00:00I haven't noticed.
00:02He left again.
00:03I'm good.
00:04Mom, you don't have to worry about me anymore.
00:06Are you guys back together?
00:07We haven't had, like, the official conversation yet,
00:09but, uh, yeah, we are.
00:1130-year-old schizoaffective male complaining of David Chespain's.
00:14Sam, what did you hear just now?
00:16Feels like ten different radio stations on at the same time.
00:18Got you a stay in a temporary housing facility.
00:20Here, we don't need money.
00:22Did Sam show back up for his appointment?
00:24Mm-mm.
00:30Matthew!
00:34Dad!
00:35Please!
00:36Matthew, we need you!
00:38Help us!
00:40Matthew.
00:42Hey.
00:43Where are you?
00:44Where are you?
00:49Dr. Wolfe, always good to see you.
00:52Matthew, how is Bronx Jen's favorite silver fox doing?
00:56Living the dream, my man.
00:58But I've got a bone to pick.
01:01Well, a few, actually.
01:02Two obstructed evacuation routes,
01:04a broken sprinkler deflector,
01:06and I'm pretty sure I saw Nurse Carter
01:08light up a pumpkin spice candle.
01:11Who's the fire marshal on this floor?
01:13I assigned Dr. Wolfe because he never leaves.
01:15I've been given the cute little vest and everything.
01:17Thank you, Nurse Silva.
01:19It's great to see you, Mr. Amati.
01:21Hey, Chris at the station
01:22wants to know why you ghosted him.
01:24Well, you can tell Chris at the station
01:26that I'm working double shifts
01:27and I don't answer you up.
01:29Noted.
01:30Well, you never set me up with a fireman.
01:32Well, take my safety course, fire marshal,
01:35and you can have your pick of a litter.
01:37Well, why don't we get you in
01:39for your monthly checkup first?
01:41How's that sound?
01:42Great.
01:43I know what y'all thinking.
01:45It's Thanksgiving week.
01:46Thorne's buying big.
01:48Let me remind you,
01:49lunch specials only.
01:51I don't make enough to be buying y'all
01:53full-size shaken beef entrees.
01:55Oh.
01:57Luckily for you,
02:00Sam,
02:01I can edit my order.
02:04What's it gonna be?
02:06Find me Sando.
02:07You know, something new.
02:10Hmm, Sam, y'all came around?
02:12Same room.
02:13Oh, I got a live one here.
02:15I need access,
02:16blood pressure,
02:17and airway ready.
02:18Yes, doctor.
02:19And blood.
02:21MCP and trauma now.
02:23Hey, Sam,
02:24talk to me.
02:30What in the hell happened to you?
02:42Mama?
02:47No.
02:48Quick, you have to hide.
02:49Why am I hiding from my own son?
02:51I don't want to confuse him.
02:52We don't know what this is.
02:53Well, I think I know what this is.
02:55Mama?
02:56Are you awake?
02:57I didn't reach the tail.
02:59What?
03:00What?
03:01What?
03:02What?
03:03One sec.
03:04Go.
03:05Can we just talk about this?
03:06I just know.
03:07Hi, honey.
03:08Morning.
03:09Oh, my.
03:09I'm so impressed.
03:10You got that on all by yourself?
03:12Not the tail.
03:13I couldn't reach.
03:14Oh, here.
03:15I'll help you.
03:16Here.
03:17Stick your arm through here.
03:18This looks great.
03:19Okay, turkey.
03:20Let's get you some breakfast.
03:21Come on, Matthew.
03:22Push.
03:23Push.
03:24All the way out.
03:25Three.
03:26Two.
03:27One.
03:28Ah.
03:29I'm so impressed.
03:30You got that on all by yourself?
03:31Not the tail.
03:32I couldn't reach.
03:33Oh, here.
03:34I'll help you.
03:35Here.
03:36Stick your arm through here.
03:37This looks great.
03:38Okay, turkey.
03:39Let's get you some breakfast.
03:40Come on, Matthew.
03:41Push.
03:42Push.
03:43Push.
03:44All the way out.
