00:00:00In the heart of Pittsburgh's busiest emergency department, Dr. Michael Rabinovich, better known as Dr. Robby, arrives for his grueling
00:00:0715-hour shift at Pittsburgh Trauma Medical Hospital.
00:00:10But today isn't just another day. It marks the fourth anniversary of his mentor Dr. Adamson's death during the height
00:00:16of the COVID-19 pandemic, a loss that still weighs on him, even as he insists he's fine.
00:00:21His team isn't convinced. Dr. Robby leads a group of dedicated, but often overworked doctors.
00:00:26Dr. Heather Collins, newly pregnant after struggling with infertility, a secret she's keeping for now.
00:00:32Dr. Jack Abbott, weighed down by grief, finishes his shift for the day.
00:00:37Before leaving, he briefs Robby on the patients expected to arrive, including a teenager scheduled for an abortion.
00:00:42Dr. Frank Langdon, an arrogant but skilled physician who has an old friendship with Robby.
00:00:47Dr. Cassie McKay, a 42-year-old second-year resident who made a late career change.
00:00:52Dr. Samira Slowmo Mohan, known for taking too long with patients.
00:00:56Much to Robby's frustration, they're joined by new interns and med students.
00:01:00Dr. Melissa King, a second-year resident, eager but socially awkward.
00:01:05Intern Trinity Santos, overconfident.
00:01:08Dennis Huckleberry Whitaker, a Nebraska farm boy and fourth-year med student, timid but well-meaning.
00:01:14Victoria Crash Javadi, a 20-year-old prodigy whose first day earns her a nickname for all the wrong reasons.
00:01:21Dana, the no-nonsense charge nurse who keeps everything running.
00:01:24Princess, multilingual.
00:01:26And Perla, her close friend.
00:01:29Kiara, a social worker dealing with the hospital's most delicate cases.
00:01:33They go on ward round, checking on few patients before chaos begins.
00:01:37Sam Wallace is rushed in with a cracked skull after jumping onto subway tracks to save a woman from an
00:01:42oncoming train.
00:01:43He's stitched up and intubated, but his condition remains critical.
00:01:46The woman he saved has a gruesome, degloving injury on her ankle.
00:01:50Worse, she doesn't speak English, making treatment even more complicated.
00:01:54Dr. Garcia wants to push morphine, but Collins warns that it'll cloud her mental status.
00:01:59They compromise on a popliteal block, numbing just the leg.
00:02:02Victoria faints during the woman's ankle reconstruction, earning her nickname when she's sent to the lounge to recover.
00:02:08Princess tries to identify the woman's language, but nothing sticks.
00:02:11The police suspect she was pushed onto the tracks, a possible hate crime.
00:02:15Hospital administrator Gloria interrupts, pressuring Robbie to reduce wait times and improve patient satisfaction.
00:02:21But he knows the real solution.
00:02:23More resources and better nurse pay isn't what the higher-ups want to hear.
00:02:27She gives him an ultimatum, step up or step aside.
00:02:30Otis Williams, a seasoned triathlete, comes in complaining of shortness of breath, then drops into cardiac arrest mid-exam.
00:02:36Dr. McKay resuscitates him, but when he crashes again, Robbie doesn't wait for lab results.
00:02:42Against protocol, he pushes calcium gluconate, convinced that he has rhabdomyolysis,
00:02:47a condition where muscle breakdown from intense exercise leading to kidney failure and sky-high potassium levels.
00:02:52Collins argues with him, but Robbie knows waiting will kill Otis.
00:02:56His call saves Otis's life, but the tension between him and Collins grows.
00:03:00Trinity bets Dennis that Victoria won't last the shift, but Dennis is more worried about his school loans.
00:03:04Trinity jokingly suggests he sell a kidney, claiming she can get him $30,000 by the weekend.
00:03:10Three elderly patients are en route from an assisted living home.
00:03:13One passes away before arrival, another is being kept alive by a Lucas chest compression machine, and the third is
00:03:19Mr. Spencer.
00:03:20However, they soon realize that the patient on the Lucas device has a DNR order.
00:03:25When they stop compressions, Robbie calls for a moment of silence, only for Dennis's phone to ring, ruining it.
00:03:31Crash, eager to prove herself, asks Robbie to let her back in.
00:03:35Instead, he orders her to go work in triage with Dr. McKay.
00:03:38Before she can protest, in walks Dr. Eileen Shamsie, a top-ranking surgeon at the hospital.
00:03:44She's here to check on her daughter, who just so happens to be Crash.
00:03:47Crash, flustered, tries to play off her nickname as Crush, because she's crushing it.
00:03:52She's not.
00:03:53It's later revealed that Crash's dad is also a doctor at the hospital, an endocrinologist.
00:03:57Robby, amused, asks Dana why she didn't warn him.
00:04:01Dana smirks, didn't want to ruin the surprise.
00:04:04A four-year-old boy arrives lethargic.
00:04:06His parents couldn't wake him up that morning.
00:04:08His labs are normal, until Langdon finds a green gummy in his mouth.
00:04:12The father suddenly remembers that his brother-in-law left gummies in his coat pocket the night before.
00:04:17A test confirms the boy ingested cannabis, and his mother is furious.
00:04:21Heather finally identifies the language of the subway woman.
00:04:24She's from Nepal, using a world map on her phone to communicate.
00:04:26Otis crashes again, this time due to fluid buildup around his heart.
00:04:31Robby performs an emergency ultrasound-guided pericardiocentesis, once again saving his life, despite Collins' objections.
00:04:38Trinity is impressed.
00:04:39Dennis handles his first case well, diagnosing a patient's gallstones.
00:04:43But when Princess suggests checking for a heart attack as well, he takes the idea.
00:04:48Teresa arrives in the ER with uncontrollable vomiting, brought in by her son David.
00:04:52But Dr. McKay notices something feels off about their dynamic, and calls in Robby.
00:04:57Alone with him, Teresa confesses.
00:04:59She faked her illness to get into the ER because she discovered a kill list in David's notebook, targeting his
00:05:04female classmates.
00:05:05David, still grieving his father's death from COVID, is deeply unstable.
00:05:09But since he's legally an adult, Robby says they can't force him into psychiatric care.
00:05:14When social worker Kiara tries to intervene, David bolts.
00:05:17Robby chases after him.
00:05:18But David vanishes into the street, and another emergency pulls Robby back inside.
00:05:23As Robby stands in the ER, exhausted, the weight of the day, and the anniversary of Dr. Adamson's death catches
00:05:29up to him.
00:05:30The walls close in, and suddenly, he's no longer in the present.
00:05:34Instead, he's back at the height of the COVID-19 pandemic, reliving his mentor's final moments.
00:05:39Robby is jolted out of an anxiety attack by Dana, just in time to face the next crisis.
00:05:4419-year-old college student Nick Bradley found unresponsive in bed.
00:05:48His pupils are unreactive, and while Narcan revives opioid overdoses, his respiration doesn't improve.
00:05:53No drugs or alcohol are found at the scene, making the case even more baffling.
00:05:58Robby presses Nick's parents for answers, but they insist he was a good kid who didn't do drugs.
00:06:03Princess arrives with the urine test results.
00:06:05Nick had fentanyl in his system.
00:06:07His parents are in disbelief.
00:06:09Before they can process it, Nick's condition worsens.
00:06:12His pupils blow, and his breathing stops.
00:06:16Robby immediately recognizes the heartbreaking truth.
00:06:18Nick is brain-dead.
00:06:20Rather than delivering the news outright, Robby orders more tests.
00:06:24Not because they'll change anything, but to give the parents time to say goodbye.
00:06:27Dr. Collins calls him out for offering false hope, but Robby sees it differently.
00:06:32Collins isn't just talking about Nick's parents, though.
00:06:34She's pointing out that Robby has been avoiding his own grief,
00:06:36refusing to confront Adamson's death and the trauma of the pandemic.
00:06:40He brushes her off.
00:06:41Robby finally delivers the devastating news.
00:06:44Nick's mother's anguished scream cuts through the ED as she begs them to shock him back to life.
00:06:49But there's nothing left to do.
00:06:51Meanwhile, another family struggles with an impossible decision.
00:06:54Mr. Spencer, an elderly Alzheimer's patient with pneumonia-induced sepsis,
00:06:58had an advance directive stating no intubation or life-prolonging measures.
00:07:01But when his children, Helen and Jeremy, arrive, they can't let go.
00:07:06Despite Robby's warnings that a ventilator will only prolong his suffering, they demand intubation.
00:07:11Collins urges Robby to fight it, to stand up for Spencer's wishes.
00:07:15But there's no time for legal battles.
00:07:17The family's decision stands, and Robby silently performs the intubation.
00:07:21Whitaker starts his shift feeling hopeful, bonding with his gallstone patient, Mr. Milton.
00:07:25The older man, proud of Whitaker's journey as the first in his family to attend college, offers words of encouragement.
00:07:32Even Robby praises Whitaker for listening to the nurses, an essential ER skill.
00:07:36But with the ED overcrowded, Milton is moved to the hallway to make space for critical cases.
00:07:41A gruesome case arrives.
00:07:42A man named Ben crashes his scooter without a helmet, suffering a Laforte III fracture.
00:07:47His face is literally separating from his skull.
00:07:49Dr. Langdon and Dr. Garcia argue over treatment, but Langdon uses the case as a teaching opportunity,
00:07:54allowing Dr. Melissa King to perform a cricothyroidomy under Robby's guidance.
00:07:59She nails it.
00:08:00However, when it's time for stitches, she freezes.
00:08:03Langdon steps in, calming her down and getting her back on track.
00:08:06Remember the four-year-old who ingested cannabis gummies?
00:08:09Kiara informs the parents that CPS has been notified, and the mother flips out,
00:08:13nearly causing a scene before Robby de-escalates the situation.
00:08:17Langdon dryly tells Melissa that if they weren't white, this would have ended very differently.
00:08:21Overwhelmed, Melissa steps outside, taking a deep breath,
00:08:24and reciting Megan Thee Stallion lyrics to calm herself.
00:08:27Her affirmation is interrupted by a van that dumps a gunshot victim, Alex.
00:08:32He's rushed into surgery immediately.
00:08:34Melissa asks if Robby doesn't need time to prep an OR, but he simply tells her,
00:08:38this is a trauma center.
00:08:40We always keep one stocked and ready.
00:08:42Melissa finds it awesome.
00:08:43The patient who was pushed onto subway tracks in episode one?
00:08:46It may have been a hate crime.
00:08:48Detective Underhill arrives to question Dr. Collins, and awkwardly flirts with her.
00:08:52Colin shuts him down.
00:08:54Robby sees the exchange and teases her, but she shuts him down just as quickly.
00:08:59Javadi shadows Dr. McKay, impressed by her skills, until her ankle monitor beeps,
00:09:03signaling she's outside her allowed zone.
00:09:05She's a single mom just trying to survive, but the alert raises eyebrows.
00:09:09The chaos hits a new level when an unconscious homeless man is brought in.
00:09:13As the doctors cut away his sweatshirt, three rats scurry out.
00:09:17The entire ER freaks out.
00:09:20Hospital administrator Gloria is furious.
00:09:22This is exactly the kind of PR nightmare that could get the ER shut down.
00:09:26Robby keeps an eye on Teresa, urging her to contact her son, David, the high schooler caught
00:09:30with a kill list.
00:09:31She still can't reach him.
00:09:33There's no history of violence, but the situation remains unsettling.
00:09:36A construction worker is electrocuted after cutting into a live wire.
00:09:40The team rushes to the rooftop to bring him down.
00:09:43He needs a fasciotomy to relieve pressure and restore blood flow.
00:09:46And Dr. Garcia suggests letting Trinity perform it.
