00:01Discuss the situation with the customer.
00:03It's not about beauty, but it's about healthy condition.
00:09Hi, my name is Anaite and I'm here to get a medical pedicure.
00:12I am clumsy and active and that's not like the greatest combo for my feet.
00:18And I don't know how I injured my left toenail.
00:22I dropped something or I was trying to jump.
00:25Today, Dr. Archer is going to see it and like let me know how to take care of it.
00:29I invest in a medical pedicure because I think it gives me a lot of security when I come here.
00:36Everything's clean and I love the staff, Tatiana and Dr. Archer.
00:41Something that best describes the way I practice is a combination of medicine and beauty.
00:46And they really are intertwined.
00:48I'm Dr. Krista Archer. I am a podiatrist on the Upper East Side of Manhattan.
00:53A medical pedicure is a pedicure as most people think of it.
00:57But it's offered in my office where we have surgically sterile instruments.
01:04And it's performed by a technician that I've trained and supervised.
01:08My name is Tatiana and I'm a nail technician and I'm working for Dr. Archer.
01:13For Anaite today, we're doing regular basic medical pedicure.
01:18It's just a part of the maintenance and to continue and to improve her feet.
01:24Okay, so good to have you here again.
01:26So I see that you have this little bruise.
01:28You didn't have it last time.
01:30What happened to it?
01:31I was kicking the sidewalk and like jumping and I also bruised here.
01:35I mean, it doesn't hurt, but...
01:37It doesn't hurt.
01:38Okay, so let's start it.
01:42She discovered she has a hematoma underneath her big toenail.
01:46The bad thing about a hematoma is that fungus loves blood.
01:49So it makes the nail very prone to getting a fungal infection or making an existing fungal infection worse.
01:57So every single instrument is separately packed and disinfected in advance.
02:03So I have my all packets of the instruments that necessary for this procedure.
02:09You're guaranteed that you're not going to get an infection or a fungal infection.
02:13It just helps them just be pain-free and fungus-free and infection-free down the line.
02:19And then do the clipping.
02:28So after the clipping, then I would examine every nail and file and make sure that corners in a good
02:33shape.
02:33The customer has a little fungus or something like that going on.
02:37So I prefer to start with the food that doesn't have that or has less.
02:42And then I change my tools while I'm working at the same time.
02:45So I don't use the same tool on the other nail that doesn't have the fungus.
02:55A hematoma takes a really long time to grow out.
02:58The nail plate grows about a millimeter a month.
03:00So a hematoma underneath the nail plate could take as long as a year to grow out.
03:07And after that, I do a little scrub.
03:10It's a glycolic peel or fruit peel.
03:13Basically, it's a glycolic wash that we use here.
03:15It's necessary to get rid of all the buildup and dry skin,
03:19a little to soften the callus, soften cuticles.
03:23And we use nail whitening.
03:26So I put nail whitening on every single nail.
03:29Usually, it's between 15 to 10 minutes.
03:36And then I brush it off with a little brush.
03:39And then I soak them.
03:47So we soak feet.
03:49Give them a little massage, a little scrub, a little movement in the water.
03:52I put soaking salt, which is Malaysian salt.
03:55That also moisturizes the skin and softens the callus or hard skin.
04:03After this, I clip all the cuticles.
04:06I clean around.
04:07I make sure that there is nothing left.
04:09No, any extra skin that shouldn't be there.
04:12I put the disinfectant tea tree oil,
04:16which is very important to disinfect to prevent the fungus
04:18and also to moisturize the skin and the cuticle area.
04:25Then after that, we use diamond cell file,
04:28which we also recommend to our customers to have at home for the maintenance.
04:31It's like the best thing you can have at home to take care of your feet.
04:35You clean it, you brush it, you rinse it,
04:37and you just keep it there in the shower.
04:38It dries and ready to use for the next time.
04:43At the end, I do a little light massage with the moisturizing oil.
04:47I use our cream that we have here, Dr. Archer's cream,
04:51with different plant extracts.
04:53And also, we add a little bit of vitamin E oil.
04:58Perfectly nourishes and moisturizes the skin.
05:03And after that, we discuss about the color,
05:06what kind of nail polish they would like.
05:08Some people prefer just antifungal base coat.
05:10They don't want any color because they want to see the condition of the nail.
05:15So all this is done in about 50 minutes.
05:18And that's what I call nice, clean, perfect medical pedicure.
05:23Okay, it's all set.
05:25I love it.
05:26Love the color.
05:27You can't see the roofs.
05:28I love it.
05:30So my feet feel great.
05:32They're very smooth and hydrated.
05:34They have a cute color on.
05:36And I did really well.
05:37It was nice and brief pedicure.
05:40Every single nail was treated well and detailed.
05:44And she walked away with some extra samples of fungal medication
05:47and instructions from me how to treat it going forward.
05:50Tatiana and Dr. Archer advised that I should keep on using the oil
05:56and then just try to not injure my feet so much.
05:59I'm so happy I got the pedicure today
06:02because I'm planning my outfits for the weekend
06:04and I'm wearing open toe shoes now.
06:06So it's awesome.
