasant

06:21has that any paid no no you know the napoleonic thing what was the spark I like the flamboyancy of
06:31the uniforms but they I mean ridiculously colored bearing in mind you are going to be shot so why
06:37make yourself red just well yeah there's there's I mean there's a whole sort of thing red red um was
06:44the cheapest ah is that why the arm the British army of the time they went for
06:53they went for the cheap cheap the cheapest option we're permanently being educated by
06:59our patients it's wonderful to have them awake and able to chat it doesn't
07:07want to come come on he's been holding on in there for a long time there's a lot of years
07:14here we go so I would imagine he's he's you know he's probably quite at home

07:23is that sore it's not sore it just feels like you can feel him feels like it's pulling on something a
07:35bit further okay round but he's just tried to as you said it was a little tethered at the base and
07:41that's what he's working out there were a few occasions um during the removal where the the
07:49pulling was quite was actually quite intense good so what is it then just a collection of fat cells
07:56that's right that the the of what but we don't know why they form uh no we don't know why they
08:06form but it's certainly genetics
08:08you can sort of tell what's normal fat and what's lipoma
08:21but that's all come out quite nicely really there's nothing left in there
08:27it doesn't feel like i've been cut open nowhere near as bad as coming out of the dentist
08:31how neat is he with this oh his stitch work is okay he's part of the wi he's okay see anyone who
08:42can hand sew and do it well should should be a surgeon they should because it's so much harder
08:47to sew clothes because every little mistake it doesn't heal whereas skin heals it's really forgiving
08:54yeah i do actually have an event to go to directly um after this so i will be a good boy i will not be
09:03um firing the musket and i will not be even taking it out of its box
09:08okay so you're ready yes ready you could lift that big reveal there you go wow there it is
09:16look at that don't ever play with it it's quite a lot in it yeah oh my god
09:22that's amazing it that is amazing yeah it was it was quite deep
09:30it looks it well i mean it looks pretty gross to be fair doesn't it
09:36you're happy to see it there in the in the tray rather than in me