00:09:49Robby and Collins object, but Garcia guides Trinity's hand, helping her complete the procedure
00:09:53flawlessly.
00:09:54Trinity immediately gloats to Javadi, who tells her to stop calling her crash.
00:09:58Trinity calls her sensitive.
00:10:00Watching from the side, Princess and Perla place their bets.
00:10:03One of them is getting slapped before the shift ends.
00:10:05Joyce, a distressed patient, is brought in screaming.
00:10:09The EMTs assume she's a drug seeker and attempt to cuff her to the gurney.
00:10:12But she's not.
00:10:13She has sickle cell disease and is in crisis.
00:10:17Dr. Mohan shuts the EMTs down and orders 20 milligrams of morphine.
00:10:21Later, she adjusts Joyce's treatment based on her usual pain regimen.
00:10:25But Whitaker makes an ignorant comment about not going crazy with the pain button.
00:10:30Mohan pulls him aside and lays it out.
00:10:32Sickle cell pain feels like glass flushing through your body.
00:10:35When he asks, what if she's faking it?
00:10:37She shuts him down.
00:10:38You can't fake a hemoglobin of 6.
00:10:40Still, she reassures him.
00:10:42Over time, he'll learn to spot the fakers.
00:10:45The shift has been overwhelming.
00:10:46But no one is prepared for the gut punch that ends the night.
00:10:49Whitaker, who started the day with so much hope, checks on his gallstone patient, Mr. Milton,
00:10:54only to find him unresponsive.
00:10:56No pulse.
00:10:57No one knows how long he's been like that.
00:10:59Desperate to save his first solo patient, Whitaker refuses to give up.
00:11:03He performs chest compressions.
00:11:06Whitaker desperately performs chest compressions on his patient, Milton, refusing to accept the inevitable.
00:11:11Dr. Robbie lets him continue for a bit, knowing that sometimes doctors need to feel like they've done everything before
00:11:16letting go.
00:11:16But there's no bringing Milton back.
00:11:19Eventually, Robbie gently tells Whitaker it's time to stop.
00:11:21They need the bed for another patient.
00:11:23This marks Whitaker's first patient death, and Dr. Melissa, in her usual offbeat way, admits she reacts emotionally to death
00:11:30but processes it differently.
00:11:32The team takes a moment to honor Milton, but it's clear Whitaker is deeply shaken.
00:11:36Robbie encourages him to talk to Kiara, the hospital's social worker, if he needs support.
00:11:41The weight of the moment is broken when Trinity, ever the ambitious intern, asks to perform a procedure on Milton,
00:11:47only for Robbie to remind her he's not a cadaver.
00:11:49Lily and John are clinging to hope for their son, Nick, even as Robbie gently but firmly tells them he
00:11:54is brain-dead.
00:11:55To give them closure, he agrees to conduct two final tests, the apnea test to check for brainstem activity and
00:12:02a cerebral perfusion study to assess blood flow.
00:12:04Chaos erupts in the ER when a frantic woman rushes in, screaming that her friend isn't breathing.
00:12:10Dr. McKay and Javadi sprint outside and find Gemma unresponsive in a car.
00:12:14They administer Narcan, and as they wheel her into the ER, she suddenly gasps back to life.
00:12:19The cause?
00:12:20A single half-pill of Xanax, laced with fentanyl.
00:12:23Back in the trauma bay, Nick's apnea test confirms what Robbie already suspected.
00:12:28Nick is gone.
00:12:29But his parents refuse to accept it.
00:12:31They ask Robbie if there's any other way to be absolutely sure.
00:12:34He tells them about a blood perfusion test, which would remove all doubt.
00:12:38Desperate for certainty, his parents request the test.
00:12:41But just as his father, John, is absorbing the devastating news, he spots a familiar face in the ER, one
00:12:47of Jenna's friends.
00:12:48Overcome with grief and rage, he storms over to her bed, demanding answers.
00:12:53Jenna yells that Nick gave her the Xanax as John screams,
00:12:56You killed my son! before the hospital staff pulls him away.
00:12:59Later, John apologizes to Robbie for his outburst, but his anguish remains.
00:13:04Why does Jenna get to live while my son is dead? he asks bitterly.
00:13:08Lily, devastated, whispers,
00:13:09None of this is fair. Nick was their only child.
00:13:13Another heartbreaking case unfolds as Helen Spencer watches her intubated father deteriorate.
00:13:17After a tense standoff with her brother Jeremy, Helen finally relents, tearfully giving Robbie permission to extubate their father.
00:13:24Meanwhile, Dr. Collins is caught off guard when the Nepalese translator, called to translate for the subway accident victim,
00:13:30immediately congratulates her.
00:13:31The victim can tell Collins is pregnant.
00:13:34Stunned, Collins quickly zips up her jacket, clearly trying to keep her secret under wraps.
00:13:38The ER is thrown into another high-stakes trauma when a construction worker, Hank, arrives with a nail gun wound
00:13:44to the chest.
00:13:45The nail has punctured his heart, meaning they can't just pull it out without killing him.
00:13:49Blood is pooling rapidly around his heart.
00:13:51Javadi intubates him flawlessly, but just as surgery is about to begin, she suddenly makes an excuse to leave the
00:13:56room.
00:13:57Given her recent history of fainting, it's not hard to guess why.
00:14:00How is she supposed to be a surgeon if she can't even handle the OR?
00:14:03Trinity, always eager for action, jumps in to take charge, having been craving something new all day.
00:14:09She and Dr. Garcia work together for a thoracotomy to suction blood and remove the nail, which Santos calls fucking
00:14:15cool,
00:14:16earning her an immediate reprimand from Collins, who reminds her this is real life, not a spectacle.
00:14:20Despite the chaos, the surgery is a success, and Hank is stabilized.
00:14:25Robbie juggles a heart attack patient, putting in a catheter, and against the odds, both cases end in victory.
00:14:31Santos and Melissa high-five, hyped about the two saves.
00:14:34But in the ER, there's never time to celebrate.
00:14:37Trinity continues to clash with her colleagues, persistently calling Javadi by her crash nickname, despite clear irritation.
00:14:43She even admits to Javadi that she only wants to befriend her so her mom can write her a strong
00:14:47letter of recommendation.
00:14:49Whitaker, still rattled from Milton's death, wants to focus on low-risk cases, but Dr. Mohan refuses to let him
00:14:55check out.
00:14:55His rough day only worsens when he spills an entire bottle of Mylanta on himself and has to deal with
00:15:01the scrubs machine,
00:15:02an embarrassing moment on top of an already difficult shift.
00:15:05Robbie notices Mohan still with the same sickle cell patient hours later, far behind where she should be.
00:15:10When he calls her out, she insists she's giving the patient the utmost care.
00:15:14Then why don't you go into psychiatry?
00:15:16Robbie challenges, but Mohan's hesitation reveals a deeper truth.
00:15:20She's lost a patient before and doesn't want to repeat the mistake.
00:15:23Robbie reminds her that in the ER, losing patients is inevitable.
00:15:27It's a lesson Mohan will have to accept if she wants to survive an emergency medicine.
00:15:31Dr. McKay continues pressing Robbie to do something about David,
00:15:34the student with the disturbing hit list, who hasn't been seen at school.
00:15:37She warns Robbie that she's been on the other side of someone who hates women and fears for the girls
00:15:42on David's list.
00:15:43But Robbie hesitates.
00:15:44If it's a misunderstanding, he doesn't want to ruin the kid's life.
00:15:47Eventually, Robbie asks Chiara, the hospital social worker,
00:15:50if there would be any legal repercussions for luring David to the hospital under false pretenses.
00:15:55He considers calling David and lying that his mother's condition has worsened to get him to come in.
00:16:00Chiara reassures him that since it's a life-or-death situation, he can do it.
00:16:04Just when it seems the day couldn't get any wilder, an ambulance is stolen right outside the ER.
00:16:09In the chaos, one of the EMTs is injured, their arm struck as the vehicle speeds off.
00:16:15Dana, smoking outside, witnesses the whole thing.
00:16:18Dana rushes in, announcing that an ambulance has been stolen from the hospital bay.
00:16:22Security immediately chastises the EMT responsible for leaving the keys inside
00:16:26before reporting the theft to the police.
00:16:28Dr. Robbie helps extubate Mr. Spencer to ensure he is as comfortable as possible in his final moments.
00:16:33For his children, Helen and Jeremy, the weight of this decision is unbearable.
00:16:38Helen, in particular, spirals.
00:16:40She can't believe today is the day they've just decided their father is going to die.
00:16:43Amidst their grief, Robbie introduces them to Ho'oponopono,
00:16:47a Hawaiian healing ritual he learned from a mentor, likely the late Dr. Adamson.
00:16:51He explains that saying four simple phrases,
00:16:54I love you, thank you, I forgive you, please forgive me,
00:16:58can ease the early stages of loss.
00:16:59The Spencers are moved to a pediatric room, decorated with a mural of cartoon forest animals,
00:17:04a choice that unsettles Robbie.
00:17:06This is the same room where Dr. Adamson died from COVID, and the memories sting.
00:17:10But for Helen and Jeremy, the setting feels eerily fitting.
00:17:13Their father spent 33 years in the art department for Mr. Rogers' Neighborhood,
00:17:17bringing the neighborhood of make-believe to life.
00:17:19Now, in a space that mirrors that whimsical world, they say their final goodbyes.
00:17:23Helen's farewell is particularly heartbreaking.
00:17:26She confesses that, as a child, she was embarrassed by her father's career,
00:17:30rejecting his world of imagination.
00:17:32Unlike Jeremy, who bonded with their dad over baseball and fishing,
00:17:35she always felt disconnected.
00:17:37So she left for college and never returned home.
00:17:40Now, regret crashes over her.
00:17:42Through tears, she apologizes.
00:17:44She asks for his forgiveness.
00:17:46And finally, she calls him Daddy.
00:17:48As she and Jeremy promise to stay close, their father takes his final breath.
00:17:52Helen gasps, the raw, soul-shattering sound of losing a parent.
00:17:56Robbie forces himself out of the room and into the bathroom.
00:18:00His composure cracks.
00:18:01He nearly has a panic attack before finally relieving himself.
00:18:04Doug Driscoll, an agitated patient with heart palpitations,
00:18:07grows increasingly angry over his long wait time.
00:18:10When Nurse Mateo finally attends to him and tells him he'll need to wait for lab results in the waiting
00:18:14room,
00:18:15Doug mutters a racist comment.
00:18:17This place is turning into a third-world country.
00:18:19Reminds you of home, doesn't it?
00:18:21Mateo keeps his cool, but the tension lingers.
00:18:24Not far away, Mr. and Mrs. Bradley walk beside their son Nick's gurney
00:18:28as he's wheeled off for a cerebral perfusion test.
00:18:31Racked with guilt, Jenna, who is being discharged, approaches Nick's dad.
00:18:35She explains what happened.
00:18:36Their econ study group had been up late, struggling with sleep, and someone suggested Xanax.
00:18:41She doesn't expect her words to ease his pain, but she offers her help in any way she can.
00:18:46Watching this, Trinity has an overdue moment of self-awareness.
00:18:50Earlier, she had dismissed Jenna's emotions, choosing to distract her with small talk rather
00:18:54than addressing her trauma.
00:18:55When Dr. Mohan had called her out on her lack of bedside manner, she'd been defensive.
00:19:00But now, seeing Jenna's raw grief, she apologizes to Dr. Samira for being, well, an ass.
00:19:06Trinity's moment of self-reflection is short-lived.
00:19:09When Wendell Stone, a festival worker, is rushed in with a tension pneumothorax,
00:19:13a collapsed lung caused by a massive speaker falling on him, Garcia lets Trinity present the case.
00:19:18She does so flawlessly, and they decide on a nerve block for pain management instead of morphine,
00:19:22since Wendell needs to stay alert to oversee his workers.