03:45Three.
03:46Two.
03:47One.
03:48Ah.
03:49Very good.
03:50Okay.
03:51Take a minute.
03:52Catch your breath.
03:53I like your new look.
03:54They call it athleisure.
03:55How we looking, Dr. D?
03:57There appears to be a significant decline since your last visit.
04:00Your lung capacity is at 55%.
04:02This tells us that your respiratory muscles are getting weaker, and we need to talk about
04:08the next steps.
04:09Fine.
04:10I'll quit smoking cigars.
04:11Test me again.
04:12I'll do better.
04:13Hey.
04:14Sorry to interrupt, Wolf.
04:15You have a visitor.
04:17Matthew.
04:18So this is where you hide out.
04:20You stood me up for our last appointment.
04:22It must have slipped my mind.
04:24Hmm.
04:25Must have.
04:26Well, consider this your reminder.
04:27I will see you later.
04:28It's a date.
04:29Okay.
04:30Uh, you two can carry on without me.
04:32I'll be back.
04:34All right, Leonard, we're in.
04:37You got it.
04:38Trigger warning.
04:39Your mom is on the premises, planning the holiday gala.
04:42Apparently, she's still cheering it.
04:43I thought she was in Turks and Caicos.
04:45She was, and now she's here, telling Silva and all the nurses about a masseuse named Baptiste,
04:49too, made her feel like she was 25 again.
04:51Okay.
04:52Tell her Josh fired me, and I moved to a yurt in the desert.
04:54Mm-mm.
04:55Not getting involved.
04:56I got my own mama to deal with.
04:57The entire Pierce posse is coming to stay with me because they don't want me and Maya
05:01to be alone on Thanksgiving.
05:02At this point, I take Morris back at the minute they stay at the hotel.
05:05Well, Carol, for shame.
05:07Wolf, my mom keeps sending me links to better mattresses.
05:10Sounds like she needs a Baptiste in her life.
05:13Hi, Mom.
05:15Hello, my guam.
05:17Oh!
05:18You look tired.
05:20Is she sleeping?
05:21Uh-uh.
05:22Not getting involved.
05:23It's nice seeing you, Muriel.
05:24Okay, these are filthy.
05:25You're always so good about keeping your glasses clean.
05:28Mom, just please.
05:29Okay, okay.
05:30Why don't you take a break and spend Thanksgiving with your mom?
05:35Oh, I can't.
05:36Uh, work, patience, saving lives.
05:38Busy, busy.
05:39I'll make my sweet potato casserole with the mini marshmallows.
05:42Come on.
05:43It's no fun cooking for one, and I miss you.
05:45Okay, well, I'll see what I can do.
05:48Okay.
05:49Hey, can you page me?
05:52Yeah, so Sam came in with sky-high LFTs and hematemesis.
05:56Liver failure?
05:57Yeah.
05:58Wow, he doesn't have any history of liver disease.
06:01Exactly.
06:02Not a drinker, doesn't shoot up, never overdosed.
06:04Is he still bleeding?
06:05Not since the endoscopy, but he is in renal failure.
06:09If this doesn't reverse itself, he's gonna need a liver transplant.
06:17Can I see him?
06:18Yeah.
06:24I know you care about him.
06:26If he needs a transplant, he's gonna need all the help he can get.
06:29Well, we got the same number three times.
06:42The test results aren't changing.
06:44This is where we are.
06:46Matthew, we need to talk about what that means.
06:48I'm not sleeping with some machine to help me breathe.
06:51That is not me.
06:53I am not that guy.
06:54Well, we don't have to make a decision right now,
06:56but the breathing test is telling us that things are moving faster than we'd hoped,
07:00and we need to make a plan before we get into trouble.
07:03Dr. Wolf, when I told you I couldn't use my hands, you said I couldn't live alone.
07:09Fine.
07:10The guys at the station, they got me.
07:13Dennis is downstairs in the cafeteria right now.
07:16He loves the egg salad here.
07:18But that's it.
07:19I'm not having them deal with strapping me in and out of my machine at night.
07:24I gotta draw the line.
