00:05What the hell were you thinking going to the house by yourself?
00:11What the hell were you thinking following me?
00:15Kovac, could I maybe grab your number?
00:17Do you have a pen?
00:19Didn't think it would be that easy, did you?
00:22You've met Dean.
00:23Mark Asher.
00:24He's not exactly a fan of our situation.
00:26Do you agree that there's value in having two parents in the same home?
00:30Hannah will be an incredible mother, with or without my help.
00:34This Archer guy is head over heels in love with you.
00:39All I'm asking is next time.
00:41Next time you're hosting folks at our place, just give me a heads up, okay?
00:46You just said our place.
00:48Rip, bring it in, man. You're gonna make me cry.
00:51Did you hear what I said?
00:52Hey, hey. Thanks again for letting us use your apartment for poker nights.
00:55Yeah. Actually, in Doris, it is, uh, it's our place now.
00:59Oh, how cute.
01:01Where are you two registered?
01:02Oh, heads up, Novak and Violet are ten minutes out with a college basketball player.
01:07Injured her knee during their morning game.
01:09I can take it.
01:11I wonder why he's so eager.
01:12It couldn't have something to do with a certain paramedic.
01:15Yeah, or because I'm just a really good doctor, so, yeah.
01:18Is that it?
01:19No.
01:19No.
01:21All right, I'm gonna take you up for an MRI shortly, Mr. Riley.
01:25You got any metal on you?
01:27Nope.
01:28Oh, wait, yes.
01:30Yes, there is.
01:32Okay, what, a wedding band, a wristwatch?
01:34It's not really on me.
01:36It's, um, in me.
01:38Um, okay, pacemaker, piercings?
01:41And, uh, where?
01:48Is it in your rectum?
01:50My what?
01:51Is it up your butt?
01:52Yes.
01:53Okay, and what is up your butt?
01:55It's a plug.
01:58I'm sorry, I didn't catch that.
01:59He said a plug, Dr. Archer, like a butt plug.
02:02Uh, uh, we were trying a new position when I fell off the bed, uh, and, uh, my girlfriend,
02:08it was her idea.
02:09It's okay.
02:10She thought I might.
02:11Yeah, yeah, sure.
02:11I'd take it out myself.
02:12No, no, no.
02:13That's, that's, that's why I'm here.
02:15All right.
02:17Let's roll Mr. Riley onto his side.
02:26What do we got?
02:29This is real white glove service.
02:31Most docs wouldn't walk the extra ten feet.
02:33Well, not most docs.
02:34We've got Kristen Thompson, 21-year-old female, pain and swelling to the left knee after a
02:39sudden stop and change in direction.
02:41It's my ACL.
02:42I know, I tore it.
02:43I heard the pop.
02:44Okay, we're going to treatment five.
02:45Right this way.
02:46We have 50 mics of fentanyl on the field.
02:48A scout from the sky is coming to our next game.
02:51No chance I'm going to be able to play.
02:53Hey, let's not get ahead of ourselves until we know what's going on, okay?
02:58Good morning.
02:59Oh, hello.
03:01What you got there?
03:02Ah, it's, uh...
03:04No.
03:05No.
03:06Nothing.
03:07So, uh, what brings you to our humble department?
03:10Well, I seem to recall that you were telling me it's sacrilege that I haven't been to the
03:13Green Mill yet.
03:14Well, as a so-called jazz enthusiast, absolutely.
03:18So I plan on rectifying that this evening.
03:22Oh, well, that's great to hear.
03:24You should try to get them to seat you in Al Capone's old spot.
03:28You might need to invite a friend or two to help you snag his booth, but, uh...
03:33You know, I, uh, I had someone special in mind.
03:36Oh.
03:37Well, that's great.
03:38Whoever they are, they're going to have a great time.
03:40It's, it's you, Dean.
03:43I'm asking you out.
03:44Uh, oh, oh, uh, okay.
03:49Sorry, I'm, uh, I'm a little, uh...
03:51Slow on the uptake.
03:52And rusty.
03:56Hi, I'm going to sneak in a quick coffee run before our next call.
03:59Do either of you guys want anything from the cafe?
04:01I'm good, thanks.
04:01Uh, I'll take a caramel frappe.
04:03Mm-hmm.
04:03Oh, and the cinnamon roll.
04:05She has the metabolism of a teenager.
04:07It's insane.
04:08Okay.
04:09So you got a sweet tooth, huh?
04:11Sweet, salty, umami.
