00:52Malcolm this looks horrendous on this episode of paramedics i reckon that's more than 20% burns
01:01all right a much-loved grandfather could lose his feet after a crab cook-up gone dreadfully wrong
01:08pain out of 10 what would you give it probably better go on there's blistering and skin already
01:14peeling off on his feet and hands Malcolm is an absolute emergency you've done a number on yourself
01:20Malcolm hello beautiful gal she's really fly isn't she intensive care paramedic lisa helps a one-year-old
01:29in trouble after she was found convulsing in her cot
01:32pausing so yeah her heart rate's up for a baby the first sign is the fast heart rate and then they drop off the cliff
01:40sorry mate sorry mate really deep breath and chris and sam fear a teenager's reckless decision
01:49to scale a spiked fence could have catastrophic consequences charlie's ripped the skin up his
01:56forearm i'm really worried that he could lose the arm altogether breathe in breathe out you're doing really well
02:13how do you get along with your fellas family and stuff do you know what they're great yeah i've um
02:20i've been in the family for 13 years now yep i'm like the daughter they never had yeah
02:24it's a quiet start to the night shift and ben's using the downtime to find out a little more about
02:31his new paramedic partner mim you'll love this they just went away for a month caravanning yeah so i
02:39bought 160 mini ducks yeah and i hid them everywhere in their house 160 yeah just for fun are you kidding
02:50no and they just keep finding them like they put shoes on they found a duck this is next level she
02:57said she's got 44 to go this is beautifully insane
03:03while ben makes a mental note to always stay on mim's good side
03:09sa ambulance service tom exactly what happened a panicked call for help is coming into the south
03:15australian emergency operation center okay so my father has just spilled boiling water all over
03:21he's burnt his pants his legs and his feet and his stomach a devastating accident at a family beach house
03:29has left a 74 year old grandfather with shocking burns we're just running water on him at the moment
03:35just keep him under that water okay yep i'm organizing help for you now oh god
03:44all righty mem we have a burns case oh yeah let's have a look as the closest ambulance to the remote
03:53coastal location mim and ben race to the time critical case we have a 74 year old male spilled boiling water
04:02on hands feet and abdomen they are estimating it's more than 18 percent of the body area burn though
04:09so that's like level one trauma yeah already my alarm bells are ringing because burn jobs can be
04:14really really complicated first of all how much damage to the tissue is there are we talking loss of
04:21toes loss of limb it's going to be a difficult one to manage potentially as well as that skin keeps the
04:28heat in the body so a burn injury can lead to someone becoming hypothermic which is a life-threatening
04:34condition well i wonder if he's maybe been cooking it's like a pot of pasta or whatever he's dropped it
04:40on him maybe yeah oh poor thing it's so painful there's a chance he'll need skin grafts yeah
04:46hello hello just this one here waiting for the ambulance are the 74-year-old's worried relatives
04:58all at the beach house for a family get together how we doing where about dizzy the crab tub did you
05:08guys go crabbing today all right instantly my question of how do we burn so many different
05:14parts of the body is answered there is a crab pot on the ground which holds at the very least 15
05:21liters of boiling hot water hello sir my name's ben i've got miriam so i'm really worried at what i'm
05:27about to see all right what's your name yeah malcolm this looks horrendous my worst fears are confirmed
05:38from what i can see i know that malcolm is an absolute emergency
05:56back in the city it's also a big night for intensive care paramedic lisa
06:00my very first shift back um i'm feeling a bit nervous but i'm excited to get back out there see
06:08my colleagues and um practice the job that i absolutely love for the past year lisa has swapped
06:15paramedic duties for a new role as a first-time mum i've absolutely loved having the last 15 months
06:22at home with my baby boy sully it's been amazing to see him grow and to see his curiosity for life
06:28um and i'm definitely gonna miss him is that your baby are you gonna feed your baby
06:39cheers for lisa and her wife pru the road to motherhood hasn't been easy
06:47i've always been obsessed with babies and children but being gay it's hard to know what that journey
06:53would look like and i knew that i had gynecological problems with endometriosis as well so it was going