00:19:25Later, when Trinity brings Wendell back from a CT scan, he suddenly struggles to breathe.
00:19:30Without consulting a senior doctor, she jumps into action and orders a BiPAP machine to help
00:19:34with his breathing.
00:19:35Unfortunately, it's the worst possible treatment for his condition.
00:19:39It makes things worse.
00:19:40Dr. Langdon rushes in and intervenes, stabilizing Wendell.
00:19:44But he's had enough of Trinity's reckless decision-making.
00:19:47She's an intern, barely four hours into her shift, and she keeps making life-threatening
00:19:51mistakes.
00:19:52However, when Dr. Garcia walks in and hears about the BiPAP error, she's furious, until
00:19:57she learns Trinity was the one who ordered it.
00:19:59Suddenly, Garcia backtracks.
00:20:01She's an intern.
00:20:02She's bound to make mistakes.
00:20:04Everyone notices the shift, even Robbie.
00:20:07Dr. Garcia isn't exactly subtle about her soft spot for Trinity, something Wendell had
00:20:11pointed out earlier when she was setting up the BiPAP.
00:20:13Trinity hadn't believed it then.
00:20:15But now?
00:20:16It's impossible to ignore.
00:20:17If anyone is having a worse shift than Trinity, it's medical student Whitaker.
00:20:22Whitaker is now pulled into restraining a meth addict, the Ratman, who has woken up violently
00:20:26thrashing.
00:20:27The nurses hold the man down while Whitaker administers psych meds, but not before the
00:20:31patient urinates all over him.
00:20:33The poor kid just cannot catch a break.
00:20:35Dr. Victoria Javadi treats Tasha, a trans woman who has cut her arm.
00:20:39While stitching her up, Javadi notices a misgendering error in the hospital records and fixes it
00:20:44and also discovers that Dr. McKay is in Alcoholics Anonymous, adding another layer to her curiosity
00:20:49about her mentor.
00:20:50Dr. Mohan and Dr. Melissa struggle to diagnose a five-month-old baby who won't stop crying
00:20:55until they realize a strand of hair is wrapped tightly around the baby's toe, cutting off
00:20:59circulation.
00:21:00It's a simple fix.
00:21:01Then another patient comes in a 12-year-old with pubic pain, GIA-Yi.
00:21:04She's diagnosed with an imperforate hymen, a rare condition that has caused months of excruciating
00:21:10pain from backed-up menstrual blood.
00:21:11Another patient is brought in, who feels there is something moving in her ear, Dr. Langton
00:21:15attends to her, and it turns out it's a dead cockroach.
00:21:18As local news covers the stolen ambulance barreling through the city, hospital staff start placing
00:21:22bets on where it will finally stop.
00:21:25Even Kiara, the sweet social worker, joins in on the gambling.
00:21:28Dr. Abbott had mentioned that a pregnant teenager was arriving for a medical abortion before going
00:21:33home. When she finally arrives with her mom, Dr. Collins performs an ultrasound before medical,
00:21:38but something seems off. While Mathia walks a patient out, explaining his medications,
00:21:42another patient in the overcrowded waiting room starts seizing. The seizing patient is rushed into
00:21:47the ER, and Dr. Langton immediately takes charge alongside Trinity. They administer lorazepam,
00:21:53but when the seizures persist, Langton orders another 4 milligrams. Trinity struggles to open
00:21:58the vial, but eventually does, warning that if it doesn't work, they'll need to prep Keppra.
00:22:03Langton dismisses the suggestion, stating that if the seizures continue, they'll intubate and
00:22:07administer more lorazepam instead. When Robby steps in, Trinity pushes for Keppra again,
00:22:12but Robby shuts her down, emphasizing that it's Langton's patient, so it's his call.
00:22:16Despite her complaints, the lorazepam ultimately works. Afterward, Langton reprimands Trinity for
00:22:22questioning him in front of Robby, reminding her that as an intern, she needs to respect the hierarchy.
00:22:26Trinity, however, is fixated on the vial of lorazepam, suspecting that the tight seal might
00:22:31indicate a larger issue. She brings her concerns to Dana, who grants her permission to investigate
00:22:36further. Whitaker, who's already changed scrubs twice, is assigned a seemingly simple case,
00:22:41a cyclist, Bob Chazin, who injured his leg 10 days ago. The diagnosis is a basic hematoma,
00:22:47requiring debridement to remove dead skin. Whitaker, frustrated with being given low-risk cases
00:22:52after previously losing a patient, insists he's fine, but the caution remains. While
00:22:57debriding the wound, an unexpected arterial pumper bursts, spraying blood all over him,
00:23:02almost getting into his mouth. Luckily, Mel walks in at the right moment, using a blood pressure cuff
00:23:06to cut off circulation and stop the bleeding. The irony isn't lost on anyone. Whitaker needs yet
00:23:12another change of scrubs. Later, Mohan slowly releases the cuff, and Whitaker is tasked with
00:23:17stitching the patient up. Understandably, Chazin is now wary of him. Whitaker's nerves are evident,
00:23:22but Mohan encourages him. When Robbie enters, Mohan reveals that the arterial rupture was actually a
00:23:28blessing in disguise. If it had happened at home, Chazin could have bled to death. Whitaker is relieved,
00:23:33but his clothing luck runs out again. He's stuck wearing morgue scrubs for the rest of the day,
00:23:37the only ones in his size. Doug has been waiting for five hours and is seething. When Dr. McKay and
00:23:43Dr.
00:23:43Javadi enter the waiting room, he confronts them. It's finally his turn, but only for a temperature
00:23:48check. Then, it's back to the waiting room. Doug is this close to snapping. Dr. McKay treats Sherry,
00:23:55an unhoused mother, with a burn injury. Sherry is exhausted, emotionally and physically,
00:23:59and after some gentle encouragement she lets her guard down. She confesses how much she feels she's
00:24:04failed her kids. McKay, who is also a recovering addict fighting to regain joint custody of her son,
00:24:09reassures her. No one has it together. But just as Sherry begins to trust her, Javadi swoops in,
00:24:15immediately suggesting a social worker. Sherry freezes. She didn't expect McKay to tell anyone.
00:24:21Furious, McKay calls out Javadi, emphasizing that impact matters more than intent. She also
00:24:26confronts Javadi about the judgmental glances she's been receiving, ever since she noticed McKay's
00:24:30ankle monitor. A surprise visitor turns heads in the ER, a teenage boy receiving hugs from nearly
00:24:36everyone. He's soon revealed to be Robbie's 17-year-old son, Jake. Instead of attending a
00:24:41music festival with his dad, he's going with his girlfriend, much to the amusement and concern
00:24:45of the staff. Princess cheekily slips Jake a condom for safe sex. When Robbie tries to give him
00:24:51something, Jake refuses, thinking it's another condom. But Robbie hands him his festival pass instead,
00:24:56warning about taking things from strangers after seeing two fentanyl overdoses. Then he bluntly asks if
00:25:01Jake is having sex. Jake, mortified. Bolts. An elderly woman, Ginger Kitajima, is brought in by
00:25:08her daughter Rita after supposedly tripping into a rosebush. It turns out Ginger has schizophrenia,
00:25:13but her only injury is an isolated shoulder issue. Melissa connects with Rita, sharing her own
00:25:19experiences caring for her sister, and advises her to seek help for caregiver fatigue. Rita is later
00:25:24called to move her car, which is blocking an ambulance bay, but hours pass and she doesn't return.
00:25:29As Ginger is ready for discharge, Persa voices a concern. She suspects Rita may have abandoned her
00:25:35mother at the hospital, possibly influenced by Melissa's advice. Robbie and Mohan check on Joyce,
00:25:40the sickle cell patient, who is becoming critically hypoxic. The transfusion meant to improve her oxygen
00:25:45levels isn't working, putting her at risk for a stroke, heart attack, or death. Robbie orders Mohan to
00:25:51get her consent for intubation quickly. Later, Robbie finds that Joyce still hasn't signed the forms.
00:25:56Frustrated, he criticizes Mohan for getting too emotionally involved. She needs to treat Joyce
00:26:01as a patient, not a friend, because every second wasted puts her life at risk. When the time comes,
00:26:06Joyce finally consents. Collins steps in to oversee the intubation, while keeping Joyce's wife,
00:26:11Andine, informed. Afterward, Mohan tries to apologize to Collins for taking too long,
00:26:16but Collins instead praises her compassion. She reassures Mohan that caring about patients
00:26:20isn't a weakness. It's what makes a great doctor. A teenage patient, Travis, is rushed in after
00:26:26suffering a severe post-tonsillectomy hemorrhage. Initially, Whitaker manages to suction the minor
00:26:31bleeding, but things take a dramatic turn. Blood suddenly pours from Travis's mouth,
00:26:36drenching Whitaker again. At this point, he practically needs an exorcism. With blood flooding
00:26:41the airway, a standard intubation is impossible. Dr. Garcia suggests a cricothyroidomy,
00:26:46but Robby and Langdon push for a non-surgical approach. Robby proposes a retrograde intubation,
00:26:51a high-risk technique involving threading a guide wire from below to secure an airway from above.
00:26:56Against the odds, the technique succeeds. Afterward, Robby surprises Langdon as the fist bump
00:27:02by revealing that he's recommended him for the Emergency Medicine Fellowship,
00:27:05a recognition Langdon never expected. The mystery of Christie's ultrasound discrepancy is solved.
00:27:11Abbott had deliberately altered the measurements. His report showed Christie's fetus at 10 weeks,
00:27:155 days, making abortion legal, while Collins' scan put it at 11 weeks, 2 days, making it illegal.
00:27:21Collins confides in Robby, fearing the repercussions of falsified records. But Robby,
00:27:26having tenure and job security, steps in. Dana, observing their dynamic, picks up on some
00:27:32will-they-won't-they tension between them. It's confirmed. Collins and Robby were a thing in the
00:27:36past, though Collins insists it'll never happen again. Dana isn't convinced, teasing her with a
00:27:41cheeky, never say never. Robby discreetly reassigns Christie's case to himself. He informs her and
00:27:47her mom that a, quote, computer glitch requires a redo of the ultrasound. Conveniently, the new scan
00:27:53places Christie at 10 weeks, 6 days, making her abortion legal. But just as Collins hands over the
00:27:58medication, Christie's real mother storms into the room, furious. The woman Christie came with was
00:28:04actually her aunt, and now her mother is determined to stop the procedure. Since she isn't 18,
00:28:08her mother's consent is required. As the staff watches the news, reports come in about the runaway
00:28:14ambulance crashing. Everyone gears up. The ER is about to get even busier. The heated argument
00:28:19between her mother, Eloise, and her aunt, Lynette, nearly turns physical, but Robby steps in before it
00:28:24gets worse. He insists that Christie and her mother talk privately. Unfortunately for Christie, as a minor,
00:28:30she has little say in the matter, despite clearly not wanting to leave. Robby runs into Gloria and the
00:28:35regional manager of ECQ America, Dr. Tracy Morris. They discuss plans to merge the ER into a national
00:28:41conglomerate, a move Robby immediately recognizes as bad news. Such a merger would prioritize profit
00:28:46over patient care, leading to staff cuts and reduced wages for doctors. Robby pushes back, pointing out
00:28:52the growing six-hour wait times, but Gloria dismisses his concerns, demanding they work with what they
00:28:57have. Just as Robby is about to explode, Dana skillfully intervenes, preventing him from saying
00:29:03something he might regret. Ginger's missing daughter remains a pressing concern, leaving Melissa at a
00:29:08loss. She's advised to talk to Chiara, but instead finds solace in Dana, who reassures her that the
00:29:13situation isn't her fault. During lunch, she steps outside to check in on her sister. Over in the ER,
00:29:19Dr. Garcia takes charge when Silas, a father in his 40s, arrives after falling from a ladder during home
00:29:25renovations. What starts as a minor hemothorax quickly escalates into a major one, requiring an
00:29:30emergency chest tube. Eager to prove herself, intern Trinity Santos jumps in, only to drop a scalpel
00:29:36straight into Garcia's foot. In a testament to her resilience, Garcia powers through the procedure,
00:29:41cutting her out of the case and letting Whitaker take over. Later, as she sutures her own foot,
00:29:46Garcia gives Trinity a much-needed lesson. There's a fine line between confidence and arrogance.