04:13I don't discriminate.
04:14Nice.
04:15You should check out my buddy Frankie's new donut stand down at Navy Pier.
04:18Got some wild flavors.
04:20Bacon, maple, dill pickle, mango chili lime.
04:24Sold.
04:25When are we going?
04:27How about tonight?
04:28Dr. Frost, need some help in here.
04:32I said she was feeling sleepy, then suddenly her sat started dropping.
04:35Okay, let's get her on oxygen and a cardiac monitor.
04:39Hey, Kristen?
04:40Kristen, wake up.
04:41Wake up, Kristen.
04:43Okay, what's going on here?
04:44No clue.
04:45She came in with a suspected ACL injury, shallow respirations.
04:49Okay, you brought her in?
04:50Yeah, she was fine and ambo, alert and oriented.
04:53She hit her head, did you lose consciousness at any point?
04:55No, mm-mm.
04:56All right.
04:58Yeah.
04:59People should pinpoint.
05:00She's OD'ing.
05:02What?
05:02Sats are down to 82, heart rate's 40.
05:04All right, bag her and give me Narcan fast, please.
05:22Narcan?
05:22Okay, push two milligrams IV.
05:24Okay.
05:28Meds are in?
05:29All right.
05:30Okay.
05:31Sats are improving.
05:32Okay, Kristen, hey, open your eyes for me, Kristen.
05:34Hello, Kristen.
05:35Yes, it's really low.
05:36She's not protecting her airway.
05:37I'll intubate.
05:3820 Atomidate, 50 at rock.
05:40They were deep in the third quarter when we got there.
05:42When would she have taken drugs?
05:43Did she mention any medication she might have taken or recent illnesses, anything like that?
05:47She had a lingering cough from a bout of bronchitis last month.
05:50She went to Campus Health yesterday.
05:52They prescribed her white venison with codeine.
05:54Maybe she accidentally took too much?
05:56Possibly.
05:57Codeine converts to morphine in the liver.
05:59Okay, let's get an EKG, a chest X-ray, a head CT, knee MRI, and then a full trauma panel
06:03with a tox screen.
06:04And let me know when she's opening her eyes and following orders so I can extubate, okay?
06:07Good.
06:09It's Violet.
06:10I gotta go.
06:11Let me know what you find out.
06:12All right, we'll do.
06:17Are we going to trauma one?
06:19Ruth Williams, 88-year-old female in a history of dementia, no anticoagulant use.
06:24Complaint of left hip pain after ground-level fall vitals are normal.
06:28Are you the doctor?
06:29I am indeed.
06:30I'm Dr. Ripley.
06:31Handsome young man.
06:33Bruce, you flirt.
06:34Hey, Mike, gonna need an X-ray in here.
06:37Let's give two of morphine.
06:38It's easy when they look like that.
06:43So what precipitated the fall?
06:45Missed the chair when she went to sit down.
06:47I think her lens prescription is out of date.
06:50I keep telling her daughter that she needs to take her mom to the eye doctor.
06:54My daughter.
06:55I'm talking about Grace, Ruth.
06:56Oh, okay.
06:58Does Grace live in the city?
07:00Yeah.
07:00I keep trying to reach her, but it's going to voicemail.
07:03I left a message.
07:04Okay, I'm just gonna gently move your leg.
07:08I'm sorry.
07:09All day.
07:10Tamika, is that you?
07:12Hey.
07:12I thought your interview was this afternoon.
07:15It is.
07:16Interview?
07:17Yes.
07:18Tamika's applying for the nursing externship program.
07:22Oh, wow.
07:22Yeah.
07:23I was just bringing in Ruth Williams.
07:25Do you remember Ruth?
07:27Ruth's apartment was a few doors down from her.
07:30Yes.
07:30Oh, dear.
07:31Hello, Miss Williams.
07:33We were at that ice cream parlor next to Sinai Gardens when she had a little fall.
07:39We decided to have dessert before lunch.
07:41Well, I'm sorry to hear that, but I hope you at least got to enjoy your ice cream first.
07:46My favorite is rum raisin.
07:49I set up for pelvis and left hip.
07:51You know, my mother used to make rum raisin ice cream from scratch that was out of this world.
07:56You know what her secret was?
07:58Tell me.
07:58She would soak those raisins for a week in my father's Appleton estate with a shot of sherry just for
08:05a little kick.
08:07Oh.
08:13Dr. Lennox.
08:14Hi.
08:15Oh, no.
08:17No, please stop.
