00:00Hey friends, I'm Stacey and this is Gorgeously Aging and today I'm going to be doing a much
00:09needed temple filler using Tesoro SubQ with lidocaine. So you can see especially when I
00:14turn to the sides that since I've my weight loss my temples have gotten quite hollow. This is
00:20normal as we age but with extreme weight loss that can be more pronounced. Filling some of
00:26that temporal hollow can give a more youthful look and smooth the contrast between the zygoma,
00:33the cheekbone, the lateral cheekbone and the temporal hollow. There are definitely some
00:38safety concerns that need to be considered. This is considered more of a high risk area to inject.
00:44Before my weight loss I was managing the volume deficit in this area using threads which is a
00:49much safer option. If you have very mild to moderate deficit in volume in the temples you may want
00:56to consider some PDO threads, screw threads or triple threads work really well, twin threads,
01:02anything that will build collagen and fill some of that lost volume in that area can help for a really
01:08long time. Now for me I probably would keep doing threads but my deficit of volume in the temples is
01:15now more than really what threads can handle. I would describe mine as moderate to severe temporal
01:20hollowing. There's a pretty good dip here and I'm starting to get that dreaded peanut head is what
01:26they call it when the temples sink in and then the zygomatic sticks out. You kind of have that
01:31concavity here and that fullness here that creates kind of that peanut shape. So we're going to go
01:36through the steps. I've already washed my face thoroughly. I'm going to alcohol swab and then I'm
01:41going to apply some topical lidocaine. This isn't always necessary depending on your pain threshold
01:47but it does make it more comfortable. We want to make sure to get all the way from kind of the arch of
01:52brow up here down to where that upper cheekbone is and just get that all cleansed on both sides
02:00and then I'm going to use a 10% lidocaine. Everything I'm showing you is from acecosm.com
02:06discount code STACY10. Now of course this isn't medical advice or instruction. There are dangers in
02:14doing your own filler on your face no matter where you're injecting it but the temporal area is a little
02:19bit more complicated. Having said that anyone can learn anything and as long as you're willing to
02:25really study and learn and pay close attention to tutorials you definitely can learn how to do this.
02:34So I'm going to apply a liberal coating of this 10% lidocaine. There are other strengths of lidocaine
02:40available on Amazon that are weaker. I think over the counter you can get 5% but you can't get 10%
02:47through Amazon or through over-the-counter retailers.
02:52So we are going to be going over safe zones and how to map safely for a temple filler. I'm going to
02:59talk a little bit about the anatomy and some safety protocol. First I'm going to let the lidocaine sit
03:03on for about 15 to 20 minutes. For those who are sensitive you can go up to 30 minutes but definitely
03:09don't let the lidocaine sit on for a prolonged period of time. While the typical lidocaine is sitting
03:14on is a good time to prepare your syringes. I like to lay down a sheet of plastic and get my syringes
03:20ready. Now for the temples you're going to want about a half of a syringe per side or a full syringe
03:27per side. Because I have a lot of hollowing I may need both of these syringes but I'm going to start by
03:33using half a syringe or half of a cc and see how I'm feeling about that before I proceed to doing a whole
03:40cc of filler per side. My guess is that I'm going to need a full syringe per side which is not uncommon
03:46in temple filler. So the topical lidocaine has been on me for about 15 minutes so I'm going to go ahead
03:51and remove it. First I'm going to use a dry cloth paper towel or sterile gauze just to remove the
03:58lidocaine and then we're going to go ahead and use some alcohol just to ensure all of the topical
04:05lidocaine is removed and the skin is sanitized. Make sure your hands are washed and thoroughly
04:12sanitized before starting. Try to avoid touching anything else while you're working on your face
04:17including your cell phone. Touching other things after your hands are clean and sanitized can lead
04:22to cross-contamination and you can put bacteria on your face in your work area which we don't want
04:27to have happen. I'm going to go ahead and put on some gloves but I want to note that it's crucial
04:32to have some kind of dissolver on hand in case of an emergency. Never never do filler with hyaluronic
04:38acid without lipo race. This is an enzyme that dissolves hyaluronic acid filler and it's important
04:45to have in case of a vascular occlusion. Now we're going to take tons of safety precautions to avoid that
04:51but even so even the most confident injectors should have a dissolver on hand in case of an
04:58emergency. If an emergency were to occur I would add two mils of saline into this vial. It's a powder
05:05mix it up and inject into the area where there's an occlusion. I do have a more thorough video on this.
