01:43already weighs 60 kilograms this is Lola she's come in because she has a fractured knee we've
01:51come today to see what Noel can do we hope we can get our water baby back Newfoundlands were
02:00originally bred to pull nets for fishermen and are as at home in the water as on land in the water
02:09they're just they come alive they're completely different breed in the water they're like half
02:16fish fish foundlands we call them Lola was three months old when she started water rescue training
02:25she absolutely loves it any splash or any boats in the water Lola was just on it she just wanted
02:31to get in and save whatever was in there the giant pup has become an enormous part of Sian's life Sian
02:38has even opened her own canine hydrotherapy center to spend more time with her Lola swims daily I can't
02:47stop her she knows the last customer has gone out end of the day and she's there ready waiting Lola was
02:55about 12 months old when she started to get lame on her daily walks I would do as much as I possibly can
03:04to get Lola out of pain she's my world so we will we will find a way
03:13now then Lola please Lola come on Lola come on Lola hello Lola hi sweetheart how are you what's your
03:21first name Ann yes very nice glad to see you from Wales are you yeah well I am well she's nice she's
03:27Lola's grandma ah grandma okay you don't look old enough grandma but I'm just saying that I'm gonna
03:34shout out the Welsh girls okay so let's track back a little bit and just tell me if you will what you
03:41know about the knee started to notice she took longer to recover after exercise yeah so I knew
03:47something was up okay so I've had a look at the scan was it categoric that they felt it was a fracture
03:52or they just didn't they weren't sure they had it possibly congenital I think almost certainly it was a
03:59developmental issue the reality is that probably she had the genetic propensity to not have a good
04:08groove for her kneecap no thinks Lola's problem is not a fracture but a slipping kneecap she'll need
04:15a CT scan to confirm his diagnosis if he can he will try to fix her knee straight after the scan are
04:23you of a mind that if I decide that I've got the equipment in today that you would want to go with
04:29the surgery today yeah she's waited too long yeah okay I'm quite happy to do the CT and the surgery
04:36after all in one go so I've been positive because she's gonna be out of pain but we're just just a
04:42little bit apprehensive trying to put the back of our minds for the rest of the day Lola is sedated for
04:48the CT scan but three hours later Sean and Anne are back Noel hasn't been able to go ahead with Lola's surgery he wants to discuss other options
05:03firstly I've gone ahead and I've woken Lola up after these images so you do not need to make any decision
05:11right now and I've done that because we're gonna need some time to think about this now then that's the
05:16way the normal groove is in this case that that bump doesn't exist so the kneecap slipping down there and we
05:24somehow got to keep it in option one would be to deepen the groove but that would leave you a rather
05:31narrow ridge which could fracture option two is you put in a new a new titanium groove so you cut a flat
05:40surface there and you put a new titanium groove there and put the kneecap into it rebuilding you're rebuilding it
05:50yeah now that's fine but this ridge is probably fine yeah so that makes me question why am I chopping
05:59it off if it's okay option three is something that I've never done it before intuitively I'm thinking
06:09what is the best option for this dog and could we build a new titanium ridge which the bone could grow
06:15on to and just replace that Ridge to save the good knee Ridge on the outside no could design a partial
06:23ridge implant for the inside of Lola's kneecap but as no has never performed this surgery before Sean has
06:31a tricky decision to make we have a few different options I like Noll's viewpoint because he's you think
06:37it's holistically he thinks of what's best for that dog so we're going away and thinking tonight we
06:41obviously want to do what's best for Lola and he's our world so we'll see we'll see how we go tonight
06:47many of the pets brought to Fitzpatrick's have complicated needs
07:10this morning a beloved Labrador has been brought in for a unique surgery that no has pioneered this is
07:20Sunday she's come down today for some surgery on her leg some of her foot replaced it's a birth defect
07:27and when we got her we didn't notice it took her to the vets and they said take her back couldn't do it
07:35we bonded with her we decided to keep her and persevere and just decided what will be will be
07:42three-year-old Sandy lives with Ian and Gail and their family my two sons live here and
07:52they know they idolise her they really do Jake and Sandy they've got it's like this bond and they
08:01both know that there's something wrong with the other her foot it's a lot thinner and shorter than
