00:03What we have found is a series of national challenges.
00:06So this is beyond just one, this isn't just one or two services, this is our patterns
00:11that we've seen across different parts of the country.
00:14And what we've found is that there are issues in terms of oversight of maternity services,
00:20in terms of culture, and the listening to women when they've got concerns, listening
00:25to staff when they are trying to raise concerns.
00:28We've also seen that we haven't seen enough progress in tackling health inequalities.
00:33We've seen some really good examples in some organisations, but that hasn't gone far enough,
00:40we have to, that has to be driven further.
