01:49you've been in before been in a lot of times a repeat offender then yeah so get ready to share
01:56a shift it's a 24-hour service seven days a week with the team at barnesley casualty
02:08it's seven a.m and at barnesley casualty sister jane is running the hub
02:14tracy it's only jane i need a female surgical bedlock
02:20i'm around down here she's taking over from a very busy night shift
02:27we just need to get them out so that we can get some space
02:32we're always taking patients in by ambulance or walking or the police bring them in
02:40and it's difficult because the department becomes gridlocked
02:46and unfortunately we're reliant on other areas helping us out to clear the department
02:55a and e has quite a reputation of just speed and pace a lot of people can be frightened by

03:04it's a daunting place to come and do a shift there's coming through two ambulances waiting to come in
03:10right have they said that they're keeping you in right i'm going to send you down surgical s deck

03:29right she can get to us sister jane moves one patient on but as one leaves another arrives
04:06this gentleman had a stroke at the end of september and since then
04:10she's been having non-athletic seizures advanced clinical practitioner chantelle will take charge

04:27where you are no you're in bansley any department have you got any pain anywhere
04:35yeah this one did you fall onto your arm
04:41you don't know have you had chest pain today whereabouts in your chest
04:56i need to get these answers from sean quickly but it's quite difficult with his condition
05:03sean was found on the floor by his wife sam did he lose consciousness and when i came down he
05:09was
05:09speaking to me i'm really glad his wife's here so we can get these answers and then i can get
05:14to the
05:14bottom of what's going off and did you not hear a crash or anything or nothing just i just looked
05:19as fast as you know and said help help like okay am i in a great so i just found
05:25downstairs
05:26he was on the floor yeah and then he were mourning his right arm were hurting him was like
05:31it's really hurting it hurts and then all of a sudden they were like his fingers have gone numb
05:35on his right side on his right side but since stroke it's his right side that it's been affected
05:41it's been affected yeah how many seizures did you often have like every day and he's ever about
05:50eight to ten a day eight to ten a day does it get like a warning sign or anything yeah
05:54normal his face twitches and it'll start going like this for his head his seizures are becoming
06:01more frequent and she's quite concerned so that's the reason she brought him to us we'll take an ecg
06:07pop a cannula into some blood and then we'll see what we find is that okay does it hurt there
06:15no what about there it hurts there is that where the pain's been coming from
06:20what about over here yeah it's tender there sean is flipping it in and out of consciousness
06:26but he is also complaining of chest pain so my job um at that moment in time is to figure
06:32out if
06:33this chest pain is contributing to his conscious level as well so it's quite important for me to
06:38establish that pain and where it's coming from mouse twitching i think i might have a seizure

07:47after his seizures yeah normal come straight out of him and he's just normal okay wife Sam
07:55has been Sean's main carer since his seizures started following a recent stroke is it only
08:05it'll have one about five seconds it'll stop then I'll go back into one and then it does that for
08:12about six or eight times and then they'll come around and they'll get up yeah but it's just
08:18continuing this is continuous is he more prone to them when he's unwell when he gets hot when he gets

08:38Sean how are you feeling you're not not feeling good there's many forms of seizures Sean these seizures
08:48can you just stop them normally like that you can stop the seizure and they'll come again and then
08:54it comes again epileptic seizures and then there's non-epileptic and there's a whole host of other type
09:00of seizures so you can control these seizures but do you think you can do it now you try and
09:08do it
09:09are you alert and aware when they're happening so what's really important is establishing what's normal
09:15for Sean and what's abnormal when you're like talking to him it'd be all right as soon as you
09:20walked out the room it set off and then a nurse came back in and he talked to her and
09:26then she'd go
09:27off and he'd start again it's like if somebody keeps in talking or whatever it seems to be all right
09:33yeah which is not classic it's not a classical seizure yeah because I won't be able to bring
09:40him out of it and he won't be able to talk to me straight away the increase in Sean's seizures
09:45could be the result of an infection so this lung sounds quite crackly and quiet so I'm gonna get
09:52an x-ray of his chest you said he's had a cough how long for about four days we're gonna
09:58come and do
09:59an x-ray on your chest soon okay you need to you need to be a little bit alert and
10:04set up for it
10:05okay so I can see if there's any infection all right you have to sit here and stroke his arm
10:12so
10:13you can keep him calm and then it didn't have a seizure that's your job Sam you're doing it as
10:19well you're a good spot there you go nice and calm but it's been in and out that many times
10:28it's like
10:29it's the norm now but this has been the worst he had a stroke about eight weeks ago two weeks
10:36in
10:36and he started with seizures he's got to go for like load of electrodes on his head to see what's
10:44happening so we are going through tests to find out exactly what kind of seizures they are it just
10:52feels useless now because they were always on the goal he never stopped around doing some gardening
10:57building and now it can't Sean hello mom hiya you all right hello hello Sean my name's

