Join me for my 6-month dental checkup with Dr. Andres Garay at Surfers Dental Centre! Great news: no new cavities after fixing three major molars! The catch? My malocclusion (misaligned bite) is worsening, leading to tongue-biting and a small white patch. Dr. Garay assures it’s benign for now, but repeated trauma could pose risks, like oral cancer. He recommends bite correction to prevent future issues. Takeaway: Regular dental visits are key to spotting problems early, and bite issues can be fixed at any age!
What is Malocclusion?
Malocclusion, or misaligned teeth/jaws, impacts 60-75% of people globally (Proffit et al., 2013, Contemporary Orthodontics). Correcting it improves chewing, reduces tooth damage, eases jaw discomfort, and prevents injuries like tongue-biting, which could lead to serious conditions if ignored. Braces, aligners, or surgery can boost oral health and well-being.

00:00I am back at Surfers Dental. This is going to be my first checkup since I had those three large molars reconstructed or they replaced those fillings. I'm pretty nervous. I'm always nervous before I go to the dentist. I'm nervous if it's bad news because then obviously they're sick with you and you sort of think about it until you get it all fixed. But today I am booked in for that checkup and a clean. So all figures crossed we'll see what happens in there.
00:30And how have you been feeling that large restoration that we did? Oh yeah like there's no pain or anything like this. It's comfortable to buy. Yeah it is comfortable to buy.
00:50Like I don't feel that this side has changed at all. As in when I bite down on this side I don't feel it higher. It bites down.
01:05So let's start checking everything here. So wide open for me. A bit of a storage. Can you relax your lips so I can pull it?
01:25I don't feel it. I don't feel it. I don't feel it. Can you open it really well for me?
01:35Now I'll poke it tight out. I'm going to put it in right here.
01:47Open it in. Get a fresh air next to the top.
01:50I'm going to start checking your teeth now, a bit of air.
02:20So what you're saying is a bit of your teeth on your teeth, top and bottom.
02:50So you're ready for this right now?
03:04Can you close halfway?
03:07And yet the erosion that I was saying is losing tooth structure because of an acid environment in your mouth.
03:21So for that one unfortunately the treatment is not only for one tooth isolated because most of your teeth are having this erosion.
03:33Can you open a little bit wider for me?
03:36Excellent.
03:37Your gums are very good though.
03:42There is no sign of inflammation so far.
03:46You do have a bit of recession.
03:49So they are not too large.
03:50Do you remember if you beat yourself here on the top?
03:51No.
03:52No?
03:53Yeah?
03:54I do.
03:55No.
03:56No.
03:57No.
03:58No.
03:59No.
04:00No.
04:01No.

04:11No.
04:12No.
04:13No.
04:14No.
04:15No.
04:16No.
04:17No.
04:18No.
04:19No.
04:20No.
04:21No.
04:22No.
04:23No.
04:24No.
04:25No.

04:36No.
04:37No.
04:38No.
04:39No.
04:40No.
04:41No.
04:42No.
04:43No.
04:44No.
04:45No.
04:46Now let me check the bite, the bite together.
04:59Can you try to grind your teeth with your right side when I'm touching here?
05:13Come a little bit more, push, stay there.
05:19The bite together.
05:20Now try to do the same to the left.
05:23Yes, stay there.
05:26All right, that's it.
05:28So can that turn cancerous if you continue to?
05:32It's a risk, yes.
05:34So when we see white patches in the mucosa, we have to always monitor it.
05:38Sometimes they can turn into something nasty.
05:41But it all depends on risk factors.
05:43So if you're not a smoker, if you're not a...
05:46Don't try alcoholic drinks, like, frequently.
05:50Probably you shouldn't use any alcoholic mouthwash.
05:54I do.
05:55I use Listerine.
05:56It's better not to use it, really.
05:58But it's like a recommendation for everyone these days.
06:01Because alcohol could be too aggressive to the mucosa and the gums.
06:05Yeah.
06:06So there is not much benefit from having a mouthwash, really.
06:10So the teeth actually will benefit more from brushing, leaving the toothpaste and flossing.
