00:00We are absolutely heartbroken by the number of times that Callicane's course could have been
00:07altered. We are absolutely heartbroken and devastated by the missed opportunities. He
00:13was sectioned four times and each time he was sectioned his treatment didn't change or people
00:19couldn't be bothered to change his treatment. To actually give someone an injectable medication
00:25that lasts a long time so you don't have to take medication daily was not sought.
00:30Community treatment orders that force a patient to take their medication in the community
00:35was not done. It is just every opportunity that could have been missed was missed. This is an
00:41absolute tragedy that this happened because I'm more than convinced that this was wholly
00:50avoidable if this person had been treated with any form of responsibility whatsoever.
00:56It was lazy and quite frankly we believe that the clinicians involved and the staff involved
01:04need to be held accountable because basically homicide does not occur without problems and
01:09delivery of standard clinical care to patients and he did not receive that and as a result the
01:15consequences have been disastrous, heartbreaking and life-destroying.
