00:00Manakam, I am Dr. Sujai Susikar, Senior Consultant Surgical Oncologist at Kaveri Hospital, Alwarvid.
00:05So, we are going to talk about pancreatic cancer.
00:08Pancreatic cancer is a group of cancers which arises from the pancreas,
00:12which is an organ which is situated deep inside the abdomen
00:15and it is mainly required for secreting digestive juices
00:19and also in the secretion of insulin which prevents diabetes.
00:23If pancreatic cancers are there, there are two main types of cancers.
00:26One is adenocarcinomas and the other type is called the neuroendocrine carcinomas.
00:32Now, the neuroendocrine carcinomas, they have got a very good prognosis
00:35because and the type of tumors, they spread very slowly
00:39and they tend to be localized in the pancreas for a very long time.
00:43But adenocarcinomas around the type of cancers are usually very aggressive.
00:47Now, what can affect the pancreas in its entire body
00:50and what is very important is pancreas or the distal pancreas.
00:55So, the head can cause pancreatic cancers usually present at an early stage.
01:03It is because it affects the head of pancreas
01:08and the head of pancreas will go through the head of pancreas
01:14to secrete bile into the intestine.
01:15Now, in the tumor, we will obstruct the bile drug.
01:18Jaundice comes even at a very early stage.
01:21So, head of pancreas cancers usually present with jaundice
01:24even when they are early.
01:26So, adenocarcinomas is identified at an early stage
01:27and they can be usually cured.

01:35and are usually identified at a later stage
01:37and so they don't have a very good prognosis.
01:39Now, treatment of pancreatic cancers
01:41there are two main types of treatments.
01:45One is surgery and the next thing is chemotherapy.
01:47If a surgery is used for curing these pancreatic cancers,
01:50pancreatic cancers involving the head
01:52we have to do a surgery called pancreaticoduodenectomy
01:57or the Whipple's operation.
01:59This is a very complicated operation
02:00because the head of pancreas is here to go.
02:02you have to remove the adjoining small intestine
02:05or the duodenum,
02:06the distal part of the stomach,
02:08you will have to remove the bile duct
02:10or the pitta nadi
02:10along with the gallbladder
02:12and also the first part of the small intestine.
02:14What is very important is that
02:17this is a very deeply situated organ.
02:19This surgery is a technically demanding surgery
02:24and can be associated with complications.
02:27So, after you remove all this,
02:28we have to bypass the pancreas head
02:30by causing an anastomosis
02:32or stitching up the intestine to the bile duct,
02:35stitching up the intestine to the stomach
02:37and stitching up the intestine to the pancreatic duct.
02:39With this,
02:39we are able to achieve a very good cure
02:41for head of pancreatic cancers.
02:43But body and tail cancers are purtavarikyom.
02:45They usually present at an advanced stage
02:47because they can usually become larger lumps
02:50and may usually present with spread to distant parts.
02:53That is,
02:53they may be spreading to the liver,
02:55to the other parts of the abdomen
02:56and this makes it incurable.
02:57In this stage,
02:58usually the treatment is going to be chemotherapy
03:00where we will try to shrink it
03:01and if it shrinks,
03:02then they go in for surgery.
03:04Early tail and body pancreas tumors
03:06can be treated by something called a distal pancreatectomy
03:09where we will have to remove the distal part of pancreas.
03:11This surgery is a simpler procedure.
03:13If a pancreas cancer is a purtavarikyom,
03:15what is very important is that
03:16we have to identify them at an early stage
03:19and treat them appropriately to achieve a cure.
03:22Though surgery is an extensive surgery,
03:24today we are able to do this surgery
03:25by means of minimal invasive procedures
03:27using either laparoscopy or robotic surgery
03:29and thus achieve a good cure
03:31at the same time preserving the quality of life for the patient
03:34and the recovery also is very quick.
