01:05he kept lapsing in and out of consciousness I'm really worried I'm gonna lose him Steven we're
01:14going boy or girl I think it's boy Ben and Mim race to a baby in a hurry I'm driving as fast as I
01:21can safely do so this is gonna be the first time I get to catch a baby and I'm really really excited
01:27and what has caused the much-loved Leo to collapse he could be having a stroke a heart attack a seizure
01:38all of these things are incredibly concerning he's on the right thing calling us I'm in to car MBA
01:59possible entrapment yeah paramedic partners Mim and Ben have only just clocked on for the night shift
02:07say Paul look like they're on scene fire look like they're on scene and their first job is a shocking
02:15car accident where is it how allow it is that speed along there it's like 90 K if that is a head-on
02:22collision that's 180 K's straightaway alarm bells are going off when you're going at those speeds you're
02:29at a very high risk of having catastrophic injuries I'm worried about spinal trauma internal bleeding
02:37but also head injuries even though we got airbags and seat belts and all side impact bars it doesn't
02:42stop things like your brain from hitting the inside of your skull your skull is a closed container if you
02:50damage or you create any bleeding inside that the blood's got nowhere to go so what ends up happening
02:56is your brain starts to push through the bottom of your skull that's called coning and it is a life
03:02threatening injury fortunately Ben and Mim are just seconds away from the crash oh here we go as they
03:12get closer they're shocked by what they say that does not look good no that's everything everywhere
03:26one for two copy that intensive care paramedic Lisa has just received her first case of the night I'm
03:41going to an 87 year old male who was walking to the toilet when he's collapsed unconscious in this age
03:47bracket there's so many things that can go wrong he could be having a stroke a heart attack a seizure all of
03:54these things are incredibly concerning as Lisa raises to the man an update comes through he's
04:03got a diagnosis of dementia so quite confused usually he's being cared for by his wife what a
04:10responsibility I can very much relate my grandma and granddad actually they both had dementia and
04:15Alzheimer's grandma's favorite spot yeah oh do you miss her very much yeah yeah every day it's been
04:27four years since Lisa and her twin sister Sam lost their beloved grandma Lenore lots of fond memories she
04:36played a crucial role in their lives at a time when their single mother was struggling to cope our child
04:43it was quite challenging mum developed a addiction to gambling so the pokies so we were left at home
04:50from quite a young age to fend for ourselves really so grandma was more like our mum she loved us to death
04:59and would do anything for us so when Lenore developed dementia Lisa and Sam immediately stepped up to care for
05:07her the twins the twins were only 19 years old when they took on the responsibility we took her shopping into all their medical
05:16appointments and she always would light up when we kind of came to visit her it was an absolute pleasure to be able to look
05:25after her in those like final years having lived that experience with grandma it's at the forefront of my mind when I go to patients now
05:34hello it's the ambulance service hi are we here for Leo if you lead the way
05:46where is he oh my gosh what's happened 87 year old Leo has been unable to move since he suddenly collapsed hello Leo my name's Lisa how are you feeling wife Helen is worried because it's his second fall of the night
06:06tonight tonight his legs just go away does he fall often no not really but you've got dementia would he know what day it is what day is Leo
06:15he would have no that's okay it's a funny one isn't it sometimes I don't even know what the day is
06:21does he have much of a medical history any trouble with his heart blood pressure bypass okay Helen tells me
06:30that he has had heart surgery before this does ring alarm bells it means that his heart is fragile and
06:37weak he's done the right thing calling us it is possible that he could be having a heart attack
06:52that does not look good no Ben and Mim are arriving at the scene of a confronting two-car accident
06:59you look at the cars and you instantly think well how can anyone survive that the vehicles just look
07:06like scrap metal there's glass oil it's just a mess you can barely tell what's what I'll just walk straight
07:13in yeah you go in hello what was your name separate yeah hey my name's Mim it's gonna get a few of your
07:23observations all right when there's a scene this large and there's multiple patients it's really
07:28important to make sure you delegate yourselves and split the crews accordingly I get assigned to
07:33the driver of the vehicle and he looks shocked the teenager was coming back from the gym when the crash