08:08her other leg she's walking very uneven she hasn't got no movement in the ankle bit she does get
08:14infections in it and the foot swells up we know she's in pain Sandy's deformed foot means she can't
08:23take part fully in family life I'm sorry Sandy you've got more of it yeah what do you love it
08:29she's stuck in the house all the time it's not fair is it there's no way she could carry on how she is
08:35we had a couple of different vets and they said end of the line is going to be a full amputation
08:41really for her it's really a last resort so we find anything but that just wanted to be able to
08:48do as much as she can in her life and just enjoy it the same as we want for Jake to give her a normal
08:55life and we just made it just mean the world mean everything I'm just like everybody in the family so
09:00happy today Sandy will have surgery to remove the deformed foot and replace it with a custom-made
09:08prosthetic which will give her a new bionic foot how you feeling I'm just trying not think about it
09:16about so I know I'll bloody cry all right then darling all right love you Sandy oh come on then girl
09:26in there then come on girl so uh how have things been not too bad she's still spending a lot of time
09:35on three legs yeah can I have a little look just hold her head there for me yeah so I suspect there
09:43is pain there with the wrist so the options are leaving her she is just giving her medicine taking
09:50the full leg off or give this dog a new foot I want her to be able to walk around and I want to be
09:57able to take a runner okay so it's primarily an issue of quality of life yeah yeah yeah okay well
10:04what we're gonna try and do here is we're gonna try and and put this implant in there like so yeah
10:11onto which a foot can be attached and that's roughly what the final foot will look like the risks are
10:17mechanical failure if this were to break before the bone grows onto it skin failure if the skin were to
10:26not bond properly onto the mesh that's the single biggest risk yeah yeah and then the fall back
10:31for all of that if that doesn't work is full of amputation yeah but you know that yeah yeah Sandy
10:38will have the life-changing surgery this evening all right so big cuddles then well done okay good stuff
10:49all right you take care now look after yourself and I will see you soon come on bubs
10:56I'm hoping that it takes she can learn to use it so she can have as normal a life as possible

11:11Sandy a three-year-old Labrador with a painful deformed poor is about to undergo surgery to have it
11:29replaced with a bionic foot this is a very challenging situation because on the one hand she would call okay
11:37with this leg on the other hand she's in quite a lot of pain all the time so it's a real ethical and
11:45moral dilemma because if I do the surgery and it goes wrong she'll almost certainly lose her leg
11:51you wake up you have a brand new foot you will 99.9% of veterinary surgeons would not do what we're going to do today when things go well I'm the hero when things go badly I am absolutely the villain
12:10okay right now the family have asked me to try so I'm gonna do my very best for them and for her
12:18to give Sandy a brand new foot no one is going to remove the painful contracted tendons and deformed
12:27poor the implant will be fixed to the radius and ulnar bones bone marrow packed between the two bones
12:37will encourage them to fuse and the metal has a special coating for bone to grow onto once the
12:44skin is sealed around the mesh dome at the base Sandy's new foot can be attached can I have a number 11 blade
12:53of the foot please the first job is to remove the malformed foot hold it there please so just chop the
13:07foot off at the wrist just put a tourniquet around the leg stop it bleeding pop the implant in
13:16then noel cuts the contracted tendons and removes the bottom part of the radius and ulnar bones no takes
13:28bone marrow from the foot that was removed and packs it between the bones and the implant now that mulch
13:36should just go into bone enjoying the radius and the honor together so that the two plates work as one
13:43with the marrow packed in place the skin can be stitched onto the mesh dome at the base of the
13:50implant to create a tight seal that will hopefully prevent infection Sandy's new foot can now be attached
13:58right sweetheart you've got a new bionic leg use it wisely
14:06it's taken just two hours of surgery but Sandy's skin needs to heal around the implant and there is
14:16still a risk of infection oh I know darling I know you could be a reception dog for the day

14:30it's been two months since Lola the giant Newfoundland puppy was last at Fitzpatrick's
14:38okay you've got a long time can you get it down there can you now you are making a bit of a mess
14:47here but never mind we'll sort it out well she's thirsty isn't she the slobbery pop has come in to
14:54have surgery on her slipping kneecap and Sian has decided to go with Noel's bespoke knee implant