06:59to be tough lisa endured the heartbreak of two unsuccessful rounds of ivf it was really devastating
07:09you do lose hope a bit yeah but finally lisa decided to give ivf one last try
07:18this time it worked can you do it wow what about this one yeah that's a good one thanks mum so it's
07:30just the best like you smile and laugh like you cannot even bottle that up oh it's just the greatest gift
07:41it means everything this one
07:57sa ambulance tell me exactly what's happened my one-year-old daughter has had a compulsion or she
08:03is still having one not so she's not responding at the emergency operation center a worried mum
08:11is on the line after finding her baby convulsing in her cot she's laying my arm is she breathing
08:19she's breathing but heavily and it's actually pulsating i'm organizing help for you now so stay on
08:26the phone i'll tell you what to do next okay
08:34on lisa's first day back from maternity leave
08:37a pediatric emergency is a challenging first-up case it's incredibly scary for the family is to
08:44see this type of thing seizures in young children have a wide range of causes from a high fever or
08:51infection to head trauma and epilepsy oh we're just about to approach any seizure in a baby can be
08:59life-threatening the shaking the loss of consciousness it means that they are not really protecting their own
09:05airways hi everybody i'm lisa where are we going and what was her name eliana if the airway is compromised
09:13for too long the worst case scenario is that that can lead to permanent damage to the brain and cardiac arrest
09:20hello beautiful gal yeah she's really fly isn't she
09:37all right we'll get a very quick couple of observations you've done a number on yourself malcolm
09:42paramedics ben and mem have just been called to 74 year old burns victim malcolm tell me the story
09:51what happened i've finished cooking for crabs yeah and i've got the boiling water that's left in the
09:56target yeah what do i handle on that lifted it out yeah yeah yeah so pain out of 10 what would you give
10:09it probably down on we'll give you some pain relief all right malcolm's trembling like a leaf
10:17i can see that the skin is already peeling off so malcolm needs to see a surgeon immediately
10:25i think he's fallen and this arm has come over the top of the hot bugger malcolm's close-knit family
10:34was enjoying time together at their beach house when the accident happened have we done any calling
10:40guys cold water 25 minutes 25 minutes thank you just with the hose his quick-thinking daughter-in-law
10:45sarah jane immediately grabbed the hose to call her father-in-law down if we go nine percent there
10:53then i reckon if we go four and a half we've got another one there that's another five it's more
10:58than 20 burns all right can i have a look at the underside of your arm yeah we've got potential
11:05circumferentials the severity of malcolm's burns means that he's at the risk of developing what we
11:11call compartment syndrome this is where the skin becomes really tight the blood is essentially
11:17cut off from the end of the limb and it will eventually die the worst case scenario is that
11:23he's going to lose his toes his feet and potentially his hands i'll get the bed if we just get him in the
11:29back and just get the heater on because he's he's cold as well as the risk to his limbs mim and ben
11:36are increasingly worried malcolm's life could be in imminent danger from a rapid drop in body
11:42temperature hyperthermia is a huge risk in geriatric burns his body doesn't have the ability to regulate
11:49his internal temperature as well as someone that's a bit younger so we need to get his temperature under
11:53control and we need to get moving now we'll get you in our ambulance and warm you up all right
12:09you're about the same age as my little one intensive care paramedic lisa is on an urgent call out for
12:16one-year-old eliana after mum vicky found her having a seizure okay what's happened mom um actually
12:24she went to bed yeah and um everything was normal then she was crying we went to work yeah and i felt
12:31she was really hot tried taking everything up and then i could tell that she was just not not quite
12:36responding yes oh you've done everything right okay i can see that eliana's seizure has stopped do you
12:45want to hold her but she's looking really floppy which is incredibly concerning so i need to
12:52take some observations to work out how serious this is all right we'll check her temperature
12:59look at her little ponytail she's got so much hair
13:0540 okay so she is very hot i'm shocked 40 degrees is extremely high extremely dangerous
13:13it's no wonder she's had the seizure my concern is that she'll have another one she probably has