06:17So the mouthwash, it doesn't do...
06:20Like I would say, the main job of the mouthwash is, like, fresh in the breath.
06:27So, and of course there are some medicated mouthwashes that they do have some deeper effect.
06:32But literally, you know, the mouthwash over the contour, they are not, like, a big deal.
06:39And even so, if you, for example, if you brush your teeth and then you do the mouthwash afterwards,
06:44what you're doing is removing the toothpaste and the fluoride that the toothpaste is giving to the tooth to make it stronger.
06:50So washing it away is just reducing the effectiveness of the hygiene process.
06:56So, I don't see much benefit from mouthwashes.
07:01Of course there are different slow thoughts, but that is my opinion.
07:04So you brush your teeth and then while the toothpaste is on your teeth, actually floss?
07:11Look, the flossing for me, the most important thing is doing the flossing, right?
07:17Yeah.
07:18But what I recommend usually is to do first flossing.
07:22So you kind of remove everything from in between the teeth and then with the brush everything is taken away.
07:28Yeah.
07:29You spit out and don't rinse it.
07:31Just leave the paste on.
07:32Oh, right.
07:33Okay.
07:34So that is the best way to make sure that the toothpaste with the fluoride is kind of strengthening your teeth against the bacteria that is causing detain or erosion as well, you know.
07:47Because the fluoride makes the teeth stronger against erosion attacks.
07:51So that is what is the deal.
07:54Oh.
07:55Yeah.
07:56Oh, thank you for that advice.
07:57Yeah, that's good.
07:58Does that white patch look worrying to you?
08:00Not at this stage.
08:01Like sometimes we see what, like we see many of white patches in mouth.
08:06And for example, a person like you, that you don't have, you're a smoker and you don't drink alcohol.
08:11So the risk, it will be reduced.
08:13But it's always a good idea to monitor.
08:16So.
08:17Could it go away?
08:18Do you think?
08:19Usually they don't go away.
08:20It's just like, yeah.
08:21But I mean, if it's something, like what we have to know is pretty much what it's because if it's something traumatic, if you're always catching the tongue in the same spot, probably your bite.
08:33And something that I was looking at the bite, I was trying to tell you is that you have what we call an anterior open bite.
08:42That means that your front teeth are not touching at all.
08:45Oh, no, I'm back.
08:46Yeah.
08:47Yeah.
08:48So this is making an unbalanced bite and that can affect your joint.
08:53So you will definitely benefit from spreading and lining your teeth.
08:57So if you want to have a consultation with an orthodontist, I can always give it a referral.
09:04I believe that will be very beneficial for you.
09:06He says you can opt between braces or Invisalign to stretching your teeth.
09:11So it's much better than before.
09:13It's a good idea.
09:14Yeah.
09:15And so many people think that this is something for teenagers or kids.
09:18Yeah.
09:19And definitely not.
09:20So there is no limitation in age.
09:22Is that your eye?
09:23Is that your eye?
09:24Is that your eye?
09:25Is that your skull changes anyway?
09:26Or your jawline changes?
09:27Yeah.
09:28Yeah.
09:29So like for example, my mom, she's 65 and she just finished a lining treatment because she was breaking
09:37her teeth in the way that the bite was changing.
09:39So she started to get some damages forces in between her teeth.
09:43So she saw her orthodontist and now everything is much better.
09:47All right.
09:48Here we go.
09:49Quite open for me.
09:50Halfway.
09:51Close halfway.
09:52Look at this.
09:53Excellent.
09:54Very good.
09:55Very good.
09:56Very good.
09:57Very good.
09:58Very good.
09:59Very good.
10:00Very good.
10:01Very good.
10:02Very good.
10:03Very good.
10:04Very good.
10:05Very good.
10:06Very good.
10:07Very good.
10:08Very good.
10:09Very good.
10:10Very good.
10:11Very good.
10:12Very good.
10:13Very good.
10:14Very good.
10:15Very good.
10:16Very good.
10:17Very good.
10:18Very good.
10:19Very good.
10:20Very good.
10:21Very good.
10:22Very good.
10:23Very good.