11:19radiology have come to do x-rays right Sean we're going to sit you forward and pop the board behind
11:25your back all right love if the x-ray reveals a chest infection Sean will need urgent treatment
11:42over in the pediatric department I've got these things to put up for the children and they're
11:50going to be like little stick puppets there's a little road out here so that they can use them
11:54to drive along the road there's like a little activity to do while they're waiting to see the
11:57doctor play leader Olivia is creating an interactive display about road safety so this is going to be
12:05like the crossroads for Barnsley Hospital there's going to be like a little lollipop man that's going
12:10to stick here and then when the cars kind of come across you're going to let the kids go fast
12:16I love my job every day is so fun they're just amazing the kids that come here they're so lovely
12:22and
12:26so friendly one girl in paediatrics today is eight-year-old Harriet who has a suspected broken
12:38tone Harriet my name's Gemma are you all right walking down yeah come this way then
12:44treating casualties younger patients today is advanced clinical practitioner Gemma okay yeah

13:00brother if you'll pop up on the bed there you go perfect so my name is Gemma what's your name
13:05mum Emma she's jumped off settee and she's banged a foot on the footstool it was sticking right out
13:13it was all red and she was crying if that was my toy it just went boom and then it
13:19hit it right there
13:20but it also hurt here and then it just went like that and it was like that the whole time
13:27and when it was sticking out did you do anything I didn't I didn't touch it and you
13:31tried to move it or anything yeah no did a little bit swollen that into sweetheart can I
13:36have a little look at you is that okay that hurt there yes sorry and just that hurt there a
13:44bit a bit
13:44okay compare it to the other side so it is sticking out so normally I would say would just strap
13:56it the
13:56fact that you've said it was sticking out that way and it is a little bit what I call angulated
14:02I
14:03think we might just need to make sure it's not like a fracture type dislocation okay so I think we
14:10will get an x-ray so give me two minutes okay and then we'll get you around okay thanks you're
14:18right yeah you've never really had any injuries before have you I've never broken a bone John
14:26follow me and I'll show you where to go you walk around if Harriet's x-ray reveals that her toe

