A combination of many factors, including mechanical and human, led to the bus crash in Gerik that killed 15 Universiti Pendidikan Sultan Idris (UPSI) students.

In a report made publicly available on the Transport Ministry website at 3pm on Friday (Jan 23), the accident was caused by a combination of mechanical, human, environmental and organisational factors.

00:00A combination of factors, including mechanical and human, led to last year's road accident in Garik Perak that killed 15 students of University Pendidikan Sultan Idris, according to a final report published on the Transport Ministry's website on Friday.
00:16The report said the bus's braking system was inconsistent and combined with driving at speeds exceeding safe limits led to the loss of control of the vehicle.
00:26The report said challenging road conditions, weaknesses in the design and installation of road barriers along the east-west highway stretch, and the structural failure of the bus cabin further increased the severity of the impact and the resulting injuries.
00:40It said kinematic analysis, together with dashcam footage from another vehicle, estimated the travelling speed of the bus entering the curve at 114.7 km per hour, whereas the speed limit for the stretch was 60 km per hour.
00:55The report said there were illegal operating practices, non-compliance with safety requirements, as well as weaknesses in regulation and enforcement that reflected systemic issues within the tourism transport industry.
01:07It said the vehicle's brakes were compromised, due to deformation and wear of the rear left brake drum, as well as contamination of grease on the brake lining and inconsistencies in the composition of the lining material.
01:19It was also found that the bus did not have dashboard cameras or GPS devices.
01:24During the incident on June 9th, last year, the bus with 42 students and two crew members on board attempted to overtake a minivan.
01:32The bus lost control and collided with the rear of a multi-purpose vehicle, killing 15 people and wounding 33 others including 4 people in the MPV.
01:41The bus driver was charged at the Garrick Magistrates' Court with 15 counts of dangerous driving.
