00:10The inquiry follows the deaths of Alistair Silva Aguiar, B.B. King and Elsie Dot
00:15Stankham at a dance class in Southport in 2024. Axel Rudakobana was sentenced
00:21to life imprisonment with a minimum term of 52 years after being convicted of
00:27their murders and attempts to murder others. The first phase found concerns
00:31about agencies handling of risk and made recommendations. The second phase will
00:36consider whether changes are needed to prevent similar attacks and to improve
00:40public safety through further scrutiny.
