A six-year-old has died after being sent home twice from the Sydney Children's Hospital.

00:00The six-year-old girl first presented here to Sydney Children's Hospital in
00:06Randwick in Sydney's east on Monday. She was initially clinically assessed by
00:11medical staff before being discharged. We understand a short time later the
00:16girl's family returned her to the hospital. Medical staff assessed her
00:21again and diagnosed her with influenza before she was discharged again with
00:27what in a statement from Sydney Children's Hospital has been described as
00:30effectively an at-home care plan. Later she was taken to Northern Beaches
00:36Hospital where she tragically passed away on Tuesday. Now we've heard statements
00:42from both at the Sydney Children's Hospital Network as well as Northern
00:46Beaches Hospital. They have both agreed to take part in a joint review of the
00:52incident and have both offered the condolences to the family of the
00:56six-year-old child. We've also heard from St Luke's Grammar School in Dewite on
01:01Sydney's Northern Beaches where the six-year-old girl was a current
01:05kindergarten student. They've described her as being full of light and joy and
01:10have said that families who may be affected are now being offered grief
01:15counselling. New South Wales Health Minister Ryan Park has also weighed in.
01:20He's extended his condolences to the family describing the incident as
01:24devastating but at the same time he said that the parents of the child deserve
01:29answers. Now this will obviously head to a review by the New South Wales coroner
01:35which is standard practice for these sort of incidents but at the same time it
01:40will also spark a serious adverse event review or SEER which is really only
01:46reserved for the most serious clinical incidents.