10:59all right so tell us what we can help you with today so i've got a hypertrophic scar on the back
11:07of my ear yeah for about three years okay it's just getting in the way yeah i can imagine it looks
11:12it looks like it might get in the way is this from a piercing perhaps i think so yes it looks like
11:17a lobe a normal lobe piercing yeah so i take my earrings out one day yeah and it just what i feel
11:23is like the back of an earring and it fell into my ear and it's i don't know it just it just appeared
11:29a back sorry yeah like the butterfly clip with an earring i'd lost it and i couldn't find it anywhere
11:33that appeared a couple of days later a keloid scar is your body's reacting to having been damaged
11:40by kind of going a little bit nuts with the scarring all right let's have a look at this ear
11:46can we have you stand up and look that way for me gosh and it's through over how long to this size
11:53um about two and a half years
11:54oh
11:56the base of the actual scar was relatively small and that's really good because that involves
12:05um less tissue being cut into to remove that scar
12:09and there's something that we can definitely do for you today i numb the ear
12:13i'm going to shave it flush to the skin either i can see the back of it when we remove it or the
12:18lab will find it if it's stuck in there i just think it's really weird if it's a piercing like if
12:23there's a back of a piercing in there
12:24it's happened to me but then you've got a keloid from that no i it's just that basically i had my ears
12:32pierced and then it got infected as it got swollen it consumes the back of the ear had to have
12:38it surgery to remove oh my god my immediate thoughts was how bad was this infection i've
12:43known dutan worked with her for quite a few number of years now and i've never heard that story before
12:47this kind of story freaked me out you will have a scar on the back of your ear
12:52that's i can't see that that's fine you can't see it's fine okay come on over
12:56i was a bit nervous hoping that it can come off and a bit of excitement as well
13:02all right so sharp scratches coming now all around you ready and deep breath sharp good well done
13:10well done it's not comfortable this in the ear well done well done i inject into the back of the
13:16ear the skin on the back of the ear breathe and that can be very painful on the ear especially if
13:21you're around cartilage because there's not a lot of fat there to kind of absorb the pain
13:25all right that's it okay yeah you're gonna cry you're okay i'm all right good
13:32close your eyes you shouldn't feel anything if you feel anything at all you tell me immediately
13:43i need to get underneath that keloid because it was really sitting over the lobe so the only tool
13:58that you can actually do that with is a derma blade it's a flexible tool like this but it's quite a long
14:02sharp edge so you can just basically slide it under the keloid and and then you can remove the keloid
14:08easily like that are you all right memphis yeah by the way you have the coolest name
14:17yeah why are you named memphis uh from the movie gone in 60 seconds is that a character's name yeah
14:23i think pretty sure it's the main character you should watch it again i can't remember i watched
14:27ages ago i watched all the um final destination movies earlier in the week the what movies final
14:35destination is that a tom cruise one you're thinking of mission impossible yes i am thinking
14:41of mission impossible
14:42don't wait we're almost there i was about to say how we're doing you're good you're doing excellent
14:57the most important thing for me is to not affect the skin of your actual year
15:00when i remove a keloid i really don't want to damage the actual the normal skin there
15:07the more you traumatize the normal skin the higher risk of it recurring so i'm really careful to make
15:11sure that the normal skin stays as kind of intact as possible are you okay memphis you
15:19yeah i just i can feel i can feel like pulling on it so it's just like weirding me out a little bit
15:24okay i have it oh it's off that's so weird yeah it's off oh my god it's okay
15:38it's okay all right now i'm gonna just do the coterie thing okay ready
15:41you're gonna hear a buzzing oh pretty good oh yeah you can smell it a little bit yeah it's just
15:47like burning flesh but there wasn't a really bad smell to it oh my god you know what it's not
15:55from the the lobe you're saying oh my god i'm going through your lobe right now this keloid came
16:00from your second piercing really from where the little keloid sticking out the front oh it's not
16:05connected at all to your other one i think it started growing from below but it's directly
16:09above the hole is still there i thought the keloid was from my first lobe and after she had taken
16:16it off and realized it had come from my second piercing instead which was really weird so i tried
16:21to move all your hair out of the way and fry your hair okay i'm getting it all cut off anyway all right
16:26oh okay i'm going for a pixie cut now that that's off oh cute now i've got my hair sorted it's just
16:34amazing it's so free in a way so how do you feel great do you want to look from the front
16:42and see what your because your ears are symmetrical like look both ears that's so weird
16:47it's incredible oh good i'm genuinely like i've waited it for this for years oh good so it's so
16:54weird to say it's finally off it's finally off best bit of my job is when i remove something like a
16:59keloid scar especially from a younger person and it completely transforms their demeanor how they
17:04feel about themselves how they put their hair up how they behave so it was really rewarding to see
17:09the immediate change in her do you want to hold it yes oh it's so it's it's less red than it was on
17:19your ear because there's no blood in it oh that's so weird yes and let's turn around if you put your
17:25right where the fat is that's that and you push it down you can't feel any metal it's just hard
17:30it's going to go to the path people it's so bad that's it that's weird it was weird because i thought
17:38it was a lot smaller but when i was able to pick it up and feel it felt bigger than it actually was
17:43when it was on my ear thank you very much you're so welcome i'm so happy thank you great

17:52i wouldn't stop smiling over because it was so relieved to just have it off after three years
17:58it was really cool
17:59i love a keloid scar removal makes me so happy i think it's the biggest one we removed i think so

