00:00Hi.
00:00Hi.
00:01How are you?
00:01Good, how are you?
00:02I'm Sandra.
00:02It's nice to meet you.
00:04What's going on?
00:07People don't come and see me and then start crying.
00:11Honey?
00:15Just a lot of emotion.
00:16OK, all right.
00:17We're just here to talk right now and see
00:19what's going on with you, right?
00:20Can you tell me a little bit about it?
00:22I have a horn growing out of my head.
00:25OK, all right.
00:27How long has that been going on?
00:28A little over a year.
00:29My family suffers from, we get these other cysts on our head.
00:33I have a couple of small ones here.
00:35I have one right here.
00:36OK, yeah.
00:37And then there's another one down.
00:38And I had one, and it broke.
00:42And of course, I squeezed it.
00:44It came to a point where I couldn't squeeze it anymore,
00:46and then this started growing.
00:48OK.
00:49That's called a pilar cyst.
00:51Well, my mother used to call them wens.
00:53Yes, she's actually right, wens, w-e-n.
00:56Yes.
00:56That is another term we use for those on your head.
00:59Right.
01:00A w-en is just another term for a pilar cyst.
01:02Just like a mole is also called a nevus.
01:05Or a skin tag is also called an acrochordon.
01:09Or a wart is also called a veruca.
01:12All right, shall I take a look at it?
01:13I'm very interested in seeing what this bump looks like now.
01:16But the way that she describes her w-en,
01:19it doesn't sound like any pilar cysts that I've seen before.
01:24Yeah.
01:27It's like you have a fingernail that's gone crazy over here, huh?
01:30Yes.
01:31We call that usually a cutaneous horn when we see something like that.
01:35That, like, ring around it is super sore.
01:38Right.
01:38And I can see how that's tender because it's really stretching your skin there.
01:42Let me take a little photo of it right now, actually, too.
01:44I mean, I've seen cutaneous horns before.
01:47I mean, but certainly, this is the largest cutaneous horn that I've ever seen.
01:51Girl, we got to get rid of that thing for you.
01:54Right?
01:55One of the things that I'm certainly thinking about with Lisa here is skin cancer.
01:59Because one type of squamous cell carcinoma called a keratoacanthoma can look like this.
02:05I'm not exactly sure what it is.
02:07I feel like you give me a story here that sounds like it could be a pilar cyst.
02:12And it has grown out of control.
02:14This is the angriest pilar cyst I've ever seen, if that's the case.
02:17Right.
02:17Because usually when we see a cutaneous horn, we do think of other things that might be a
02:22little bit more concerning, you know, that can happen with types of skin cancers.
