02:44a whole different ballgame as Sean has an appointment with dr. Dev Shah

03:03got a large a lipoma mass under my right armpit okay it's about the size of a
03:09cricket ball and basically it's just an obstruction that gets in the way does it
03:14stop you from doing anything and I used to be a very good swimmer I still am but
03:19obviously I can't do the stroke movement I used to do right and obviously
03:24sporting wise it it just gets in the way what sports you like playing badminton
03:30swimming cricket but gave that up lately ten years ago now are you right-handed
03:34yes bowling yes so it's sort of right is it only right yes only right a lipoma is a
03:43group of cells in your body that are made up of fat so we all have fat in our
03:50bodies and it grows at a at a normal rate with your body but a lipoma will grow at
03:55an abnormal rate how is it making you feel it's getting me down if I go on
04:01holiday you can't take my shirt off because you know people are looking at
04:04you and staring at it when I'm turning I'm knocking things off shelves when I'm
04:09going through doorways doors and narrow then I have to obviously turn sideways so
04:14it's obviously the back of your mind all the time and has anyone ever said
04:18anything to you they have they've basically pointed out what is it it's
04:22ugly you know you need to get rid of it so over the years obviously I've seen the
04:26local NHS doctor but obviously because it's cosmetic they wouldn't refer me on
04:31the NHS so okay all right fine because they're not cancerous and they're not
04:37going to kill someone in any way or ulcerate or bleed they are not deemed to be
04:42treatable on the NHS would you like to take your shirt off and then we'll have
04:47a little look and then we'll see what we can do for you oh wow right it's big
04:52isn't it yeah it is big is and you've had that fur it started when I was young at 14
04:57and grown over the years we're looking at maybe so I'm 57 now
05:03wow it must impede you so much yes that really does get in your way yeah if you put
05:12your arms up right okay so what full movement there's no restriction of the
05:17arm but it's just yeah yeah yeah I can't we're gonna have a little have a little
05:23look that's okay just feel it yeah it doesn't hurt I've squeezed it it's like a
05:27stress ball you're gonna miss it have you got a name for it it's the third boob the
05:31wife called it the third boob due to the fact that it's got a little nipple on it
05:35these eyes anatomically correct okay there we go that's good isn't it all right
05:40there we go let's have a little look then what we're doing I think Sean's
05:44description of a third boob was probably summing it up it was quite
05:49dramatic I'm just impressed how he's lived with it for so long is that not
05:54hurting you no what you're looking for when you're seeing these soft things on
06:00the on the body is whether they are actually soft whether you can feel the
06:05base because if it was connected to his lungs for example could it be something
06:10very odd and a bit dodgy having the lipoma removed today would be a big
06:17impact especially on family life the wife is obviously worried about it it is
06:24actually still growing if you don't mind we'll get our colleague in as well just
06:28to have a look just to see you know sorry most of us in dermatology work in
06:35groups and teams and we would always call each other in for interesting
06:40patients and it was a interesting presentation of something quite common
06:44a more comfortable like yeah I'm not going to peel you any grapes okay

07:30difficult and tricky position on the body as well coming up that's the big weird
07:39looking beast isn't it yeah it's just like a little creature that looks amazing
07:44it's mid-morning at the new clinic and skin A&E is as busy as ever but proceedings have been
08:10paused in treatment room one as dr. Dev considers one of the largest lipomas he's ever come across I
08:17would say that I'm feeling apprehensive so it's coming out of the body rather than going in and
08:26that's unusual for lipomas to be so obvious with 57 year old Sean waiting on the treatment bed
08:34Dev has asked dr. Adil to help him weigh up the decision oh it's gonna feel they can squeeze as
08:41hard as you want we've had the squash and a squeeze so stressful it felt very heavy when I initially tried
08:49to lift it up and it was going to be a challenge for Dev the lipoma was very big it was the biggest we've
08:59treated and the one thing I knew I could do for Sean if I couldn't remove the whole thing was at
09:08least to make it smaller we let you know we'll probably be a couple of hours so you know keep
09:15our lunch warm while dr. Dev and nurse cherry prepare for an epic procedure hi how can I help