07:43happened I can see that he's so remorseful that he's turned into oncoming traffic and it was not his
08:00intention but I think he's realized that one lapse of concentration changed everyone's lives forever
08:07while Mim continues assessing sebbet Ben needs to check on the driver of the other car
08:15Stoic Shane was on his way home after a few days away when the accident happened how fast do you
08:30reckon you were going I guess 30 managed to hit the brakes or anything like that nah you're gonna need
08:35checking over mate he's telling me he's all right but one of my concerns is with the adrenaline that's
08:41probably flowing through his veins he could have absolutely anything wrong with him and have no
08:46idea just because he's so shaken up watch yourself in the cars yeah we're also on a busy road I don't
08:55want him to walk into a truck so I'm trying to get him to another ambulance where he can get seen and
08:59assessed while Shane is checked over by another paramedic crew sebbet is still concerning Mim how do you feel
09:09good Samaritan Desma witness the crash and still can't believe anyone got it alive I just came up to
09:25this intersection here ready to stop at a red light and heard a horn beep and like looked up to see
09:31where it was and just saw these two cars come together here smash both went up in the air I didn't even turn
09:37the car off and just ran across to them tried to get the door opened it took a while but got the guy
09:42out of the driver's seat and he was in a pretty bad way have you got any pain anyway it's um just
09:49some slight bleeding right over here okay yeah it's more like a graze or something okay did your airbags go
09:58what yeah yeah our patient is complaining that he's got these really quite bad abrasions from
10:05the seat belt from the high impact do you think you'd be all right to stand up and have a seat on
10:09the bed for me but I just know there are so many other things that could be wrong with him okay well
10:15done good work good we need to get him in the back of the ambulance and do a full head to toe check on
10:21him to make sure there's no life-threatening injuries has he been well lately before this
10:35happened he's been okay so just tonight he's been very weak yeah just tonight intensive care paramedic
10:42Lisa is urgently assessing Leo to find out if a heart attack caused his sudden collapse tonight
10:48do you come in my house today we did come into your house tonight the 87 year old who also has
10:55dementia fell on the way to the bathroom and hasn't been able to get up now can I check your temperature
11:02wife Helen has been trying to keep her husband calm but with Leo's history of heart problems
11:09everyone is worried it is possible that Leo is having a heart attack that's why it's a priority to have a
11:16look what his rhythms doing to work out if he is having one or not all right so you're in a nice
11:26looking rhythm nice heart rate readings good I'm ticking off my checklist I can see that his heart
11:34looks okay his temperature looks okay and now I'm looking at his mobility can you wiggle your toes yep good
11:42can you lift your arms up thank you we're doing a bit of a dance aren't we perfect Leo doesn't appear to
11:52have any obvious injuries but Lisa can't rule anything out so another paramedic team has arrived to help get
12:01the 87 year old off to hospital the muscles is here hey welcome hello this will be good mate this will get you up in a flash
12:12mark will be using a special lifting device called a razor to help get Leo off the ground safely
12:18make sure he does his job well is that what you're saying the longer we spend with Leo he's becoming quite cheeky and
12:29his personality is coming out can you hold my hand and we're going to come over this side he really
12:36reminds me of my grandma she was incredibly sweet and kind and cheeky and so Leo has a lot of those
12:42characteristics with Leo finally off the ground the team can get him onto the wheelchair and into the
12:57ambulance we'll take him up to the Queen Elizabeth Hospital sadly Helen can't go to the hospital with her
13:06husband she needs to look after her 94 year old mum Christine who also has dementia you're doing an
13:12amazing job if you're caring for both of them that's right yeah it's a lot isn't it I can only imagine how
13:20much both of them mean to her but also it's a full-time job she's got to get them both up dressed
13:27showered make sure that they're fed we'll get you onto our bed okay your wife Helen after years of
13:36caring for my grandma I can truly appreciate her level of commitment and responsibility she's pretty
13:42amazing there you go good job but leaving his wife of more than 40 years is distressing for Leo
13:49Helen yeah I think anyone with dementia they want to stay with their loved one they want to stay where