15:00been waiting for her custom fit rich to be 3d printed so she's having surgery hopefully this
15:06evening so it'll be a bit of relief hey I'm a super dog yeah and he's a super vet yes now Lola please
15:16oh hello hiya how are you I'm good hi babe how are you lost a bit of muscle mass haven't you sweet
15:24I'm worried about her right four though so I want to ask you about um when she's been
15:29swimming she gets onto the ramp there's a click you'd like me to have a look okay fine cool well we can
15:37do that so as CT shoulders and elbows okay so big cuddles then good girl my darling hey thank you
15:45before the knee surgery can go ahead Noel wants to investigate Sian's concerns about Lola's front
15:52right leg so Lola's shoulders and elbows are scanned to check for any problems
15:57okay so we've got some major issues with Lola both shoulders are affected by a condition
16:07called OCD whereby the cartilage does not develop properly in the shoulder very unlucky
16:12hiya hello you're right you do have front leg lameness okay and unfortunately you've got multiple
16:26problems here do you know what osteochondritis dissecans is so you've got that in both shoulders
16:32unfortunately both shoulders yeah right okay what should we do about it well I would probably recommend that we scope both shoulders to determine how extensive the lesions are I'm assuming that you want to carry on and treat her and do the best you can for her
16:49definitely definitely okay yeah she's my world so do everything we can and you know just yeah I'm so
16:57sorry I didn't expect for her to have problems on three legs no will investigate Lola's shoulder
17:06problems with an arthroscopic camera but first Sian wants him to proceed with Lola's knee surgery as planned
17:13no will save Lola's healthy knee Ridge but replace the other malformed Ridge with an implant he's designed
17:21specially never done this operation before and to my knowledge nobody ever has the first job is to
17:30position the new knee Ridge but it's immediately clear that there's a problem unfortunately the front one
17:40of the two cruciate ligaments is partially torn afraid like a shoelace so we're now going to have
17:46to do two procedures on this knee unfortunately with this kind of knee disease inflammation is severe and a
17:54damaged ligament is always a possibility but it will mean a more complicated and lengthy surgery for 20
18:00month old Lola first Noel addresses the knee Ridge that Ridge should be as high as that Ridge so I'm
18:08going to put a new Ridge on here he drills the fibrous surface off the damaged Ridge so bone can grow into
18:15the new implant this is a custom-made metal Ridge I'm going to take bone graft from the other side of
18:21the bottom of the femur here and pop it in here so that they can grow into this mesh and so that the
18:28Ridge will become part of lawless body forever now when we pop the kneecap back in it should stay and
18:44not slip out so we'll just stitch it up now and after that part of the operation done give me four metric
18:51yes please no now needs to tackle the fraying ligament I'm gonna make a cut in the top of the tibia
18:58using a curved saw blade and then I'm going to take the slope on it and rotate it from there to there by
19:06altering the angle of the top of the tibia it should prevent the femur slipping and straining on the damaged
19:12ligament noel can then secure the position with a plate unfortunately for Lola that's just one joint
19:25sorted out before Lola is woken up noel takes a look at the shoulder OCD lesions with an arthroscopic
19:36camera okay we're gonna scope the right shoulder and there isn't too much inflammation in this joint
19:44and I think the cartilage hasn't actually flapped off so I think this shoulder will be okay okay let's
19:52flip so this lesion looks more pervasive there's quite a lot of cartilage lifted off the bone there
20:01that's unlikely to heal on its own so it looks like the left shoulder is going to need surgery
20:06by quarter to ten Lola is finally out of theater thank you but with the prospect of yet more surgery
20:14it's not going to be an easy road for Lola and her best friend Sean arriving at Fitzpatrick's from the
20:32Highlands of Scotland is black Labrador Moogie and his best friend Toby who's taking the low road
20:38oh there you are it is a little enlarged situation going on Moogie and Toby are rescue dogs taken in by
20:49David and Alicia we just locked eyes with both dogs and thought that's it I just fell in love with them I
20:56couldn't say no when the couple adopted two-and-a-half-year-old Moogie ten months ago they
21:03began to notice a limp he's got an old injury to his left elbow how it was inflicted I don't know it he
21:12hasn't had a good life in the past we are his third owner the first two owners were hard on him if you
21:17were sitting over an evening and you moved your feet the dog would run out his bed and tuck his
