02:06a big impact on my life lost 20 kilos or so in the past year just from walking the dog okay Adrian the

02:22oh hi is it Adrian hi hi how are you come on in and have a seat thank you I'm Dr Emma this is Donna
02:30hi yeah hi yeah so Adrian what can we do for you uh I've got a lump on the back of my neck I had it
02:37there for 20 odd years oh long time yeah yeah absolutely and has it been growing the whole time
02:44yeah it's getting bigger and bigger started off really small and it grew would it be okay if I had
02:49a little look at it yeah yeah get some gloves that's on the back of it yeah yeah oh yeah just just poking
02:56out from your collar there all right okay it's quite mobile the skin over the top of it is not so
03:04it's a fair old size isn't it yeah yeah it was quite symmetrical which would put you in mind of a
03:13cyst uh sometimes lipomas are a little bit more asymmetrical it was pretty huge uh in terms of cyst
03:20sizes particularly in in that region does it cause you any pain or no discomfort on a day-to-day no I don't
03:29tend to feel it it's just now uh I've lost a bit of weight recently and it's just yeah it's just
03:35stopping collars actually closing yeah yeah shirts that fit it's adding to your collar size and then
03:41your shirt's too big yeah yeah exactly how much weight have you lost uh 20 odd kilos in the past
03:46what's that like three stone oh I don't know in old money yeah oh I only know old money I still haven't
03:53got into killing I'll try not to look in stone because it's more frightening than kilos all right well
03:58I think it's going to be tricky okay but I think we could give it a go that'd be great if we could
04:04yeah so what we have to do is we'll have to obviously make quite a sizable cut in the skin I think
04:09because it's going to be it feels really quite stuck in the whole sort of middle segment off it and
04:15and then we have to sort of shell it out and try and get it out whole that's it a little perturbed
04:20saying that it may be a bit tricky as it wasn't moving under the skin very much but to be actually
04:26told yes they could hopefully take it out was absolutely great so this is a bit of a beast
04:33isn't it it's a big one put these on in the hope that it's we're going to get a squirt
04:37yeah usually there's like a little punctum in the middle which is the hair follicle that
04:43it's arisen from but I can't really determine that I wonder if it's that bit there never been
04:50any discharge from it yeah I might just remove a smaller lips like that because I like to remove
04:57a little bit of the extra skin I still did an ellipse you know if it's central hopefully that's
05:02going to be roughly where the punctum is and so if I remove the punctum then hopefully the cyst
05:07doesn't come back there's no scars about that big yeah no okay yeah no worries I don't see it so
05:13yeah it'll be hidden by your shirt collar hidden by your shirt collar that you'll be able to fasten
05:18absolutely right I'm going to start this is a bit stingy okay
05:23I got Adrian to set up for this procedure because if he lay on his tummy that would have put pressure
05:32on the cyst which would make it more likely to pop on me but it also would have made all the skin
05:37wrinkle up and it would just have been a bit of an uneven surface to work on right Adrian does this
05:42feel sharp anywhere no no no no
05:45can you see it not yet not yet oh drum roll
05:57well it definitely is a cyst not lipoma so almost as soon as I cut into the skin
06:05um I could see that sort of silvery shiny uh cyst wall there it is
06:11and that is a good thing because lipomas in that area often get like stuck down to the muscle
06:18underneath I've gone through the wall so this is probably going to give us the right treat
06:24right shall we yeah Donna loves it when they burst so I just thought Donna's got to get first
06:32squeeze on this one a little bit of me inside thinking oh come on baby come to mama
06:40I'll come back into the light a wee bit that's it whisk oh you beauty
06:50are you okay we're not pressing too hard no not at all oh gosh it's like you know that mac and cheese
07:00oh don't eat oh no it doesn't look like that beautiful I didn't really know how it was going
07:11to come out whether it was just going to be one of those splurters or over it would come out in one
07:16solid ball coming up watching your face it is amazing just made a lovely home for it doesn't want to
07:26leave because I didn't want it to stay we've literally been pulling stuff out of this for 10 minutes
07:33oh this is a gift that keeps on giving
07:35it's a packed day at the clinic
07:48hello in surgery room two dr emma is treating 51 year old adrian's large nexus
07:57just wonder if we do that lift the capsule up
08:04oh oh wow oh that did not disappoint
08:12it's nice for the pathologist if we can keep it a bit intact it is a really good size
08:24I was really surprised by the sheer volume of contents of adrian's cyst
08:38I think the capsule wall is there it's like it's quite shocked that it got to this size and actually
08:45not caused him any symptoms oh gosh oh oh this is a gift that keeps on giving
08:51having four hands back there trying to extract it I was having to push back to counter out
08:58they're pushing on my neck it was hard battle for for the pair of them