10:24Very good.
10:25Very good.
10:26Very good.
10:27Very good.
10:28Very good.
10:29Very good.
10:30Very good.
10:31Very good.
10:32Very good.
10:33Very good.
10:34Very good.
10:35Very good.
10:36Very good.
10:37Now, I'm going to explode.
11:06I'm going to go around the floor to take care of the irritants in the teeth.
11:24I know, it's hard. Yeah, that's sort of changed too. Like, it's hard to get a floss through the teeth for some reason, Mum.
11:32Yeah, that's probably because of your bite.
11:36And you know what? Your teeth are getting the erosion on the top.
11:41They're getting very sharp corners that they cut the floss.
11:58You can have a little picture, please.
12:06You're making weapons for the fire nation.
12:11You're making weapons for the fire nation.
12:15You're making weapons for the fire nation.
12:20You're making weapons for the fire nation.
12:24You're making weapons for the fire nation.
12:33You're making weapons for the fire nation.
12:35You're making weapons for the fire nation.
12:37Now!
12:38You're making Otherwise, they'reター!
12:40You're making weapons for the fire nation.
12:44One more?
13:06Yep.
13:14So now we're going to do the turvish.
13:21This is now the least uncomfortable.
13:24Yeah, this is the easiest one.
13:27Yeah.
13:29Can you close it?
13:33Yeah.
13:48Okay.
14:11Okay.
14:15Okay.
14:22Okay.
14:24Okay.
14:26Okay.
14:29Okay.
14:34Okay.
14:36Okay.
14:45Okay.
14:46It's a slow opening there.
15:16So now I'm going to slice away the tape, so I'll open it this way.
15:39You can take another good rinse in there and then we do the fluoride for things for you.
16:01Oh, thank you.
16:02Well, if you were going to take an x-ray, would you do it now, like at this point?
16:07Yeah, so it would be really good to take the full mouth x-ray.
16:11So if, I mean, do you think they look like you won't find anything horrible in the spring?
16:19I don't know, I'm not a wish.
16:21But look, it's always a good idea.
16:23I think you need the x-ray for multiple purposes.
16:26You need to make sure that your body is in a good state.
16:30You see, obviously, you have any decay between the teeth that we cannot see with the naked eye.
16:35And also, that full mouth x-ray will help to analyze your structure for, like, an orthodontic treatment.
16:44And therefore, you will benefit from it.
16:46Because since all your teeth are getting worn down from your body, from your ocean, in the position of your teeth, definitely the first, one of the main important steps would be realigning them to a better position.
17:00So, of course...
17:01Oh, before you do any reconstruction of it.
17:03Yeah.
17:04So, are you saying that even if there is a little bit of decay, you think it's more important straight to get in the line?
17:09No.
17:10So, dental treatment should be by phases.
17:12So, phase number one always is to control if you have any active decay or any active disease.
17:17Right.
17:18So, with that x-ray, we can analyze that one.
17:21You have some things that I have recommended you to replace and to resolve.
17:25So, all those things would be ideally done before you start any orthodontic treatment.
17:31But, for example, having the consultation with the orthodontist is always a good idea.
17:35So, you can know what is, what would be your best option, what is your plan, what you want to do.
17:40If you want to go ahead with braces, or if you are eligible for clear aligners, those type of things are always going to be able to roll out as early as possible.
17:49Ah, right.
17:50But, definitely, before we start the treatment, having all the things done, like, feelings and decay, are important.
17:56Well, I might just hold off on the x-ray for now until we can sort of brace ourselves to get more work done on the teeth first.
18:05Yeah.
18:06Right.
18:07And where would be the orthodontics?
18:09So, usually refer to the guys, I think they have different offices around both of us.
18:15Yeah.
18:16So, like, is there one sort of choice, is there?
18:18I have to double check the addresses, but yeah, yeah, definitely.
18:21Like, I think they have in Robina, they have in different spots.
18:24Okay, yeah.
18:25Yeah.
18:26I might, um, so I might just hold off on that.
18:29Yeah, but don't be worried, just let us know.
18:31Is this the point that you know that you should do it?