00:29:51The mystery of the stolen ambulance is finally solved. It turns out two frat boys took it for a
00:29:55joyride before crashing into a tree. Zach escapes with minor lacerations, but his friend Miles isn't
00:30:01so lucky. His collarbone is pushing against his windpipe and requires immediate intervention.
00:30:06It's a gruesome but effective fix, and with a dislocated hip on top of that,
00:30:10Javadi steps up and performs her first successful hip reduction, with Langdon guiding her.
00:30:14She's proud of herself, and rightly so. The ambulance theft also settles the ongoing bet in the ER.
00:30:19Collins wins $850 and debates splurging on a fancy stroller. Dana, one of the only people who
00:30:25knows about her pregnancy, encourages her to treat herself. Robbie checks in on Mohan's patients,
00:30:30including a woman suffering from a botched butt injection. When asked where she got the procedure,
00:30:35the woman reveals it was done by a guy from TikTok, who may have injected her with silicone
00:30:39caulk from Home Depot. Robbie can't help but exclaim, Jesus. Later, Robbie reluctantly holds a team
00:30:45meeting, trying to appease both the administration and his overworked doctors. His new directive?
00:30:50Discharge patients before taking on new ones to reduce wait time and meticulously document every
00:30:55treatment, most likely to maximize billing. The doctors, especially Langdon, are not happy about
00:31:01the extra workload, but there's not much they can do. Things take an awkward turn when Dr.
00:31:05Eileen Shamsi returns to the ER to teach Javadi and Whitaker. Javadi pointedly refuses to answer her
00:31:11medical questions, prompting Eileen to remark, Honey, we read this at home. Whitaker is shocked
00:31:16to realize that Eileen is Javadi's mother. Noticing the tension, Dr. McKay swiftly fakes a patient page
00:31:21to help Javadi escape the situation. Trinity redeems herself when she notices something others missed in
00:31:27Silas. Irregular breast tissue enlargement. It could signal alcoholism, a pituitary tumor,
00:31:32or another hormonal imbalance that may have contributed to his fall. Robbie is genuinely impressed by the catch.
00:31:38Later, Whitaker checks in on Trinity, and when Javadi overhears, she asks what happened.
00:31:43Trinity initially offers a half-truth, saying Whitaker took her chest tube opportunity,
00:31:47but then admits she dropped a scalpel into Garcia's foot. Given how much she's teased Javadi and
00:31:52Whitaker in the past, her screw-up shifts the dynamic. They tease her back, joking that it's
00:31:56worse than fainting. Trinity takes it in stride, calling them her fellow losers before walking off.
00:32:01The results of Nick Bradley's cerebral perfusion test confirm the worst. He is completely brain-dead.
00:32:07Robbie breaks the news to his devastated parents, introducing them to a grief specialist.
00:32:12Though they begin to accept Nick's passing, things get complicated when they learn he's an organ donor.
00:32:17His mother, Lily, protests. In the midst of all this, Dylan, a new patient, arrives with a twisted
00:32:22testicle. Collins attempts to correct it, but turns it the wrong way first. Ouch. Thankfully,
00:32:28he's okay in the end. McKay also convinces Robbie to keep David's mother, Teresa, in the hospital for
00:32:33observation, after she took pills to make herself sick. She argues that she could be a danger to
00:32:38herself. But we know it's not out of concern for the woman. Once David's mom leaves, they'll lose
00:32:43track of him. She also urges Robbie to report David to the police. But Robbie is still hesitant.
00:32:48Robbie arranges for her to see a psychologist at 1.30pm. Langdon handles a bizarre case involving dead
00:32:54brain worms in a patient's brain. But the biggest shock of the day comes when the team receives a
00:32:58generous lunch delivery. Everyone digs in without checking who sent it, until Robbie finally reads
00:33:03the card. The food is from Shelby Adamson, the sister of the late Dr. Adamson, who sends lunch
00:33:07every year on the anniversary of her brother's death. Back in Christy's case, Christy tells her
00:33:12mom she's nauseous and proceeds to lock herself in the bathroom, refusing to leave. As Eloise and
00:33:17Collins bang on the door, Lynette arrives and calls out her sister's hypocrisy. She's treating Christy
00:33:22just as cruelly as their own mother treated her when she was young and pregnant. The argument escalates
00:33:26until Eloise shoves Collins. It's not a hard hit, but it's enough for Collins to decide she's done
00:33:30playing Peacemaker. Christy remains locked in the bathroom, refusing to face the reality of her
00:33:35pregnancy, while Robbie vents his frustrations to Gloria about the overwhelming patient load
00:33:40and long wait times. Gloria steps away, and Dana playfully teases Robbie about being hangry.
00:33:45A new patient arrives. Terrence, an autistic man with a sprained ankle. Langdon struggles to communicate
00:33:51with Terrence, overwhelmed by the patient's barrage of questions. Seeking a breather outside the room,
00:33:56he runs into Melissa, who immediately understands what's happening. She offers to take over and
00:34:01handles Terrence with remarkable ease, impressing Langdon. Melissa later reveals that her skill
00:34:06comes from experience. Her sister is on the spectrum, and they share a close bond. When Trinity informs
00:34:11Silas's wife, Susan, of his breast enlargement, Susan confesses something chilling. She has been
00:34:16secretly spiking Silas's coffee with progesterone to kill his libido. The reason? She suspects Silas is
00:34:22molesting their teenage daughter, Alana. He goes to her bathroom when she is showering and gives her
00:34:26unwanted massages, and Alana has been struggling in school. As Trinity is processing this, another
00:34:31emergency rolls in. Nandi, a beauty influencer, arrives in a manic state after screaming at traffic
00:34:37and behaving erratically. Robbie pushes for a psych consult, but Mohan picks up on small neurological
00:34:42symptoms that suggest something else is at play. Digging into Nandi's skincare routine, she suspects that
00:34:48one of the products contains dangerously high mercury levels. Mohan knows Robbie won't approve
00:34:53the heavy metals test, so she gets Collins to sign off instead. When Robbie finds out, he explodes.
00:34:58Don't ever do that again, he snaps at Mohan, but his real rage is directed at Collins. He accuses her
00:35:04of being rigid and overstepping. Collins fires back, calling him out for his erratic behavior in the way
00:35:09he's been undermining Mohan, shaking her confidence. She demands that he leave his baggage at the door,
00:35:15like everyone else. Dr. Collins approaches Christy's mother, Eloise, in the waiting area,
00:35:20where she remains distant and firm in her stance. She believes Christy has had ample time to come to
00:35:25terms with her pregnancy and should move forward. Eloise reflects on her own experience. She was also
00:35:30pregnant at Christy's age, and while it was challenging, it ultimately turned out to be a
00:35:34positive experience, at least until now. However, Dr. Collins speaks candidly, cautioning Eloise that
00:35:40pressuring Christy may only drive her further away. Just because motherhood was the right path
00:35:45for Eloise doesn't mean Christy will feel the same. The words strike a chord, and after some
00:35:49consideration, Eloise relents, agreeing to let Christy take the pills. The subway hero Sam regains
00:35:55consciousness and insists on seeing Minu, the young woman he saved. Though they don't share a language,
00:36:00their gratitude is evident as they clasp hands. Sam vows to help the police find the attacker.
00:36:06Back in Silas's case, Trinity feels strongly that Susan is right about the molestation,
00:36:11but without Alana's testimony, they have no concrete evidence.
00:36:14Taking matters into her own hands, Trinity subtly asks Alana if her father has been abusing her.
00:36:19The uncomfortable confrontation sends Alana running back to her mother.
00:36:23Ravi and Kiara inform Susan that because she admitted to poisoning Silas, which caused him to
00:36:28fall from the ladder, the doctors are legally obligated to report her to the authorities,
00:36:31even though she was acting out of desperation. Now, if she's arrested, Alana will be all alone
00:36:36with Silas at home. Trinity isn't willing to let things slide. With Silas sedated but conscious,
00:36:41she leans in close and delivers a chilling warning. I know what you are. I know what you've done.
00:36:46And if you ever touch Alana again, I will make sure you pay for it. The weight of her words
00:36:50suggests
00:36:51a painful past of her own. Harvey Chang arrives in cardiac arrest and goes into V-fib. Despite multiple
00:36:56rounds of CPR, including mechanical compressions from Lucas's heart machine,
00:37:00Harvey remains unresponsive. The team resorts to ECMO, a life-saving procedure that oxygenates
00:37:05the blood outside the body when the heart and lungs fail. When Whitaker asks why ECMO wasn't
00:37:10used to save Mr. Milton, Langdon explains the strict criteria. It requires CPR within five minutes
00:37:16of collapse, and they didn't know how long the earlier patient had been down. As the team preps
00:37:20the ECMO, Robbie is hit with a brutal flashback. During Adamson's COVID treatment, he had been on ECMO
00:37:26for 17 days with no improvement, only for the machine to be taken away to save a 12-year-old
00:37:30child instead. Amid the chaos, Collins has been quietly enduring stomach pain throughout the
00:37:35shift. When it becomes unbearable, she stumbles into the bathroom, where her worst fears are
00:37:39confirmed. She's having a miscarriage. Alone, she processes the devastation as she cries out.
00:37:45After sobbing alone in the bathroom, silently processing the miscarriage she suffered,
00:37:49Collins wipes away her tears and heads to a ward, where she performs an ultrasound to confirm the loss.
00:37:54Just as she's finishing, Robbie walks in and notices something is off. Though he doesn't fully
00:37:59understand what he's witnessing, he can tell something is wrong, but she brushes him off and
00:38:03returns to work. Louis is back in the ward again, blind drunk, and Langdon notes that he was given
00:38:0820 librium that morning. But when Trinity checks, there are only 10 left. She presses Louis, who is
00:38:14too intoxicated to give a straight answer. Langdon shrugs it off, suggesting that Louis might have pawned
00:38:19the pills for alcohol. But Trinity isn't convinced. She's already had suspicions, since she struggled to
00:38:25open a vial earlier. Determined to dig deeper, she consults Dana, who shows her the hospital's
00:38:30automated drug storage system. Everything appears normal. Before Santos can investigate further,
00:38:34she's pulled into an urgent amputation case. During a VY flap procedure, a surgical technique used to
00:38:40close gaps after an amputation, Rocco, a psychology student and HVAC technician, seizes the opportunity to
00:38:46flirt with Collins, casually asking about her plans after work. Collins shuts him down with a blunt reply.
00:38:51Sleep. A new patient arrives in excruciating pain, but insists only Dr. Shamsi treat her.
00:38:56She's allergic to morphine, complicating pain management. Garcia and Trinity pull Javadi into
00:39:02the case. While Shamsi orders a scan, Javadi digs deeper, questioning the patient about her routine.
00:39:07She soon discovers that the true cause isn't her Crohn's disease. It's a black widow spider bite.
00:39:12As she treats the patient, Shamsi arrives, impressed by Javadi's sharp diagnostic skills.
00:39:17Willie, an elderly patient, is rushed in with a dangerously low heart rate of 30 BPM.
00:39:22Despite receiving three atropine doses, his pacemaker isn't working properly. Surprisingly,
00:39:28Willie displays impressive medical knowledge, even inquiring about Dr. Adamson,
00:39:32a name that visibly unsettles Robbie. The team soon discovers the issue.