13:18a underlying illness it's a matter of trying to work out what that might be all right so we'll monitor
13:23her heart now with her temperature skyrocketing lisa's worried eliana's body is fighting a
13:30potentially life-threatening infection mama's here i'm just gonna put this one here
13:37if her heart rate is elevated and with her temperature so high these are signs that she
13:44could be in a state of sepsis which is when nasty bacteria has got into the bloodstream
13:53poor thing so yeah her heart rate's up eliana's heart rate comes back at 200. this is double what it
14:00should be we want to get panadol into her the next thing that could drop is her blood pressure
14:08or her level of conscious state that's incredibly dangerous now that that paracetamol's gone in
14:15it should take effect and start lowering that temperature lisa has called for urgent ambulance
14:21backup to transport eliana to hospital fortunately it's only minutes away
14:26you've got a lovely family around number five oh my gosh you are amazing
14:39vicky is a rock she's being incredibly strong but i can see some really concerned eyes
14:45i really feel for this family and the concern that they've got for eliana right now
14:50if anything happens like she has another seizure i'm right here okay so i i can definitely support you
14:55through that we've got medication if we need to i'm really worried for eliana if she has a really
15:02nasty infection that has got into her bloodstream organs can't cope and they start to shut down
15:17malcolm what are you gonna do pop it in your mouth take a couple of sips on it that's it
15:22paramedics ben and mim are anxious to get burns victim malcolm into the warmth of the ambulance
15:29before his body temperature becomes dangerously low do we want to get some plastic bags on his feet
15:35and then he can take a stand because otherwise they're going to get dirty and infected but before
15:39the 74 year old grandfather goes anywhere mims come up with a practical solution to protect his raw skin
15:46from infection multi-purpose bin bags hey look like cinderella perfect size burn injuries have lots
15:55of serious complications if they develop infections they could end up with sepsis it's gonna need a good
16:01clean so plastic bags for his feet is not textbook but you know what a brilliant idea let's take that
16:09just have a step up and then pop your bum up here oh don't put your pressure on your hand that's all
16:14that's right that's all right yeah yeah yeah that's it anxiously watching on a malcolm's wife faye
16:21his children and grandchildren we're going to go to the royal adelaide just because the burns it's
16:26the only place that deals with it really seeing their beloved papa go to hospital is the last thing
16:33they expected when they arrived for a get together at the beach we'll do cling film on the legs then we
16:39can go blank it over the cling film because otherwise it's going to get stuck high tech stuff
16:45we all love him in this family he means a lot to um all of us keep breathing on the whistle
16:53malcolm loves his shack he doesn't need to go anywhere else in australia because this is his little piece of
17:00paradise definitely was not the end to the day that we would have hoped for how are we feeling still
17:08nine out of ten pain oh probably eight where's the worst pain days around the knees right right
17:16foot all right let's get you going i reckon i am genuinely concerned that malcolm's at the point now
17:23where he's going to lose his feet that is a huge disability and my heart goes out to him let me
17:30know when you're ready to go i'm ready to just go yeah all i can do now is treat his pain and give him
17:36the quickest access to a burn surgeon that he can get and hopefully that will be enough
17:53oh the poor little one looks like the ambulance has just got here actually intensive care paramedic
18:04lisa is moving fast to get baby eliana and her mum vicky to hospital we're going to rug her up and
18:11we've got the heater on in there they urgently need to find out what's causing the one-year-old's
18:16dangerously high heart rate and alarming temperature if you give her lots of kisses we'll just get this
18:24harness on mommy's here
18:29it's been a distressing night for the whole family after witnessing their baby sister having a seizure
18:37bye mama hello beautiful yeah i'm just gonna sit here so i can keep an eye on things
18:43how long in total do you think the shaking lasted yeah i am fearful that eliana is fighting a nasty
18:51infection a young person definitely doesn't have as many coping mechanisms as an adult the first sign
18:59is the fast heart rate and then they drop off the cliff so i'm watching eliana like a hawk