14:34dislocated it will need to be manipulated back into position 33 have got no beds at the moment and
14:53I've got this neck of femur who they do know about and there's potentially another neck of femur that's
14:58coming in the hub the pressure is not letting up for sister Jane casualty is still jam-packed from a
15:05busy night shift it's looking busy at the moment there's 63 patients in the departments at the minute
15:13for 11 o'clock in the morning it's not great some of these patients have been here 14 15 16
15:20hours so
15:20it's not really very good we're not a ward and we need to move patients out to get more patients
15:26and
15:27it's just a vicious circle really it's just never ending you still have a duty of care towards those
15:34patients to ensure that they're safe they've come to a hospital to be treated for their illness and
15:40they need to be safe and sometimes it's just difficult we've been in there all morning nobody
15:47wants them in here for 12 hours because they're on trolleys but everybody's doing their best hi it's
16:00a patient with a history of heart attacks has arrived 79 year old Tommy is taken straight to the
16:08ambulance bay so granddaughter's phone today because he's got this normally chest pain okay
16:15fine granddaughter Lauren called the ambulance you had a bit of a chest pain yeah yeah I'd be chest pain
16:24again I've not been very good for a long time Tommy is all too familiar with Barnsley casualty I've been
16:32with a few times you had a gash on your back you're ready to hear you found you don't want
16:36to fold in
16:37the stairs yeah yeah cut your head up and in yeah yeah so you want to have it clone a
16:42great good job
16:43that was good coming there that was good hey Tommy my name is Jess am I okay to have look
16:50see if I can
16:50get a cannula in to take a test you can what you allergic to I'm allergic to I used to
16:57be allergic to the sun
16:58to the sun I wasn't going to get that much of that now I wasn't in here but a week
17:05and a half ago I think
17:05no two weeks was it yeah I was just going to go and get you shopping for you before you
17:12decided to
17:13feel unwell again I've been staying with him for a few days we take it in turns because he's on
17:19his own
17:19now since he lost his wife my mama since then he's gone down also he's got a lot worse sir
17:29so he's got 21
17:33grandchildren and 16 great-grandchildren so we all take it in turns don't we granddad
17:53Tommy must wait for the blood test results to find out if he's had a heart attack if he has
17:59he
18:00will need to stay in for observations so Ellie's coming in at 2 and then they put a message on
18:11for
18:11any band 3's they have sent me somebody down from the ward but only this morning in the hub sister
18:17Jane not only has a jam-packed casualty she's now two nurses down we've had some sick calls today
18:26when staff are tired they become susceptible to catching illnesses so we all struggle really
18:37pretty rubbish we're not going to close the department for no staff it's a 24-hour service
18:45seven days a week so you just have to plough on
18:59in resus Sean is still having seizures advanced clinical practitioner Chantel has the results of
19:06his chest x-ray his infection markers are up a little bit yeah but there's no pneumonia on his
19:12chest x-ray which is good so I'm going to give him some antibiotics to clear that chest infection
19:17so I have told the doctors upstairs about him yeah and they're happy for him to go upstairs when there's
19:22a
19:22bed yeah do you think the infections cause sometimes yeah sometimes if he's been a bit
19:28unwell and a bit feverish can upset him a little bit and hopefully antibiotics yeah yeah yeah yeah and
19:35if he's okay tomorrow they might convert them to oral tablets and then he can take them at home yeah
19:41right I'll leave you with him okay hopefully good get the antibiotics in and hopefully get him back
19:49home and get back to well normal as possible Sean spent the night on the ward and was released the
19:59next day he's still undergoing investigation for his seizures there is no let up for advanced clinical
20:13practitioner Chantel as one patient leaves another arrives paramedics are bringing to resus a 94 year old
20:24woman with a high heart rate and dangerously low oxygen levels
20:41right we're going over onto here okay so a little ride are you ready ready steady slide oh you're light
20:50as a feather paramedic Marcus hands over her care what any other medical history so Alzheimer's and
20:59osteoporosis as well how are you feeling at the minute you feel a little bit frightened our priority
21:09in emergency medicine is to find out the emergency but it's really important to try and make a patient
21:14regardless of age not be scared off right and especially when they're confused as well so
21:20Margaret do you know where you are at the minute I do like to give comfort with touch do you
21:37want me to remind you
21:38yeah so you're in Barnsley hospital at the minute can you remember now don't worry you're fine and

23:29we all rely on a certain number of nurses to get work and jobs done and obviously if you haven't
23:34got
23:34that nurse there if you want down then it falls on another nurse or two nurses to share that
23:40responsibility so it does make it difficult Tom in rapid or Lisa in resource take your pick but let
23:48me know you can do rapid with me have you seen how many there is to assess walk-ins are

24:18practitioner Chantel needs to find the cause of Margaret's dangerously low oxygen levels
24:28they're not very good oh see what I can do I'm quite good you know I'll not be good now

27:07she's definitely drafted it if you look at these ones we look for nice smooth smooth lines and on
27:14this one she's got a little bit of a step there but she's also got a bit of a bend

