00:00Chronic liver disease are one of the leading cause of major health problems worldwide.
00:06Very often, skin manifestations are the first sign of liver disease.
00:11Few skin lesions are typically seen in liver disease, but some skin changes are general and can be seen in many other diseases.
00:20So let's discuss the signs one by one.
00:24Yellowish discoloration of skin is a typical finding of liver disease.
00:28It is evident when the serum level of bilirubin exceeds 2.5 or 3 mg per dl.
00:36The color of the skin varies from yellow to brown, depending on the amount of bilirubin in your blood.
00:43The white portion of the eyes also turns yellow.
00:47This skin changes is due to excess accumulation of bilirubin under the skin.
00:54Next important skin manifestation is spider angioma.
00:58You may see web-like clusters of blood vessels under your skin, which is called spider angioma or spider tailingectisia.
01:05The lesion contains a central red spot and a reddish extension, which radiate outward like a spider's web.
01:16They may appear as multiple or solitary lesions and it is painless.
01:21But remember, it can be seen in many other conditions like thyrotoxicosis, rheumatoid arthritis and pregnancy.
01:29However, multiple spider angiomas are characteristic of chronic liver disease with a specificity of 95%.
01:37Palmar erythema, also called liver palms, is a condition where your palms, dot sum of hands, finger tips and nail bed becomes red.
01:48Similar changes can also be seen over soles.
01:52This erythema or redness blunches on the pressure and flushes synchronously with pulse rate.
01:59You may also experience throbbing and tingling sensation.
02:03Next important skin sign is paper money skin.
02:07Patients with cirrhosis have many randomly scattered thin superficial capillaries over the upper trunk in association with spider angiomas.
02:18This resembles the silk thread in American dollar bills and hence is the name dollar paper marking or paper money skin.
02:27Sometimes, you may notice painless solen veins around your belly button.
02:34It is commonly seen in liver cirrhosis.
02:36Scarring in liver cirrhosis makes it harder for the blood to flow through the veins of your liver.
02:43Which leads to backup of blood in your portal vein.
02:47The increased blood in your portal vein then leads to portal hypertension.
02:53With nowhere else to go, some of the blood tries to flow through the nearby veins around the belly button called the peri-umbilical veins.
03:01Which can be seen through the skin.
03:05Next important thing is easy bruising or bleeding.
03:09The liver cells are involved in synthesis of most of blood coagulation factors such as fibrinogen,
03:15pothrombin, pothrombin, factor 5, 7, 9, 10, 11, 12 as well as protein C and protein S.
03:25Those factors help in blood clotting.
03:28In chronic liver disease, there is decreased production of coagulation factors.
03:33As a result, you can experience easy bruising or bleeding.
03:37The bleeding can occur from your nose or gingiva.
03:41You may see tiny red spots on your skin.
03:44But again there are many other causes which can cause bleeding from your nose and gum.
03:51Sometimes you may notice small irregular shaped hypopigmented patches over arms and legs.
03:59These are considered to be due to venestasis and are known as bear spot.
04:05Their characteristic feature is that they disappear when pressure is applied or when
04:10the affected limb is raised from the dependent position.
04:16Next is Genthalisma.
04:18Genthalisma is a harmless yellow growth that appears on or by the corners of your eyelids
04:24next to your nose.
04:26It is due to cholesterol deposition under your skin.
04:30Having Genthalisma could be a sign of liver disease but it is associated with other conditions
04:36such as diabetes, hyperlipidemia and thyroid problems.
04:43Nail clubbing is also an important manifestation of chronic liver disease or cirrhosis.
04:49Nail clubbing occurs when the tips of the fingers enlarge and the nails curved around the fingertips.
04:57It is also seen in many other conditions like lung disease, heart disease and inflammatory
05:02bowel disease.
05:03Another important sign is Terry's nails, in which most of the nails appear white except
05:09for a narrow pink band at the tip.
05:12It can be a sign of serious underlying conditions such as liver disease, congestive heart failure,
05:18kidney failure or diabetes.
05:22Now one more thing I would like to mention here is liver spouts or age spouts.
05:28In other conditions you may find small flat dark areas on the skin.
05:33They vary in size and usually appear on areas exposed to the sun such as face, hands, shoulders
05:40and arms.
05:42Previously it was believed to be caused by liver problems.
05:47And that's why the name liver spouts.
05:50But they are physiologically unrelated to the liver.
05:54Age spouts or liver spouts are actually caused by overactive pigment cells.
05:59Ultraviolet light speeds up the production of melanin.
06:03And it is common over the age of 50.
06:07Many people are currently suffering from fatty liver disease.