08:19Yeah, yeah, yeah.
08:19Hey, hey, no.
08:20She hates this, so, you know.
08:23Louder.
08:26I didn't think you were coming back until next week.
08:29Uh, we were planning a big party.
08:31Well, now I'm very glad I came in early.
08:36Hey, how's your, uh, mandated two-week vacation?
08:39I was going crazy at home.
08:41You sure you took enough time to...
08:43Organize my spice rack and clean up my closet?
08:45Yeah.
08:45I was gonna say heal.
08:47I'm healed.
08:47And process.
08:49Process what?
08:49Hey, Dr. Lennox.
08:51We have a pregnant patient in four with peripheral edema.
08:54Dr. Asher had to run up to L&D for a delivery.
08:56Just let me put my stuff down.
08:57I'll be right there.
09:01One plus pitting edema bilaterally.
09:03No calf tenderness.
09:05I'm just the oven.
09:07And favorite future auntie.
09:09It's Jacob's sperm in this five-foot-nine goddess from Harvard's egg.
09:14I swear, she looks just like Beyonce.
09:16Well, swelling can be a completely normal byproduct of pregnancy, but I'd like to do an ultrasound
09:20to check on the baby.
09:21Of course, yeah.
09:23And to be clear, my husband and I chose our egg donor for her scholastic achievements.
09:28Total lie.
09:29He just doesn't want you to think he's shallow.
09:31The aesthetics were simply a bonus.
09:34This might feel a little cool.
09:36I take it you two have been friends for a while?
09:39He's been stuck with me since the first day of freshman orientation.
09:42When she demanded I be her friend.
09:43Best friend.
09:46Is everything okay?
09:49You said this was a single pregnancy?
09:52Yes.
09:52A boy.
09:54Why?
09:55I am definitely seeing a second fetus.
09:58Shut up.
09:59We're having twins.
10:00Surprise.
10:02Yeah, the second fetus must have been hiding behind their brother during previous ultrasounds.
10:07Oh, I can't believe this.
10:08Would you like to know the second baby's sex?
10:10Yes.
10:11Yes.
10:11Please.
10:12It's a girl.
10:13Oh, my God.
10:14We're having a boy and a girl.
10:16We have to FaceTime Ryan.
10:18Ryan's the other daddy.
10:19He's on a business trip.
10:20I can never remember.
10:21Is China 13 hours ahead or behind?
10:23Who cares?
10:23Wake him up.
10:24Let's get a CBC CMP fetal echo and level two ultrasound.
10:28Is something wrong?
10:29Oh, given the new finding of twins, I want to make sure that the female fetus has a thorough
10:34assessment.
10:40The female fetus looks considerably smaller than her brother, right?
10:43She could have selective fetal growth restriction from twin-to-twin transfusion syndrome.
10:48Let me know when Dr. Asher's back from L&D.
10:50We need to loop her in on this.
11:00All right, everyone.
11:02We will finalize the capital investments proposal at our next meeting.
11:06Please send your suggested edits to Tiffany ASAP.
11:14Sharon.
11:15Yes.
11:15I understand you're overseeing the selection committee for Gaffney's nursing externship program?
11:20Yeah, that's right.
11:21I was just speaking with Jessica Ewing on the committee, and she mentioned that you have
11:24one spot left to fill.
11:26I happen to have the perfect candidate.
11:28My nephew, Colton, graduated from UPenn, Bachelor of Science in Nursing.
11:33Uh, so did Colton submit an application?
11:37He missed the deadline.
11:38The truth is, I didn't even know about the program until Jessica mentioned it.
11:41But I was hoping that his resume and a good word from a board member might make up for
11:45it.
11:46Uh, I'd be happy to meet Colton.
11:48Uh, have him call Tiffany to set up a time this week.
11:53Any chance at all you could squeeze it in for today?
11:55Colton's flying to Aspen tomorrow for a ski trip.
11:57It's a belated graduation present from his aunt and uncle.
12:03Okay, um, have him come by my office at, uh, one?
12:08You're the best, Sharon.
12:15I took two doses of cough syrup at halftime.
12:19But the doctor said I should do that if my cough started acting up during the game.
12:23Okay, so you took 10 mils last night, 10 when you woke up, and then 20 at halftime.
12:29Was that too much?
12:30No, no, no.
12:31Not nearly enough to cause an overdose.
12:34Do you have any history with seizures, Kristen?
12:37No.
12:38Fainting or any heart arrhythmias?
