00:01I went for a regular dental cleaning they took a picture they saw pink which
00:06I've had different doctors tell me was the nerve coming through they found an
00:09x-ray and then they saw all this gray stuff so that's where the whole process
00:12started of seeing different doctors and discovering that I needed to get my
00:15tooth removed one of the biggest things when patients come in to see me is they
00:21have a big fear that they're gonna be walking around without their front tooth
00:25as long as you're working with me you're never gonna go without a front
00:29tooth hi my name is Megan I'm a line producer at refinery 29 today I have to
00:40get my front tooth extracted and I'm going to tell you if it's worth the time the
00:45money the pain and what I recommend this to someone who has to go through the
00:48same thing it's my left front tooth I remember a time when I was nine years old
00:56and I was at a park and I ran into another kid's head and I hit my mouth all
01:00the doctors seem to be in agreement that it could have been that impact something
01:04about the roots and how they get established and then it could have just
01:07been slowly deteriorating since then so I found these doctors who have a
01:11different solution that allows me to have a tooth that doesn't come in and out of
01:15my mouth at all I think success looks like people can't tell that it's a
01:19different tooth when the new tooth is put in my OB and all the dentists said you need to
01:23handle this before getting pregnant to prevent a situation where you wouldn't
01:27have a front tooth the gums would collapse or you would have to have a
01:30surgery while you're pregnant Dr. Andy and Dr. Simon are just the best they are
01:35so caring compassionate and gentle they understand the importance of like having
01:39a tooth in the front of your mouth and why that would matter and why you'd be
01:42embarrassed to take a tooth out every time you eat for up to a year I'm hoping
01:46that other people know there are doctors with other techniques they don't have to
01:50have an Essex or a flipper they can find a doctor who does the ACE technique you
01:54just might have to shop around and interview a bunch of different doctors
01:57to find a doctor that's willing to do a different technique I'm Dr. Zeve Simon
02:03I'm a periodontist and implant specialist practicing in Beverly Hills California
02:07I'm a dental surgical specialist I specialize in replacing broken teeth damaged
02:13teeth with dental implants I also treat gum disease different types of oral cancer
02:18and oral medicine conditions in the mouth I've been in a practice in Beverly Hills
02:22for about 25 years our practice has been in its existence for close to 45 years
02:28extracting a tooth in the front which we call the aesthetic zone is probably one
02:33of the most complex things in dentistry why because it's the front of your face
02:38that's your smile that's how you present yourself to the world and having a
02:42front tooth missing not only affects your function but affects your confidence the
02:48other thing is more biologic Megan's tooth came with a condition called
02:52resorption when we lose a tooth the gum and the bones start to shrink and collapse
02:58because the tooth is no longer there and what that creates is basically a big
03:03defect that's going to complicate things later on we will extract the tooth in a
03:08way that will preserve the soft tissue in the bone if you have surgical
03:12intervention or any type of infection in the mouth we need to take medications
03:17that might affect IVF so we are timing the procedure before the IVF and then Megan
03:23will be on a holding pattern until she conceives until she has her baby and then
03:28she'll get a tooth after that and Dr. Andy will place an immediate provisional so
03:35Megan she can have still a confident smile without the tooth while we're working on
03:39replacing it I'm Dr. Andy Jean Miro and I'm a cosmetically driven dentist located in
03:45Beverly Hills California my job would be to photograph the teeth to replicate the
03:53details that we're seeing in the natural tooth into the final implant
03:57restoration one of the most common things in dentistry that we use to replace a
04:03tooth is something called a flipper now for a certain clientele that speaks for a
04:09living or is in front of other people having something that's removable for
04:13me really isn't an option for Megan's case we were able to make something
04:18called a Maryland bridge basically a fake tooth that attaches to the two
04:24adjacent teeth for stability it's fixed in place it doesn't come out and this
04:29allows our patients to heal and have something that's really final for the time
04:34that they're healing in the temporary phase
04:39once we did the simulation we 3d print a surgical guide and that's placed in
04:45Megan's mouth and through that guide we are extracting the tooth and also placing
04:50the implant so it replicates what we did in the software in the patient's mouth
04:55and that gives us the most precise solution I'm gonna take Valium and
05:00laughing gas so I'm gonna be a little bit relaxed while this is happening and
05:05I'm a little nervous to see when I'm done with the day and I'm realizing the
05:08implant did or did not happen because that means that I have to have a dental
05:12surgery once I've had a baby that is stressful to me the first thing we do is
05:16called the timeout protocol part of this protocol is to mark the tooth to avoid
05:22any medical errors and then we start local anesthesia and we wait a few
05:26minutes to make sure that the anesthetic takes effect and then we start the
05:31procedure the first step is to create a few small sections in the tooth to
05:41separate the crown part and the root part from the part that is facing the gum
05:46we leave something called the anti-collapse extraction segment and that's
05:50gonna fool the body in thinking that the tooth is still there while we remove
05:53about 95% of it the next step we're going to place the implant through that
05:58surgical guide an implant is an artificial root and it's basically a tiny little
06:02screw made out of titanium it fits inside the jaw bone and once the bone
06:07heals around it then the dentist can build a tooth on top of it we'll mix blood
06:12with a bone graft that's going to create sticky bone and we'll take this sticky
06:16bone and we're going to place it inside the socket and that's going to create
06:20the foundation for an implant and preserve it we don't want anybody to know that
06:27she has a fake tooth in the front of her smile we wanted to have a tooth that she
06:31can bite with and feel confident with and that's what an implant does her