00:39:36Willie's pacemaker was twisted, causing the malfunction. Once corrected,
00:39:40his condition stabilizes. It turns out Willie isn't just any patient. He was part of the Freedom
00:39:45House Ambulance Service, the pioneering paramedic team composed entirely of black medical professionals.
00:39:50Mohan, after testing Nandi's skincare products, finds dangerously high mercury levels in one of
00:39:55her face creams, explaining all her symptoms. She also helps the influencer record a PSA warning
00:40:00her followers. But Dana and Robbie let it slide, Dana joking that he pushed for higher patient
00:40:05satisfaction scores anyway. Rita didn't abandon her mother after all. She had fallen asleep in
00:40:10the car after parking. Mel and Chiara help her explore Medicare options to lighten her burden,
00:40:15and she cries tears of joy as she never expected it. Now that her relationship with Garcia has improved,
00:40:20Trinity confides in her about her suspicions that Langdon might be stealing drugs. However,
00:40:24Garcia isn't interested in entertaining accusations, reminding Trinity that she's only been at the pit for
00:40:29seven hours. She defends Langdon, saying, I insult the shit out of him, but he's a great doctor,
00:40:34shutting Trinity down. The trauma of this episode centers on six-year-old Amber, who was brought
00:40:39into the ER in cardiac arrest and hypothermic after nearly drowning. The doctors fight desperately to
00:40:44revive her, trying to raise her body temperature in hopes of restarting her heart. Amber's grandmother
00:40:49and little sister Bella arrive, and the terrible truth unfolds. The girls had climbed the fence to
00:40:54retrieve a soccer ball from the pool. The water was too deep, and Amber drowned. When Amber's parents
00:40:59arrive, the team continues fighting for her, but her blood work confirms the heartbreaking reality.
00:41:04Her potassium level is at 12.2, a fatal number. With quiet empathy, Robbie makes the final call.
00:41:10Amber has died. Later, Collins, Chiara, and Melissa visit Bella, who reveals that she was the one who fell
00:41:16into the pool. Amber saved her, but couldn't save herself. Bella, still hopeful, colors a rainbow for
00:41:21her sister to give her when she wakes up. Melissa gives Bella a teddy bear and tells her that if
00:41:26she
00:41:26speaks into it, Amber will hear her. Bella, not yet grasping the loss, whispers into it, thanking Amber
00:41:32for saving her and calling her her best friend. Dr. McKay takes on a troubling case involving 18-year-old
00:41:37Piper, who arrives with her overbearing boss, Laura. Laura answers all of Piper's questions for her,
00:41:42immediately raising red flags. McKay and Dana step in and find a way to separate Piper from Laura by
00:41:47scheduling a CT scan, which Laura, who is pregnant, must avoid due to radiation exposure. In private,
00:41:53they inform Piper that she has chlamydia. Piper insists she contracted it from an ex-boyfriend
00:41:58back home, but Dana notes that her address matches Laura's. Piper claims she's just staying with her
00:42:03due to housing issues, but McKay suspects a trafficking situation. With Piper unwilling to talk,
00:42:08they come with another test to still keep her in the hospital. An honor walk is held for 18-year
00:42:12-old
00:42:12Nick Bradley. His parents, John and Lily, have made the selfless decision to donate his organs
00:42:17after speaking to their pastor. The hospital staff and friends from college lines the hallways as
00:42:22Nick is wheeled to an ambulance, draped in a stealer's blanket. RIP Nick. The trauma of Amber's
00:42:28death still lingers in the ER as the cleaning staff arrives, picking up the last remnants of the tragedy,
00:42:33including the tiny hair clip Esme finds on the floor. The doctors wrestle with their own grief.
00:42:37Langdon calls home just to hear his son's voice. Mel loses herself in calming ocean sounds.
00:42:44Javadi, shaken from her first major loss, finds unexpected reassurance from Trinity.
00:42:49Dr. Robbie, the team's emotional anchor, shares a deeply personal story about his first pediatric
00:42:53loss as a resident, how he coped by metaphorically entombing his emotions. But before he can finish,
00:42:59the ER pulls him back in as a new case arrives. Collins finally confesses to Dana that she had a
00:43:05miscarriage. She reveals what she saw on the ultrasound, an empty uterus. I shouldn't have
00:43:10bought that fucking stroller, she mutters, as Dana holds her in quiet support. Later,
00:43:15Robbie notices something off about Collins and asks Dana about it. Dana retorts,
00:43:19you just gave a speech titled, How to Literally Bury Your Feelings. In the waiting room, tensions
00:43:24escalate when one mother offers another a mask for her coughing child. The anti-masker insists it's just
00:43:30allergies, but when tempers flare, she throws a punch, only to end up with the other woman's tooth
00:43:35lodged in her fist. Dana storms in. You're in a hospital. There are kids around. You should be
00:43:40ashamed. The women are taken into the ER, much to the outrage of Doug Driscoll, who's been waiting
00:43:45all day. So all I have to do is get in a fight to cut the line. Langdon takes his
00:43:50own approach when
00:43:51treating the anti-masker's injury. He calmly offers her the option of a mask-free surgery. After all,
00:43:56he says, the hospital respects her views. The sarcasm is razor sharp, and the woman immediately
00:44:02backtracks. No, no, I'll take the masks. McKay and Dana try to stall for time with Piper, the young
00:44:07woman caught in a sex trafficking situation, as Laura, the woman controlling her, searches for her.
00:44:13McKay discreetly hands Piper a pen with a crisis hotline number hidden inside. Piper insists she
00:44:19wants to leave with Laura, but later, when they check her ward and realize she's gone with Laura, the pen
00:44:23is missing, looks like she took it. A new case arrives. Walter Purnell, a 52-year-old struck by a
00:44:29car,
00:44:29his leg covered in injuries. But the driver, Paula, is in worse condition. She was actually
00:44:34seen earlier that morning by McKay for what was thought to be a simple UTI. When Paula suddenly
00:44:39goes into shock, it becomes clear she has sepsis. The real cause? Endometritis, an infection of the
00:44:46uterine lining that occurs after childbirth. Paula had just given birth, and McKay might have caught
00:44:50it if she had performed a pelvic exam instead of assuming it was just a UTI. Collins subtly implies
00:44:56McKay was biased due to Paula's weight. McKay takes the criticism head-on, vowing to do better.
00:45:01The ER is thrown into crisis when a young woman overdoses on MDMA at PitFest, arriving with a core
00:45:07temperature of 107 degrees Fahrenheit. The team works quickly, submerging her in an ice bath. When
00:45:12she begins seizing, Trinity quickly recognizes the signs of hyponatremia and tells Mohan to push saline,
00:45:18before the labs even confirm it. It's a risky call, but it saves the patient. Langdon, however,
00:45:23is livid that he wasn't called immediately. His frustration turns into something harsher. Anger,
00:45:29belittlement. He tears into Trinity, calling her reckless, arrogant, and stupid. Then, Robbie steps
00:45:35in and pulls him aside. Langdon is trying to justify his actions, when Robbie tells him to shut the
00:45:40fuck up. The entire room goes silent. He calls out Langdon's pattern, pointing out that he rides
00:45:46Trinity harder than anyone else. Shaming and belittling are not effective teaching tools.
00:45:50You are senior leadership here. Step up and act like it. The words hit hard. Later, Langdon finds
00:45:57Mel, whom he had earlier told to take a break after Amber's death. She's sitting with Walter's
00:46:01dog, Crosby, regrounding herself. Langdon is gentle, encouraging. You're all pretty green,
00:46:07but you've grown on me, he tells her. When she confesses that the sisters in the drowning case
00:46:12hit too close to home, he listens. This job is hard, but you belong here. And with that,
00:46:18he hands her Walter's road rash case. Whitaker is informed that Ratman, the patient who once
00:46:23urinated on him, wants to see him. Expecting the worst, Whitaker masks up, as if preparing for a
00:46:29COVID case. But when he enters the room, he finds a completely different man, stable, lucid. The
00:46:35patient explains that he has schizophrenia, and when he's off his meds, he spirals. But he's homeless,
00:46:40and without money, he can't afford to keep up with treatment. Whitaker arranges for him to get
00:46:44medication, and personally signs up for the street team to deliver his meds every 30 days.
00:46:49Javadi takes a chance, asking Matteo if they can hang out after work so he can show her around.
00:46:54He gently lets her down, stating that he doesn't date co-workers. Watching the whole thing unfold,
00:46:59Earl, the man casually eating his sandwich, chuckles. This is like a rom-com. Thankfully,
00:47:05Dana swoops in just in time to save Javadi from further embarrassment. They proceed to the waiting
00:47:09room, where Doug finally snaps. He insults the other patients, scoffing that his tax dollars pay
00:47:15for their health care. Javadi informs him that they're waiting on his troponin levels to rule
00:47:20out a silent heart attack. Dana calmly explains that patients are seen based on severity. But Doug
00:47:25isn't having it. Langdon hands him an AMA form. If you leave, you're doing so against medical advice.
00:47:31We are actively working here. Doug stays put, grumbling. Turns out, Santos has experience with
00:47:38drug overdoses. She's seen a friend suffer from one, which is why she knew what to do earlier.
00:47:42Mohan reassures her that Langdon shouldn't have spoken to her like that.
00:47:46You're a good doctor, she says, and it means a lot to her. One of the rats, from earlier escapes.
00:47:52Crosby chases after it. When Whitaker gets there, he throws a blanket over it and, yes,
00:47:57snap its neck with his bare hands. The ER erupts into applause. Robbie notices the cops speaking to
00:48:02Teresa. Did McKay call the police? Dana steps out for a smoke break,
00:48:07unaware of Doug approaching. He punches her, knocking her down and leaving her nose bleeding.
00:48:12Tossing his AMA form at her, he sneers,
00:48:14Busy my foot, mocking Langdon's claim that they were hard at work.
00:48:18I'll take my chances, he mutters, walking away.
00:48:21Robbie is immediately annoyed when the police tell him McKay was the one who called them to
00:48:25question David's mother, Teresa. He barely has time to process his frustration when Dana walks into
00:48:30the ER, blood running from her broken nose. The sight shocks Javadi, but the rest of the team
00:48:35barely flinches, each chiming in with their own stories of being assaulted on the job.
00:48:39Dana initially refuses to name the patient who attacked her, but Ahmad insists that whoever did
00:48:44this needs to pay. Finally, she reveals the name, Doug Driscoll. Ahmad wastes no time,
00:48:50alerting the police and surrounding hospitals to be on the lookout.
00:48:53News of Driscoll spreads quickly, and it doesn't take long for Gloria to appear,
00:48:57concerned about violent patients roaming freely and attacking staff. But Robbie immediately puts her on
00:49:02the spot, reminding her that he's requested increased security three times in the last four
00:49:06months, only to be denied each time. He leaves her to handle the angry nurses, while he slips away.
00:49:11Thankfully, Dana's CT scan shows no skull fractures or serious damage.
00:49:16Collins delivers the good news, and the two share a moment, reflecting on the chaotic day.