07:25Matthew, what about your family? Have you told them yet?
07:27What's it matter?
07:28It matters because Alice isn't something you survive alone.
07:33And sometimes, lying to your loved ones can hurt them more than the truth.
07:37You're talking about my family or yours.
07:40Nobody survives this.
07:44We both know that.
07:47And I know how to take care of my family.
07:51So I guess you heard about the breathing test.
07:58That was a total bust.
08:00You're up against some hard decisions.
08:02I also heard that your family still doesn't know about your diagnosis.
08:07My daughter has one semester left at Amherst.
08:10I am not about to do anything that's gonna distract her.
08:14So you're not gonna see them over the holidays?
08:16They take one look at this.
08:19My ex will take me back out of pity.
08:21My daughter will fail her finals, lose her focus.
08:24So, yes, that means I don't see them.
08:28Didn't you say Thanksgiving was your holiday?
08:30The one you all will do together even after the divorce?
08:33Not this year.
08:35I know who's to say there's a next year.
08:38But if there isn't, then this is the gift I give them.
08:41One more happy moment.
08:43Not that voice banking app you keep recommending to me.
08:48I understand how considering a future where you can't talk might feel intolerable.
08:54I do.
08:55But, Matthew, this is the umbrella, not the rain.
08:58We're giving you more power over this disease, not less.
09:01And I think telling your family might serve a similar purpose.
09:06You know what?
09:09There is something I need to tell them.
09:13Okay, Siri?
09:15Text Alicia.
09:16Okay.
09:17What do you want to say to Alicia?
09:19I won't be home for Thanksgiving this year.
09:22I gotta work.
09:25Sam is in a fulminant hepatic failure.
09:28And we don't know what happened.
09:30Erica, this is...
09:31Yeah, I know.
09:32I'm gonna call the liver transplant team to evaluate.
09:34Hold on, not yet.
09:35What is his neuro exam?
09:37He's slow, but he's awake and alert.
09:39Have you ever treated acute liver failure before?
09:41These are some of the sickest patients you'll ever see.
09:43Your job is to ensure this doesn't become a neurological emergency.
09:46I want you to think of Sam's mental status as a key prognostic sign.
09:49Once that declines, his time is even shorter.
09:51But if he's awake, we can afford to wait a beat before calling transplant.
09:54With this team, we have one shot.
09:56If they say no, they will never reassess.
09:59So what do we do?
10:01I have an idea.
10:04He's just not a good candidate.
10:07Look, I know Sam's young, and you all care about him and have developed a relationship with him during his many visits to RER.
10:12But that committee will see an unhoused man with unstable psych disease and zero family support.
10:17Someone who won't take the meds that will keep him from rejecting his new liver.
10:20I know what the chart says, but that's not the whole story.
10:22I had just started seeing Sam.
10:24He had good pre-morbid function, was a college student when he had his first break, and he wanted to get better.
10:31I was hopeful.
10:32You'll get the chance to talk to the transplant team.
10:34I don't think that'll be enough.
10:35Sam still has no family here, which means there would be no one here to help him take his meds or get him to appointments.
10:40Whether right or wrong, you simply cannot get a transplant if you're entirely alone.
10:43So for Sam, being alone means death?
10:46And I'm supposed to accept that?
10:48Transplant decisions can be capricious and outright biased.
10:51We've all seen candidates rejected because of something that feels unjust, but these concerns are valid.
10:56Patients like Sam get overlooked all the time.
10:59But he wanted to get help.
11:00Dr. Pierce said it. He told us that in his own way each time he came into our ER.
11:06I'm not giving up.
11:07Neither am I.
11:08Okay, we need to talk to Sam.
11:09Figure out if a transplant is even something he would want, and we need to do it now before he loses his mental status.
11:19Wolf, Carol, you know this. Even if this is what he wants, he's just not a good candidate on paper.
11:25Then we make him one.
11:30I can't believe Sam is in lip for failure.
11:36He'll be a hard sell to the transplant committee, but I'll do what I can.
11:40Hey, how is Matthew? I'm worried about him.