19:04about 20 years ago and then this last few years it has gone and has it ever been painful or boozy or
19:12red no no anything come out of it no no have you ever had a no no no no no no okay the grandchildren
19:21they're always looking at it saying oh i think then it's getting bigger and bigger does it cause
19:27you any trouble sort of day to day no sometimes it's a bit itchy or sometimes when i lay on it
19:33and is there anything that sort of prompted you to well my grandson's getting married oh so um as
19:41you can see when i go out in the wind it just blows and shows right hi yeah my neighbor one day we
19:50was talking and it was quite windy and all of a sudden she went oh goodness whatever's the matter
19:55with your head it is embarrassing do you mind if i have a little look no no no no is it sore today
20:02nope nope that is a beast season
20:08i thought that must be really uncomfortable you know how did how does she sleep
20:14i was quite surprised that she'd had that for so long we can remove that no problem that should
20:19be super simple to do hopefully when i was excited i can have it removed
20:26that's it i'm just going to give you a little bit of a superhero cake the hardest bit of any procedure
20:34is getting the patient in the right position yeah we need to make you the glamorous granny the wedding

26:13this pretty obvious i would have thought
26:15yeah
26:16yeah um yeah i've been had it for about nine ten years now
26:20ten years and it's got bigger in recent years
26:23it must be having quite a big impact on you
26:24well it does to a degree
26:26i forget about it but then i get reminded when people look at me and do a double take
26:31yes and that's going to affect your confidence isn't it
26:34recently a little girl was facing backwards on the bus
26:38and she was going mum mum look at this you know that sort of bothers me
26:44how does it affect your work or your hobbies
26:46well it doesn't affect my work because i work on a computer most of the time but
26:50it does affect my hobbies i'm into amateur dramatics
26:55um and you know i'm hoping to get some extra work maybe for fun
27:00yeah but um of course anything like that you've got to have a portfolio of photos
27:04i thought of photoshopping the lump off but i thought that's no good because you turn up
27:10for an interview and you're out of luck
27:13okay it's most likely either a harmless cyst or a harmless growth of the fat cells
27:24caused a lipoma i can remove it for you today if you would like
27:27yep it'd be nice to look in the mirror and not see that
27:32i don't think anyone would really want to walk around with this lump
27:36on the head when you're ready lie yourself back onto the bed there
27:39yeah you're all right you're okay in that position
27:43i'm just going to pop some local anesthetic in there so just be patient with me i do understand
27:48it's a little bit sore
27:50see once this is in you won't be feeling anything else painful or sharp
27:56i thought it looked more like a cyst because it was a very rounded
28:01um shape but when i was touching it was a little bit more spongy
28:05um so i was thinking there's a possibility it could could also be a lipoma
28:10if someone said put a bed on i'd say yeah i i think that's a cyst
28:15there we go
28:17all okay for you yeah now if any point you do feel anything you just tell me immediately
28:25have you been in a film before yes i was in a horror film
28:29you're in a horror film i was um an extra basically driving a camper van with my supposed
28:37family yeah and uh they filmed that and then they filmed us in the van
28:43yeah with our necks pierced by a blood-sucking um a dwarf vampire basically
28:51oh it sounds brilliant
28:52but yes it's available on dvd i have to check that one out
28:56well it was all filmed around southampton where was it and uh so it was interesting from that
29:02point of view as well yeah i am not a horror fan at all i'm absolutely terrified of scary films
29:11well it's not a cyst
29:12probably looks like a lipoma yeah
29:17as soon as i took the top layer of the skin off i could see immediately that there was fat cells
29:25so it was very clear to me that it was a lipoma rather than a cyst
29:29i think i feel some drips yeah it'll be a little bit bleeding there will be there will be yeah i
29:37could be filmed for that scene again
29:39yeah being honest this would be a good time to film it but pop some gloves on for me angela and we'll
29:45just get a bit of pressure on there but it's coming out nicely actually i have to say so that's
29:51one good thing good that's coming out very nicely
29:54yeah i mean these things they do bleed i'll be honest it's just that is what they're going to do
30:03there's no yeah no you can't head away from that especially in this area yeah it's going to bleed
30:08so hopefully it won't yeah take too long that should be fine
30:12it is going to bleed so i'm going to need angela to press on that firmly for me
30:16there we go great the difference with a lipoma versus a cyst is that the lipoma does have a blood
30:24supply needs to get a little clip on it there we go yeah there you go you are expecting to see some
30:31bleeding and quite often it does take longer to deal with the bleeding than any other part of the
30:36operation you're going to hear a bit of a buzzing noise just sealing things off
30:43it's essential that all of the blood vessels are completely sealed either with cautery or with
30:49a stitch of their larger blood vessels because otherwise you can get bleeding after the operation
30:53which can be dangerous for the patient
30:54you can make a good suit or something can you yeah with your sewing skills
30:59okay so we're all done
31:12so do you want to have a look at it then oh yes yes so grab that mirror there
31:17yeah so you can see there it's all come out and that scar is kind of in your crease there
31:23yeah well done okay thank you thank you very much
31:27they did a very good job cleaned me up nice with all the blood on me and you know it's very good
31:36so everything went smoothly it we were going to send it for analysis it looked more like a lipoma
31:43which was maybe not what we were expecting um did you want to have a look at it before you go
31:48at all or what the the lesion that was removed um can do out of interest totally up yeah if you
31:53want to bring it over for you yeah there you go so that's that's it there it's quite big actually
31:59that is yeah that is it that's surprising did you want to touch it we can give you some gloves
32:03it's just a relief in a funny sort of way really a good luck with everything yep thanks bye

