"At normal speed?" the AVE driver asked. Adif's control centre said yes. That is the 08:36 exchange on the leaked recordings published by El País, Cadena SER and El Mundo on 28 Oct 2026.

The signalling (LZB) on the Madrid–Seville high-speed line had failed at about 08:30, and every train on the section stopped. After a failure like that, the rule is to restart "on sight" (marcha a la vista), at 40 km/h at most. Twenty-five kilometres ahead, on the same track, an Iryo train was standing still. The AVE went past 200 km/h over some of those kilometres, Transport Minister Óscar Puente said, and near Urda (Toledo) it ran into the stopped train. The order said "normal" where the rule said "on sight".

The investigation (AITAT) and a court in Orgaz are still working on why. Victims, crew and control-room staff are not named.