21:22head and his tail in and run in a blind panic through the house so we knew someone had probably
21:27just booted him kicked him it's really awful he's a beautiful dog with the injury severe enough to
21:36affect Moogie's daily life the couple have made the eight-hour journey to see if Noel can advise them
21:42on a joint replacement he's a young dog and he's absolutely bristling with energy he's full of
21:49energy but sadly because of this leg we can't exercise him as much as he really needs it doesn't
21:56go to meet a happy chappy hey thank you good morning how are you doing good morning little and large
22:03folks hi pleased to meet you pleased to meet you what ended you up here hey the local vet referred
22:10this right to here right so you were already aware then it's quite severe yes do you know anything at
22:16all about his background hey the first donor we believe kept him in a cupboard for 10 months oh wow and
22:24speaking for myself I get the feeling that's where the injury occurred somebody who's going to keep a
22:31dog in a cupboard for 10 months as well to do anything okay I do not think that this dog's been
22:37hit oh that is developmental in my opinion all right yeah the humerus doesn't fit in the all that
22:45and the radius we need further diagnostics to determine what the possibilities are I can tell
22:51you right now that he's already beyond some surgical techniques that we would use right right so that
22:57leaves you with partial or complete joint replacement or fusion fusing him solid all right
23:03moogie will have a ct scan and an arthroscopy to determine what route surgery will take all right big
23:12guy off you go then okay it's all yours yeah bye take care of my dog will do yep you wait for your brother
23:26you're gonna have gravy down oh yeah I like it's gravy bones there you are go in then it's okay thank you
23:33very much are you eating it can't take her in if you're terrible it tastes like cardboard I know
23:41they do actually I feel the bacon ones oh no now moogie please moogie hi guys hi the situation is a
23:54little better than than we might have expected but it's still bad okay okay CT scan clearly shows
24:01collapse what you can see on arthroscopy there's a hole there in the humerus and a hole there in the
24:10ulna but as it transpires you still have cartilage over at least 80% of the radial head so my
24:20recommendation is going to be that instead of considering a total elbow replacement which has
24:26high risk associated with it that we consider a custom medial compartment replacement ie replace
24:34that with metal and that with plastic a resurfacing to jack that up from its collapsed position right
24:41after a night in wards moogie will go home to wait for the partial elbow replacement to be custom made
24:49yes better result than we expected it's good that he's not been hit or anything like that that was a
25:01main worry let's put our mind at race that the dog hadn't been you know abused we're really delighted now
25:06with the outcome of it i think we're going to have a wee bottle of wine or something maybe several
25:12so our longer-term patients definitely become part of the fitz family we visit them sit with them every
25:29morning every evening before we go home talking to them every day we start to treat them like family and
25:35and i think they kind of start to see us as part of their family
25:42hello sandy hi bobby who's a good bobby three-year-old sandy is still recovering in kennels after having
25:51been fitted with a bionic foot four days ago okay it's not too far out i think today noel is checking the
26:03golden girl's new foot to make sure it's the right length to start walking on it's actually more or
26:09less now isn't it right okay so maybe we'll leave it and see when she's walking properly
26:17infection is a big risk so noel also needs to check how the skin is healing to the implant
26:23so i would say so far so good on that in another couple of days we'll probably be able to take the
26:30bandage off completely and providing that the skin sticks to the metal and no infection gets in hopefully
26:37she can go home all right sweetheart good good girl does lola enjoy the swimming just a little bit
26:56four weeks after knee surgery water baby lola is back making a splash in her home hydro center
27:02oh you clever girl lowly struggles on land as long as she can swim she'd be happy so our aim has always
27:11been to get her back to water rescue from the lakes we will get there sean's been doing an exhausting
27:19six hours a day of hydrotherapy with her she's a good girl but i hope noel's happy with our rehab i'm
27:27happy with what we've done and lowe's lowe's getting there she's getting happier yeah
27:39ten weeks later and back on dry land sean has brought lola to see noel today we're here to do
27:47a checkup on the knee and also to talk about that left shoulder i'm reluctant for surgery because it was
27:53so horrendous i just don't want to put her through that again if we don't have to