09:08yeah nuts to this
09:14I knew this was going to be a corker and then that's the best part of my job
09:29do you know that bit alone would have been impressive yeah we thought we'd like we've
09:36literally been pulling stuff out of this for 10 minutes and then there's another whole massive
09:43bit that yeah you know when a magician has the flags yes just keep coming out and coming yeah it's like that
09:49right Donna have a bit of a poke around in there wow holy moly
09:55if you can sort of lift your head up straight for me yeah so we're going to put some stitches in
10:04underneath it's quite a deep space there it went down about three four centimeters which you wouldn't
10:10think you have that space on the back of your neck no no so when we have a big sort of void like that
10:15left under we will put stitches in underneath to sort of close that space down and bring everything
10:20together so if it bled into it it can expand and form a hematoma but because this is the
10:26back of your neck you're gonna have to be really careful with what you do yeah for the
10:30next few weeks really okay all right yeah all right so you've got three stitches in there
10:37yeah in fact shall I take a photograph of it oh yeah that'd be good and one from the
10:46side there's probably a tiny bit swollen because of the local and a seven yeah yeah but oh wow that's
10:56better isn't it a lot better totally flat yeah put my hand at the back of my neck and actually not
11:04feel it it's weird it's gonna take a little bit of getting used to but be good collars done up soon
11:09so are you ready for the the big reveal yeah okay oh there's bits of it that's a bit weird yeah
11:20it's like bits of fat on a bit of steak it was oh pop that in a pot so we can send it off and
11:29just get it double checked by a pathologist maybe a slightly bigger pot yeah right you can wave him
11:38after 20 years living with that lump back there it's weird to be without it you'll buy some yeah

13:10you today um I've got a lump on my right shoulder okay so I first noticed it about seven years ago okay
13:18um but over the last couple of years it seems to have grown slowly growing yeah does it hurt is it
13:24it's uncomfortable more than anything yeah against the backs of chairs and you know in the car when
13:31I'm driving it sometimes bothers me okay um started off about walnut size and it's now about tangerine
13:37size all right yeah let's have let's have a look at this so can you see it through your clothing we
13:42will look at that yes I'm beginning to feel quite conscious of it particularly when I'm on holiday or
13:50you know during warmer months when you're wearing sort of thinner clothing or strappy tops okay so it's
13:55right where your bra strap kind of pushes on it yeah yeah this is probably a lipoma I think you might
14:02have realized that yeah yeah it's so unfortunate it's right above your top do you know what I mean
14:07yeah okay um right we'll have a seat for a second
14:11so we can definitely remove this for you today great yay um do you have any questions
14:18no I'm looking forward to having it removed there's such a build up to this point and yeah
14:25to hear those words it was lovely so just rest your arms by yourself
14:30it's really important to mark up the edges of lipoma because when you put anesthetic in the
14:37whole area can become swollen and then you won't be able to feel where the where the margins are
14:41so by marking it in the beginning I know roughly how far I have to go
14:46to get that whole lesion out
14:48all right so I'm just you're gonna feel a sharp scratch and sting if you want to move your arms
14:55around move your head you just tell us okay you can move in whatever way you want
14:59and you're gonna feel a sharp scratch now ready and sharp good well done and now a sting
15:05a little a little bit I mean I don't like them does anybody like needles but um I've gone
15:21through childbirth so I think once you've experienced that anything else just pales into insignificance
15:27so I'm gonna get started now if you feel any pain you have to tell me okay okay
15:34how are you doing Sue yeah I'm good thank you
15:42just trying to stay relaxed
15:45are they quite common yeah well in our world yeah we see we see them all the time
16:03how's it looking looks good we're just unsticking it so it's sort of contained is it yeah yeah I think
16:14this is one ball I wanted to make sure you couldn't see the scar if she was wearing a strappy top or
16:20something in her back but I have to think about where I am on the body and what will give the best

17:40she watches all of it all of the dating ones in particular

18:14it's a couple little fascia bits sticking it down still
18:19so you've just made a lovely home for it doesn't want to leave

18:26I didn't want it to stay
18:28I find that it's easier to remove lipomas in someone older who has more skin laxity
18:34the skin's very tight or the person's very skinny
18:37then there's not much room there for me to get into that area
18:41you don't want to let go of it because if I let go of it then I'll lose the base of it
18:46oh it's hanging on for dear life this one
18:53just my look

18:55it's like I don't want to go stop
18:58we never leave anything behind

22:14I've suddenly gotten these things under my breast area

22:17and they're like moles or skin tags
22:20yeah
22:20I'm not quite sure what they are actually
22:21um I have had about three or four of them taken frozen off by the doctor
22:27okay yeah
22:27uh but they are so annoying when I sweat
22:30yeah
22:30say I go on for a long walk
22:32they get so red and sore
22:34the itching is unbearable
22:37you can't scratch them because you know that you make them bleed
22:40or make them worse and it'll take longer to heal
22:42maybe if we get you to change into a gown would you be okay with that
22:45yeah
22:45so if Angela will just give you a gown
22:48yeah
22:48wearing a bra is a nightmare when it gets like that
22:52even if they take a few off
22:54and just make it more bearable and you know more comfortable
22:57so which are the ones that bother you
22:59that one there

23:07what they are they're called seborrheic carotaceae
23:09so they are completely harmless skin growths
23:11but as you found they can be quite annoying
23:14I can remove some of them for you

23:17seborrheic carotaceae are growths that arise from the surface layer of the skin
23:23so down here
23:24it's a tendency that you have
23:26and if you have that tendency
23:27then generally you will slowly develop more over time
23:30okay let's just start doing some injecting

23:34this is just a bit slightly uncomfortable
23:36so just be patient with me
23:38sorry
23:38the injection was a little bit stingy in certain areas
23:43but otherwise it was okay
23:45it's not as bad as a dentist honestly
23:47I used to be a dental nurse

23:48are you still working or what do you do now
23:50oh no I'm retired now
23:52it is pretty busy when you're retired
23:54have a weekend away with the girls
23:57and we love going on long walks
23:59I mean like 10k walks
24:01so it would be a relief
24:04not to have this problem

24:11painful during these procedures