05:12I'll link it in the description below. So now that we've cleaned, numbed, and alcohol swabbed the area
05:17I have my gloves on we're going to do some mapping and discuss safety and anatomy. So what we want to feel
05:22for is the temporal fusion line. That's basically where the frontalis ends and the temple begins.
05:29You can kind of feel a little bump there and if you open your mouth you can sometimes feel it even
05:35better by going...
05:40You can feel that temporal muscle there moving.
05:43So I have a pretty easy time finding my temporal fusion line. It's right here. So we're going to take an
05:50expo marker and I'm just going to show you how we determine our safe zone. Now the next point we're
06:00going to find is our zygoma. So the top of the cheekbone and we're going to mark that
06:07and then this I'm just going to also mark.
06:15So this is the temporal area. Now it's important to determine where the safe zone in the temple is
06:21because there are there is a lot of vasculature there. So we're going to mark this into quadrants.
06:28The middle here and in the middle here. Now the areas we want to avoid I'm going to mark in red like a stop sign.
06:37So in my case you can actually see one of my arteries pretty clearly through the skin. I don't
06:42know if you guys can see it. It kind of runs zigzag through here and down into here.
06:46So this is a stop sign area. No injection here. Stop sign area. No injection here. And here.
06:58So some of you may have heard of the one up one over technique. This is our go area but I'm going to show
07:04you the one up one over point. So if we go one centimeter up and one centimeter over this is our
07:13safe injection point. And if you feel it's very bony and when you go below this it's very it gets
07:21softer. We want to inject right in this area down to the periosteum. That means the bone.
07:26And this is our safe zone. I'm going to probably hit right above that black dot.
07:31Now something to note when doing this is you're going to go in hold steady and we're going to
07:36aspirate on an unprimed needle. So if we pushed filler up through the needle this would be primed.
07:43The filler would be going up there. Right now there's just air there. It's not primed.
07:48But by priming it you're making it harder to see if there's a vascular occlusion. So for an
07:53extra step of safety we're going to go into our spot and we're going to hold steady pull back
07:59count to ten on an unprimed needle because it'll be easier to see any blood that might come into the
08:04syringe. If blood comes into the syringe it means the tip of the needle is in an artery and then we
08:09don't want to inject there. So that is a very important safety measure. Not just with temple filler
08:15but with any filler. Even more safe is using a cannula but with a cannula you can't get down to the
08:21zygoma. Cannula injections into the temple region tend to lay a little more superficial and it can
08:26be a little lumpy bumpy. It's still very useful and a lot of injectors do use a cannula but you have
08:32to do it right to get good results. A combination of needle and cannula can also be used. So we have
08:38our markings. You can see where our green light in our safe zone for injection is. I'm just going to take
08:44some of that marking off here before I do the injection on this side. So I'm just going to take off a little
08:49bit of this marking here. It's really more of a visual aid for you guys but I don't want to push
08:56any of that marker into my skin. Another important safety measure is hold your finger and just feel
09:02for any pulsation in that area. Take your time and just allow yourself to feel. Right now I'm not feeling
09:08any pulsation which is good and another safety measure to take for temporal filler. Now I want to
09:14show you on my mannequin that there is a lot of vasculature. Now this doesn't necessarily show the
09:20depth. Some of it is more superficial and some of it is deeper in the face. But if we go to the temporal
09:25crest which is right where this frontalis muscle ends and we go to the base of that here, we go up
09:31and over. That's exactly where this is and you can see it's a safe zone free of any arteries or vessels.