09:46for you today I've got a skin tag on my leg all right okay it's quite a big one oh let's have a
09:51look oh my oh right that is big oh it is a big one yeah yeah I think that's maybe the biggest one
09:59I've ever seen it's the size of a large grape I'd say it's rather soft and squishy lift it up does
10:13it have it's got very narrow stalk hasn't it yeah well that is a beast yeah yeah do you know how long
10:19you've had that one for I don't know it's been a few years we tend to get skin tags in areas of
10:25friction maybe for our clothes rub or where we have folds in our skin where they rub together and
10:31the more friction that encourages them to grow has it ever been infected or inflamed or no it just
10:37pulls it's it's uncomfortable it it gets caught it hurts hopefully you've not had anybody pass
10:44comment on it or just my wife cause it my leg willy oh yeah I was thinking doesn't really look
10:57like a willy it's reminding me more of another part of the male anatomy I'm presuming your wife
11:03will be quite happy if we get rid of it for you yeah she will yeah not happy that I told you that
11:08have you got any of the skin tags or is this just the only thing that's ever bothered your skin I
11:16got one under my armpit here that's very small that one off as well if you want at the same time
11:27and make sure it doesn't turn into something like that because it's another area where you've got
11:30friction yeah brilliant two for one right so on to how we're going to remove it so we just numb it so
11:38you won't feel anything and we'll literally just snip off with a pair of scissors and then cauterize
11:43the base with a little electric current which just creates like a little burn in the area and stops any
11:47bleeding okay when dr. Emma said she could remove that today it was you know it was brilliant no you
11:54know just what I wanted to here so do you twiddle it when my granddaughter comes around I go look
12:00my granddaughter thinks it's disgusting but then so does my wife and daughters
12:10okay so it does sting a little bit as it goes in but it goes on really quickly okay yeah all right
12:24you did smart bit but you know it would have been worse if she'd have cut the skin tag off without
12:33it that's it well that's the worst bit done I'm going to test it with the needle now to see if it's
12:47numb can you feel anything sharp no perfect yeah I'm just going to snip it off so is that okay yeah
12:59oh okay all it's tough there we go go on sounded like a clunk yeah you could hear it it's because
13:13it's really it's like gristly oh thanks Donna there was a big clunk and then that it was off and I
13:21actually felt the clunk not painfully but it was weird it's not really bleeding is it no it's not
13:29I'll just give it a little it's just as quarter isn't it that's it seconds of brain it yeah it doesn't
13:41look like the other thing that you want let's just quickly whip that one off as well for you
13:57right Donna here we go this is going to be a good combination right give us a little blast

15:24from one sizable skin tag it's just like a little creature to an even larger lump in treatment room
15:36one after a thorough examination dr. Dev is ready to start work removing Shawn's enormous lipoma we're
15:45going to local eyes everywhere but I'm hoping to give you a scar that sort of looks like that okay all
15:51right okay the reason that we proceeded with doing this is that I could feel the base and it very much
16:03felt that it was encapsulated outside of the body and I knew the main problem would be to get the
16:10cosmetic scar looking right so it's not too lumpy sharp scratch coming up okay this is one of the
16:19biggest procedures I think I've ever been faced with I would say that I'm feeling apprehensive this
16:27is about the limits of what dermatological surgeons should be doing and could be doing I'm very reassured
16:34with how outside of the body it is so we're not near really dangerous structures but it's really how Sean
16:44will cope with a lot of digging and a lot of pulling still more going in are you feeling this
16:49still you will feel pulling and pushing you'll feel pressure yeah that's all I was very nervous first
16:58operation I've ever had so is it one of the biggest you've seen absolutely okay you're in charge if you feel
17:11anything no Sean you let us know okay we'll stop and put more local anesthetic okay anything sharp no
17:41anything sharp no nothing
17:52bit sharp there it was okay I'm just going to put some more local in
18:20anything sharp no no no no no good I feel it's just going to come away in my hand the majority of my job
18:38was to ensure that the gravity pulling the weight of the lipoma didn't impede what Dr. Shah was doing
18:48um in removing it it was incredibly weighty and was pulling on the skin which made Dr. Shah's life a
18:58little difficult as long as you know where my fingers are yes once I had started the incision I
19:08was really scared because I couldn't find the depth of the lipoma I was scared that we couldn't get an
19:15easy plane and we would go deeper and deeper towards the lung I needed to find where that plane
19:20was and getting to that point was really nerve-wracking is it just one then I was in nodules on it no no just
19:30one okay let's get this out right you okay not feeling anything sweetie no well done Sean you're doing really well
19:43I'm feeling feeling that a bit tugging okay wait a minute sharp yep okay it's just more local he's putting in well done thanks for letting us know in skin surgery size doesn't really matter what what matters is how much local anaesthetic you can give someone generally the anaesthetic lasts for about two hours the more you dilute
20:13the more you dilute your work you know she'll take your toes so you're sort of working against time how we doing there

21:52I've turned 40 and I want to sort out little bits of my life
21:57that need crossing off my list.

