Continuing my May 17, 2023 phone call with Christ Health CEO Dr. Robert Record MD re: 2019 visit with their Nurse Practitioner Anna Musgraves & Dr. Terry Daniel Smith MD. Dr. Record asked about my medical history and while I was reluctant to share that information I offered to send him CT / MRI and X-Rays that would confirm the seriousness of the broken skull injury I took into their clinic in 2019. Despite being referred to Christ Health from an Ascension Saint Vincent's East Emergency Department visit (and Christ Health accepting me as a patient) Dr. Record tells me their clinic staff could not look at MRI CT & X-RAYS of my brain and skull. He said they are only experienced with basic X-Rays such as chest and arms. Dr. Record tells me he will contact Director, Administrative Services Deborah Pickens to look at my information and come up with a way "forward" and that she will contact me. 3 weeks following this call I have not been called back by this person or Christ Health.

2 days later I found American Health Imaging in Birmingham who agreed to do the MRI for $500 cash and a referral. After paying I had AHI call Christ Health Woodlawn where the receptionist was placed on hold 15 minutes before someone at Christ Health hung up the phone. I did not get the promised referral. AHI agreed to do the MRI without the referral. Radiologist Angus Baird MD (of Birmingham Radiological Group) did not acknowledge the several inch diameter injury between my skull and cervical column of my neck that appeared in the exact area where I complained of pain but it can be clearly seen (surrounded by infection) in those MRI images (and correlates to broken bone in the underside of my skulll that can be seen in a CT scan of my head and neck from an Ascension ER visit weeks earlier).

Regarding visits I had at community health clinic The Christ Health Center (Birmingham, Alabama). I was referred here by Saint Vincent's East Hospital (Ascension) after a 2018 X-Ray (the injury was overlooked by reading radiologist Dr. Julian Patrick Druhan MD (also a member of Birmingham Radiological Group)) I had done was said to be "normal". Doctors Terry Daniel Smith ("T. Daniel Smith") did the encounter with me in May, 2019. Attempts to obtain a referral for an MRI to reveal the injury (I needed one that day, immediately) were not facilitated and he wanted to go a different route with the treatment. I feel I was not treated as a human being, my opinions were ignored, he levied an ultimatum, and so I walked out. I was charged for medical procedures not performed on me (see records and bills). Days later, I was able to secure an MRI on my own and was told by Nurse Anna Musgraves from a subsequent visit (several days following the first chc visit) at Christ Health to call their office for the promised referral. American Health Imaging did this after I paid them $500 cash and they were put on hold for 15 minutes before Chirst Health hung up the phone on their end.

00:18Oh, yes. It's been four years. I've been trying to get that issue resolved for four years, actually. I saw
00:25Dr. Terry Daniel Smith, MD, and he was at the Chalkville, Old Springville Road Christ Health Center.
00:34My issue with my Christ Health visit, 2019, I believe I received no measurable level of service in my visit.
00:43And I had two visits, okay? The one with the doctor, Terry Daniel Smith, and that visit was $109. I
00:52had a visit with nurse, NP, and Musgraves, $107.
00:58I've got a question to ask you. Do you have any doctors at Christ Health Center that can look at
01:03x-rays, MRIs, or CTs?
01:06We look at basic x-rays, like chest x-rays and arms, but brain and skull would be outside of
01:11ours. That's why we depend on them.
01:13We get them from the, we get, when we say we reviewed the imaging report, we reviewed the imaging report
01:20that the radiologist read and gave to us.
01:22What I'm interested in is, I've got to have somebody, a medically licensed doctor in the state of Alabama that
01:32works for you guys.
01:33You've got to look at my records. You've got to look at those images, or you've got to get in
01:37touch with somebody who can look at those images from 2019
01:41and getting me a correct diagnosis on the state of my physical condition when I brought it in to your
01:48medical practice.
01:49Okay, so let me get with this team member who is very good at finding this forward for us.
01:54I can't tell you what time she's going to call you back because I've got to go find her, which
01:59won't take me long to find her,
02:00but I have to go bring what you said to me to her and then have her call you.
02:06She probably can leave the exact return number of her phone.
02:09Okay, what's the name of this person?
02:12Debra Pickens.
02:13Is she a medical doctor, licensed in Alabama, nurse?
02:17She's our compliance officer.
02:19She's not medically licensed.
02:23She's not medically licensed, but she speaks for all of our medically licensed professionals.
02:27Okay, there's a big difference between that.
02:29You, on the other hand, sir, are a CEO of a company and a state of Alabama licensed medical doctor,
02:35and I think you could do a lot more for me, but it seems like you're not that willing to
02:41help out.
02:41Well, I don't read TT scans.
02:45At the end of the day, I don't read TT scans at all.
02:47But I think that you could get in touch with somebody to correct this issue for me because it's huge.
02:52With no appointment, I've spent the last 30 minutes completely trying to listen.

03:02I've heard everything you said, and I'm going to take it immediately to our compliance officer and to our quality

03:14Yeah, and the key to it is you need to get those records, the images from Ascension, the MRI scans,
03:23et cetera.
03:23You need to look at those pictures, and you will see what happened to me.
03:27It's totally unacceptable.
03:29I will bring this to our team within minutes of getting off the phone with you.
03:35And what I can promise you is our team is looking into this right now.
03:38And I can tell you, I am sorry for what you went through.
03:41I don't know what happened to fracture your skull, but it doesn't, no matter what happened,
03:46I am so sorry you've had to walk through what you've had to walk through.