18:33I should do it.
18:34And then I know there's feelings that will need changing and all that.
18:37Yeah.
18:38And ideally, like, for example, as we did in this part, that we changed the feeling in this part, which is the green.
18:44Yeah.
18:45We have the x-ray of that area, so we know how deep we can go.
18:49So, for the other parts, it would be ideal to have x-ray beforehand as well.
18:52Oh, before you change the leaves.
18:54Yeah.
18:55So, are you saying that I do already have the leaves that would be an idea to change?
18:58Yeah, yeah, for sure, because they are worn down, the edges are kind of, kind of debonded
19:03from the actual margin, so these, when we see, feelings that these are deteriorated,
19:09they are effective, they are immediately replaced.
19:12Yeah.
19:13Yeah, yeah.
19:14So, this one is a fluoride treatment.
19:15Yeah, yeah.
19:16So, we're going to put this one inside your mouth, we're going to leave it in there for
19:20a couple of minutes.
19:21Yeah.
19:22And then you'll speak everything out without rinsing your mouth.
19:25Can you point out, is it possible to get a close-up of this white patch?
19:29Because my hubby's a doctor as well.
19:31Yeah.
19:32And I'll get him to...
19:33Yeah, for sure.
19:34He, I mean, he's not a dentist, he's an obstetrician, but he's still medically trained.
19:38Yeah, yeah, for sure.
19:39He might still be done without rinsing.
19:40Yeah, yeah.
19:41We will take a photo and give it to you.
19:42Oh, a photo, that would be great.
19:43Yeah.
19:44Yeah, yeah.
19:45So, you're sure it doesn't look alarming at the moment?
19:46Yeah.
19:47Not at the moment.
19:48Yeah.
19:49Because that's not a lot.
19:50No, it's...
19:51I will take a photo and that is the idea to check any changes in the future.
19:55Right?
19:56Right.
19:57So, I'm going to put this one inside your mouth.
19:58Right open for me.
19:59Excellent.
20:00Keep it in there.
20:01Okay.
20:02Okay.
20:03Okay.
20:04Okay.
20:05We set the...
20:07Oh, yeah.
20:08Yeah, open.
20:09Now we do...
20:10All right.
20:11That's right.
20:12So, this one, what it does, is it?
20:14Yeah.
20:15Okay.
20:16Okay.
20:17So, this one, what it does, it will make the surface of your teeth more resistant against
20:23acid attack and bacteria activity.
20:28So, that's why we do it every six months.
20:30Yeah.
20:31So, you have some tissues here.
20:33Okay.
20:34You want to take it out.
20:37And then you're going to move that gear around the teeth like if it was a mouthwash.
20:42Oh, yeah.
20:43That's too bad.
20:44Give it the four minutes and then you can spit the excess into this cup.
20:47Right?
20:48Okay.
20:49Because cancer, oral cancer, that is one of my massive fears.
20:53And I'm always, I'm always grateful when it is because my gums look fine.
20:57Yeah.
20:58But now, if I've got one starting to fester on my tongue.
21:00Yeah.
21:01But look, it's, the tongue usually gets a lot of stuff because it's always in the middle
21:04of grinding and things.
21:05I know.
21:06I know.
21:07I know.
21:08Do you have anyone in your family that has suffered from cancer or anything like this?
21:17No.
21:18I mean, my granddad died from leukemia, but that's not related to gum, you know.
21:24All right, you can spit down in there.
21:25And I'm going to take the photo.
21:26But that, that shows why it's good to have regular visits to the dentist.
21:27Exactly right, yeah.
21:28If you did have one that was...
21:29Yeah.
21:30So lay back for me because you should...
21:31I mean, I do have one, but you're saying that...
21:32You should have asymptomatic, you know, that's the thing.
21:33Yeah.
21:34So...
21:35Quite open for me.
21:36Poke your tongue out.
21:37It looks very, like, it's just something very superficial in there that probably it's
21:39just that you cut your tongue in there while biting.
21:40So it doesn't look concerning.
21:41Yeah.
21:42Yeah.
21:43Yeah.