06:37original tooth was a little shorter so we're actually going to make it the same
06:41length as the adjacent tooth and then on the inside these are the two little
06:47attachments that go on to the adjacent teeth from the kind and allow us to get
06:53more stability of the restoration what really brings it to life is these
06:59amazing technicians bringing it to life with beautiful color so that it matches
07:05the adjacent tooth and doesn't stick out
07:12literally seen this in five minutes her teeth have these white hypo calcifications
07:18and now we have it in the temporary I'm using the blue etch this opens up the
07:24porosities of the tooth I then put some bonding agent getting it ready to place
07:30that final restoration I'm putting some cement on the bridge placing it into
07:36place and then curing it with the curing light the curing light will make the
07:40material hard and now this temporary is fixed in place for the next two and a
07:46half months to three months while she heals and really working to shape that
07:51tissue and make it the most ideal for our final restoration you can hold that up
08:01wow so this is not my actual tooth okay you didn't do that do you see how it looks so
08:07much like that's so crazy it will get better and it will all heal in it just
08:13gonna take a little time I'm gonna give you some instructions so that you have
08:16great healing how to brush your teeth what to do and what not to do and the
08:21best you follow the instructions the faster you heal thank you so much I
08:25really appreciate it my pleasure dr. Andy and dr. Simon I think of them as my
08:30angels these doctors have a plan that preserves my smile and helps me prevent
08:34that embarrassment so success rates with implants are very high there around 93
08:3994% the health of the implant needs to be checked at least once a year you still
08:44need to get your regular cleanings and your checkups with your dentist to make
08:47sure everything is long-lasting our patient is going through some fertility
08:52treatments and trying to get pregnant now that the implant is done she doesn't
08:56even need to be anesthetized anymore when she comes back in three months we
09:02don't have to use anesthesia we basically will take a little impression of the
09:07position of the implant and then the next week she'll have her new front tooth it is
09:12about five weeks post tooth extraction and some things that I've noticed are no
09:18one can tell that I have a fake tooth the only thing that's like a little bit
09:21different is that I eat on the sides like I don't put food directly right here
09:25right now healing process is going great I don't have any sensitivity swelling
09:29anything like that it was a little bit uncomfortable I was probably taking
09:32Tylenol and Advil for a week it took like a stronger than an Advil medicine one
09:36time it was not as painful as I was expecting it to be and I was able to heal a
09:40lot faster I would say it was easier than getting wisdom teeth removed I guess my
09:44lips maybe were like a little bit bruised or swollen for a couple of days
09:47day-to-day the temporary tooth I don't even notice it's there the only thing is
09:51I can kind of feel the cement used on the back of my teeth to hold it and adhere
09:55it to other teeth so I feel that a little bit with my tongue but I have
09:58learned to work around it up close you can kind of see the different artistic
10:02designs that Dr. Andy put into it from the moment I found out I needed to have my
10:06tooth removed until this surgery happened I was really struggling with
10:10sleep and I don't think I realized how stressed I was and then once they told me
10:14they could put the tooth in so really I'm back here Dr. Andy's office I have my
10:18temporary tooth and they're gonna put in the permanent tooth today they're
10:21taking out my temporary tooth they'll unscrew it pull it out and put in the
10:25permanent tooth that I should have the rest of my life fingers crossed I'm
10:28really excited because this is the end of the process and now I'll have a really
10:31strong tooth in my mouth this tooth looks so good that no one has noticed if
10:35anyone knows that I have a fake tooth it's because I said to them check out
10:38this tooth it's not the real tooth I even got family photos done I didn't
10:42think about that twice because it looked to me just like my regular tooth I'm
10:45nervous to be here today because it's not so comfortable to take a tooth out and
10:50put it in Dr. Andy's the best she goes really slow so it's not painful and she
10:54makes it really easy but it's just I'm a little bit nervous the fun part of this
10:58process today we're gonna give Megan her implant crown and she doesn't even need
11:03to be numb for this we essentially are going to unscrew our temporary and we're
11:09gonna screw in this new tooth and she'll go home today does not even have to be
11:14numb I actually put a little bonding on her adjacent tooth because when we were
11:18designing this restoration we want to make sure that the front two teeth are
11:24mirror images of each other as we were starting to design the restoration
11:28I noticed that her other tooth was a little bit smaller in dimension so we
11:32added a little volume so that we can make it a little bit more comparable to
11:37each other now that that permanent tooth is in there are really no
11:40restrictions for Megan she can go live her life bite into food unrestricted and
11:47really be able to enjoy her smile as it was before the gum and the bone around
11:53the tooth really dictate how long this restoration will last and so in Megan's
11:57case I am hopeful that it will last 20 plus years but it is always possible that
12:03we may have to change it in time as her other teeth change around it and that's
12:08not something that is uncommon when we're just doing one tooth all right you
12:13ready ready I love it it fits perfectly it's really smooth so much smoother than
12:22the temporary right yeah it's even just in the back and the front all of it just
12:26feels more like a real tooth and it's like you it's the same length as the tooth
12:31which my other one wasn't it looks great thank you we did it Dr. Andy and Dr. Simon
12:38were the best that I could have ever had they listened to me they advocated for me
12:42they understood the importance to me and they also worked with my timeline of
12:44knowing I wanted to do IVF have a baby but I couldn't have this like potential
12:49tooth problem in pregnancy so I needed to get it done quickly they work with my
12:53different doctors and the different medications I needed so overall an
12:56amazing experience totally would recommend these doctors totally would
12:59recommend advocating for yourself to find a solution that is not a temporary