19:04it's so heartbreaking when you hear them cry like this isn't it tonight is lisa's first shift back
19:15after being on maternity leave with her son sully eliana's inconsolable so i'm thinking what have
19:22i learned in this last year of motherhood it's okay my dear what tricks do i have to try and calm her
19:29down does she watch any tv like bluey if i could break the road rules then i would put eliana back
19:35in mom's arms but we have to transport eliana safely we're almost here cuddles are coming my dear
19:46luckily the crew are just moments away from hospital the best medicine for her now is going to be a cuddle
19:52from you the nurses and doctors will get to the bottom of what's caused this temperature and why she's
19:57had a seizure here's mommy only time will tell if she's going to get worse and that's why it's
20:04important that she's wanted to closely oh see mom she just wanted your cuddles i'm so sorry i know it's
20:10just stressing with eliana now safely at hospital lisa's mind turns to another precious one-year-old
20:18i'm gonna go home tonight and even if sully's asleep i might poke my head in and hold him close and just
20:23appreciate those happy healthy moments all right i'm gonna give you some morphine get rid of the pain
20:41i'm gonna give the hospital ring and let them know you're on your way all right
20:45paramedics mim and ben are transporting malcolm from his coastal beach shack to a city hospital
20:54as quickly as possible uh yeah the 74 year old sustained shocking burns when he tripped over
21:02carrying a crab cooker full of boiling water so what are we bad we've got bilateral feet
21:09legs abdomen right arm is it all partial thickness or is there some full oh it's mixed we've got
21:17potential for a circumferential foot burn and around the fingers on the left hand okay a circumferential
21:24burn is time critical because if you burn your limb all the way around that can stop your blood supply
21:30due to the swelling which can cause tissue death so malcolm could lose a limb that one looks like the
21:36worst i reckon your skin's coming up all over it all the way up to the nail yeah there we go what
21:43a stoic man i'd be crying for sure you're a pretty brave dude yeah you've impressed mim
21:50you had a successful day crabbing did you yeah yeah 28 perhaps it's pretty good yeah fair
21:56yeah we're roughly pulling in now ben and mem have done their job keeping malcolm's mind of his
22:07horrific burns it's now time for the doctors to take over you're a tough cookie but you've done
22:13yourself a number yeah i have no doubt that the doctors will take one look at malcolm and rush him
22:20straight to surgery and the recovery process is just going to be brutal has he fixed you yeah just
22:29you're just thinking about those crabs you've missed out on i think oh that's what we want
22:35i just really hope that malcolm can be home with his family eating those crabs like he deserves
22:41let's get you in malcolm
22:50i'm going to tell you something and you're going to tease me about it hit me with it housemates and
23:02paramedic partners sam and chris know a lot about each other but there's one secret chris has never
23:08revealed about his university days until now when i was studying kimber movement we did a topic called
23:16skill acquisition okay and i was tasked the skill of rhythmic gymnastics i think i pulled off a pretty
23:24good uh routine i started off with a bit of a jumping i don't know what they call it okay a bit of a
23:31twirl twisting around yeah i think my most impressive move was holding the ribbon up spinning it around
23:38over me and just letting it fall to the ground as i slowly knelt down and i had to feel myself film yeah
23:46where is it oh i don't know i need it i don't know chris tells me some weird things but you know
23:53this is right up there the thought of chris in some lycra spinning around with his twirling ribbon
23:58she's like i can't get that out of my head oh that video will never see the light of day
24:02say ambulance tell me exactly what's happened yeah hi uh my mate just jumped the fence and cut his
24:13eye over me sorry take a breath at the emergency operation center a distraught teenager is on the
24:20line he's just witnessed his mate's disastrous attempt to scale a two meter high mental school fence
24:28the gate to the back usually they unlock it but it was locked so he tried jumping over and the spikes
24:33on the top of the gate cut straight to his arm just confirming where is the wound on his arm his entire
24:39bottom of his arm is cut open it is bad okay i'm going to get you some help just stay on the line
24:45please just try and be quick it looks really bad all right sam we are off to a college for a 17 year
24:55old male there's not much information but it says lower arm is ripped open and they can see the muscle
