UK Justice Secretary David Lammy has revealed that 12 prisoners were wrongfully released over the past three weeks, with two inmates still at large.
The disclosure follows a string of high-profile errors, including the accidental release of Algerian national Brahim Kaddour Cherif, who was later rearrested, and Hadush Gerberslasie Kebatu, captured after CCTV and bodycam-led tracing operations.

00:01I made it clear in Parliament that on a paper-based system there will always be human error and
00:07there has been a human error for all of my lifetime until we move to a completely digital
00:12system.
00:13I think it's impossible to rule out error but there has been a spike, it's on a downward
00:18trajectory.
00:19There have been 12 since I made that statement, at the moment two are currently at large.
00:30I made clear to Parliament when I spoke to them that the trajectory began to increase
00:52in the last four years of the last government because of their new release schemes and the
00:58complexity of that system, a system that we're simplifying with the sentencing bill through
01:03Parliament.
01:04That does mean that every week there are one or two cases.
01:14It places huge demands on our police, particularly as we re-arrest these people.
01:20That's why I've put the systems in place to see the number come down but we would have
01:25to move beyond a paper-based system.
01:27I've asked Lynn Owens to look at this.
01:30She was former director of the National Crime Agency.
01:33She will do that work and we will build on that work so we see that number come down to
01:38more traditional levels.
01:39It's now 17.15 and I'm arresting you on suspicion of sexual assault on a female under 16 years
01:44old so you do not have to say anything but it may harm your defence if you do not mention
01:47when questioned, something which you later rely on in court.
01:49Anything you do say may be given evidence, we're arresting you for a prompting and effective
01:52investigation by being to intervene to you and prevent harm to yourself or others.