12:39I see you were prescribed an SSRI about six months ago.
12:43Yeah, after my grandma passed away.
12:46We were really close, and I had a pretty hard time with it.
12:50But the medication didn't actually help, so I stopped taking it.
12:54And how have you been managing your mental health since stopping?
12:57Once basketball season started and I got back in my routine, I began to feel better.
13:03Have you ever taken any medication that you weren't prescribed to help regulate your moods, like, uh, Oxycontin or Percocet?
13:10No, never.
13:13Listen, Kristen, everything that you tell us is confidential.
13:18Nothing will be reported to your parents, your coach.
13:21I've never done drugs.
13:23Not even weed.
13:24My brother got hooked on Vicodin after he had back surgery for a volleyball injury.
13:30He's better now, but I would never put my family through that again.
13:36I swear.
13:38All right, Kristen, they're ready to start your MRI, okay?
13:45But was it really necessary to call an ambulance?
13:48She couldn't get off the ground.
13:50We didn't want to make it worse by trying to move her.
13:53Hi, uh, you must be Ruth's daughter.
13:56Grace Evans.
13:57Nice to meet you, Mrs. Evans. I'm Dr. Ripley.
14:00What is the issue?
14:01Your mother's x-ray confirmed that she has a fractured hip.
14:05Isn't he handsome?
14:07He's nice, too.
14:09Can you see I'm trying to talk to the doctor?
14:11When's lunch? I'm hungry.
14:13I'll go grab you a snack, Ruth.
14:16Uh, the next step is to get a CT scan.
14:19Why?
14:20You already know what's wrong.
14:22Yes, but orthopedics needs more detailed imaging in order to determine if it will heal on its own or if
14:27Ruth will need an operation.
14:30An operation?
14:31I think I want a turkey sandwich.
14:32You're kidding me.
14:33No mayonnaise.
14:35This is not a restaurant, okay?
14:37You're in a hospital, and they serve food.
14:41We'll know more after the CT.
14:43How long is that going to take?
14:45I'm supposed to volunteer at my granddaughter's preschool at one o'clock.
14:49Well, I doubt radiology will be able to read the results that fast.
14:55Okay.
14:56I will call my son and let him know that I'm not going to make it.
15:01Excuse me.
15:02I'm sorry.
15:07Here you go, Ruth.
15:09Don't look like a turkey sandwich.
15:10Well, they'll bring you lunch soon.
15:12Don't you worry.
15:24Couldn't help but notice.
15:27Notice what?
15:28Well, you know, just, uh, you and Kingston look pretty cozy this morning.
15:33You should ask her out.
15:36Actually, she asked me out.
15:38Wait, she did?
15:39That's, that's great.
15:41Yeah.
15:42I declined the invitation.
15:45Why?
15:48Because.
15:50Because of me.
15:53Yeah.
15:55You know, it wouldn't be respectful to you in your condition.
16:01I'm sorry, my condition?
16:04That's not the right words.
16:05I didn't mean to say that.
16:06What did you mean?
16:08I mean, you're not, you're not dating currently, so...
16:11How do you know I'm not?
16:13Oh, uh, are you?
16:15No.
16:16But it doesn't mean I couldn't.
16:18I wanted to, and it certainly doesn't mean that you can't.
16:20I mean, it's not like we're together.
16:23Right.
16:23Yeah, we're just two friends having a baby, remember?
16:30Yeah.
16:32Oh, good.
16:33You're back.
16:33Dr. Lennox is looking for you.
16:35Okay.
16:44I think we can rule out twin-to-twin transfusion syndrome.
16:48I mean, these babies both have their own placentas and sacs, so they're unlikely sharing any blood supply.
16:52So what's causing the size difference?
16:55I'm not sure.
16:57Can you pull up Olivia's last ultrasound?
17:08Oh, my God.
17:10What is that?
17:12I see why they missed the second fetus on her prior ultrasound.
17:17It's not clearly visible yet, but...
17:21I don't think that the female fetus is growth-restricted.
17:24I think she's younger than the male fetus.
17:26You lost me.
17:26These are not twins.
17:28These babies are of different gestational ages, about four weeks apart, I'd estimate.
17:35How is that possible?
17:37I think that this is an extremely rare case of superfetation.
17:42Olivia became pregnant after she was already pregnant.
17:56Hey, Kristen's tox came back.
17:57It's positive for opioids and fentanyl.
18:00I'm inclined to believe her.
18:01The only opioid she took was codeine.