11:42His disease is progressing faster than I had hoped, but he's still in denial.
11:46Some aspect of denial is necessary. Adaptive, even.
11:49Not when it's unsustainable.
11:51Matthew's stubborn, and I don't blame wasting what time he has left avoiding the people who are closest to him.
11:56I'm gonna go down to the station and check on him there, see if I can make any progress.
12:00That's a good idea. But I also know you want to see some firemen in action.
12:04Carol, do not objectify New York's bravest. But the 2026 calendar did come out. Remind me to show you Mr. October.
12:12Sam. Hi. Can you hold your hands out like this?
12:28What's going on?
12:33That's called asterixis. It's a sign of liver failure.
12:36Sam, I'm not gonna sugarcoat this. We need to get you a new liver.
12:40But in order to do that, I need to know some things first.
12:44What happened since the last time we saw you here?
12:48I ain't mad about your credit card. I'm sorry.
12:52Hey, I'm not mad. You got yourself some stuff you needed. That's why I gave it to you.
12:58No. I didn't use your card for anything.
13:04The meds helped. But the voices are. I yelled at them.
13:11Then these kids started beating me, hitting me, kicking me.
13:14Why didn't you come here? We could have helped you.
13:17They took you a card. I told you I'd bring it back.
13:27I don't wanna...
13:33I had my meds, though.
13:36Took them every day. Meds for pain, too.
13:40What were you taking for pain?
13:42Red. I took them all.
13:45Acetaminophen. That's what caused the liver failure.
13:49Is there any way you could get in touch with your family?
13:52It would really help if they could be here.
13:55Already dead to them.
13:57I don't wanna die.
14:06I don't wanna die.
14:07Okay, junior firefighters. There are four very important things you need to know when caught in a fire.
14:20Number one, stay low to avoid the smoke.
14:24Number two, to raise the alarm, pull the fire alarm.
14:28Or yell for help.
14:30Number three, exit the building as quickly as possible.
14:33And number four, never, ever hide in a burning building.
14:40So just remember, Fred, floor, raise the alarm, exit, don't hide.
14:52Which reminds me, there is a very special member of our team I think you'd really like to meet.
14:57So I'm gonna bring him out now. You ready?
14:59Yeah!
15:00Okay, Fred, where you at, boy? Come on out.
15:07Come on meet Fred, y'all. Come on.
15:14Where is he? Where is he?
15:17Matthew, we need to talk right now.
15:20Where do you get off not coming for Thanksgiving?
15:23Our little girl's coming home and you're blowing us off?
15:25Leisha, can you not do this here?
15:29Uh, I'm so sorry to interrupt.
15:31And who are you?
15:32I am Dr. Oliver Wolf.
15:36Perhaps we should all talk?
15:45How could this happen?
15:47I cooked healthy meals for the family every night.
15:49I've always had a cruciferous vegetable at the table and this is what we get?
15:53I can assure you that this is not about what you did or did not do.
15:56Alicia, I'm the one who's dying, not you.
15:59No, Matthew, don't say that. No one is dying.
16:02How could you have kept this from us?
16:04Let's all just try to understand the position Matthew is in.
16:08There's no right or wrong way to process this diagnosis.
16:11So what do we do? How do we fix this?
16:13Um, unfortunately we can't fix this.
16:19ALS is a progressive disease.
16:22I did some Googling on the train.
16:24I want a list of all the medicines that he's taking, uh, upcoming appointments.
16:27I read that there are only a few FDA-approved treatments,
16:30so I want him to get pushed into a trial as soon as possible.
16:32We are exploring every trial, and yes, this is a devastating diagnosis.
16:36But I want you all to know that research is accelerating.
16:39There are thousands of people all over the world studying this disease,
16:42and there is real hope for effective treatment on the horizon.
16:46In the meantime, our goal is to help Matthew live the best life possible.
16:50Right. So Matthew is gonna live with me. I'm taking care of him. No questions.
16:54Are you guys done speaking about me like I'm not even in the room?
16:58And there's plenty of resentment there. I'm not moving in with you.