00:49:21Robbie corners McKay about calling the police, but their conversation is interrupted when McKay's son
00:49:25Harrison shows up. Chad, McKay's ex and Harrison's father, is brought in after a skateboarding
00:49:31accident. His ankle is broken in three places, and while he's given pain meds before surgery,
00:49:35they make him loopy enough to confess he still has feelings for McKay, despite having a girlfriend
00:49:39Chloe. He also mentions taking his son skateboarding just because Mateo did, though unlike Mateo, Chad
00:49:45can't skate. Robbie and McKay finally hash out their argument about David. Robbie is furious that she
00:49:50went behind his back, convinced she's overreacting, and ruining a confused teenage boy's life. McKay,
00:49:55however, is focused on the potential victims. Why is David's life more important than the safety
00:50:00of the girls on his list? She demands. The argument is cut short when a 16-year-old baseball prodigy
00:50:06is rushed in after taking a devastating line drive to the eye. He only has light perception,
00:50:11and there's dangerous blood buildup behind the eyeball. Despite their earlier fight, Robbie doesn't
00:50:16let personal conflicts interfere with patient care. He lets McKay take the lead on a critical
00:50:21canthotomy procedure to relieve the pressure. When she admits she thought he'd bench her over the
00:50:25David situation, Robbie surprises her. This is a teaching hospital, he says. Not just for residents,
00:50:31but for him as well. He admits he was so focused on David that he didn't even consider the girls
00:50:36in
00:50:36danger. Once the boy is stabilized, Javadi loses it on his father, Greg, who is more concerned about
00:50:41when his son can get back on the field than, you know, his actual well-being. She outright scolds him,
00:50:46and McKay later has to remind her to keep her cool and separate personal feelings from professional work.
00:50:51Things get awkward when Mateo and McKay plan to spend time together outside of work. Javadi jumps
00:50:55in announcing she's free too. McKay is happy she can babysit her son, but that's not what she wanted,
00:51:00though. The team takes on a heartbreaking case. Teddy, a farmer who suffered catastrophic burns and a gas
00:51:05tank explosion. 90% of his body is covered in full thickness burns, including internal burns in his
00:51:10throat, which means he needs a breathing tube. His pregnant wife, Amy, arrives, and Whitaker does his
00:51:15best to comfort her. She shares that they're childhood sweethearts. Their bond deepens when Amy invites
00:51:20Whitaker to visit their farm anytime he misses home. As the situation worsens, Teddy's skin tightens to
00:51:26the point where he can't breathe. Langdon steps up and calmly guides Whitaker through an emergency
00:51:31procedure, cutting into Teddy's chest to relieve the pressure. Even Robbie is impressed by Langdon's
00:51:36teaching skills, but the victory is short-lived. Langdon privately tells Whitaker that Teddy's chances of
00:51:42survival are less than 10%. He advises Whitaker to prepare Amy for the worst and to call Kiara,
00:51:47as a psychologist may be needed if Teddy doesn't make it. As Langdon steps out of Teddy's ward,
00:51:52he sees Santos speaking with Robbie and assumes she's throwing him under the bus. He launches into a
00:51:57rant, accusing her of cherry-picking cases, bullying teammates, and not being a team player. Robbie simply
00:52:03responds with, good meeting. But what Langdon doesn't realize, Santos wasn't talking about him.
00:52:08The ER takes in another patient, a Jane Doe in her 20s, who has had a stroke and is apparently
00:52:13a
00:52:13gamer. The stroke neurologist Dr. Mehta is called in, and his endless dad jokes make Melissa cringe.
00:52:19However, Mohan explains that everyone has their own special sauce for handling ER stress. When Jane Doe
00:52:25suddenly has an allergic reaction, Melissa considers intubation, but Mohan steps in with IV epinephrine
00:52:30instead. It works, and Robbie, impressed, tells her, slow-mo no more. He jokes that Dr. Mehta's
00:52:36humor is rubbing off on him. Later, Melissa apologizes to Mohan for not trusting her,
00:52:41and wonders what her own special sauce is. Mohan reassures her that she'll figure it out in time.
00:52:46After his conversation with Langdon, Robbie meets with Santos privately. She hesitates,
00:52:50but then pulls out a set of missing pain med vials. Now Robbie knows something is seriously wrong.
00:52:55Robbie immediately confronts Langdon, accusing him of stealing medication. At first, Langdon looks
00:53:01confused, but when he gets defensive and refuses to open his locker, Robbie doesn't hesitate.
00:53:05Open your fucking locker, or I will have security smash it. Inside, they find a bag of pills,
00:53:10likely Louie's Librium, the ones meant for the drunk patient. Langdon insists he was weaning
00:53:15himself off pain meds for an old back injury, but Robbie isn't interested. He orders Langdon out of the
00:53:20building. I do not like that they had to prove Trinity right. Robbie heads back into the ER,
00:53:25still on edge from the Langdon incident. He tells Dana to go home, but she refuses. Instead,
00:53:30he gives her a task, run a pharmacy audit on Langdon's prescriptions for the last three days.
00:53:35He doesn't tell her everything, trying to keep himself from spiraling, but it's clear he's
00:53:40struggling, as he has another flashback in the bathroom. Robbie then informs Trinity that he had
00:53:44sent Langdon home. Santos jokes about it, saying she wanted to make a big impression on her first day.
00:53:49But now that they're down a senior resident, everyone needs to be all hands on deck. Robbie tells her to
00:53:54keep the situation quiet, but she immediately calls Dr. Garcia. Princess, who witnessed Robbie
00:53:59and Langdon's confrontation, is already gossiping with Perla in Tagalog, assuming no one could
00:54:04understand them. But Santos, to their surprise, spoke the language fluently. They were shocked.
00:54:10Given her appearance, they had no idea she was Filipina. Garcia comes down after receiving Santos's
00:54:15message, and Santos asks her to keep the Langdon situation quiet and that she was right. Garcia coldly
00:54:20tells Santos she's trouble and refuses to associate with her. David posts a cryptic message on Instagram,
00:54:26something that suggests he's either going to kill someone or himself. The police say it's not enough
00:54:31for a bolo. Robbie reassures her that everything will be okay. But later, Teresa decides to place
00:54:36David on an involuntary psychiatric hold. It can't be anonymous. She has to step up as a witness,
00:54:41and they need a social worker and psychologist to sign off on it. Natalie, a surrogate for her best
00:54:46friend Justin and his husband, arrives at the hospital already crowning. The baby's head is
00:54:50halfway out, but it suddenly stops moving. The shoulder is stuck against Natalie's pelvic bone.
00:54:55Robbie has to reach inside Natalie and manually rotate the baby to pull it out.
00:54:59It's a terrifying moment. For a few seconds, it looks like the baby isn't breathing,
00:55:04but Dr. Collins and Dr. King manage to resuscitate it. Just as everyone starts to relax,
00:55:09another crisis hits. Natalie begins hemorrhaging after delivering the placenta. Blood gushes out in an
00:55:14alarming amount. Collins acts fast, inserting a uterine tamponade to stop the bleeding.
00:55:19Natalie pulls through. Not gonna lie, this is intense. Women are fearsome beings.
00:55:24Dr. McKay deals with another crisis, a woman with hepatitis B whose cirrhosis has led to severe
00:55:29esophageal varices. The veins in her esophagus have ruptured, and blood is pouring from her mouth.
00:55:35Robbie arrives just in time to assist McKay with inserting a Minnesota tube,
00:55:38a tamponade device meant to control the bleeding. The woman is going to need a liver transplant.
00:55:43Dr. Whitaker and Dr. Mohan take on a jittery 55-year-old patient, Ivan Pugliese, who is
00:55:49experiencing stomach pains and suggests they give him morphine. Mohan suspects withdrawal,
00:55:54even though he insists he doesn't drink or do drugs. Mohan tells Whitaker,
00:55:58remember I told you when you see fakers you would know. This is one. Mohan administers buprenorphine,
00:56:03but doesn't fully explain its purpose. She just tells him it's like morphine. His symptoms disappear
00:56:09soon after. When Robbie finds out, he's pissed. Mohan should have told him first. This is becoming a
00:56:15pattern. And just when they were finally getting along, Robbie and Mohan return to talk to Ivan.
00:56:19They tell him the drug that helped him is a withdrawal medication. Ivan flares up,
00:56:24insisting he's not an addict. He has a good job, a family, a life. Robbie tells him that doesn't mean
00:56:30he can't be an addict. If he wants their help, he can speak to Mohan. Before leaving, Mohan shares
00:56:36something personal. Her father died young. Ivan, who is in Pittsburgh for his daughter's wedding,
00:56:41might be able to walk her down the aisle, but Mohan hopes he'll be around for a lot longer than
00:56:44that.
00:56:45McKay and Chad are at odds after he took their son, Harrison, skateboarding without a helmet.
00:56:49They agree Harrison will stay with McKay until Chad recovers. But then Chad's girlfriend, Chloe,
00:56:54arrives, wearing a bonus mom t-shirt, as if she's marking her territory. She tells Chad Harrison
00:56:59doesn't need to stay with McKay because she can take care of him. Chad agrees. McKay is about to
00:57:04go off, but she's pulled away to see a patient. Later, she finds Chloe alone. They aren't supposed
00:57:09to be in the same space due to Chloe's restraining order against McKay, but McKay doesn't care.
00:57:14She tells Chloe that Harrison is her son, and she's a damn good mother. If Chloe really cares
00:57:19about him, she'll back off. Oh, and if she ever wears that shirt again, she'll regret it.
00:57:23Robbie gives Louise back his medication that Langdon stole.
00:57:26With Natalie's crisis over, one of the husbands holds the baby against his bare chest for skin-to-skin
00:57:31contact. Collins watches for a moment before rushing out of the room, near tears. Outside,
00:57:36Robbie calls his son Jake, who introduces him to his new girlfriend over video chat.
00:57:40It's a rare moment of peace, until Robbie spots Collins sitting alone in the ambulance. He sits
00:57:45beside her, gently insisting that she needs to go home and take a break. No phone, no distractions,
00:57:50just time to process. Collins finally admits why this miscarriage hit her so hard.
00:57:55This was her last round of IVF. Not just because of the cost, but because she can't take the emotional
00:58:00toll anymore. Robbie reminds her that he's never known her to give up. That's when Collins reveals
00:58:05something even bigger. This wasn't her first pregnancy. Years ago, she got pregnant, but she
00:58:10never told the father, because she was scared. And the father? It's Robbie. The weight of the
00:58:15revelation settles between them. Robbie tells her to forgive herself, and they embrace. Melissa goes
00:58:20to find Langdon to tell him about Natalie's birth, only to learn that he's gone. She's disappointed he
00:58:25left without saying goodbye. She decides to tell Trinity instead, but she completely ignores her,
00:58:30still feeling burned from earlier. Instead, Santos goes to Robbie, who reassures her that she has a
00:58:35strong moral compass, and did the right thing. Throughout the shift, Langdon has been calling
00:58:40Dana and Robbie non-stop, with none of them picking up. Dana is on her smoke break, and she's being
00:58:45guarded by Ahmad, who doesn't want the same thing repeating itself. Dana is exhausted. She tells Robbie
00:58:50she's quitting. This job is all she's ever known since high school. And she's done. Then, their pagers
00:58:56go off. Multiple gunshot victims are en route. There's been an active shooter at Pit Fest. Robbie
00:59:01realizes Jake and his girlfriend are there. And the shooter? Who wants to bet it's David?
00:59:06A code triage is called, and Robbie and Dana immediately take control,
00:59:10locking down the ER, organizing teams, and preparing for the surge of casualties.
00:59:14Even Gloria Pink Blazer proves invaluable, ensuring that every bed is prepped and every supply is fully
00:59:19stocked. To make room, patients are quickly discharged, transferred, or moved to family medicine,
00:59:24and the waiting room is emptied entirely. The TV is shut off. No one wants panic spreading
00:59:29before the real chaos begins. Despite everything happening, Robbie still can't reach Jake. As the
00:59:34night shift arrives, including John Shen, the ER shifts into full triage mode. Dr. Jack Abbott also
00:59:40arrives after hearing about the shooting on a police scanner. Patients are to be tagged with color-coded
00:59:45snap bands, prioritizing treatment without time for x-rays or scans. This is pure instinct-based
00:59:50battlefield medicine. Every department has a designated primary lead, each wearing a vest,
00:59:55and the ER is divided into color-coded sections. Yellow. Extremity wounds. Stable vitals. Mel. Santos.