25:02oh wow yeah this patient is at a real risk of going into hypovolemic shock meaning he's lost so much blood
25:11that he can no longer supply enough blood to all of his major organs which will mean they will start
25:16to shut down uh clear left as well as extreme blood loss the boys are worried about the long-term
25:24damage to the teenager's arm they've already got arteries veins nerves the other thing is this could
25:30be a huge infection risk as well he's only 17 and to potentially have to lose your arm at that age
25:36that's that's catastrophic i'm really worried that he could one lose function in his arm or two
25:43potentially lose the arm altogether which would completely change the course of his life
25:48is this guy for real wow
26:04it's an ambulance service okay tell me exactly what happened i just picked up a big prompted
26:10my leg hurts so i can't get up i can't stand up a distressed woman is calling in to the emergency
26:17operations center after an agonizing fall in her front yard
26:29try to stay still as you can until we can get that help out there okay
26:36intensive care paramedic bill is now on his way lights and sirens to the 72 year old
26:42they're concerned this patient might have a fractured femur the femur is that big long bone
26:48that makes up the majority of your leg and if you do break that that's extremely painful and you can
26:53actually lose a lot of your blood volume into that leg if it is broken sometimes the bone inside the
26:59leg can actually sever an artery oh there they are poor lady she then will have a huge lack of blood
27:06supply and oxygenation to that leg and worst case scenario she could lose her leg
27:13hello what's your name carolyn carolyn my name is bill lovely to meet you i'm just going to hold
27:18on to your wrist for a quick tick what happened well i just took the rubbish bins in and tripped over
27:23that bit of concrete there oh do you mind if i have a bit of feel down here this all feels okay
27:26that's okay i didn't lose consciousness you didn't hit your head no loss of consciousness lovely
27:31well you've got all the medical terms are you medical or it's hard now so good thing to do
27:38there's no pain um anywhere except here nothing in your lower back or in your bottom
27:43no and if i feel your hips no it's fine
27:49that feel right to you that sharp notch there because i mean right there that feels like that
27:53could be a potential bone sticking out as i'm feeling carolyn's leg there's something that's not quite
27:59right should be smooth and rounded but this is actually quite jagged and sharp this is definitely
28:04a shocking break g'day deep how are you a backup ambulance crew has now arrived to help bill with
28:12what will be a difficult extrication just take a piece of pus into for your mouth and that for your
28:17nose all right moving carolyn onto our bed is going to be excruciating so we've got to try and treat her
28:23pain from a multitude of levels so we'll draw up fence and we'll go 100 mics and two mils if that's right
28:28just a bit of pressure here do you take any regular medication at all no fit and healthy
28:34and you're retired how many years 10 years yeah my job is to have coffee with the girls
28:43that sounds like a lovely life all right we're going to give you some fentanyl we'll start you
28:49with 25. right jolly good with the pain relief taking effect the team can begin the challenging process
28:58of moving carolyn all right take a few deep breaths in for us my love all right now this one here
29:04and i want you to take 10 really big breaths in through your mouth out through your nose i've given
29:09carolyn a hell of a lot of pain relief but this is not going to be comfortable so she's going to have
29:14to be brave ready on three yeah one two three
29:29so it says a student will flag us down paramedics chris and sam are racing to help a 17 year old
29:36who has ripped open his arm while trying to climb a metal fence at school all right this is it
29:45i can see the fences and to me they just look like giant metal spears he is so lucky that he
29:52hasn't taken his arm right off g'day charlie my name's chris this is sam what's happened today mate
29:59i'll just jump the fence okay is that one of those fences that we saw when we yeah came out okay and
30:08what's happened to your arm as you've jumped the fence it's too low okay how much pain are you in
30:13at the moment made out of ten seven seven okay charlie seems quite disengaged and i'm still trying to
30:21work out is that because he's in so much pain or is it because the injury is so severe that he's starting
30:26to lose blood supply to his brain has it been bleeding through quite a bit yep okay 120 on 80.