17:01Nobody resents you. And I'm gonna come back home, too.
17:04I only have one semester left of school, so I can go back and finish whenever.
17:07What does that mean, whenever?
17:09Dad, don't be ridiculous. I can't just stay in school.
17:11The hell you can't.
17:13You can't do this on your own. Be for real, Dad. You live in a walk-up.
17:16I live in a fire station, Gabby. I can manage.
17:19Remember, we fight the disease, not the tools we have to help us.
17:23No. I'm not letting anybody uproot their lives from me.
17:27I could fight this on my own. End of story.
17:36Eastgate Medical didn't have any family contact for Sam.
17:39And he is too confused now to be able to tell us anything, even if he wanted to.
17:42Neither did St. Elizabeth's, Queensbridge, or anywhere else Sam's been hospitalized.
17:46I'm striking out here, too.
17:48That's wild how hard it is to get a transplant.
17:50I don't think I'd even qualify. Two people, really?
17:53I'd vouch for you.
17:54I wouldn't let you. You'd have Liam.
17:56How about you, Erica? You think you'd make the list?
17:58Um, I don't know. My parents wouldn't move.
18:00Would you even want one?
18:02Immune suppression and infections the rest of your life, fearing rejection?
18:05If the alternative is death, yeah, I would take it.
18:09The thing is, we would be listed.
18:11Even if we drank or don't have social support, we'd be listed,
18:14because we're the people they make exceptions for.
18:16And that sucks.
18:18Privilege.
18:19Hey.
18:20I think I found the number.
18:24Green beans, uh, sweet potatoes.
18:28Oh, two pie crusts.
18:29Look at this big boy.
18:31As if we needed that much turkey.
18:33Uh-huh.
18:37You kids, you're getting cold for Christmas.
18:39Hey!
18:40Yeah.
18:48Hello?
18:49Hi, I'm calling to be with Claire Mamesbury.
18:51Yes, this is Claire.
18:52Who is it?
18:53This is Dr. Martin Kinney from Bronx General Hospital.
18:55I'm one of the doctors who've been taking care of your son, Sam Mamesbury.
18:58What's going on?
18:59He's in her ICU and very sick with liver failure.
19:03I'd like to call you to update you and answer your question.
19:06Answer what you're doing.
19:07It's Sam.
19:08You know, I don't know what's more shocking,
19:12that Matthew has ALS or that he's seeing a psychiatrist.
19:15Reluctantly seeing a psychiatrist.
19:18Well, that tracks.
19:20If your sessions are anything like our marriage,
19:22you ask him how he's doing,
19:24he mumbles fine, cracks open a beer,
19:27and plops down on the couch to watch a Jets game.
19:30Oh, honey.
19:31If he's a Jets fan, he's never fine.
19:33He does talk.
19:37Sometimes I can't get a word in.
19:40Huh.
19:43I see that strikes a chord.
19:46Can you tell me why?
19:48My sister set us up on a blind date.
19:53I was so pissed at her at first.
19:57She knew I liked the strong, silent type.
20:00And Matthew would not shut up.
20:03We closed the restaurant down, and when he kissed me goodnight,
20:06he whispered,
20:08we will never run out of things to say to each other.
20:13Well, sometimes marriage is all about how you navigate the silences.
20:17How do you navigate an abyss?
20:19Mm.
20:20It wasn't all his fault.
20:21We went through a lot early on.
20:239-11.
20:25We had friends whose husbands didn't come home that day.
20:29So how could I complain?
20:31My husband did.
20:33But he didn't really.
20:43I would look at him and say, Matthew, where are you?
20:48He never let me back in.
20:53It's why we split.
20:55So to hear that he talks to you.
20:58What he practices in this office is what he'll bring to his relationships outside of this room.
21:04Including his relationship with you.
21:06That's all I ever wanted.
21:09Was for him to tell me that he wasn't fine.
21:13Yo, guys.
21:23What are you trying to do?
21:24Burn this place down?
21:25Guys.
21:30Thanks for the patch up.
21:44So how did you fall?
21:46I was being careless.