14:07it's familiar as soon as you take them out of that home environment they become even more confused but
14:13we need to get him to hospital for a thorough assessment Ananya's gonna sit in the back with
14:18you and take you off to the hospital all right I'm gonna say goodbye you're in good hands though okay
14:23I think the best case scenario for Leo is that he returns home and can be with Helen it's cleared the
14:30love between them but I am fearful that if he has injured himself he might have to go into a nursing
14:36home because I can't imagine that Helen would be able to continue to look after him at home
14:48I'll quickly do head to toe and then I'll get a set of obs nice deep breath in for me feel the lungs
14:56any pain when you breathe in good paramedics Ben and Mim are urgently assessing sebbet any pain through
15:04that and out the 19 year old was driving one of the cars in a high-speed accident after seeing the
15:12carnage of the two smashed up vehicles Ben is concerned his patient could have suffered serious
15:18injuries I need to check the injuries that can kill our patient so I work head to toe assessing every
15:26inch of his body as I go any pain through there just still just lower area all right I'm just gonna
15:33pop this over your top so it's gonna have a look at your hip oh yeah pain through there yeah just a
15:43corporate area all right the worry is sebbet's pain could indicate a broken pelvis or internal bleeding
15:51pain through here no nothing well good excellent he's got pelvic pain but it's like seat belt external
16:01trauma I don't think we need to go behind her after I assess his pelvis it looks pretty stable it feels
16:08like it's intact your blood pressure looks good I look at all of his observations everything is
16:13maintaining really stable so I'm not too concerned that he is bleeding but I need to keep it in the back
16:19of my head and I need to monitor him closely you let me know when you're good to go just go yeah
16:24it's now crucial the team gets sebbet to hospital tests and scans will be the only definitive way to
16:31confirm what damage has been done you look like you could be an athlete mate were you coming from the gym
16:40I used to have a body like that back in the day men there's an Olympia I just don't believe it Ben
16:54true story trust Ben to try and relive his Olympic glory days and bring it back to how great his body
17:01used to be but I'm sorry Ben stop living in the past this guy does not care how's that pain going
17:08creeping up yeah does it feel like to feel a lot more like in that is it in your abdomen or is it
17:14in your pelvis pelvis you just got pelvic pain yeah once this adrenaline starts wearing off you're gonna
17:21be sore I can give you something for the pain I've got the cannula in you reckon yeah there's no tick box
17:28for being a hero fortunately the team are now arriving at hospital we'll go in with you straight
17:35away you're what's called a level 2 trauma because you like quite a fast impact head-on speed we'll get
17:40you in the room have the doctors look at you they're gonna do a full head-to-toe check on him if they find
17:47anything they might take x-rays they may even take some bloods they're definitely gonna monitor him
17:52for a good few hours just to make sure that if there is any internal organ damage or bleeding that
17:58they can keep on top of it he's been a top patient and I really hope the best for him let's get you in
18:17it's an ambulance tell me exactly what's happened severe chest pain and blacking out
18:22is that you or someone else someone else it's late morning at the emergency operation center
18:29and a worried caller is on the line she's just arrived at her friend's house and found him in big
18:36trouble is he breathing normally with difficulty he's got severe jaw pain he's left down he's feeling numb
18:42Steven Steven suddenly the man's condition deteriorates even further Steven come back is
18:52he climbing while having cold sweats yes and he's just passed out again
18:55intensive care paramedic bill is now racing to the critically ill man
19:09he'll be providing specialized backup to a crew who have just arrived on scene there's a paramedic
19:18group that has already done an assessment and I found the patient is in rapid AF which is definitely
19:26dangerous a normal heart rate would sit around 60 to 70 beats a minute but this bloke's in what's
19:31called rapid AF or atrial fibrillation so he's a sitting about two to three times the speed that it
19:37should be the longer it goes that fast the more dangerous it is because your heart gets tired it
19:45doesn't get as much blood supply and as an added negative you can get formations of small clots in
19:50your heart that can then be distributed to the rest of your body including your brain which can often
19:55result in a stroke
20:02Eloise copies what the dogs do so the dogs if they weigh on the grass she has to weigh on the grass