28:01today she will have her walking analyzed and have scans to check her knee and the ocd in her shoulders

28:13hey sean all right so it's all all good so there's the kneecap there yeah and there is the new ridge
28:26yeah and that's nice you've done a brilliant i would say fabulous job at the rehab nobody could
28:33have done it as well as you did it so i'm very proud of you sincerely you've been the difference
28:38between success and failure here i think i know i know but my goodness you've put in the hours
28:44i mean you've got that swelling down that's a brilliant result and without your hours sorry
28:50at least our tears of joy i think without you doing what you did you would not have got that result so
28:56you should be very pleased with yourself yeah she's everything you're fine almost i almost had a neck
29:05fracture there that's all that's why i can't operate for the rest of the day so let's talk about the
29:11rest of the body i did a repeat ct scan of the shoulders today so that i could ascertain what to do
29:17next with the left shoulder i'm definitely worried so the options are do nothing just give medicine or
29:26taking the cartilage flap away and poking the hole underneath in the hope that a scar forms in
29:31or we could fill that hole with either a synthetic or a stem cell seeded plug so with the shoulder you
29:38do have a choice and you could do nothing and you might be okay and i was hoping that that perhaps you
29:46thought we could could conservatively manager but we just don't know do we okay well have a think about
29:52what you want to do yeah she's right outside yeah brilliant okay all right come on bub there you go
30:03okay good yeah doing good we got some good news today lola's all good on that left stifle everything
30:10that should be oh brilliant we just had discussion about her left shoulder uh where to go i kind of
30:17feel we've done this much we should get to the end of it and and then she's set for life hopefully
30:31you've been here before moogie haven't you oh please i know moogie the black labrador from the scottish
30:44highlands has arrived for his elbow surgery today's the day he's going to get the job done i'm looking
30:51forward to the next few months so slowly but surely be able to start building up up again and then it's
30:56back to the hills back to where he was born for me moogie has had a custom-made implant created from
31:06his ct scan so right i'm going to try and replace the inner compartment of moogie's left elbow so that
31:14ultimately we end up with metal articulating on plastic rather than bone articulating on bone okay
31:23excellent right okay any questions what worries me is it'll be clyde built won't it this thing
31:31this implant you're putting in that's a very good scottish phrase it's clyde built it comes from the
31:36shipyards i love it yes it's clyde built which means that it's super strong and made with super strong
31:43scottish workmanship going into the ship yes i i got it straight away so my plan is to put celtic
31:51workmanship in so that it won't budge good yes that's my plan come on then get some scottish
31:56girders in your arms go on then off we go he's in good hands and he's in good hands full of faith
32:05full of confidence but it's never a happy time i felt like traitors yeah it's a shame
32:18but no it's all going to be worth it it's all going to be worth it i'm going to get my
32:22good morning left here we go no will be resurfacing the inside of mugia's elbow where the cartilage has
32:40worn away we can see the eroded surface of the humerus right here and we're going to drill
32:48a recess into which we're going to put the metal component of the implant
32:58this implant has a trabecular metal surface onto which the bone should grow
33:03and a super smooth surface on which the joint can glide
33:14right now we're going to put the owner component in
33:23we're going to put this figure of eight shaped plastic implant with a metal backing
33:30into that hole once you grow into the metal backing over time moogie will spend the next few days
33:37recovering it will be some time before it's clear if the rescue dog with a troubled cast
33:43has a brighter pain-free future ahead
33:50basically the list she's given us here she's given us a bit about her and what she likes and
33:55she's given us the feeds um the goat's milk is in the fridge
34:00sandy the golden labrador has been at fitzpatrick's for two weeks now recovering from the surgery to
34:06replace her deformed foot with a bionic one that's excellent straight line please so far all the signs
34:15are that the surgery has been a success it's important to recognize that this operation does not
34:22always work and sandy's success certainly is the result of both successes and failures in the past
34:30hopefully as time moves on we'll develop better and better implants just like this for more dogs
34:37but today's a good day today biology smiles upon us come on let's go sandy will now be able to