32:21there niggling me you know whenever i look in the mirror or someone looks at me and gives a double
32:27take hopefully it just looked like a wrinkle there there you go great yeah so yeah i really thought
32:35that would be a cyst yeah but you do never know um thanks to dr magnus clive's scar will be hidden
32:47in a frown line but that's not always possible luckily the skin a and e team have some top tips
32:55for helping any scar heal get your stitches removed on time make sure you avoid the sun
33:01often advise patients to put vaseline onto the scar to massage it and you need to use a fingertip a good
33:08firm fingertip so not like this like you're pointing and circular movements little and often

34:07for you so i have a cyst or a lump on my right shoulder okay that's been there since 2018 it
34:15started off just like a normal skin lump yeah and i think it's been slowly getting bigger over the period
34:21of time um and also looking a bit more angrier it's been there for seven years and i think it's
34:26time for it to go you know it's been there for a very long time and uh i'm worried what other people
34:31might think about it if they see it it might go you know i've had a few things like that so yeah
34:37not fun i can't wear like certain shirts because sometimes you'll see like a little lump over there
34:42yeah i worry about dating with the lump because you know if i take my shirt off and a girl sees it and
34:48they think crikey what's what's this lump on his shoulder what have i got myself into so uh
34:53does it ever leak no and have you ever had it treated before no i've had a scan on it last year
35:00sure and that came back harmless all right if i can have a look at it yeah
35:04are you okay taking your shirt off yeah yeah
35:08yeah yeah obviously it's i mean it's pretty visible isn't it yes yes it's more annoying than
35:18anything you know is it painful no i mean crikey my mum's tried to press on it and
35:25i keep to that and no don't press on it make it bigger

36:22capable hands to sort the job out right now step one is anesthetic this bit stings and then it will go
36:28numb yeah that's fine um and then you shouldn't feel anything once the anesthetic's gone in no
36:32problem okay right so when we put the anesthetic in it will pressurize it slightly yeah and sometimes
36:38we get a little bit anesthetic squirting through which sort of shows that it is a cyst okay right

36:45you okay yeah all good yeah there we go so we can already see the contents trying to come out
36:57it essentially squirted out of the original opening at the top this confirmed that it was a cyst
37:06now i'm concerned that it may erupt when i actually cut into it coming up the longer i've left it the
37:13worst it's probably got

37:31cysts are sacks or balloons under the skin that contain dead skin and debris when i injected the
37:41anesthetic the anesthetic the increased pressure caused some of the contents to start oozing out
37:46even before i'd start cutting
