02:57She's been in labour for some time...

05:21She's showing all the characteristics of wanting to give birth, but she's not able to give birth.

05:36There's something gone wrong with her birth mechanism.

07:37Over in Huddersfield, Rohin wants to do everything in his power
07:51to help a guinea pig called Vader.
07:54There's a large wound on her back that's not been healing.
07:58It was initially an abscess that burst,
08:00and these things can often get better with medical treatment,
08:03but she's been unlucky, and it hasn't.
08:06To try and solve the problem, Rohin's thinking outside the box.
08:10Looking at Vader's back, there's not much skin to work with.
08:13It may not be possible to stitch that together
08:15in the way we normally would.
08:17We may have to do something called a skin flap,
08:19a rarer technique borrowed from human plastic surgery.
08:25Vader, who was brought in by Amanda,
08:27is one of a pair of guinea pigs who are doted on by her children.
08:32She's the quieter one of the two guinea pigs.
08:35She's named after Darth Vader.
08:37She's my little boy's guinea pig, and they're lovely.
08:40They're a really nice pair.
08:42Aw, she's a sweet little thing, isn't she?
08:44She is.
08:45They're really important to the children,
08:47and they would have come with me today to drop them off,
08:49but they just felt a bit too nervous about it.
08:52To help Vader, Rohin can't summon the Force.
08:56So, I'm expecting a big squeak here when we give this.

09:02But he does need Nurse Jade.
09:04Guinea pigs are one of the highest risk animals for anaesthetic,
09:08so we need to be very careful how we go about it.
09:11I've wrapped her little feet up in bubble wrap.
09:15Because she might be asleep for quite a while,
09:17it's really important to keep her warm,
09:19and they do lose a lot of heat out of the feet.
09:23I have to be quite careful when clipping it,
09:26because if you squeeze it, the cheese wants to come out.
09:30Ooh.
09:31Oh, it smells as well.
09:33It looks even bigger now.
09:35You can see all the hair's gone around it.
09:37There's infected material coming out of it, yeah.
09:39I mean, this is the issue we've got,
09:41that we're going to have to remove the infected material.
09:44The wound's probably going to become even bigger at that point.
09:47To actually get this closed is going to be our biggest challenge.
09:52Step one's just tackle this nasty infected stuff, isn't it?

09:58Guinea pig pus is thicker than human pus.
10:01It tends to form this solid cheese-like material,
10:04and often it does require removal under surgery,
10:06like the way we're doing now.
10:09If we can't get this wound to heal,
10:11there's a chance she may have to be put to sleep,
10:13which would be devastating.
10:15We have broached the subject with our owners.
10:17If the wound is not going to heal properly,
10:19she won't get a quality of life,
10:21and it does mean that it's quite a high-stage surgery.
10:24I feel really nervous for her,
10:26because she's only little,
10:27and obviously it comes with risks,
10:29but hopeful that we can come to a resolution
10:32and she'll recover.

18:47There's a lot more than I thought there would be at the start, but we've committed now.
18:51The more I remove, the more nervous I get about how to get this wound closed, but it has to be done.
18:57She's doing fine under her anaesthetic.
18:58It's a lot riskier than a dog or a cat, but she's doing really well.
19:06It's a big undertaking this, isn't it, especially for such a tiny animal.

19:26Removing the infection isn't Rohin's biggest battle.
19:28The more I take off, the more risk we're creating, but this is all infected skin, so it has to come off, otherwise it can't heal.
19:39I think we've got it all now.

19:43The challenge now is wound closure, isn't it?

19:46And to close up the huge wound, he's thinking laterally.
19:50So the idea is we will create a second wound in the shape of a V that will allow the wound to stretch more.
19:57We have more skin to work with.
19:59We do end up then with two wounds to close rather than one, but the amount of tension on the first wound is much reduced.
20:07It's all in the measurements.
20:09I'm happy with the lines I'm drawn.
20:11I'm going to make my second cut now, and we can do this skin flap.

20:25So now we can close up the original wound.
20:28That will stretch the V into a Y.
20:31We then close up our Y, but the overall tension on either of them is less.

23:12...has been showing signs of lameness.
23:15She's certainly got arthritis done these toes.

23:23That's quite knobbly, and that's quite knobbly.
23:26They're the two where it's painful.
23:27Lucy and I have taken quite a few X-rays of Emmy's legs.
23:32There's quite a bit of arthritis around this joint here, which...
23:37It's not that common in dogs, but you do seem to get a disproportionate amount of pain with foot issues like this.
23:43So my plan is I'm going to do an injection into this joint of a steroid and into this joint as well, that will reduce some of the pain and some of the inflammation, which hopefully will help.

23:56You're waking up a little bit.

24:02This is a long-acting steroid.
24:04It works quite well for the little joints of the foot.
24:07It's not something that we do very often, but for foot pain, it's quite good, a bit like when people get steroid injections.

29:00It's not something we do commonly and not something I've personally done before, but it
29:04should all be worth it if we see it today and everything's sealed okay.