18:03I'm the father of an addict.
18:05I got an extra sensitive nose for bull.
18:07Of course, that leaves the fentanyl.
18:08We know where that came from.
18:10Novak gave her 50 mics in the field.
18:12That's what Novak said she gave her.
18:15You're not suggesting that...
18:16Well, listen, man, you know, I hate to go there, but paramedics are human.
18:20Mistakes happen.
18:21And in the absence of any other explanation...
18:24You think Novak overdosed here?
18:26Uh, seems like the most likely possibility.
18:28But obviously, before we want to chain to paramedic chief Robinson,
18:32we should probably notify Novak as a professional courtesy, right?
18:36You two seem to have a rapport.
18:39Maybe you want to talk to her.
18:41Yeah, sure.
18:46Well, I see you have certifications in advanced cardiac life support,
18:51and you helped administer the vaccine to at-risk communities during COVID.
18:57Yes, that's right.
18:58Mm-hmm.
18:59So, uh, what did you take away from that experience,
19:02being on the front lines of the pandemic?
19:05I learned firsthand how critical it is for hospital systems to have the proper infrastructure to respond to a pandemic.
19:12No, no, no.
19:14Personally.
19:15Ah.
19:15What did you learn about yourself?
19:17I learned how to work well in a team and implement new protocols quickly.
19:23What drew you to nursing?
19:26Well, my mom's a CRNA, and my dad's a radiologist, and as I'm sure you know, my Aunt Miranda has
19:32worked in the public health sector for decades, so I guess it's kind of in my DNA.
19:37You know, Colton, your resume is very impressive.
19:45So, I hope I'm not speaking out of turn here when I say I'm getting a sense that this isn't
19:54something you actually want to do.
19:56Am I wrong?
19:57Correct me if I'm wrong.
20:01No.
20:02You're not.
20:03Oh.
20:05My family pushed me to apply to the BSN program, but after I did my clinical rotations, I realized I
20:13don't want to be a nurse.
20:15Please don't tell my aunt.
20:16I know she pulled strings to give me this interview, and she'll tell my parents, and it'll be a whole
20:19thing.
20:19No, no, I won't.
20:21Thanks.
20:23So, what is it that you want to do?
20:27Tell me what brings you joy.
20:31I, um...
20:34I really love photography.
20:35Yeah?
20:36Yeah.
20:37One of my photos was actually just accepted into a show in a gallery.
20:40Really?
20:40Yeah.
20:40It's a small gallery, and I only get paid if it sells, but...
20:43An artist has to start somewhere.
20:45Yeah?
20:46Yeah.
20:47Show me one of your photos.
20:50Okay.
20:51Yeah.
20:56Oh, wow.
20:59So, after Olivia was implanted with our embryo, she became pregnant with her own biological baby?
21:06Yes, exactly.
21:08Usually, the pregnancy hormones prevent the body from being able to get pregnant again, but Olivia's body managed to bypass
21:14that system.
21:15I thought we agreed that you weren't going to sleep around while pregnant, and if you did, you would at
21:19least use protection.
21:20I didn't sleep around.
21:22Who's the father?
21:23It's...
21:24Zane.
21:25The dates line up with when we were on again.
21:28Zane?
21:29Olivia, I...
21:30If I put this out later, there is more we need to discuss with you.
21:34Your fetal echo and level two ultrasound revealed that the female fetus, Olivia's biological baby, has a congenital pulmonary airway
21:41malformation.
21:42It's a cyst on the lungs putting pressure on the baby's heart.
21:45This is causing a buildup of fluid that your body is essentially mirroring, and that's why you're experiencing swelling.
21:52Okay.
21:52So, so, what do we do?
21:56How do we fix this?
21:59Well, we can try an ultrasound-guided procedure to drain the cyst, put in a shunt, and relieve pressure from
22:05the heart.
22:06And then the baby will be okay?
22:07That's our hope, yes.
22:09But there are risks that the shunt won't work, and the cyst will return.
22:13And if that happens, what do you try next?
22:18If this procedure is unsuccessful, it is highly unlikely that the female fetus will survive.
22:26So, this is the only option?
22:29It's our best option.
22:32There are other risks associated with this procedure that you both should be aware of.
22:38What risks?
22:40Any in-utero procedure puts Olivia in danger of her water breaking, sending her to preterm labor.
22:47With your baby at 21 weeks and Olivia's at roughly 17, neither would be viable.
22:56We lose them both?
22:58I'm so sorry.