09:38And that is this point right here. The Tesoro Sub-Q with lidocaine does come with two needles
09:44to choose from. One has an orange cap and one has kind of a gray cap. The gray cap is a 27 gauge 13
09:50millimeter and the orange cap is a 25. So a little bit bigger opening. Now something that I'm planning
09:56to do, there is a bevel on every needle. I'm not sure that you'll be able to see this. If the bevel is
10:03let's see, facing you and it's the part where the filler comes out from. So when we put our bevel
10:11in, I want the bevel facing out, not this way. We want the bevel or the opening on the needle
10:18facing out and I'm going to have it facing this way and then as I inject I'm going to slowly twist
10:26so that the filler is getting moved a little bit into this area. So we have a more even distribution.
10:32It's important to go slowly and not to press hard on the plunger. Take your time in finding the right
10:38amount of pressure for a nice slow steady injection. So again, we're going one up, one over
10:48and I'm going to feel to make sure the periosteum is there. Visually inspect.
10:54And when I say one up, one over, I'm talking about a centimeter and we're going to lightly twist. You
10:58might hear a little like crunchy sound, snap, crackle, pop, I call it. And I can feel the tip
11:05down to the bone and we're going to very slowly pull back on this plunger. You want to keep the
11:14needle steady. You don't want to pull it away. We're going to pull back for about a count of 10.
11:22So I don't see any flash of red in there.
11:28And I'm going to grab, without moving the needle tip, you want to keep it down on the bone.
11:32And we're going to slowly find the right pressure. And it takes a minute to figure that out.
11:46And as it starts moving down that plunger, I'm going to keep a close eye on it.
11:54Not press hard, just press steady. And we're going to move the bevel. I'm starting to twist.
12:02And move like this as well. But you just don't want to pull that needle head back. You want to keep
12:11the tip on the bone. And I'm being patient. There is lidocaine in this. So as it goes in,
12:26it's continuing to numb the area inside, where the topical lidocaine, twisting a little more,
12:34where the topical lidocaine only really numbs the surface of the skin. This numbs a little deeper
12:40in the skin. I want to get about half of this syringe into this temple area.
12:46Nice and steady, not pressing too hard, finding that right amount of pressure.
13:02Okay, you can see that bolus there is there. And I'm going to
13:06start to press it down a little bit. Now, this one doesn't flow as much, but it's nice
13:14and thick and will stay in this area
13:19without sliding into areas it shouldn't be.
13:23I'm not pressing on the soft spot. I'm pressing above on that bony part to distribute this a little
13:29bit better. This also will help give a nice little brow lift and support. I'm going to take and roll
13:38just a little bit, not pressing too hard. Now I'm going to move my point of entry down just a little
13:45bit. It's still on the bone. It's still in that safe zone. I'm going to feel for any pulsation.
13:53Twist a little to go in. You hear a little cracking.
13:56I'm going to feel that I'm on the bone there. We're going to aspirate again,
14:01which should be done every time you reposition. And hold five to ten seconds.
14:15And I'm going to go and do the other half of the syringe, finding the right pressure.
14:27Little twist.
14:45Steady.
14:45Sometimes you can hear the filler going in a little bit.
15:01Okay, pulling out. I'm going to clean this whole side here.
15:06The marker kind of makes a mess. You certainly don't have to do that at home. That was more for
15:10demonstration. Now we can see how fast a full syringe goes in this temporal area, especially
15:17when you have a lot of volume deficit like I do with my 80-pound weight loss. So you can see that
15:22that gave some nice fullness through here. But we do still need to address this area, which may be
15:28safer to address with a cannula going from here and fanning this upward. The other option is to go in
15:35even slightly lower just at the low point of that safe zone with a filler that flows a little bit
15:42better. I do have the Tesoro Deep, which is not quite as thick as the Tesoro Sub-Q. You do want a
15:50filler that has some thickness to it. Otherwise, it's just not going to stay and give you the volume
15:55restoration that you want in those deeper areas where you're going down to the periosteum or the bone.
16:01So now I'm going to move on to Tesoro Deep with lidocaine and I'm going to use a cannula and go in
16:08and soften up some of this area here. So we have definitely volumized in this region and now I just
16:14want to kind of feather in some of this thinner Tesoro Deep just to kind of smooth everything out.