21:48Matthew, I'm glad that you have support at the fire station.
21:51But I am worried it's not a safe long-term solution for you.
21:55What's long-term here?
21:57I'm not sure I can keep fighting it.
22:04I'm tempted to tell you how resilient you are.
22:08To talk about adaptation.
22:09To tell you it's incredible what people can learn to accept as their new normal.
22:16But none of that's gonna help right now, is it?
22:18I feel like every time I learn to adapt to a new loss, I lose something else.
22:31From where I'm sitting, all of the core elements that make you, Matthew, are still intact.
22:36Then let me keep this one.
22:38I don't care about me.
22:39But my girl Gabby, and even Alicia, I need to protect them.
22:45Sam, there's some people here to see you.
22:51Sam.
22:52Did I ruin your Easter?
22:53It's Thanksgiving.
22:54No way, kid.
22:55This is the only place we wanna be right now.
22:56With you.
22:57Hey, Doc.
22:58Sorry to just surprise you like this.
22:59Oh, uh, you're the chief.
23:00Mr. October, right?
23:01No.
23:02I'm sorry.
23:03I'm sorry.
23:04I'm sorry.
23:05I'm sorry.
23:06I'm sorry.
23:07I just surprised you like this.
23:08hey doc sorry did you surprise you like this oh uh you're the chief mr october right
23:28you can call me dennis what can i do for you dennis uh look we all knew maddie living at
23:34the station was temporary but we thought it was safe i uh i shouldn't have left him alone don't
23:40blame yourself you got him out of his apartment you helped him maintain something that felt
23:44normal for a little while longer that's not nothing yeah maddie's the best we got you know
23:51he trained me my first weekend i thought there's no way i could do this but maddie made me realize
23:56i could he'd always say alone we could do so little but together we could do so much it's a
24:04quote from uh helen mirren i think yeah it's helen keller but easily confused i just wish we could do
24:15more for maddie actually i might know a way you can cow's milk is back right thanks i've been reading
24:25about stem cell clinical trials is that something that my dad could do uh it's a possibility but
24:31ultimately that's for him to decide
24:38you and your dad seem close i mean he talks about you all the time
24:44i've always been into modern art
24:47it's really not his thing but he used to keep a calendar on the fridge of openings happening in the city
24:52and every weekend he'd take me to a new one and when i got a phone he started texting me these random
25:01photos and captioned them art or not
25:04he's one of a kind i can't imagine that what your dad needs is for you to keep going
25:18one good thing about modern art you don't need fine motor skills to enjoy it
25:22keep going together
25:32talk to me sam open your eyes sam sam
25:35what happened he's unresponsive he was talking 30 minutes ago he wouldn't wake up we called you
25:44right away i need a rapid response to room 12 in the icu i'm sorry we're gonna need the room
25:50wait what's happening to it we're taking care of him okay sam sam
25:57puke was equal and reactive but he's uptunded and he's dissatting he can't protect his airway
26:01severe hepatic encephalopathy likely cerebral edema
26:04erica prep an airway now i'm ready for etomidate and sucks
26:08you told me that my job was to keep this from becoming a neurological emergency is there
26:12anything i could have done differently you were monitoring him and you gave him the only antidote
26:16for acetaminophen overdose but as his liver failed and ammonia accumulated brain swelling became
26:21inevitable which can lead to brain herniation and death what's the immediate treatment hyperventilating
26:25him and dosing him with mannitol and hypertonic saline will lower the icp as a temporary measure
26:30but only a new liver will stop the swelling permanently and we don't have long to get it for him correct
26:41so he's on the list for this new liver not yet unfortunately it's been
26:45difficult because of sam's housing situation the committee is reticent to list sam
26:49if he doesn't have a support system people committed to caring for him after the transplant
26:53well he has that he has us we used to be so close but when he got sick we became the last
27:01people that he wanted those thoughts in his head all that paranoia the voices it turned him against us
27:09he didn't want to stay with us he said it felt like prison i mean i i went to the city once to bring
