00:00Hey friends, in today's video we are going to do a little jawline support using Tesoro
00:05implant with lidocaine. This comes with two syringes, one mil each, so this is perfect for
00:11the jawline area. We could be using a full syringe on each side, so part of that I will decide as I
00:18treat the area. There are a few things we should talk about as far as jawline filler goes for both
00:24safety and a positive outcome. Men and women have different jawline structures. A woman has a more
00:30open angle on the jaw where a man has a more closed angle, kind of more of a 90 degree, where a woman's
00:36is slightly more tilted back. Also when treating a man it's okay to treat on top of the bone out here
00:42to make the jawline wider, but on a woman we don't want a wider jawline. So often on a woman you're
00:49treating underneath the actual jawline so that you're not causing more width. Now initially there
00:54might be a tiny bit more width because of swelling, but placement just underneath the actual jawline
01:00is important. There is an area that I want to point out that should be marked before any jawline
01:05treatment. Now this video is meant for education, this is not medical advice or instruction, so please
01:10consult with a doctor in your area. So you can see when you run your finger down your jawline there's a
01:16little notch and then it kind of gets smooth again. So again run your finger down your jawline, find that
01:22little notch and we're going to mark it out. It's kind of a finger's width. There are some important
01:27arteries that run up through that notch. We want to avoid that area. So again we're finding the notch
01:33here and I'm just going to take and mark this area in red and this is a caution area. So we're going to do the
01:45same thing on the other side. Find that notch and this is a caution area. Now that doesn't mean we can't
01:55go through that area with a cannula. We definitely want to avoid that with a needle. Now I have treated
02:01the jawline with a little topical lidocaine for about 15 minutes. That is completely optional. I do
02:06like to do that, but it is up to each individual person whether they need topical lidocaine or not.
02:11This tesoro product does have lidocaine in it so once it is used in the area it does give some
02:18numbing to that internal structure. I'm going to open my first syringe of tesoro implant with lidocaine
02:25and it does come with two needles. So I decided to do a voiceover for the actual injection part of this
02:30video just so I can give you plenty of information. Here I am finding the posterior angle of the jawline
02:37and I'm going underneath the jaw as opposed to on top of the jaw because I don't want to create
02:44width. I am pulling back on the end of the syringe to aspirate to make sure that I'm not pulling any
02:50blood into the syringe checking to make sure that the needle is not into a vein or vessel. And then very
02:57slowly with gentle patient pressure I'm going to put a bolus of about 0.2 into the posterior angle
03:06of the jaw. This kind of creates an anchor point or a pulling back point and the other injection sites
03:14are going to have less of the filler. It's really helpful to have a magnifying mirror when doing this
03:19and to remind yourself to have a slow and even pressure on the syringe. I like to use my fingers to
03:25mold the shape and press back on the filler gently to give a really nice angle to the jaw. You can
03:32already see that the one injection point makes a subtle but impactful difference and this also helps
03:39to lift the sagging in the lower part of the jaw line. So even just treating the posterior jaw in the
03:46one place is an impactful treatment. Here I am marking out the no-go zone just for clarity so
03:54everybody can see. And then I'm putting smaller boluses on the lower part of the jaw line underneath
04:03instead of on the top of the jaw line. And these are about a 0.10 ml of product and just small boluses.
04:14Again I'm pulling back on the syringe looking for any flashback of blood making sure the needle is on the
04:21bone and then with slow gentle pressure applying a very small amount of filler. This treatment not
04:29only sharpens up a sagging jaw line but helps to make the jowls look less jowly. It's really a great
04:37treatment for jowls. I'm now moving along lower on the jaw line to create a little more shape and
04:45sharpness in that lower jaw. This overall will help that elongating and tightening effect of the jaw line.
04:53Again these are smaller boluses just about 0.1 ml in each spot at the bone on the lower side or
05:02underneath the jaw bone. For safety I aspirate every single time I do an injection to make sure that no
05:10blood comes into the syringe and then I go ahead and slowly inject a small bolus into several points
05:19along the jaw line. I personally have a very elongated jaw and a pointier chin so I don't go too forward
05:27onto the chin when I'm doing these injections because my chin is already pointy and my jaw line is already
05:34long. If you have an underdeveloped or shorter chin you can definitely come more forward into the chin
05:41to create more elongation from the posterior angle of the jaw into the chin. For me that's just something
05:49I don't want because I would definitely get more of that witch look because I already have such an
05:55elongated jaw and pointier chin. This treatment should be customized to each individual. You may not have as
06:02many injection points if your jaw structure isn't as elongated and you may have more injection points
06:09depending on your bone structure. I've done this treatment in the past using a cannula which is
06:14always the safest way to go with any type of filler treatment. However for precision a needle is always
06:22able to get a more precise application of filler especially when doing this type of technique. Again for this
06:30technique I am using the Tesoro implant with lidocaine and the reason I'm using that is that it's a very
06:37robust a thicker filler that stays put on the bone and it really gives the sharpness that I want in the
06:45jaw line but also isn't so soft that gravity pulls the filler down. This really does meld into the bone very
06:54nicely and stays in place. Please keep in mind that with any filler treatment there is the risk of vascular
07:02occlusion. There are safety protocols in place that help to mitigate the risks that come along with
07:10vascular occlusion. When done properly with an understanding of anatomy and safety techniques such as
07:18aspiration the risks are substantially mitigated. As part of a safety protocol it is essential to have
07:25a filler dissolver on hand any time you're doing a filler treatment even when using a cannula. I always
07:32have a lipo race and sterile saline on hand and ready to mix in case of a vascular occlusion. I've
07:40personally never had any kind of vascular emergency or occlusion where I've had to dissolve it in an
07:45emergency situation but I always have it on hand just in case. Here I'm massaging the jaw line and
07:53just smoothing out the filler. Now even though I'm doing some massaging and smoothing the area will feel
08:00a little lumpy and bumpy for a few days. There will be some inflammation and tenderness in the area and I
08:08usually will have that for almost a full week after a jaw line filler. It takes anywhere from two to four
08:15weeks for the filler to really integrate into the bone and for that inflammation to settle after
08:22treatment. I repeated the same thing on the other side and you guys will see in the next video I'm
08:27going to release I'm doing a little cheek filler touch-up. Thank you so much for watching and spending