23:00We know that this is a lot to take in.
23:05Oh, Olivia, I'm so sorry.
23:12Ruth has avascular necrosis in her left hip.
23:15The blood supply to the head of the bone has been disrupted, essentially causing it to disintegrate.
23:21Orthopedics is recommending a hip replacement as soon as possible to prevent further damage.
23:27What are the other options?
23:29There really aren't any. Without a hip replacement, Ruth's going to be bedridden and unable to walk.
23:34Which would be such a shame.
23:36Ruth is very mobile.
23:37She goes on excursions, takes weekly dance classes.
23:40So she gets the surgery.
23:42She spends, what, a night or two in the hospital?
23:45And then she can go back to Sinai Gardens?
23:47Not right away, no.
23:49She'd need to spend at least a few weeks at an acute rehab facility.
23:52Which will cost a small fortune, I'm sure.
23:55I'd be happy to arrange for you to speak with someone from case management.
24:01Forget it.
24:02Just keep her off her feet, keep her in the bed, and when I get back, I'll take her to
24:06get a second opinion.
24:07When you get back, where are you going?
24:09I told Denise months ago.
24:11John and I are leaving Monday on a three-week cruise.
24:14This really can't wait weeks.
24:16I'm in charge of Ruth's medical care.
24:19And I want her discharged back to Sinai Gardens.
24:30I've seen what happens when patients become immobile.
24:33They develop bed sores, pneumonia, infections.
24:37You're not wrong.
24:39I just...
24:40I just wish there was something we could do.
24:58You think I overdosed her?
24:59I think if you did, it was a total accident.
25:03Kristen's fine, by the way.
25:04I probably should have led with that.
25:06I didn't.
25:07I gave her 50 mics.
25:08I'm positive.
25:09That's good.
25:10Look, I believe you.
25:11Okay, I do.
25:12It's just, she OD'd on something, and...
25:15Okay, look.
25:16You're a doctor.
25:17I get it.
25:18When the math isn't mathing, you blame the paramedic.
25:21No, that is not what I'm doing.
25:23Okay, that's actually what Archer is doing.
25:25I'm trying to prevent this from going up the chain.
25:28I don't need your protection, okay?
25:30If Archer wants to file a report with Robinson, then he can go ahead.
25:33I'm not worried.
25:35Maybe there's another explanation.
25:37Is there something else that you observed at the scene, or...?
25:40There definitely is another explanation.
25:43But I don't have it, okay?
25:44I told you everything I know.
25:46My work was thorough, and I stand by it.
25:48Maybe you should double-check your own.
25:52You're taking this personally.
25:53I absolutely am not, okay?
25:55I'm just hungry.
25:56All I've had today is a frappe, a cinnamon roll, two bags of Pirate's Beauty, and some red vines.
26:01I am starving, and it's deep dish for lunch.
26:04It's that insane metabolism, right?
26:23Wait, you're petitioning to have a judge remove me as my mother's power-up attorney?
26:29Mrs. Evans, I understand you're upset.
26:32You got some nerve, you know that?
26:34We're concerned about your mother's welfare.
26:36Medical neglect is a form of elder abuse.
26:39You think I'm abusing her?
26:42Why don't we take this to the consultation room?
26:44You don't know what you're talking about.
26:47You don't have a clue.
26:49You look at my mother, and you see this feeble, sweet, innocent old lady.
26:54But that's not who she is, who she was.
26:58Most times, dementia turns people mean, right?
27:01Yes, unfortunately, I experienced that with my late ex-husband.
27:05Well, it had the opposite effect on my mother.
27:08What the doctor called it, pleasant dementia?
27:11Well, let me tell you, Ruth Williams, the real Ruth Williams, was anything but pleasant.
27:21When she was drunk, which was most nights, I was her ashtray.
27:28And she was even nastier when she was sober.
27:32The only names I heard growing up was fat, ugly, and stupid.
27:39She said she wished I was never born, that I ruined her life.
27:46Are you a mother?
27:47I am, yes.
27:49Can you imagine saying such hateful things to your own child?
27:53No.
27:55She never hugged me?
27:57She never told me that she loved me?
28:01So you all can stand there and judge me like some terrible person?
28:08We're not judging you, Mrs. Evans.
28:10I'm nothing like her.
28:12I am a good mother.
28:15And I am a good wife.
28:16Mrs. Evans, I am so sorry what you went through.
28:21I know.
28:23I know that I should be able to forgive and to forget.