30:35chris urgently wants to check how much blood charlie has lost but first needs to get the
30:40teenager's severe pain under control take small breaths in through your mouth and out through your nose okay
30:50my mates do you it was charlie's best friend jacob who made the panicked call for an ambulance
30:56after witnessing the horrific accident i looked down and his arm is just bleeding it's sliced open he's
31:04holding it he's looking at me he's freaking out he didn't know what to do my immediate thought was
31:10he's gonna lose his arm to be honest it was terrifying seeing one of my best friends go through
31:18something i would rather die than experience was very tough
31:21unfortunately i'm gonna have to just unravel it a little bit mate so i can have a look
31:28take five really big breaths for me keep keep breathing keep it in your mouth don't take it out
31:35of your mouth well done mate all right mate as i peel that last layer of bandage off i can see that
31:43charlie's ripped the skin up his forearm i can see his muscle clear as day as well as all the fatty
31:50tissue underneath fortunately no severed blood vessels really deep breaths okay i'll try and be as
31:56gentle as possible this injury is even pretty gruesome for me to see i can't imagine how charlie's
32:03feeling knowing that one bad decision could potentially alter his life forever breathe in
32:08breathe out you're doing really really well
32:10all right darling you okay you can do this retired nurse carolyn suffered a disastrous fall
32:27when she tripped and fell while bringing in her rubbish bins so everyone ready on three yep one two
32:33three bill's worried the feisty 72 year old has broken the bone that joins the thigh to the hip
32:40and we can go down two three lovely that's the worst of it mate good friend and church pastor will
32:48is supporting carolyn through the ordeal and is concerned about the impact on her life down two
32:55three beautiful piece of cake carolyn's a force of nature that lady she's a very active person she's
33:03always out and about doing stuff and the idea of her being confined i i don't think it'll suit her
33:08well so that worries me well let's have a quick look yeah yeah i'd say we've just done a high
33:14extremely hot she's always stoic she'll always find the the positive in something so seeing her in
33:20pain yeah it's hard thanks for all your help will don't worry i'll get over this what we do mate you
33:30take care i'll see you soon i will god bless you thanks all right i reckon we are ready to rock and roll
33:39carolyn's potentially got some bleeding internally that we can't see the damage that this could do to
33:43her long term it could be irreparable and for someone like carolyn who's so fiercely independent
33:48this could be devastating now how is your pain going oh i can feel the floor yeah good can you
33:55wriggle your toes lovely
33:57you guys do a great job oh we say the same about you nurses really i actually started nursing later
34:09in life i was a financial planner for a mate and one day i said to myself i don't want to do this
34:16anymore if you touch i want to be a nurse really put myself through yeah right carolyn love being a nurse
34:23and helping people in their time of need and i can relate to that because it is exactly what i'm
34:28doing right now all right let's get you some panadol and it's a privilege to be able to care for
34:33someone that has cared for others for 40 years and absolutely loved it have you been back to
34:38lindland i'm flying back in july i'm a londoner i'm from surrey with the pain relief kicking in
34:45carolyn's curious to learn more about paramedic deep who is a fellow bred i'll bring you back
34:51some bits from marks and spencers all right oh fun facts and probably a bit personal about me but my
34:56mum always used to buy me jocks from marks and spencers because they were the best and i reckon
35:01i still have some in my wardrobe to this day and the last time i got them was about 15 years ago

35:12all right i think we're just coming into the limac although carolyn's in good spirits she knows the
35:21serious injury could put her cherished independence at risk see you later carolyn have a nice day

35:38carolyn's going to need every ounce of her feistiness to get her through this
35:41surgery and rehab is only the beginning she might have chronic pain who would have thought
35:46you pull your bins in you have a fall and then your life changes forever
35:49hey mate listen to me do not take it out of your mouth i promise it will help and then we've got
36:01something stronger coming all right 17 year old charlie is in agony after ripping open his arm
36:07while attempting to jump a metal fence in your mouth mate you're doing really really well the wound is
36:15extreme and chris and sam are worried about potential loss of function in his arm it's really
36:21important especially with an injury this severe to try and work out if he's lost blood flow to the
36:26fingers or if there's any nerve damage i'm touching your hands here does that feel the same yeah on all
36:32the fingers yeah okay can you wriggle your fingers yes okay charlie's got good sensation in all his
36:39fingers his fingers are a good color so i'm a little bit more comfortable that he hasn't done damage to
36:45any of the nerves or any of the major underlying blood vessels so the plan will be we're going to
36:51bring our stretcher all the way to here we'll help you do you think you'll be up to taking a couple of
