01:34I felt deformed and now I feel human.

01:50Have you had to deal with a lot of patients that have trauma?
01:54Um, yeah, a lot.
01:55Because I trained in the Bronx for medical school and then at the LA County Hospital for residencies.
02:00And I did three years of full general surgery.
02:02So, lots of gunshot wounds, lots of stab wounds, car accidents, motorcycle accidents.
02:09Oh, man.
02:10I am Dr. Sheila Nazarian.

02:15I am a mom of three, an entrepreneur, a business person.
02:19I grew up in Iran in the year of the revolution.
02:23And during the Iran-Iraq War, my family escaped to bring us to America for a better life.

04:03So, we know a little bit about your back story.
04:06And we were actually just talking about dealing with patients that have gone through trauma.
04:11And our philosophy is your skin tells a story.
04:14And I think some of these scars tell a story you don't want to tell anymore.
04:18Yes.
04:19I had a traumatic incident and these scars are one of the last things left for me to begin my
04:26healing.
04:32On March 23rd, 2017, I was shot nine times.
04:38I was shot in my face.
04:40I was shot on every limb of my body except for my right leg.
04:45I came home from work just as I normally do.
04:49And my ex-husband had a gun in his hand.
04:52I heard a bang.
04:55And my husband walked upstairs, continued to shoot.
05:00He noticed me crawling toward the door and took his own life in front of me.
05:08When I look at these scars, that's what I think of.
05:11I think of lies, betrayal, hurt.
05:16I deserve to be happy again.
05:18I deserve a second chance.

05:30I'm looking at some old pictures of us from the concert from a couple weeks ago.
05:35I'm smiling.
05:36It was a good time.
05:38Currently, I am in a relationship.

05:43It's been a little over a year.
05:45And we make each other happy off of the littlest things.
05:49When I get back, you have to take me to another concert.

06:13These scars remind me of everything that was taken from me.
06:20But these scars do not define me.
06:23They are not who I am.
06:25They're just a part of what happened to me.
06:30I would like to find out how I could get rid of my scars.
06:34I just don't want those signs.
06:36I don't want the evidence.
06:38Mm-hmm.
06:39Another thing is my tattoo of my ex-husband's name.
06:43I want that gone.
06:44Yes.
06:44The scars are mostly on my torso.
06:47I have a couple of marks on my arms and legs and hip area that I would like your help
06:56in hiding.
06:58And it's all about, you know, having that confidence.
07:00Not having to always have it in the back of your mind.
07:02Oh, I can't wear that even though I love it.
07:04It's so cute.
07:05I'm going to be showing somebody's going to ask me what that is.
07:08All the time that happens.

07:14And I don't want to tell my story to everyone.
07:17My story is my story.
07:19And I want to share it with whom I want to share it with when I am ready to share

07:24So we are going to just send you out for a little bit to get some scans and some images
07:28taken.
07:28And we're sort of going to look at everything and discuss what we think will be the best plan for
07:33you.
07:34We're going to get a game plan.
07:35It's going to be on.

07:49My tragedy was the lowest point of my life.
07:52It changed my life in many ways.
07:56I remember waking up at the hospital.
07:59I found out that when my ex-husband stopped shooting me, he took the lives of our children.
08:10I didn't find out my children were dead until four or five days after I woke up.
08:16I had to plan my children's funeral from a hospital bed.
08:22Every day is a constant reminder of me and my children, my children that I don't have anymore.
08:30I described my traumatic event as my rock bottom.
08:34But when I finally just really looked myself in the mirror, I said to myself, you can choose
08:41to cut yourself off from the world or you can appreciate what you still have.
08:48Yes, I lost a lot.
08:51But I still have my life.
08:52I still have my health.
08:54And with that, I do have a second chance.
08:58I finally feel like I have the courage to help take these marks of sadness off of me.
09:07Now I'm ready.
09:15So there's a lot of different ways that this can be approached.
09:19Yes.
09:19How would you deal with her scarring?
09:22I could definitely do some laser resurfacing, microneedling, a little bit of pigment laser.
09:29But the bigger scars would definitely be a cut.
09:32So I would defer to your expertise on those.
09:35The thing that bothers me a lot, especially in dark skinned people like myself,
09:39is when sutures are placed outside of the skin.
09:42Right.
09:42And like you can tell on that left shoulder scar that it's got those dot, dot, dot, dot, dot, dot.
09:48Yeah.
09:48And you see the pucker.
09:49Yeah, I call that train tracking.
09:50And it's like a pet peeve of mine.
09:51But in this situation, I mean, this was a trauma.
09:53They were just trying to get her closed.
09:55Just stop the bleeding, survival mode.
09:57I feel like if I can just cut them out, start from fresh, and then control how that scar heals,
10:03I think in my hands I get a better result.
10:07For the wide scars that she has, we are going to surgically remove them,
10:12suture it back together, and leave her with thin scars.
10:15For the midline scar that she has, I'm going to actually turn that into a fleur-de-lis tummy tuck.
10:22So I'm going to pull her waist together and then pull her down and get her flat.
10:26So we're turning a scar in the midline into something beautiful.
10:30It's actually going to help me pull her waist in and give her a beautiful hourglass shape.
10:35And plus we can do, like, so much more cherry on top with it.
10:39Oh, yes.
10:39She's going to get the stretch marks cut out and in the trash.
10:41You know, with the tattoo of her ex's name on there.
10:45Yes.
10:45Just, you know, laser that out.
10:47Disintegrate that.
10:48Yeah, that's got to go.