01:00:02Whitaker. Pink. Critical injuries that could be fatal within an hour. McKay. Javadi. Night shift staff.
01:00:09Red. Immediate life-saving intervention required. Jack. Mohan. Robbie. The first victims are rushed in by
01:00:15police vehicles. They are immediately color-coded for treatment. Robbie tells the officers to talk to
01:00:21Teresa, believing her son might be connected to the attack. Three more patients arrive in a van.
01:00:25Sylvia, a mother who was hit by a car while escaping, is focused only on her son Omar. Omar,
01:00:31who is deaf, has been shot in the chest. The third patient is D.O.A. Omar is bagged,
01:00:37given an intraosseous bone marrow blood transfusion, and rushed to surgery. Garcia later confirms Omar will
01:00:43survive, but Whitaker and Mel find Sylvia unconscious, with a liver laceration. She receives
01:00:49a bag of O-negative blood, but it's not enough. Seeing her fading fast, Mel offers to donate her
01:00:54own blood, sparking an ER-wide blood donation effort. Even Dr. Jack Abbott donates, taping a blood bag to
01:01:00his legs so he can continue working. Mel manually squeezes the blood bag to push the transfusion faster.
01:01:05Langdon is back, and watching the ER function at its peak. Robbie is not happy to see him,
01:01:10but they need all the help they can get. Santos isn't thrilled either, but she's too busy dealing
01:01:15with a patient bleeding from the head to care. Mel, however, is relieved he's back. Javadi
01:01:20treats a trampled woman while dealing with her overbearing mother. This time, Javadi stands her
01:01:24ground, earning McKay's approval. When they run out of chest tubes and thoracels, she instructs her
01:01:29mother to use an ET tube and a urine output bag instead. Mateo calls it genius, and Javadi blushes.
01:01:35Langdon diagnoses a coma patient at a glance. Noticing a glucose monitor, he suspects hypoglycemia
01:01:41and administers AMPD50 glucose injection, despite Garcia wanting to confirm first. His hunch proves
01:01:48correct, and the patient wakes up. Robbie takes on a patient struggling to breathe. The patient is
01:01:53secreting too much fluid, making intubation nearly impossible. Robbie asks McKay to perform CPR while he
01:01:59uses a bubble intubation technique, following the air bubbles to guide the tube in. Mohan calls Robbie for
01:02:04assistance. Her patient has subcutaneous emphysema, meaning air is leaking from his chest into his
01:02:09skin. Mohan suggests making an infraclavicular incision, below the collarbone, to release the
01:02:15trapped air. Robbie gives her the go-ahead, and the wound bubbles as the air escapes. This is exactly
01:02:20what he's been pushing for. Residents taking initiative but checking in first. Santos stabilizes
01:02:25her patient but notices another patient missing, the same one who had earlier refused treatment.
01:02:29She finds him secretly filming inside the hospital, confirming he's a reporter. When she tells him to
01:02:35stop, he runs, only to slip in a puddle of blood. His hoodie is soaked in blood, but it's not
01:02:40his.
01:02:41Santos has him admitted for a head injury, with restraints. Then, in a savage move, she dumps his
01:02:47phone into a mop bucket full of bloody water. Chad sneaks onto the ER floor, completely overwhelmed by the
01:02:52chaos he's seeing. McKay finds him frozen in shock, and orders him to the staff lounge. Inside, Chad
01:02:59pleads with Harrison to come home with him, but Harrison refuses. He wants to stay and wait for
01:03:03McKay. Instead, the two settle in to watch a movie together. Lupe and Chiara speak to the families of
01:03:09the Pit Fest victims, informing them about a new website they've set up. Families can scan a provided
01:03:13barcode to access the site, and upload photos or distinguishing features of their loved ones who
01:03:18attended Pit Fest. This will help the hospital identify each victim more accurately. Patient with
01:03:23a neck gunshot wound has a growing hematoma, making intubation tough for Mohan. Robby steps in to intubate
01:03:29using a three-step method. Scalpel for incision, insert finger to open guide placement, then inserts
01:03:35bougie into the hole. Once the bougie inflates, the patient is bagged. Abbott controls the bleeding by
01:03:40inserting a Foley balloon into the bullet wound and clamping it. Robby steps outside to help with
01:03:45incoming patients, but is met by SWAT officers. The cops reveal that David's phone was pinged at
01:03:50the shooting location. Rich Stefano, a cop, is rushed in as a critical code read. The bullet
01:03:55went through his neck and out his cheek. But then comes the real horror. SWAT has intel that
01:04:00the shooter may be headed toward the hospital. SWAT is inside the hospital, hovering near the police
01:04:04officer with a nasty gunshot wound who's being stabilized. Dana wants them out, but they won't leave,
01:04:09and their presence sends a wave of paranoia through the ER. Whispers ripple among staff
01:04:14that the shooter might be headed to the hospital. Eileen quickly shuts it down, warning Langdon not to
01:04:20spread rumors. Up on the rooftop helipad, Javadi and Whitaker receive a fresh blood delivery being
01:04:24airlifted in. They rush back to trauma to get it into the bodies that need it most, but everyone needs
01:04:29blood, and they scramble to distribute it. Dana's relieved to see night shift nurse Bridget Young arrive.
01:04:34She immediately puts her in charge of watching over the new med students. The pressure's on for
01:04:39everyone, especially Whitaker, who's working on another gunshot victim, a clown named Buster Pirelli.
01:04:44When Whitaker uses an IO, intraosseous access, drilling into bone to deliver meds, and the man
01:04:50screams, Melissa pulls him aside for quick lesson. IOs are for unconscious or unresponsive patients only.
01:04:56Robbie struggles to intubate the injured cop due to excessive bleeding. There's no bougie for a
01:05:01cricothyroidomy. But Dr. Abbott, ever the ex-military trauma doc, pulls out a tactical airway kit,
01:05:06the kind used in combat, and quickly stabilizes the officer. Dr. Mohan, watching, wonders why they
01:05:12don't have more of this life-saving gear. Robbie simply says, not in the budget. Down the hall,
01:05:18Kiara photographs unidentified deceased patients, their defining features saved for grieving families
01:05:23searching for answers. A patient Langdon is treating suddenly regains consciousness and reaches
01:05:27for his concealed gun. The entire floor hits the deck. SWAT storms in, but the man falls back
01:05:33unconscious. He seems to only respond to pain. IO doesn't work due to the man's size, and they can't
01:05:38intubate because all the laryngoscopes are dead. Robbie jumps in with an old-school tactile intubation,
01:05:44feeling for the airway by touch, and gets it done. Still, the patient's crashing. Langdon makes a risky
01:05:50call, inserting a supraclavicular subclavian line, an advanced central line placement, something he
01:05:55learned from an EMRAP podcast. It stabilizes the man, but barely. SWAT checks the man's ID. He has
01:06:02a permit, and his gun doesn't match the wounds. He's not the shooter. Then McKay's ankle monitor
01:06:07blares. Without missing a beat, she grabs an IO drill, bores into the device, and silences it in
01:06:13a spray of sparks. It's reckless, sure, but lives are on the line. She'll deal with the consequences later.
01:06:20Kiara goes through photos sent in via the barcode system created last episode.
01:06:24She identifies one of the deceased, Vincent Rivera, and breaks the news to his wife,
01:06:29who then reveals her brother was also at Pitfest. Kiara hasn't seen him in the system,
01:06:33which at least means he's not among the dead. Whitaker checks on Grayson. The patient Trinity
01:06:38had been stitching up when she first realized Langdon was back in previous episode. He seems fine,
01:06:42asleep. But when Carmen, a woman with a thigh gunshot wound, is rushed in, Whitaker moves to help. The wound
01:06:48is too high for a tourniquet, so Mel suggests using hemostatic bandages. Robbie steps outside for air,
01:06:53and hears that they've had 85 patients so far, and the hits keep coming. He compliments Dr. Shen and
01:06:59Dr. Ellis on their steady hands under pressure. But then, he hears a familiar voice. Jake. Robbie's
01:07:06pseudo-stepson is unhurt, but his girlfriend Leah has just arrived, the 86th victim. Robbie drops
01:07:11everything. He intubates her. Starts compressions. But it's not going well. The nurse can't feel a
01:07:17carotid pulse. Dana checks for ephemeral pulse. Nothing. Still, Robbie keeps going. He pours in
01:07:24plasma, platelets, time, and energy. He's ignoring MASH triage protocols. It's personal. Carmen is still
01:07:32bleeding through her bandage. Whitaker and Mel work to help, but Trinity notices Grayson isn't moving.
01:07:38Whitaker thinks he's asleep again, but this time, he's wrong. There's no attending available.
01:07:43Everyone's focused on another emergency. Whitaker checks Grayson's retina and confirms increased
01:07:49intracranial pressure. His brain is bleeding. There's no time. Mohan drills directly into
01:07:54Grayson's skull to relieve the pressure, a move she read about once but never expected to try.
01:07:58Shocked but impressed, Javadi, Whitaker, and Santos look on as Grayson stabilizes. But Carmen is fading
01:08:05fast. Her gunshot wound nicked an artery. She collapses from blood loss. Leah's pulse flickers and
01:08:11vanishes. Robbie still believes. But Dr. Walsh and Abbott know better. Blood is for the ones we can
01:08:17save, Abbott says. They've poured over four units and a cell saver into her already. Abbott lays it
01:08:23out. The bullet tore through her heart. In any other scenario, she'd be pronounced dead in the field.
01:08:29Even ECMO wouldn't be enough. We're gonna lose ten other patients if you put all your efforts into
01:08:34saving this girl. Robbie finally stops, only to see David, the suspected shooter, being tackled by SWAT.
01:08:40He came to see his mom. Robbie checks him out to make sure he doesn't have a concussion,
01:08:44and David is locked in a guarded room. Robbie takes Jake into a quiet room and breaks the news.
01:08:49Leah's dead. Carmen's bleeding internally. Trinity tries to get an attending, but everyone's occupied.
01:08:55With no help coming, she makes a bold decision. Perform a Reboa. Resuscitative endovascular balloon
01:09:01occlusion of the aorta. A high-risk trauma procedure to stop internal bleeding. All the interns protest,
01:09:07saying only an attending can do it. Trinity does it anyway. And it works. When Abbott finally arrives
01:09:13and sees the results, he scolds her for breaking protocol, but then leans in and whispers,
01:09:17you're a badass. Jake is with Leah's body. Still in shock, he turns to Robbie. Why couldn't you save
01:09:24her? I mean, this is what you do. But there is no good answer. Would she have survived any other
01:09:30day?
01:09:30Maybe. Maybe not. Robbie, overwhelmed by grief and PTSD, breaks down sobbing. Oh my Shayla.
01:09:38As chaos simmers in the ER, word spreads that the shooter might be in the hospital.
01:09:43But Trinity steps up and shuts that down. In a quiet corner, Mel sits beside a woman who went
01:09:48into shock after her husband who was wheelchair-ridden died in her arms. A bullet entered her arm and
01:09:53continued into her husband's head as she tried to protect him. The woman, shattered, asks Mel how she
01:09:59copes with the pain and death all Mel can say is she doesn't know either. Later, after reuniting the
01:10:04woman with her daughter, Mel breaks down in front of Robbie. She apologizes, but Robbie gently tells
01:10:09her never to apologize for caring. The clown patient is freezing, and the only blankets are in pediatrics,
01:10:15which is now housing the dead. Whitaker loses a rock-paper-scissors match with Trinity and has to go
01:10:21fetch them. But while Whitaker's away, chaos breaks out in the ER. Surgeons are needed. Patients are
01:10:27crashing. And everyone keeps asking, where's Robbie? Whitaker finds Robbie in pediatrics,
01:10:33sobbing and praying. Not knowing what to say, Whitaker sits beside him. When Robbie says he has to
01:10:39leave, Whitaker responds, no. We need you. He tells him the truth. Without him, the team is screwed.