27:20we're looking for it's not nice and smooth so it looks like she yeah she's fractured it

27:39out of line so we don't it's not what we call dislocated so what we need to do is just
27:44strap

27:54I would um avoid it for a few weeks well just um healing
27:58and then there's one other question what about boxing because I do that
28:02boxing you don't box with your feet do you
28:06no as long as you're not in pain then you should

33:35happened um i came that track i went into woods yeah and i went straight into a tree at some

33:46obviously stopped impact yeah fell off the bike yeah okay did you get back on the bike or not
33:52i like fell off and then i started screaming me like me like okey doke and did you get pain
33:57straight away then when you fell off i got pain and then pain all the way back and pain all

34:01back did you have to take painkillers or anything when you got home off no i've just took some
34:05thingamajigs thingamajigs iprofen today yeah yeah has that helped or not it's just stuff robbing

35:14if you're looking at 15 mile an hour and then just comes to a sudden stop that's quite a big
35:20impact on
35:20that area he's weight bearing he's walking up down so i think it's unlikely that he has got anything
35:27fractured there however we have had people that have walked in before with broken femurs
35:33if the x-ray shows his femur is broken caden won't be getting back on his bike anytime soon
35:53so your patient has just come to you from high dependency two
35:58they've pulled his bed so we can go up to the ward now lovely thanks charlotte do you need a

36:06you're on the ball with casualty full and her staff working non-stop

36:34kenny's sugar hit is good we have our sweet drawer which is generally always full and if it's not
36:40then uh there's words to be had the specialities come down and take our suites and that's not fair is

37:05in cubicle 10 tommy is hoping to go home but dr tet has the results of his blood test
37:14i would like you to keep in amu admission water why because he's got the chest pain and the first
37:25heart attack mother is
37:28raise i'm just worried that the heart attack is back when you get back home and then you need to

37:34back here okay i think it is best recommended to keep you
37:38we've got to stay in granddad he's said this morning it was just a pain but obviously it won't
37:43they're thinking it might be might have been an attack
37:47so so his granddad here weren't even going to come in
37:51i didn't know what it was i just had a bit of pain i've had pain before
37:55but we used to it because he's had that many heart attacks
37:59he's got 16 cents in his heart because he's had that many heart attacks so

38:24tommy spent the night in hospital and he's now back at home being cared for by his family

38:52i was just going too down there too fast i was up to him yesterday so it's trying to go

38:57heart attack and it fell off three times yeah it's just it's just part of the sport really having fun

39:15we do see a lot of sports related injuries ones that are the highest risk are certainly speed is
39:23related i've had a look at the x-ray no fracture on the x-rays but it's nice and clean
39:28all the way
39:28down down to the knee and this is from side on and there's your knee itself so that's good news
39:35but of
39:35course it's not only the bones that are you know at risk so um we need to have a quick
39:39look at the
39:40general function if you want to pop up on the bed how long we've been racing for i started this
39:48year
39:49oh did you yeah but you are you serious stuff or is it serious is it yeah very good well
39:56that's
39:56getting into it all now he just started with bmx but he's starting to get into some of the more
40:00stunts and wants to do the downhill stuff i think historically we haven't been necessarily as
40:04good as we should be at encouraging physical activity so when we do see cases of of sports
40:11and exercise related injury i think it's really important we congratulate them for being active
40:17lift this leg straight off the bed that's as far as i can get where's the pain there all in
40:23that
40:23using bending the knee lift as high as you can right so with the knee bent it's all right yeah
40:29a bit
40:29tight but not as hard as with the knee straight okay back down and no numbness down the leg at
40:36all
40:36no feels normal both sides pull those back on and then you can get your shoes back on when's your
40:41next race october okay and what do you do for work build motor arms oh good brilliant that's great
40:54busy busy busy lads i don't think there's anything we need to do maybe for the first 48 hours a

41:00of frequent icing will help just bring the swelling down so all in order all right nice to meet you
