A report has found unacceptable failings within the New South Wales department of communities and justice. As a result of those errors two foster children were allowed to live under the same roof as a convicted killer.

00:01This all began late last year, around Christmas time, and the Department of Communities and
00:06Justice, the DCJ, got wind that a child who was living in foster care was living under
00:11the same roof as a convicted triple killer.
00:14Now, this person is called Regina Arthurall, previously known as Reginald Arthurall.
00:20And the 79-year-old was convicted over the 1995 murder of a former partner, and they'd
00:27also been convicted over two separate manslaughter charges.
00:31So we're talking about three separate homicides.
00:34So what happened when DCJ found out about this living situation?
00:38Well, it turns out, according to this report, not much happened at all.
00:42The information was kind of triaged, it was peer reviewed, but it wasn't escalated.
00:48And that inaction basically led to another child then moving into the same home in March
00:54this year.
00:55So look, the Premier was asked about this today.
00:58He says it's a very, very serious incident, but he's hoping the government will be able
01:02to learn from the mistakes.
01:04This is a serious, serious situation.
01:07Whilst it's a difficult job, we know people who work in that part of the New South Wales
01:12government, by and large, do an incredible job.
01:14But in this instance, we want to make sure that the public's got confidence that even for
01:20these cases, or particularly for these cases, we're taking the warnings in.
01:24There's a range of recommendations have been made to try to prevent this reoccurring.
01:30There'll be better protocols when it comes to, I guess, the triaging of this kind of information
01:36when it comes.
01:37Better policies when it comes to escalating potential critical incidents.
01:41So, you know, essentially when DCJ finds out that children are living with a convicted
01:46triple killer, that actually gets run up the chain.
01:49And the report did find that there are already existing policies in place that could have
01:55prevented this, but they weren't always followed.
01:57And so, while the report found there were systemic issues within DCJ, it also found that the individual
02:04actions of some DCJ staff contributed to this scandal.
02:08And as a result, they'll be facing misconduct proceedings.
