A six-year-old has died after being sent home twice from the Sydney children's hospital.

00:00The six-year-old girl first presented here at Sydney Children's Hospital in
00:06Randwick in Sydney's east on Monday. She was initially clinically assessed by
00:11medical staff before being discharged. We understand a short time later her
00:16family returned her to the same hospital. She was assessed again by staff where
00:21she was diagnosed with influenza before being given what's effectively been
00:26described in a statement by the Sydney Children's Hospital Network as an at-home
00:30care plan. We understand she was later transported to Northern Beaches Hospital
00:35where she tragically died on Tuesday. In a statement the Sydney Children's Hospital
00:40Network says they express their deepest condolences to the family for the loss
00:45of their child. They say they're progressing an initial review into the
00:49patient's care while at the Children's Hospital. Northern Beaches Hospital has
00:54echoed the same sentiment. They say alongside the Sydney Children's Hospital
00:58Network where the child had received care earlier in the day they will
01:02undertake the appropriate investigations in line with New South Wales health
01:06policy. Staff at St Luke's Grammar School in D.Y. in Sydney's Northern Beaches where
01:11the six-year-old was a kindergarten student have described her as being
01:15free-spirited, bright and full of life. They're also offering grief counseling to
01:22affected families. In an email informing the D.Y. school community of her death on
01:28Tuesday night the school says no words can express the heartbreak the parents
01:32are currently experiencing. New South Wales Health Minister Ryan Park has
01:37described the incident as devastating and says he will work to find out how and
01:42why the girl's death occurred and find answers for her parents. As well as a New
01:48South Wales coronial review of the girl's death the incident has also triggered a
01:54serious adverse event review which is only reserved for the most serious clinical
02:00incidents.
