01:39And heavy snowfall is creating white-out conditions
01:42on the slopes.
01:44At Coronet, we don't really have trees above snowline.
01:48And when those white-out conditions come in,
01:51you know, you've got white clouds and white snow.
01:54And, you know, it can be actually quite scary
01:56for a lot of our guests skiing in those conditions.
01:59It doesn't take long for the calls to start coming in for ski patrol.

02:08This has had reports of an injury
02:10at the top of the hurdle of the M1 intersection.

02:17Patroller Finn is responding to an injured guest on the M1.
02:21The M1 is a two-kilometre run
02:23for intermediate skiers and snowboarders.
02:28Hey, guys. You have a knee injury?
02:31Yeah.
02:3122-year-old Izzy was on day one of her skiing holiday
02:35when she tripped on a thick patch of snow.
02:38Hey, I'm Finn. What's your name?

02:43So can you tell me what happened?
02:45I think I was turning and then I somehow got myself caught
02:51and then took out my knee and then popped.
02:54Did you feel a pop, hear a pop?

03:33The problem is, to move her safely, her leg has to be straightened and put into a splint.
03:39It's a painful but necessary step.
03:42Trying to minimise the amount of movement is really kind of key.
03:46You're in the rig, we try and go as slowly and carefully as possible but, um, yeah.
03:51It's always going to be bumpy, um, so those kind of movements can be really painful.
03:57Cool.
03:58So, um, in this little rig pack, um, we've got this splint, which is what I'm going to put your
04:03knee in.

04:05So, what we're going to do is I'm going to get some pain relief down here.
04:09Um, just because I think getting it into the splint is going to be really quite difficult.
04:13Um, and I don't want to move you until we get some, like, pain relief and get you nice and
04:18comfortable.

04:40We are getting two wheel drives to start fitting chains at Chain Bay 4.

04:48The heavy snowfall means it's all systems go for Harry and his roads team.
04:53So, yeah, some days we can get on it in terms of road management and making sure that the road
04:58is clear and accessible and chain free in the mornings.
05:01But other days, you know, we can't control the weather.
05:04We can only watch it and try and make a plan according.
05:09And, yeah, some mornings it comes in really hard and we're left kind of chasing the weather and a bit
05:14behind.
05:15And, um, it can cause a bit of chaos.
05:19We are all hands on deck at the moment.
05:23Um, getting everybody up the mountain safely.
05:26We are currently preparing for 15 potentially centimetres throughout the day.
05:32Yeah, we can definitely see the, uh, snow changing at the moment.
05:38It takes a small army of dedicated roads crew to keep the access roads safe in these conditions.
05:45Tyre chains are a must on days like today.
05:48Here in New Zealand, there's only usually one standard set of tyres.
05:53Um, and they're not made to withstand ice and snow.
05:57What happened?
05:59Oh, this is a nice sideways slip!
06:03So, when it becomes slippery, the tyres are just naturally going to slip out and you're going to lose traction.
06:09So, chains just help in giving that extra bit of grab onto the road.
06:13So, we're hoping people will be bringing up chains today.
06:17So, we're going to set up a, not a chain bay as such, but just a checkpoint.
06:20So, the cones will hopefully slow them down so we can ask them whether they've got chains on or not.
06:24If not, we'll send them back down so I'll tell you, James.

07:52When I came back to, like when I came down to him, lifted his helmet and he was, his eyes
07:56were rolling and he lost consciousness.

08:30On the M1, 22 year old Izzy has injured her knee after a slow twisting fall.
08:36She's in nine out of ten pain.
08:38Yeah, she kind of just got stuck in the loose stuff and then, um, sort of just got bogged down
08:44and then fell over and kind of rolled over an air wee bit.

08:47Ski patrol need to get Izzy's pain under control before they can splint her leg into position.

11:21Tristan's complaining of back pain.
11:23And that's a red flag for a spinal injury.
11:26Yeah, so he was indicating that he had some midline point tenderness, which means he had some pain directly on
11:31his spine.
11:32That mixed in with a concussion where someone can't give us a clear indication of what's going on with their
11:39spine means that we have to put them in full spinal mobilisation.
11:42It's hard to tell if his back is realistically really sore or not, but we'll take the precautionary measure.
11:49We'll get a vacuum mattress down here, we'll get a toboggan down here.

11:52And we'll attach this into our medical centre.