27:13him home but he wouldn't come he thought we were trying to hurt him told me never to call him again
27:20i had his siblings back home i had to be there for them too doctor i feel guilty and scared for sam
27:27every single day can i just bring him home i'm afraid to transfer to another hospital now particularly one
27:34farther away is it's too risky i know that you have a life back in ohio but if there is any way
27:43that you could be here for sam we can rent a place here whatever sam needs
27:46he's he's our son we let him go once we're not doing it again
28:00you watch all your patients while they sleep or just your favorites no just my favorites
28:15how are you sleeping terribly
28:20keep having this dream wakes me up most nights
28:24i'm uh in my old house with my family and there's a fire and i can i can hear them calling for me
28:40i i try to save them but i can't that sounds terrifying
28:46i wonder whether your weakness and trouble breathing at night may be causing this vivid dream
28:53your oxygen level drops carbon dioxide climbs your REM sleep is in disarray the good news
28:59is there's a solution that bypass machine i mentioned will help you breathe deeper while you sleep
29:07i'll take your machine fight the disease not the tools yeah
29:16you're going to discharge me now now tell me more about this dream first
29:24walk me through it careful what you wish for
29:30please matthew we need you matthew where are they matthew we need you i don't know
29:37i don't know i can't move i can't save them
29:44please come out daddy what if you're not supposed to what if they're supposed to save you
30:14you
30:37okay
30:39Oh, my God.
31:09Look back at all the chances I missed narrowly
31:19But I'll move along, can't keep on looking by a glance
31:29There's a light, there's a light up ahead
31:38The transplant committee said no
31:44What happened?
31:46I told them that Sam had just started a medication regimen that I hoped would work for him
31:52The meds he's been taking since discharge and that he had a supportive family
31:56I also told them that the acetaminophen was an unintentional overdose
32:00And that Sam truly wanted to get better to live
32:03But he was on the margin, Erica
32:05And they had to take a leap
32:07Sometimes they take it
32:08Sometimes they don't
32:09But I thought when we got his family
32:11They would see that he is someone
32:13Someone worth saving
32:14Just because they declined him doesn't mean all programs will
32:17I've already called my friend at Columbia
32:18We'll call Cornell as well
32:19We just need to find a program that isn't so risk averse
32:22But even if they say yes
32:23What are the chances they decide in time?
32:25Okay, how long does Sam have?
32:27He needs a new liver faster
32:28He won't make it
32:29For now, we manage Sam's ICP as best we can
32:32And we hold out hope
32:32Mm-hmm
32:33Uh, hey, Silva
32:40Um, where are you going with that pie?
32:45Uh, bringing it to our staff at the potluck
32:47I know this is weird
32:49But I could actually use it for a patient
32:50If that's okay
32:51Okay
32:53Yeah, anything for your patients
32:55Thank you
32:56Um, I finally convinced Matthew to attend his family's Thanksgiving
33:02He asked me to go with him
33:03So I can't show up empty-handed
33:10Is it too late to bail?
33:14Uh, yeah, it sure is
33:16Besides, I went to a lot of trouble to get that pie
33:19I want to eat it
33:19Why don't we just run down the street to the pub and grab a whiskey?
33:24Okay
33:24Dr. Wall, please have a seat
33:27Thanks
33:28I just want to say how much it means to me to have all of you around the table
33:35This year, what I'm most grateful for is second chances
33:40Dr. Wolf, would you like to go next?
33:47Uh, I guess I'm coming to appreciate unexpected reunions
33:53Hmm
33:54Matthew
33:58Yeah, um
34:00I'm grateful for my family
34:05And that, uh, I haven't been a burden on them
34:09Matthew, please
34:10I can't live in my apartment anymore
34:14And I can't live at the station
34:16It's too much
34:17So
34:19I made arrangements with a facility
34:25A home
34:26A place I can move to
34:28I'll be safe
34:29You know, for as long as I can tolerate it
34:32But
34:32My ship
34:35Is sinking
34:37And I'm not taking any of you with me
34:41Dad, we want to help
34:42That's not your job, honey
34:43It's too much
34:44It's not, though
34:45It's not
34:46Because we wouldn't be doing it alone
34:48What do you mean?