20:10paramedics Mim and Ben are back on shift together and waiting for their first case to come through
20:16if the dogs have a poo on the grass she now has to do one today's topic of conversation is Mim's
20:23two-year-old daughter Eloise who's going through an interesting phase so the other day she went to do
20:30one didn't realize the dog was behind her and she literally pooed on the dog's head I might go and retrieve it
20:37it's like he couldn't care less anyway this is the life of a child see things like that then kind of
20:44see I get bullied about whether kids are on the way all the time but after this whole situation I'm not
20:52in a rush to have a baby while Ben considers what the future might look like SA Ambulance can you tell
21:01me exactly what happened at the emergency operation center a panicked call is coming through my wife
21:09was broke and you felt that for the effect that something could be an expectant father desperately needs
21:17help for his pregnant wife whose labor is progressing too quickly to get her to hospital
21:22how many minutes are part of the contractions
21:26so listen carefully lay her on her back in the center of the bed or the floor
21:33and I'm going to tell you how to help deliver the baby
21:36no Ben and Mim are one of two nearby ambulance crews who are now racing to reach the couple before
21:55their baby enters the world second pregnancy she's got the pushing feeling oh my god
22:02for Mim it's a case that's close to home she's 20 weeks pregnant with her second child
22:09are we gonna have a baby hopefully not mine hopefully just hers
22:14hopefully it's a smooth delivery yeah there's always things that can go wrong
22:21usually births go to plan but the baby might be a breech birth so coming out backwards or even
22:28having a cord wrapped around their neck but one of the most serious complications
22:31is blood loss and if there is any bleeding the bleeding happens quickly so when things do go
22:39wrong they can go wrong pretty drastically suddenly there's another update
22:45so there's an imminent birth we're crowning it's happening the baby's coming out Mim just wants to get
22:51there I just want to get there I'm driving as fast as I can safely do so this is going to be the
22:56first time I get to catch a baby and I'm really really excited I'm guessing it's this one intensive care paramedic
23:14bill has just arrived at the home of a 63 year old man with atrial fibrillation knock knock a dangerously
23:23fast and a regular heart rate which can lead to a stroke or cardiac arrest
23:28so this is Steve he looks so we've been in an AF rates of up to 200 okay initial blood pressure was
23:38about 110 Stephen is coughing he looks like he's in pain he is red his heart rate is through the roof
23:46he's on a thinner you're going to have PEs okay what was his temperature
23:5130 31 yeah this guy is febrile he's got a medical history of having PEs which are clots in the lungs
23:59he doesn't look well basically a one-day history of left-sided chest pain sharp going through to his
24:04back with numbness on his left arm okay not wanting to worry his wife who just left on an interstate trip
24:13Stephen phoned his good friend and retired nurse Jenny for help it's all right I'm with you
24:20I'm with you I was quite unnerved when I came in the door and saw how unwell Stephen was and I felt
24:29his pulse and it was pretty thready and then he kept lapsing in and out of consciousness and I'd be
24:36shouting at him Stephen Stephen so how did this all start occurring so he's given his friend a call
24:43complaining of the chest pain okay Jenny has checked his heart rate on the watch it with 140
24:48okay I've got to get to the bottom of this quickly because he's really unwell and I'm concerned that his
24:54heart's gonna tire he's gonna throw clots off and have a stroke or it could go into worst case scenario
24:59and have a cardiac arrest see see Ben
25:02I love that they're just so exciting aren't they we're going boy or girl I think it's boy
25:14paramedics Ben and Mim are just seconds away from reaching a mum in the very late stages of labour
25:21come on people a second ambulance crew has just arrived at the house any updates no other updates
25:31other than the imminent birth I know that this baby has its own time that it wants to enter this world
25:39but deep down I'm thinking just wait a minute I just want to be there next one do you want to grab
25:45the maternity care I'll just walk straight here you go in but as Mim hurries up the driveway there's
25:53an unmistakable sound paramedics Chelsea and Wycliffe arrived just in time to help so Ling and Jonas
26:12deliver their second child I literally was seconds away from catching a baby but a screaming baby is
26:28one of the most beautiful sounds you can hear because that means everything's probably gone
26:33well little boy a little girl no boy it's very cute have you got a little name both parents are
