07:01Matt's on more familiar ground with his latest patient.

07:15Poppy is a Rottweiler and she's a lovely girl.
07:18She's unfortunately been diagnosed with a cancerous tumour on the side.
07:25This lump, it's appeared quite quickly and it's really hard and firm.
07:29We've tested it and we know it's what we call a mast cell tumour.
07:32Now, these tumours can spread rapidly and they can be really quite nasty.
07:36And the best way to get rid of this lump is to excise the entire thing.
07:42This isn't ideal timing for Poppy or her owners, Lauren and Tracy.
07:47Today, it's Poppy's seventh birthday,
07:50so she will be spending it here having her operation.
07:54It's definitely words you don't want to hear when your dog's got cancer,
07:58especially with her being a rotty and being seven-year-old now.
08:01She's not exactly young, so it's always a worry.
08:05Come on, sleepy time.
08:07Even though this lump doesn't look massive,
08:10it's going to be a really big wound
08:11because I need to take three centimetres circumference around it.

08:17Because that's how far the cancer seeds from the actual tumour itself will spread.
08:22So, to make sure this doesn't come back,
08:23we're going to have to take a big chunk of tissue away.
08:26So, this is certainly one of the difficult parts
08:30as I don't want to bust a disc because she's a good 50kg.

09:06but I have to say, drawing on a dog is a little bit different to painting on paper.

09:14how much tissue needs to come out.
09:16Because even though the tumour's here,
09:18the tumour cells could be anywhere within this circle.
09:25Unfortunately, it's the mast cell tumour.
09:27We do know that it can spread quite rapidly around the organs.
09:30And from the tests that we did,
09:31we know it's quite a rapidly dividing tumour.
09:34We've picked up on it early.
09:35It's not spread yet to the liver or spleen.
09:37But you never know.
09:39Biology always likes to throw you a curveball.
09:41But this is the best that we can do for Poppy.
09:44She's such a happy dog.
09:46And she loves playing with other friends at her daycare.
09:48And she loves spending time with family.
09:51And she's always got so much love.
09:53And just hoping that she comes around well after the anaesthetic and stuff.

10:59So my nerves are starting to build.
11:02And I thought, well, the best thing I can do is come to see a friend of mine.
11:05He's an expert on showing.

11:09This farm on the hills outside Halifax
11:12has been run by Stephen Short's family for over a century.

14:04Because I'm taking such a big tissue mass out, there's going to be a lot of blood vessels that I'm
14:11going to come across,
14:11then it's going to look quite bloody.
14:14It clouds the surgery field and does make everything a little bit harder.
14:17But step by step and hopefully we'll get the mass off all as one.
14:25Now I've made my incision, you can see how massive a surgical area this is.
14:30It just illustrates as to how nasty cancer can be and how quickly it can spread.
14:36We need to find out exactly where that tumour sticks to and remove all of its base.
14:43So what I'm going to do is pretty much mine underneath the tumour.
14:47Be careful not to touch it because I don't want to rupture it or touch it and risk seeding anything
14:51out.
14:51And take the whole lump off as one.
14:56Beautiful.
14:57Look at that, that is exactly what we want.
14:59It's coming away really nicely.

15:11I think the plant is, I want to run some of this skin and I think I've got enough tension
15:14on it to pull everything up together.

15:18The wound that I've just created is massive.
15:21Now it becomes the trickier bit and the longer bit.
15:23The only way that I can bring that wound together is pulling together all the fat from the surrounding tissue
15:29and bringing those edges of the skin closer together so that it will make for much easier stitching.

15:40Unfortunately, when you've got wounds like this, if they're under a lot of tension, if you don't manage them correctly,
15:44they can break down and that's the last thing that I want.
