A major fertility group has incorrectly transferred the wrong embryo to a patient. It's the second incident that Monash IVF has apologised for in recent months.

00:00The IVF provider which has clinics around Australia has admitted to another bungle in a statement to the ASX Monash IVF says that a patient's own embryo was transferred to that patient instead of their partner, which is what was agreed on in a treatment plan.
00:20Now, the IVF provider has apologized to this couple and says it's doing an internal investigation. It says it's also going to expand a review into an IVF bungle that was reported earlier in the year, that one in Brisbane where a woman gave birth to a baby who wasn't hers, again because of the incorrect transfer of an embryo.
00:41The IVF provider says that was due to human error. Now, with this latest incident, it says that it's going to do another internal investigation and also will be introducing a bunch of safeguards around patient confirmation and additional verification processes to restore confidence in patients and clinicians about what they do.
01:03It's also, it says that these are going to go above and beyond current practice and hopefully will make some change.
01:11Now, it says they've also notified industry regulators about this latest incident and is continuing to support the couple, the couple that have been affected by this.