26:47thrilled with Carla's arrival but he did give them a massive scare hide myself ready to deliver the baby
26:54myself so the moment I saw the paramedics in my shoulder fell like a hundred ton of weight just drop off
27:01I feel so relieved to see them have we got any baby blankets or like a beanie the biggest risks
27:11with babies is that they can't regulate their own body temperature and keep you super duper warm
27:16they've come from this really warm nice environment to the outside world they're gonna get cold really
27:22quickly okay there we go have you got any more urges to push you do but just as everything seems to be
27:30under control things kick off again so Ling's done an incredible job birthing this little baby boy but
27:40she's only halfway through she still needs to deliver the placenta and I can see her contractions are ramping
27:47up again how's your pain mom out of 10 if 10 is really bad it's pretty bad yeah the placenta is an
27:53organ that provides oxygen and nutrients to the growing baby in the uterus once the baby is born the
27:59placenta has to come out but it can lead to a life-threatening postpartum hemorrhage you're doing
28:06really really well the biggest risk for suling at the moment is whether she's gonna have a catastrophic
28:11bleed after we deliver this placenta even if we put this under mum just to catch how much blood
28:17loss we are getting women can lose litres of blood from this hemorrhage and that's really risky
28:23you're doing so well
28:27intensive care paramedic Bill and the team are urgently assessing Steven I'm just noticing now
28:43that sometimes the 63 year old has atrial fibrillation a fast irregular heartbeat which
28:51can cause blood clots to form and potentially lead to a stroke Steve yeah can you push down up you're
28:59pressing on the brake can you do that with your left leg as well you're still able to do it but it
29:05definitely is weaker on that left side yeah can you feel me touching does it feel different yeah
29:12less on the left yeah all right Steven has chest pain with a rapid heart rate and now he's got left hand
29:19weakness and he also can't move and push back his feet and his knees as he normally would either
29:24which basically means this guy does tick the boxes for a potential stroke now too so chest sounds good
29:30yeah the sats are dropping yeah home alone with his wife of more than 30 years away interstate
29:38Steven turned to good friend Jenny for help
29:41immediately the retired nurse knew how dire his condition was I was worried that I was going to lose
29:51him I was really worried being a nurse watching what's happening is really different in a home
29:59situation to what it would be in a hospital we've just got some fluids running through your arm at the
30:04moment okay now I'm also going to give you an aspirin it's really hard when it's somebody really
30:10close to you it's a scary situation
30:17so I think we just get him onto the green chair and then let's go through the max
30:25with Steven's heart still racing and the possibility he could also be having a stroke
30:31treatment at the hospital is needed urgently before we move here I just want to quickly
30:36take your blood pressure again Steven's not in great shape and we need to get him out of the
30:40house and into the ambulance and getting to hospital quickly but we need to make sure that
30:44we are three steps ahead of any possible scenario
30:46cool 160 or 90 if we stand Steven up and he dumps his blood pressure and he goes unconscious it's going to be a big problem
30:53good job good job good job you're doing really really well did you want to try and breathe on that whistle while you're pushing
31:08Mim and Ben's patient Sue Ling has just given birth to her second child a baby boy named Kyla
31:16but now she's enduring another round of contractions to deliver the placenta
31:22you're about to give you some pain relief in your arms all right
31:25husband Jonas is watching on anxiously
31:29Sue Ling is a great wife and a fantastic mother
31:32she's pretty much the blue that makes us all stay together as a family
31:36so I feel a sense of relief knowing the paramedics are taking control of the situation
31:42making sure that my wife is in good hand that's it any more pushing or you feel
31:47not that's so good but until the placenta is out and they've made sure Sue Ling doesn't have a serious postpartum bleed
31:55no one can relax
31:57I think we've still got maybe another little something in there
32:00yeah I don't want to pull it because otherwise we'll retain the placenta

32:04this is a really important process because we need to know that that placenta is intact and whole
32:10sometimes tissues can be left behind this can cause bleeding and an infection which
32:15can be really difficult to treat and these are huge complications down the line