16:21Now it's always important on the temples to underfill rather than overfill. You definitely don't want
16:27this bulging. You want to underfill as opposed to overfill that area. This is a 22 gauge 50 millimeter
16:36cannula. We're going to go in right about here and then I'm going to use the cannula to feather in
16:42some of this Tesoro Deep. Tesoro Deep is a little bit softer than the Tesoro Sub-Q, but still has some
16:50body to it to volumize the temples. Using a cannula is much safer than using a needle because the tip
16:57of the cannula is blunt, meaning it's not going to puncture an artery. Now it still can, but it's really
17:04unlikely. So the cannula pack comes with a needle for piercing our entry hole. I'm going to set that
17:10aside. I'm going to take this Tesoro Sub-Q and we're going to twist this on. I already took the cap off
17:17that. Twist this on nice and tight and you can see that's a nice long cannula. I'm going to take this
17:25needle tip and we're going to make a pilot hole, but first I'm going to take an alcohol swab
17:31and swab this cheek area right about here and just go in at the same angle you plan to go in with your
17:47cannula. All right, so I'm going to take my Tesoro Deep with the cannula and we're going to go right
17:59into this area. Being patient, getting the tip in there, a little tiny twist and you don't want to force.
18:14You want to gently allow that to go into the tissue. I'm at about a 35 degree angle I'm guessing here.
18:23So we can see when we pull up where that tip of the cannula is.
18:38And I'm going to want to feather this in. I'm still going to aspirate a little bit,
18:43holding that for about five seconds,
18:51twisting, grabbing here and slow pressure, moving downward.
19:06And then I'm going to take pressure off the plunger while I reposition.
19:09repositioning again,
19:25not forcing.
19:35I'm going to pull that out here and we're going to gently work that in.
19:52I wanted to save the other half of this for the other side.
19:56So I put the one full syringe up here, doing a bolus,
20:00and then we kind of feather it in a little bit down lower with a cannula for safety.
20:06You can also supplement threads. So if you do some up higher and you want to supplement some threads
20:12down in here to build natural collagen, that's a great option.
20:17Not everybody feels comfortable with a cannula and threads are a very safe way to supplement filler
20:25in the temporal area. So I'm going to fill the other side off camera. So this video doesn't get too long.
20:30You can definitely rewatch this side and just really study that area. But I'm going to do this
20:36other side off camera and I'll be back to talk a little bit about how I'm feeling about how it looks
20:42and what I might tweak in the future. Okay, so you can see where I have treated this side of my face
20:48up here in that safe zone in two spots. One a little higher, one a little bit lower. One full mill,
20:55or one box, of tesoro. This one is the sub-Q. I had to look at the box to remember. Tesoro sub-Q with
21:03lidocaine. The whole syringe is up here in that primary temple area. Then I went in and made a pilot hole
21:10and took the cannula up here with some tesoro, the other half of that tesoro deep. That's the blue box.
21:18Tesoro deep. So with the cannula, I went up just to fill a little bit more of this area here. Only
21:25half a mill with the tesoro deep and a full mill of the tesoro sub-Q, both with lidocaine. Now I'm
21:32going to wait for about a week and reassess how this looks. I'll share some before and after pictures

21:57After this settles in and gets comfortable, I'll take a good look if I need more or maybe want to
22:04add or supplement some threads in the temple area. I can already tell it looks a lot better. I was
22:10definitely very hollow right here and that has filled in nicely. So I don't want to overdo it.
22:16It's always better to underdo the temples and reassess after a week or two and see what additional
22:23supplementation may be needed to restore volume. So are you guys seeing the difference already up in
22:28this area here? So there's still a little bit of hollow here, but again, it's better to err on

I just did my first plla in my temples, I've done my botox and a syringe of ha, and I think its safer to diy because u can feel it, and u learn ur face, also, we care more. So, do ur homework on anatomy and use universal precautions to prevent infection and i love doing it myself!