01:10:46That finally pushes Robbie to grab his hand, stand up, and head back into the ER. When Whitaker returns,
01:10:52clearly shaken, Trinity checks in with him for real. No jokes. Is Trinity finally showing a crack in the
01:10:58armor? Gloria returns, thrilled that new supplies have arrived, only to find out the doctors donated
01:11:04blood without clearance. Before she can rant about legality, Robbie loses it. Go back to your
01:11:09micromanaging ivory tower. Dr. Abbott immediately sends him outside to cool off. It's clear. Robbie told
01:11:16Jake the truth about Leah. Just when they think it's over. Ella scolds Dr. Shen for tempting fate by saying
01:11:22it's quiet. And boom. A car crashes into the ambulance bay. The driver? A Navy medic with a GSW
01:11:29to the leg who's been patching up people all day. In the back seat. A young man turning blue from
01:11:34an
01:11:34overdose. Trinity and Langdon has to team up for this. Awkward. Ellis quizzes Trinity on the diagnosis,
01:11:41but Langdon interrupts. Trinity snaps. Are you running this case or is Ellis? Ellis reminds her, senior
01:11:47resident's job is to teach. Trinity nails the diagnosis, methemoglobinemia, a condition that
01:11:53prevents red blood cells from carrying oxygen. The team administers methylene blue, which restores the
01:11:58patient's color. But the cause is unclear. It'd take a gallon of poppers to do that kind of damage.
01:12:03They'll have to wait until he's conscious. Ellis clocks the tension between Langdon and Trinity.
01:12:08Jake is in tears, staring at a photo of him and Leah. Langdon comforts him, reassuring him that
01:12:13Robbie did everything he could. Robbie watches from afar, consumed by guilt. The Navy medic turns
01:12:19out to be Vincent's wife's brother from the festival. She's brought in to see him and begs
01:12:23him to fight like the stubborn bastard he is. But Mohan and Abbott realize something's wrong.
01:12:27His oxygen is plummeting. Diagnosis. Intracardial air embolism. Air in the heart blocking blood to the
01:12:33lungs, likely from running on a leg injury. There's no time for a CT scan. Instead, Dr. Abbott talks
01:12:39Mohan through inserting a five-French pigtail catheter into the heart to pull the air out.
01:12:44The other doctor objects, but the team pulls it off just in time. McKay ignores a call from the
01:12:49courthouse and keeps working. Her dad shows up to pick up Harrison, sees Chad in the lounge,
01:12:54and delivers a line for the ages. A douche name for a douchebag. Honestly? Iconic. Gloria confirms the
01:13:01shooter is dead. Self-inflicted. It wasn't David after all. The AR-15 and duffel bag full of ammo show
01:13:07he
01:13:08could have done far more damage, but the worst is over. McKay and Robby deliver the news to David.
01:13:13Though he's not the shooter, he still has to stay for a mandatory 72-hour psychiatric watch.
01:13:18Teresa arrives, pleading with him to let her in. But the moment he finds out it was his mom who
01:13:23told
01:13:23the doctors about his list and signed the order for his 72-hour psychiatric hold, he completely
01:13:28shuts her out. Robby blames McKay for how David was brought in, handcuffed, and getting David help is
01:13:33her problem now. Mateo praises Javadi's resilience and calls her by her first name, Victoria, handing
01:13:39her a juice box. She lights up. Don't play with her heart, Mateo. Janie, Jake's mom, arrives after
01:13:44seeing the news. Robby tells her Jake is fine, but Leah is dead. She assures him that Jake will
01:13:49understand someday. It wasn't his fault. The gist is that Janie and Robby used to date, and during that
01:13:54time, he treated her son like his own, often bringing him to the hospital with him. As the hospital
01:13:59reopens, Shen jinxes it again by saying it's quiet. Fate laughs, sending in an unresponsive
01:14:05boy, Flynn, with strange markings and unresponsive. Shen and King are stumped. But Robby knows the
01:14:11answer. Measles. His sister caught it, too, after a trip to Orlando, but her immune system
01:14:17fought it off, so it wasn't serious for her. It happens. Flynn's parents didn't vaccinate their kids.
01:14:23His mom, a self-declared medical researcher with a phone, refuses the spinal tap the doctors need
01:14:28to confirm his condition. Acute disseminated encephalomyelitis, a rare but serious post-infection
01:14:34brain condition with a 20% death rate. She insists on steroids instead and cites Google as her source.
01:14:40Robby, already exhausted, finally explodes and storms out. To wrap up the chaos, police arrive
01:14:46for Cassie McKay. She's handcuffed and arrested for tampering with her ankle monitor, no explanations
01:14:51allowed. Robby intervenes during McKay's arrest, trying to get the cops to back off. When they refuse,
01:14:57he pulls in Officer Harrelson, whose team saw McKay saving lives. Harrelson convinces them to let
01:15:03McKay go. Later, Gloria checks in with Robby and Abbott, acknowledging their massive efforts.
01:15:08With 112 patients treated and only six deaths, she commends the staff and instructs Robby to hand
01:15:14daytime duties over to the night shift doctors so the exhausted team can finally rest. The fight
01:15:19continues to save young Flynn Edwards. His mom still won't allow a spinal tap to rule out measles
01:15:24complications, and Robby, frustrated, takes Flynn's dad to the makeshift morgue to show
01:15:29him what's at stake. But the father calls Robby an asshole, and storms off. That stunt eventually
01:15:34pays off, though he later consents to the procedure while the mother is away. By the time she returns
01:15:39mid-procedure and tries to stop it, the spinal tap is already done. Dr. Santos and Ellis investigate
01:15:45the mystery of the blue-skinned boy. They consider multiple causes, including nitrate or nitrite
01:15:50poisoning, commonly found in food preservatives, and sometimes linked to suicide attempts.
01:15:55After speaking with the boy, Santos senses it was a suicide attempt, even though he won't confirm it.
01:16:00In a rare moment of vulnerability, she opens up about her own childhood trauma. She and her best
01:16:05friend were abused by an older man, and her friend later died by suicide. Santos, usually cold and brash,
01:16:10gently consoles the boy, holds his arm, and assures him she's scheduled him to see a psychiatrist.
01:16:15Dr. Mohan is still riding an adrenaline high, frantically trying to track more at-risk patients.
01:16:21McKay and Ellis gently confront her about the blurred lines between work and life,
01:16:24reminding her that she needs to take care of herself too. Later, we see Mohan crash in the
01:16:29bathroom, sobbing uncontrollably as the weight of everything she's been through finally catches up to
01:16:33her. Langdon corners Robby, asking him not to report his malpractice. Robby urges him to go home.
01:16:39Langdon later approaches Dana, fishing for whether anyone's spreading rumors about him,
01:16:43and asking if she'd vouch for him if necessary. But Dana has her own concerns. She's considering
01:16:48quitting, overwhelmed by everything that's happened. Langdon urges her to stay, insisting the hospital
01:16:53needs her. Another emergency arrives, a pit worker pinned by a supply truck. He has a crushed pelvis.
01:16:59Robby and Abbott stabilize him, before handing him off to Ellis and Shen. The team transfuses blood
01:17:04per protocol, but the patient begins crashing. The blood pressure shoots up, breaking up clots that
01:17:10were helping stop internal bleeding. Ellis feels responsible, but Robby points out that Shen,
01:17:15who was supposed to be overseeing, has been missing in action. The team stabilizes the man with
01:17:19pre-peritoneal packing, a procedure that applies pressure internally until surgery can be done.
01:17:24It works. Barely. Whitaker, now known as the hospital's unofficial bloodhound since killing the
01:17:29rat, is sent to track down a missing patient. He finds her outside, smoking, and wheels her back inside.
01:17:34Langdon later confronts Robby again, outside the ambulance bay. Still refusing rehab, he takes a
01:17:40low blow, referencing Robby's panic attack in the pediatric morgue by saying,
01:17:44I never had a complete meltdown. He even mentions someone saw Robby talking to cartoon murals on the
01:17:49wall, hinting others may know about his mental break. But Robby doesn't engage. Robby regroups and
01:17:55gives a heartfelt speech to the remaining staff, thanking them for everything they've done. Afterward,
01:18:00he quietly slips away to the roof, where Abbott follows. Alarmed, Abbott finds Robby standing on
01:18:05the edge, mirroring the way they first met in the show's pilot episode. Robby is mentally unraveling,
01:18:10haunted by his failures, especially the death of Leah, Jake's girlfriend. During COVID, Robby lost his
01:18:15mentor, Dr. Adamson, and today's events have reactivated all that trauma. Abbott reassures him,
01:18:21they're built to return, no matter how rough it gets. He reminds Robby that his brief panic in the
01:18:26morgue was human, not weakness. Robby approaches Jake, hoping to clear the air. Jake shuts him down,
01:18:33still grieving. But Robby feels it's better Jake blames him than blaming himself. Then,
01:18:38Leah's parents arrive, another emotional blow. Robby steadies himself and walks in to face them.
01:18:44McKay finally makes a breakthrough with David. She tells him, even if he's innocent,
01:18:48that darkness can creep in if he doesn't talk to someone. What if it was your mom next on that
01:18:52death
01:18:53list? Her words land. David tears up and agrees to see a professional. Afterward, Santos catches
01:19:00Whitaker slipping through a staff-only door and follows him, discovering he's secretly living in
01:19:04one of the hospital's unused rooms. He's broke and homeless. In a touching twist, Santos offers
01:19:10him a room in her apartment, rent-free, just help clean up and fix things. It's the warmest gesture
01:19:15we've seen from her yet, hinting at how much she's grown since the season started. As the shift winds
01:19:20down, Mel finally reunites with her sister Becca, putting on a brave face despite everything. Dana,
01:19:25fed up after being assaulted by Driscoll, who is now in custody, contemplates pressing charges,
01:19:30and Robby encourages her to do it. In the final scene, a few staff members gather at a park across
01:19:35from the hospital, drinking beers under the night sky. Mateo invites Javadi to join, her very first shift
01:19:41finally over. Robby laughs, saying, the next one will be easier. It's the first time we see Abbott's
01:19:47prosthetic leg, a quiet reveal that ties back to his hinted military background. They all raise a toast,
01:19:52to the people they saved, and the ones they couldn't. So do we think Dana is really quitting?
01:19:57From the very first episode, I was hooked. The show wastes no time, starting off with a gruesome,
01:20:03degloved leg? Yeah, I was sad immediately. The way they dive into the medical procedures is absolutely
01:20:08insane, in the best way. That childbirth scene? Brutal and raw. And the gunshot wounds? Whoever's in charge
01:20:15of makeup and special effects deserves a serious raise. Now let's talk about Langdon. I really liked
01:20:21him from the jump. But then the writers had to go and prove Trinity right. Why did they make that
01:20:25man steal drugs? That was such a letdown. Whitaker, though. I'm genuinely praying to the medical gods
01:20:32that he hasn't picked up any diseases from all the bodily fluids he's come in contact with. That man is
01:20:36always drenched in something. As for David, I'm still unsure how I feel. McKay did what was morally right,
01:20:42but that choice definitely made it harder for David to get treatment. Then again,
01:20:46David could have easily ended up as the Pit Fest shooter, so it's complicated. I read online that
01:20:51this show is actually based on the real lives of medical workers. And if that's true, wow. I would
01:20:56have passed out by the third shift. Massive respect to all healthcare professionals. And I'm calling it now.
01:21:02Mateo and Javadi are definitely going to be a thing eventually. Overall, great show.