11:07So we went over some surgical options, some non-surgical options.
11:11And on your case, I want to take you for both.
11:16So to start off with the non-surgical, I would say, obviously, the tattoo removal.
11:21That's something that we have to get off of your person.
11:24Yes.
11:24Too much baggage with it.
11:26It has to go.

11:28Obviously, we're going to treat the scarring that you have on your body, on your hip, on your arms, in
11:33your abdomen especially.
11:34I do want to get you tight, tucked, and feeling really good.
11:39We want to make sure that nobody's asking you about what is this, what is that, that you're able to
11:43feel comfortable and normal, like you said.
11:46I want to really get you feeling lifted and pumped to look in the mirror.

11:59versus just talking to someone who is just listening to me vent.

12:23When I was in the hospital, recovering, there were times that I would have nurses just, gloves, putting them on,
12:31just the sound of the rubber glove popping.
12:33I would just get anxiety.
12:35Okay.
12:36The gloves.
12:37I knew after the gloves was always pain.
12:40Finally, one doctor said, this is for your health.
12:43This is to keep you alive.
12:45And I started to tame my anxiety.
12:48One, two, three.
12:49And now, knowing that there are ways to get rid of the evidence of my tragedy.
12:56I'm so deep for us.
12:58I finally feel like I have the courage to go through it voluntarily, going to a surgery,
13:05going to see a doctor.
13:07This opportunity is my chance to be happy and my chance for peace.
13:16Okay, let's take all the sheets off.
13:18Here.
13:19Okay, I'm gonna just inject her abdomen first, and then we'll flip her.
13:25One, two, three.
13:30So you can actually see this hole.
13:33So just so you guys know, she was shot nine times.
13:36She was shot?
13:37Yeah.
13:37So can you pull her towards you?
13:38And then I'm gonna slide this under.
13:40On Katrina, I look at the bullet wound on her back that looks like a deep, dark hole.
13:44She feels most insecure about that one.
13:47So we're gonna get rid of it first.
13:48Okay, so this is the bullet wound.
13:52And you can see it was just scarred in.
13:54When you release it, it comes back.
13:56It's almost not like a belly button when it was scarred in.
14:00Let me get my stitches, please.
14:01I'll take the PDS.
14:03Now that the scar has been cut out, I'm gonna get her sewn up with long-lasting sutures deep in
14:08the wound
14:09and shorter-lasting sutures right underneath the skin.
14:12Super important not to place outside sutures because I don't want her getting train tracking around the scar.
14:18And then I was thinking of just knocking these two out at the same time since she's already flipped.
14:24This is my last one I'm taking off.
14:26So those are the two bullet wounds removed.
14:30So we squish up the skin on purpose like that to take tension off of the epidermis.
14:36So I know right now it looks a little smushed, but that's on purpose.
14:41You wanna attach here to here like that.
14:44And that way there's no tension on that top part.
14:47You don't want your scar to look like this when you first sew it.
14:49You want it to look like this, like a hill.
14:51And that way when the sutures dissolve, it just goes like that.
14:54And it's thin.
14:55So it's cool.
14:57Next, I'm gonna flip her and get started on her horrible scar in the midline of her abdomen.
15:02I'm actually gonna turn that scar into an opportunity.
15:06So what I'm going to do is a fleur-de-lis tummy tuck.
15:10And that allows me to pull her waist in and then also pull the skin down.
15:15And that's gonna allow me to cinch her waist and flatten her tummy and really give her a beautiful hourglass
15:20shake.
15:25Right now it's just sewing till the end, my friend.
15:28We should weigh how much skin came off of her.
15:311025, that's 2.26 pounds.
15:35See, 2.2 pounds was taken off.
15:37That's actually a lot.
