02:52And they're always a concern to a veterinary surgeon
02:54That she's not quite getting on with it
02:57She's been in labour for some time
02:58But she's not progressing
03:00We got her in a couple of hours ago
03:04To have a feel
03:04And she's just not opened up at all
03:06Good girl
03:07In these cases, time is of the essence as well
03:11I am concerned that she has been on stuff for some time
03:17And the significance of that is
03:19That it is possible that the calf can die off inside
03:22So I want to get there as soon as I can, really
03:24She's a bit unsettled, really uncomfortable
03:31She's a limousine crossbowl
03:32So that can sometimes throw a big calf
03:35And that's always a worry

07:51To help a guinea pig called Vader
07:53There's a large wound on her back
07:56It's not been healing
07:57It was initially an abscess that burst
08:00And these things can often get better with medical treatment
08:03But she's been unlucky
08:04And it hasn't
08:06To try and solve the problem
08:08Rohin's thinking outside the box
08:10Looking at Vader's back
08:11There's not much skin to work with
08:13It may not be possible to stitch that together
08:15In the way we normally would
08:17We may have to do something called a skin flap
08:19A rarer technique
08:20Borrowed from human plastic surgery
08:22Vader, who was brought in by Amanda
08:27Is one of a pair of guinea pigs
08:29Who are doted on by her children
08:31She's the quieter one of the two guinea pigs
08:35She's named after Darth Vader
08:37She's my little boy's guinea pig
08:39And they're lovely
08:40They're a really nice pair
08:41She's a sweet little thing, isn't she?

08:44They're really important to the children
08:47And they would have come with me today
08:49To drop them off
08:49But they just felt a bit too nervous about it
08:51To help Vader

08:56So, I'm expecting a big squeak here

09:04Guinea pigs are one of the highest risk animals
09:07For anaesthetic
09:08So we need to be very careful
09:10How we go about it
09:11I've wrapped a little feet up in bubble wrap
09:15Because she might be asleep for quite a while
09:17It's really important to keep her warm
09:19And they do lose a lot of heat
09:21Out of their feet
09:22I have to be quite careful when clipping it
09:26Because if you squeeze it
09:28The cheese wants to come out
09:30Oh, it smells as well
09:33It looks even bigger now
09:35You can see all the hair's gone around it
09:38There's infected material coming out of it
09:39Yeah
09:39I mean, this is the issue we've got
09:42That we're going to have to remove the infected material
09:44The wound's probably going to become even bigger at that point
09:47To actually get this closed
09:49Is going to be our biggest challenge
09:50Step one's just tackle this nasty infected stuff, isn't it?

09:58Guinea pig pus is thicker than human pus
10:01It tends to form this solid cheese-like material
10:04And often it does require removal under surgery
10:06Like the way we're doing now
10:07If we can't get this wound to heal
10:11There's a chance she may have to be put to sleep
10:13Which would be devastating
10:14We have broached the subject with our owners
10:17Because if the wound is not going to heal properly
10:19She won't get a quality of life
10:21And it does mean that it's quite a high-stage surgery
10:23I feel really nervous, Farrah
10:26Because she's only little
10:27And obviously it comes with risks
10:29But hopeful that we can come to a resolution
10:32And she'll recover

18:52The more nervous I get about how to get this wound closed
18:55But it has to be done
18:56She's doing fine under her anaesthetic
18:59It's a lot riskier than a dog or a cat
19:02But she's doing really well
19:04It's a big undertaking this, isn't it?
19:07Especially for such a tiny animal

19:29The more I take off, the more risk we're creating
19:31But this is all infected skin

19:43The challenge now is wound closure, isn't it?

19:51We will create a second wound in the shape of a V
19:55That will allow the wound to stretch more
19:57If we have more skin to work with
19:59We do end up then with two wounds to close rather than one
20:02But the amount of tension on the first wound is much reduced
20:05It's all in the measurements
20:08I'm happy with the lines I'm drawn
20:10I'm going to make my second cut now
20:12And we can do this skin flap
20:15Right, so far so good
20:21So now we can close up the original wound
20:28That will stretch the V into a Y
20:30We then close up our Y
20:32But the overall tension on either of them is less
20:35Very impressed with that, Rowan

23:27Lucy and I have taken quite a few x-rays of Emmy's legs
23:32There's quite a bit of arthritis around this joint here

23:36It's not that common in dogs
23:38But you do seem to get a disproportionate amount of pain
23:41With foot issues like this
23:43So my plan is I'm going to do an injection
23:45Into this joint of a steroid
23:47And into this joint as well
23:49That will reduce some of the pain
23:50And some of the inflammation
23:51Which hopefully will help
23:53All right, so
23:55She's waking up a little bit
23:57If I come round this way

24:01This is a long-acting steroid
24:04It works quite well for the little joints of the foot
24:07It's not something that we do very often
24:10But for foot pain
24:12It's quite good

28:53And creative stitch-up of her infected wound
28:56It ended up being quite a radical surgery that we did with Vader
29:00It's not something we do commonly
29:02And not something I've personally done before
29:03But it should all be worth it if we see her today
29:06And everything's healed okay