34:49We all agree that living at the station isn't a solution
34:53But the guys
34:55They want to help out as much as they can
34:58Matthew
34:59I want you to move back into the house
35:02I won't be doing it alone
35:03And neither will you
35:04And don't even try to argue
35:06Dennis has planned a month's worth of frozen meals
35:10He's one full Martha Stewart, okay?
35:13I'm Betty Ann
35:13You have a lot of good people around you
35:15Who want to help out as much as they can
35:17Take a look
35:21Yeah, Matthew
35:23Fantastic
35:25Yeah, you're awesome
35:26Yeah
35:28Appreciate you, Matt
35:31Wait
35:31Who are all these people, Dad?
35:34People from work
35:36And lives he saved
35:37Sometimes letting people help
35:41Is the greatest gift you can give them
35:43Thank you, Matthew
35:45Thank you for bringing all this stuff
35:54Yeah, thanks, Auntie
35:55Everything looks great
35:56I'm sorry I had to work on Thanksgiving
35:58No worries
35:59We got you
36:00Always
36:01Thank you
36:02Okay
36:02Hey, hey, hey
36:04Okay
36:05Word on the floor
36:06Your sweet potato pie is to die for
36:08It sure is
36:09It's an old family recipe
36:10Anthony, this is my sister
36:15My sister Helen
36:15Anthony runs our emergency room
36:17Lovely to meet you
36:19Any plans for the holidays
36:20Or is it all gunshot wounds
36:22And severed limbs?
36:24Mostly cuts and burns, actually
36:26But I'm headed out
36:27Kids are with my ex
36:28So it's going to be a quiet one
36:30But you know what I am thankful for this year?
36:33An entire Thanksgiving in the ED
36:34Not one patient spoiled the Cowboys game
36:36The Cowboys?
36:39Oh, no
36:40And I was just starting to like you
36:41Look, ladies, look
36:42I'm not that bad
36:43Tell her
36:44I'm not that bad
36:45Oh, my God
36:47Who made this?
36:49I told my staff
36:50Best dish gets Christmas off
36:52You realize this is our first Thanksgiving as a family?
36:56I wish I could have made it special
36:58This is special
37:00We're together
37:02Matthew?
37:23Hey
37:24Got you a little something
37:27If you want to take a break from athleisure
37:29Hey
37:39Where are you?
37:45I'm right here
37:46The act of rescue is often linked to things like heroism and bravery
38:05But for guys like me
38:10I've always looked at it as an act of caring
38:13I don't get it
38:16Dad, it's art
38:18If you say so
38:21I guess I never saw myself as the one who needed to be cared for
38:30But I know now that even on my hardest of days
38:52You and your mom will be there to save me from any fires that may come our way
38:59Mom
39:00Oh, Oliver, it's too soon
39:02I wanted to surprise you
39:04I brought Thanksgiving to you
39:07Ta-da
39:08I saw your ferns, Oliver
39:23I've been so busy at work
39:27I know
39:27So, you know, I just
39:28I took some of them upstairs to the bathroom
39:30I ran the shower for a while
39:31The steam should perk them right out
39:32They're dead
39:33They'll be better tomorrow
39:34You'll see
39:35And now
39:36We are going to get this big boy out
39:40Woo!
39:41You know
39:42It was always his job to carve the turkey
39:45And our friends would applaud
39:46Because he made it look so easy
39:48Ugh, he used to drive me crazy
39:50I do everything else
39:52Mom
39:52Sometimes the most heroic thing you can do isn't running into the burning building
40:08It's quieter, smaller, but in a way even harder, at least for me
40:16Letting you and your mother in
40:22Accepting help
40:24Agreeing to this voice begging business Dr. Pierce has been talking about
40:29Allowing myself to be rescued
40:32Even when I don't want to
40:34For me, this is the bravest thing I've ever done
40:38And it's scary as hell
40:43But it is worth it
40:51If it means more time with you
41:13Greg, we'll get ahead
41:40You
41:42You
41:44You
41:46You
41:48You