34:44continue her recovery at home with her family it's been horrible to be honest really horrible
34:50i've been away from her i just can't wait now to have her back it's going to be quite emotional
34:57actually seeing her and seeing what it looks like and yeah it is a big operation she's had

35:15is stitched down over here yeah so warning number one is don't let her lick that yeah warning number
35:22two do not under any circumstances let her run jump slip or slide because this all has to become one

35:36hello hello who's here who's here who's here it's just amazing it's you know it's takes a bit of
35:51sort of like you know getting used to but it's what it's going to do for her it's just fantastic
35:57five months after her successful knee surgery giant newfoundland lola has returned for stage two
36:10we're back to have shoulder surgery so her ocd implant she's struggling a little bit at the moment
36:16and we're finding it difficult to rehab her hind leg still because the front leg is struggling
36:21um so hopefully this is the last of her ops fingers crossed touch wood
36:28lola yeah we're coming if we can get her out come on sweet unfortunately 60 kilogram lola has other
36:39ideas come on then come on come on then prima this is a prima this is come on she said i've been in there
36:49before before she says i know what's going on in here come on lola let's go go go thank you
37:09lola will have surgery this afternoon to insert a specially designed plug into the ocd lesion it's
37:19only one surgery compared to three four she had last time in one go so hopefully the rehab's going
37:25to be a little easier as well if successful it could be the end of a very long journey for young lola
37:31if any animal's going to get through anything it's this dog because she's so strong-willed
37:44okay so we've identified the hole in the head of the humerus we're now going to tidy up the edges of
37:50that and we're going to drill a hole in here which is 10 millimeters in width and six millimeters in depth
37:59noel must make the hole precisely the same size as the state-of-the-art plug he's created for lola
38:08right this is what's called a biphasic implant it's like trifle it has a pink lower layer
38:15and a yellow upper layer the upper layer has been seeded with lola's own cells which have been
38:21specifically differentiated into cartilage precursor cells that we hope will form a new layer of very
38:30smooth cartilage and now we're going to put glue this is a biological glue in on top just to seal the
38:38edges before we close to our knowledge it's the first time in the world this specific scaffold has
38:46been used the advent of 3d printing technology that allows us to create man-made materials which we can
38:55use like a climbing frame for stem cells you in there missus there you are good girl
39:03noel faces three months of recovery and rehab before noel will know if the surgery has been a success

39:3012 weeks ago sandy the labrador born with a deformed front paw underwent life-changing surgery to fit a
39:37bionic foot
39:44today ian has brought the three-year-old in for a check-up
39:48she's coping really well with it i think she thinks it's like it's always been there
39:52she's jumping up everybody you just have to warn people when they come in watch it because she's

40:13what the goal is for a dog or for a human with a limb amputation prosthesis is a permanent seal
40:20between the skin and the metal with no discharge no discharge means no bugs no bugs means no infection
40:28no infection means longevity so that's really good at the moment and that's the way we hope it'll stay

44:25finally the paddling pooch is back where she belongs
44:28lola's doing phenomenal um she's back to training in the water um it's her first session and i can't
44:36stop smiling i think something special about being in the water with these guys she just wants to swim
44:41it's what they're bred for it's what they're meant to do so it's kind of just fulfilling the dream of
44:46having our new feet oh come on then lola has been working towards human rescue to retrieve and come back
44:52to the handler it's so emotional being rescued by your own dog it's pretty special i recommend it to
44:59everyone
45:03the future's looking really promising this is why we went through all the surgery
45:07this is why we went through all the rehab and the tough times to enjoy your day like today
45:12and in scotland on the shores of the firp of clyde black lab moogie the rescue dog is doing some
45:27splashing of his own he's getting better and better all the time he's full of energy full of life
45:34you know that's what it's all about seeing him in the water enjoying himself it's great to see him
45:40not being in pain now he's running about and being a nuisance as they should be
45:50i just see my old dog coming back i love him to bits you've got no idea how much i appreciate what
45:55it all stands for the dog and for for me it's definitely clyde belt now right down to the sand