15:48So it's a really unique opportunity for med students to spend some time with us up the mountain.

15:57It's a very unique environment and it helps them to develop their pre-hospital skills.

16:26So there's lots of structures in a knee that can get damaged when you're skiing or snowboarding.
16:31The meniscus can get torn.
16:34Then you've got ligaments and they can be sprained in a minor way or have a full sickness tear.

17:49He has pain in his C-spray in his back, but what we'll do is we'll put him in the
17:53back mat just to keep him comfy.

17:59Head injuries can start to deteriorate very fast, especially mixed with that spinal pain.
18:06We really want to make sure that we get this patient down to see our doctor and stabilised as fast
18:12as possible.
18:13Can you give yourself a big bear hug like I'm doing?
18:16Lift those arms up mate.
18:17Cross your arms.
18:18Good boy.

18:19If Tristan has fractured his C-spine, he could be at risk of paralysis.

18:26A slow and steady extraction is what's required.

18:32With spinal injuries, if we are to move that patient too much, you know, we could definitely cause more damage.
18:40Right behind your cable and I'm going to come with you.
18:42For Tristan's mum, Catherine, this is every parent's worst nightmare.
18:47We use our vac mats to keep them as immobilised as possible.
18:50The risk is if we don't decide to use that immobilisation, that we can cause further damage to the spine
18:57on transport.

19:02Any of that bumping around as we head down towards the clinic actually could cause damage to the spinal cord
19:09and have real bad repercussions for a patient.

19:23She's done her, she's done her nice and now we're in the mid, mid-team thing.

19:33Kiwi couple Izzy and Jackson's skiing holiday has taken a painful twist.
19:38So, Izzy was skiing on the M1 and her skis crossed and she fell over and heard her pop.
19:47Is there any effort sore?
19:48Yeah, it's quite sore.
19:49The popping sound Izzy heard could indicate a ligament tear.
19:54So, when I examined Izzy's knee, it was hot and swollen.
19:59I saw her on this side.
20:01She couldn't move it properly so she couldn't flex it to 90 degrees.
20:06And are you able to bend it?
20:08I don't know.
20:09I don't know.
20:10Is it there?
20:10No further?
20:12It feels.
20:12Same.
20:13What sort of?
20:13OK.
20:14That can indicate a lot of different things, anything from a fracture to an ACL or a bad sprain.
20:22Do you want to try and see if you can stand on it?
20:26I'll give it a go.

20:31Whether or not Izzy can weight bear will help diagnose the severity of her injury.
20:40OK.
20:42So, with Izzy, she also couldn't put her weight through it.
20:47Not going to happen?
20:49I don't think so.
20:52And the mechanism of the injury made me suspicious for an ACL.
20:58So, what you'll need to do is go down to Queenstown and get an X-ray of your knee.
21:02We just need to make sure that there's no breaks to any of the bones in your knee.
21:08Izzy and Jackson's ski holiday is over.
21:11But the blame game is just getting started.
21:14Yeah.
21:15All his fault.
21:16He took me up it, thinking I'd be able to do it.
21:20I was literally saying I'd be so proud of myself if I get down to the bottom.

22:05And we're just running on our feet.
22:06And cars start slipping, getting stuck before we've even had a chance to try and litigate that issue.

25:16I'd say no more than a minute, actually.
25:18And he had C-spine pain and back pain, but it was hard to gauge the pain amount.
25:25So when Tristan came in, we took it really seriously and had him in full spinal precautions.
25:31And did you see him fall?
25:33Yes, I did.
25:33When he fell over, basically he would scream really loud.
25:37Then after that, he was out of it.
25:39Okay.
25:39Yes, yes.
25:40Because when someone has midline tenderness, that raises the concern of a spinal fracture.
25:47Dr. Nicky will need to conduct a full spinal examination to assess the severity of Tristan's
25:53back.

35:13I'd send it with my brother, decided to duck off the edge of the actual run to hit a massive

35:20Went a little too fast and just macked it way too hard.
35:24So the collarbone, it's kind of like the bumper of your car absorbs some of the impact and can quite
35:30easily break.
35:32Unlike young Tristan and Sam, Darshan's injury appears to be quite obvious.

35:43It was quite an easy assessment for Darshan.
35:46I could see quite clearly there was an obvious step deformity in his collarbone, so he had broken that.