32:19there's a little bit of blood loss but with all things considered everything is going well and the
32:38body's doing what it needs to come here little guy go for a quick cuddle with dad
32:42here you go dad have your first little cuddle as well

32:59what a beautiful moment you've got the whole family at home multiple generations all there supporting each
33:05other looking at this baby in this situation kind of am a little bit clucky oh we're tired already
33:13it's been a big day for this little man
33:16while Kyla's pretty comfortable Ben and Mim can't relax just yet it's still important they get mum to
33:23hospital for a checkup to make sure all the placenta has been removed all right mum that's it so if there's
33:31a retained placenta your body will start to reject that and an infection can occur and your body can
33:36turn septic really quickly so it's really important to make sure everything came out all right let's get you in the back get you warm
33:50see this green chair will sit you forward and then you put and then you plonk your bottom right on that
33:54intensive care paramedic bill is worried steven could be having a stroke brought on by his dangerously
34:02fast and irregular heartbeat you can hold my hands you're okay the 63 year old needs to get to hospital
34:09urgently but moving him is risky i've got you're okay don't throw your arms out you just hold on to
34:15your wrists and i'm going to drag you backwards as a paramedic it's important to reassure our patients
34:21so i'm talking to steven nice and calmly because i don't want him to know or to see how worried i am
34:26about him good man well done with steven's wife interstate his friend jenny will be following the
34:36ambulance to hospital i'm really worried if something happens how am i going to ring and tell steven's wife
34:43leone they have an incredible bond and she would never forgive herself if she wasn't here if something
34:52happened all right steven can you hold my hands and squeeze them as hard as you possibly can squeeze
35:00squeeze squeeze squeeze squeeze squeeze squeeze squeeze i really want to be sure that we're not
35:03missing anything here so i do another stroke assessment can you push your feet away that's significant
35:09left side though isn't it yeah and i do find again that he has left arm and left leg weakness so he's
35:15got classic signs and symptoms of someone who is having a stroke ready rock and roll so we need to
35:20get him to hospital now so your wife is in tassie i'm on tour of my stories and my wife
35:32was this tactical to get him back i was looking forward for the holiday with steven so unwell it
35:48looks almost certain his wife leone will soon be on her way home i think we're just coming in to
35:54galamak hoping doctors can find some much needed answers we're going to go straight through to the
36:00resus room where there's going to be cardiologists and neurologists waiting for him to try to
36:05pinpoint what's actually going on hopefully he's not having a stroke but i know that steven's extremely
36:12unwell and i'm really worried about him and then your lovely friend jenny is calling your wife
36:17is it often for people giving blood their performance yeah a lot of the time your second
36:30pregnancy body knows what to do and then it just happens so quickly and yeah paramedics mim and ben
36:38are on the way to hospital with new mum su ling and proud dad jonas after their second child kyla was born
36:45unexpectedly at home how's your pain at the moment although things appear to have gone well mim won't
36:53be completely happy until su ling's had a thorough examination it's really important to get the
36:59midwives to really inspect the placenta at hospital because if there's retained placenta inside a woman's
37:05body you can become septic from an infection oh so snug in there this does bring back all the memories
37:14my little one's two and i'm currently 20 weeks pregnant i see the way that su ling's looking at
37:18kyla and i just know that she has been through an enormous amount of pain but right now all she
37:25cares about is this newborn bubble and just soaking it all in perfect you should be so proud of yourself
37:32honestly your wife deserves the biggest present ever it's crazy to think that in a few months time this
37:40will be me i get to relive all of this again and have the newborn cuddles and the newborn smells and
37:46i just can't wait all righty let's get you in while everyone appears to be doing well mum and bub still
37:55need to be assessed by the specialist medical team who are standing by oh little kyla what a special
38:02entrance into the world that's definitely a story to tell isn't it
38:1363 year old steven gave everyone a fright when he developed a dangerously fast and irregular heartbeat
38:20and showed signs he could be having a stroke

38:35bill managed to get steven to hospital but no one could be sure what the outcome would be