21:44Yeah.
21:45Yeah.
21:46Yeah.
21:47Yeah.
21:48Yeah.
21:49Yeah.
21:50Yeah.
21:51Yeah.
21:52Yeah.
21:53Yeah.
21:54Yeah.
21:55Yeah.
21:56Yeah.
21:57Yeah.
21:58Yeah.
21:59Yeah.
22:00Yeah.
22:01It doesn't look concerning, really.
22:02It doesn't look concerning yet.
22:03It doesn't look concerning yet.
22:04It does not.
22:05All right.
22:06All done.
22:07Thank you so very much.
22:08Yeah.
22:09I really appreciate that.
22:10So our next steps then are full mouth x-ray, work out what else needs doing in my...
22:17Yeah.
22:18Because I know very well that once I have the x-ray I'll get all the news of everything that
22:23needs doing.
22:24So I think I need to get my head around the fact that I am going to need work.
22:28Yeah.
22:29Then I'll get all the news at once type of thing.
22:30Right.
22:31Whenever you're ready.
22:32Yeah.
22:33I don't know.
22:34I don't want to push yourself out yet.
22:35Like whenever you're ready then start.
22:36We'll take that x-ray.
22:37And then um...
22:38And then once all that work's done I'll go to the...
22:40Because I look at...
22:41Dentists have been telling that though for years about my bite.
22:45Like...
22:46Yeah.
22:47And I've always had a loose joint.
22:48Mm-hmm.
22:49So yes.
22:50What I can see now on your teeth is you know like...
22:53When you see the...
22:54The top of your teeth when you open you have this thing that you have like some little dishes
22:58like this.
22:59So this is called by motion and that is kind of removing your teeth away.
23:04I know.
23:05And then at the same time they are biting quite strongly because they don't distribute any
23:10loads to the front.
23:11So they are exposed to more wear.
23:15Thank you so very much.
23:16Take care.
23:17I really appreciate it.
23:18Thanks.
23:19Bye bye.
23:20Not like I've got any beds or vases on or anything.
23:21No.
23:23Take care.
23:24Thank you so very much.
23:29Have a good day.
23:30Bye bye.
23:31I'm excited.
23:38So there are two really important takeaways from what just happened in there.
23:43The first one is the importance of regular dental visits.
23:48Like I never would have known about that white patch that's apparently now sort of developed
23:53on my tongue.
23:54No symptoms.
23:55No pain from it.
23:56I did realize that I was biting down a bit more on my tongue.
24:01And that is actually a really common issue.
24:03Bites can change.
24:04I have had dentists in the past talk to me about this alignment.
24:08And I've always had, ever since I was young, I've always had a bit of a loose jaw.
24:12I'm not sure one dentist did have a term for it.
24:16But so I do know about this issue.
24:18So I do think it is probably about time I did get something done about it because that
24:24is true.
24:25Regular trauma in your mouth can lead to something really nasty as in oral cancer.
24:30And that is obviously cancer is a big fear for everybody.
24:34So it's always good to see any warning signs of something that might develop into something
24:39not so good later.
24:40So again, if I hadn't have come here today, I never would have known about it.
24:45So go and see your dentist regularly.
24:48That developed in just six months.
24:50So I was here just six months ago.
24:52And dentists do say that once every six months to keep on top of anything that's developing.
24:57Second really important takeaway is like he said, you're never too old to get your teeth
25:03realigned.
25:04And in fact, when you're older is probably when you're going to need your teeth realigned.
25:10Like he said, his mother who's 65 now needs her bite realigned.
25:13And that is actually a very common problem that your bite does change.
25:18So you don't have the one bite in your twenties and that lasts forever.
25:22Not the case.
25:23Nothing stays the same.
25:25So look, that was positive though to get that.
25:28So I do understand that before you get your teeth realigned, you probably do want to just
25:33fix up any issue.
25:34So I will get that x-ray and I will find out what else needs doing.
25:39And I will do it and I will then go and get my teeth realigned.
25:44So like I said at the end of last video, lucky I've got a dental playlist because the journey
25:51I think will just continue on and on and on forever.