28:29I mean, look at her.
28:35But there are some things we can't forgive.
28:42Your mother needs to be cared for.
28:45I know.
28:46But it's okay if you can't be the one to take care of her.
28:51We will look after her.
28:54I promise you that.
28:57Absolutely.
29:18Hey.
29:21The doctors are going to take care of you.
29:33That lady looks so sad.
29:37She going to be okay?
29:40Yeah.
29:55Hi.
29:57So, have we made a decision?
30:02Yes.
30:03I, um...
30:06I want to do the surgery.
30:08Okay.
30:10I've already spoken to our fetal surgery team, and there's an OR available in the next few hours,
30:14so we'll move you to pre-op shortly.
30:16I'm just going to call Ryan to update him.
30:26Hey.
30:28You okay?
30:32He doesn't agree with my decision.
30:36He said that?
30:38He didn't have to.
30:39Jacob's always wanted to be a father.
30:42Ryan and I joke that he's the quintessential dad just without the kid.
30:46Yet, he takes care of everyone.
30:49His family, his friends, me.
30:53Especially me.
30:55I was so happy to finally be able to do something for him.
31:00Look, I know that the last thing you would ever want to do is put Jacob's baby at risk.
31:06But you can't feel guilty for wanting to do everything you possibly can to save yours.
31:16Lab tests have revealed that you're what's called an ultra-rapid metabolizer.
31:20So you have a genetic variation, which causes you to metabolize certain medications faster than the general population.
31:26Leading to two potential outcomes, depending on the drug.
31:30An overdose, like you experienced today with the codeine.
31:33Or a complete lack of therapeutic effect, which is why your antidepressants didn't work.
31:39I've had this my whole life.
31:41Why haven't I overdosed before?
31:44Well, ultra-rapid metabolizers don't experience these effects with all medications.
31:47Right.
31:48It's actually a fairly narrow list of drugs you need to avoid going forward.
31:52Codeine being the most obvious one.
31:54And that's the good news.
31:55So, as far as your knee goes...
31:58Yeah.
32:00Orthopedics has looked at your MRI, and you're going to need surgery to repair the tear.
32:06There goes the season.
32:09Probably the rest of my basketball career.
32:12Look, this is not going to be easy.
32:16That's not what I'm saying.
32:17All right?
32:17But a lot of players return to the court after ACL surgery.
32:21Yeah, maybe.
32:22Look, I know this is a huge blow.
32:25So, I got you a little something to hopefully cheer you up a little bit.
32:34Oh, this is so cool.
32:37Salutes, like, my idol.
32:40Hey, I'll be, uh, I'll be right back, okay?
32:44What's next?
32:46Okay.
32:47There you go.
32:49Kristen, I would like you to meet.
32:51Salute!
32:51Hey, Kristen.
32:53You know Courtney Vandersloot?
32:55I know a guy who knows a guy who knows Courtney Vandersloot.
32:58I'm so sorry to hear about your knee.
33:00I tore my ACL this past summer in a game against the Indiana Fever.
33:03Yeah, I watched it happen, right as you were going in for a layup.
33:06Yeah, it was a huge bummer to have to miss the rest of the season.
33:09But I'm working really hard post-surgery, and I'm going to be back on the court next season, stronger than
33:13ever.
33:14That's awesome.
33:15And you will, too.
33:16And when you're ready, we'll get you down to Winters, and you and I will get some shots up.
33:20I'd love that.
33:22This is for you, by the way.
33:24Wow.
33:26Thank you so much.
33:34Courtney Vandersloot.
33:36So, the judge granted our emergency motion.
33:40They'll assign a new court-appointed conservator in the coming days.
33:45We're admitting Ruth to the orthopedic unit while she awaits surgery.
33:48They're hoping to get her on the schedule before the end of the week.
33:54Where am I going?
33:56We are taking you upstairs.
33:58I'm going to go with you, okay?
34:00I'll come visit tomorrow.
34:01Maybe I'll sneak in some rum raisin, okay?
34:03Okay.
34:06You know, it's always made me sad that I don't really know who these people used to be
34:11before dementia took their identity.
34:14But maybe it's a blessing.
34:16It lets me care for them without being burdened by their past sins.
34:22You seem to have a lot riding on this not being Novak's fault.
34:25Well, there was a potential date depending on it, but I'm not so sure about that anymore.
34:30Not that I particularly care, but I thought that you and Dr. Howard were an item,
34:35or at least on your way to becoming one.