22:43I would say I was around 15.
22:47It really makes me feel a bit vulnerable.

22:53and you're meant to like your hair.
22:55I used to wear my hair really high up here.

22:59Scraped back with a lot of hairspray.

23:09I used to use a very thin comb to comb my hair back
23:13so it looked really tight.

23:23I was a bit concerned that it may be too late to help Candy
23:27because she'd had the problem for so many years.

23:43I've never seen any hair grow there for years.
23:46So I'm looking with this light thing.

23:50Obviously at the hair.
23:51And then I'm looking at the, I'm looking closer up at the actual scalp to see if you, I can see hair follicles there.

23:59If I can see little hairs.
24:00So you have little follicles everywhere and there's little hairs as well.
24:09But I think they're so small that you can't see them.

24:13Like tiny, tiny, tiny ones here.

24:20It was good news when I could see follicles in small hairs because that meant that it's a non-scarring hair loss,
24:26which means that there's a potential for the hair to grow back.
24:30I think you probably had alopecia areata.
24:32Mm-hmm.
24:33This is nothing that you have done.
24:38I think all the time it's my fault that I've done it.
24:41Oh, no.
24:42Yeah.
24:43No, that's terrible.
24:44So I think that's really important actually that we touch on that.
24:45Because it's definitely not what something you've done.
24:47I'm glad to hear that actually.
24:48Yeah.
24:49Alopecia areata is an immune problem of the hair follicles.
24:53Your body has decided it doesn't like those hair follicles and attacks them and kind of stops them from working.
25:00There's something that didn't like the hair at the time.
25:03Yeah.
25:04And put those follicles to sleep.
25:05Okay.
25:06And hasn't been woken up again.
25:07Yeah.
25:08So though I can't guarantee that I can make the hair grow again, I think it's worth trying a treatment for this.
25:14Mm-hmm.
25:15When she told me that there was something that could be done, it was very relieving.
25:21The treatment I would suggest is to have some steroid injections into the scalp.
25:27So the idea there is for that inflammation and the immune system response to be lessened by the steroid.
25:33Mm-hmm.
25:34And then for the hair to then wake up again.
25:36We could do that today.
25:37Brilliant.
25:38Is that okay?
25:39Yep.
25:40Just a bit overwhelmed really that finally somebody's listened to me.

26:16You can think about the steroid injections as waking the hair follicle up.
26:19But what it's doing is it's dampening down the inflammation and stopping that immune process
26:23from causing the hair to not grow.

26:36I have a few patients who have this in their beard.
26:38And I have to go around and do this in the beard.
26:41And they're always like, oh.
26:45That's the whole treatment.
26:46That's it.

26:48From my own clinical experience, I think there's about a 50% chance that Candy's hair will grow
26:52back after the injections.
26:53And she should see that happening within two to four weeks after the injections.
26:59What we're looking for is to see patches of short regrowth.

27:02Because it's going to be short hair.

28:11Luckily today isn't one of those days for Dr. Dev because he definitely needs Cherry by his side as he's removing Skin A&E's biggest ever lipoma.
28:25They're halfway through Sean's procedure but have hit a complication.

28:48I kept touching the nerve with a swab and it would fire off an impulse and make Sean quite uncomfortable.
28:55And I hadn't quite realised what that was until I saw this sort of elastic band grey tube that was clearly a nerve.
29:04I'm so sorry.
29:06We've got like 80% of it done.
29:09And it's just that last bit that's causing a problem.
29:13The lipoma was connected to one nerve ending, which sent a sharp pain, like a bee sting, but even 100 times worse.

31:25When you get skin together at either end, you create redundant areas of skin.
31:31You look like a patchwork quilt.
31:34No, you're looking fantastic.
31:37And unless you remove that excess skin, you'll have big lumps which would look probably just
31:44as bad and has a big risk of infection.
31:47We're just going to take a bit more out.
31:50Just not a lipoma, but it's just a redundant skin.

31:55Just because otherwise it's going to be a bit baggy for you.
31:58But you have to remove that to make the scar look flat.