34:38Yeah, that's what I was hoping, but long story short, it just didn't work out.
34:42Yeah.
34:43You move on fast.
34:45Carry a torch too long, you're bound to get burned.
35:01Oh, Tamika.
35:03Oh, good.
35:04I was hoping I'd catch you.
35:05Everything okay with Ruth?
35:07Oh, yes, she's fine.
35:08On behalf of the selection committee, I'd like to offer you a spot in Gaffney's upcoming nursing externship.
35:16I got in?
35:20Thank you for everything.
35:23This was all you.
35:25Huh?
35:26Congratulations.
35:28Thank you for your license.
35:39Oh.
35:40How'd it go?
35:41Is Olivia okay?
35:42Olivia did very well, as did your baby boy.
35:46And Olivia's baby, were you able to remove the cyst?
35:53Um.
35:53We initially thought that the repair was successful, but when we did our fetal heart check after, she had passed.
36:02I'm so sorry.
36:04There was just too much pressure on her little heart.
36:08Does Olivia know?
36:10No, she's just waking up.
36:12I'm about to go in and talk to her now.
36:14Can I tell her?
36:15Do you mind?
36:17Sure.
36:19I'll be in after to answer any questions.
36:25Thank you, Dr. Asher.
36:27Okay.
36:55So, if it's not too late, or, uh, you haven't invited anybody else, I'd love to go to the
37:01Green Mill tonight.
37:02What about your plans?
37:05Well, I could lie and say they fell through, but I, I don't know, I think I'd like to try
37:12honesty instead, so, um, I'm in a somewhat complicated situation, too complicated, I
37:23thought, to date, but I, I'll let you be the judge.
37:27I thought you said you were single.
37:29I am.
37:30Are you gay?
37:30No.
37:31Celibate?
37:32Definitely not.
37:34Okay, well, I'm okay with complicated.
37:38Why don't you tell me more over drinks?
37:40Okay.
37:50Colton said you went with another candidate.
37:53Oh, yes, but I truly enjoyed meeting with him.
37:57Tamika?
37:58Tamika, yes.
38:00Yeah, well, I'm glad you got the candidate you wanted.
38:03What an amazing opportunity for her.
38:04Yes, it is.
38:05Tamika is highly qualified, which is why the committee unanimously voted to accept her.
38:11Well, I'm sure a recommendation from you didn't hurt either.
38:14Miranda.
38:15No, I get it.
38:15I understand you throwing your support behind someone who would benefit from the same opportunities
38:23you were given.
38:26You know, I'm not gonna dignify that statement with a response, but I will say this.
38:31I think you should have a conversation with your nephew.
38:46Hey.
38:48So how was it?
38:50Your first day back?
38:51Fine.
38:56You sure you're good?
38:57I need an elevator.
38:59Forget it.
39:00I'll take the stairs.
39:04Hey.
39:08Okay, what's your deal?
39:10You think showing any sign of emotion is a weakness?
39:14You went through something really traumatic.
39:17I mean, we both did.
39:18I've served in combat zones.
39:19This was nothing.
39:20I understand that, but just...
39:21What do you want?
39:22You want me to start crying?
39:23Tell you I can't sleep?
39:25I'm having nightmares and I keep flashing back to that night?
39:28I can't stop thinking about it.
39:29Well, I'm sorry to hear that.
39:30I've moved on.
39:31I don't buy it.
39:32You're not a robot.
39:33Something is going on with you and has been for a while.
39:35You keep throwing yourself headfirst into these dangerous situations like you got a dip.
39:54Well, thank you for letting me attempt to make up for today.
39:58Oh, don't sweat it.
39:59I knew I had nothing to worry about.
40:01And watching you grovel is fun.
40:03Yeah?
40:05How are you so confident?
40:06The six rights.
40:07Yeah, it hasn't let me down yet.
40:09Right patient, right medication, right dose, right route, right time, right document.
40:13Go through that checklist every time I dispense meds.
40:15It's very efficient.
40:17Yeah.
40:22What do you say we get out of here?
40:23No.
40:24No, no.
40:24Okay, trust me.
40:25These ailments are worth the wait.
40:27There are other ways to make up for today.
40:36You want to go back to your place?
40:38Yes.
40:39Yes.
40:40Just give me one second.
40:42Yep.
40:42Yeah.
40:43Okay.
40:44Okay.
40:46Okay.
40:50Okay.
41:04Okay.
41:05Okay.
41:07Okay.
41:11Okay.
41:18Okay.
