01:49Never went to a doctor. I wasn't allowed to go to the doctor because of the abuser.
01:55So I haven't been back to the doctor because I don't want to relive that shame.

16:52the pandemic hit, and so they couldn't do it.
16:55I never went back to have the surgery because the doctor, he told me I have stage five of my
17:02kidney disease. Stage five.
17:05Can't catch a break. The one thing is something else. Now, I'm just tired. And I'm tired of this bump
17:11on my back.

29:57I developed a really high level of expertise in rock climbing,
30:02so I've been able to take that same method and try to apply it now to dermatology.
30:08I want a protege for Dr. Lee because I'm at a stage in my career where I have everything to
30:15learn about dermatology,
30:17and I want to learn it from the best.

30:53I have a little bit of experience in dermatology, but I'm still really green.

30:58I'm just so ready to learn and so excited.
31:01And that's amazing because I want to show you things that I use as a dermatologist that
31:05you don't really know unless you think about it yourself or you watch somebody do something
31:09like that.

31:13This is like my favorite kind of thing.
31:15Dr. Lee gets a lot of extreme cases.
31:18My mind's already kind of going like, what is this going to be?
31:21So let's start with, we have a patient here today.
31:24I haven't seen him.
31:25So we're going to see what he's got going on.
31:28It's not like a giant bump.
31:29He doesn't have an extra head because we're going to start off easy, but I'll give you
31:32some tips.

31:34Dr. Lee told me we're starting you off small today.
31:38So I don't know what's to come.

32:24Mama's very, very squishy, like clay dough.

32:32How long have you had these bumps?
32:34When I was nine now.
32:36And have they grown bigger just as you've grown?
32:38Yeah, especially this one.
32:40My mama grew no bigger than baby.
32:42Okay.
32:43They grow.

32:44Well, do you mind if we take a look at them?
32:46Of course.
32:46Would you like a gown to cover you?
32:48Sure.
32:49I don't know if you want to describe it.
32:51Yeah.
32:51Like a resident would.
32:53Yes.
32:53So I think it's like about two centimeters fleshy, well circumscribed nodule.
33:00It's soft.
33:01There's no secondary changes on the skin like scale.
33:05Okay.
33:06Can we take a look at the other one?
33:08This is mama.
33:10Yeah.
33:11So this one, this is like a pedunculated nodule, also fleshy, but it's kind of on a stock.
33:19I'm going to feel it too.
33:20More like a pretty wide, but pedunculated stock.
33:23And then this one would be more sessile, I would say.
33:26All right.
33:26If you don't mind having a seat, let's just talk about it a little bit.
33:29Do you have any thoughts about what these might be?
33:32Oh, no.
33:38I wasn't ready for this.
33:40When a doctor asks you, okay, well, what do you think this is?
33:44It's kind of the thing that sends a shiver down medical students' spines.
33:49I think.
33:59Do you have any thoughts about what these might be?
34:02Oh, no.
34:04I'm here with Fode, his two bumps, and dermatology intern, Dr. Tolson.
34:09I've just put Dr. Tolson on the spot.
34:11I'm really kind of curious to see if she's able to accurately diagnose what Fode's bumps are.
34:18I think they kind of look like lipomas.
34:21They feel very soft and very, like, rubbery.
34:25I don't really think that that's what that is.
34:28I think there's a chance this one here can be a neurofibroma.
34:33A neurofibroma is just a benign growth that sort of has nerve tissue in it.
34:38And the one under here, it might be like an acrochord on too, which is just a big skin tag.
34:45Because usually a lipoma is under the surface.
34:48Okay.
34:49Dr. Tolson thinks that these are lipomas, but I would say technically no.
34:53She really hasn't started learning specifically about dermatology, so this is really understandable.
34:58I'm more, you know, agreeable to remove mama.
35:02I can remove baby, but if it's a neurofibroma, it can live under your skin a little bit too,
35:07and I would try to make it as smooth as we can, but you might start to raise up again
35:12a little bit.
35:13I'm happy to do both.
35:14Yeah, yeah. Actually, I want both of them to go away.
35:18Okay, well, I'll give you a moment to say your goodbyes to them, okay?
35:21Yes.

35:24Yeah, I'm a little bit nervous about removing mama and baby.
35:29Like Dr. Tolson said, baby can't go back, you know.
35:33That's the only thing I'm going to nervous.
35:41So I'm going to put a little numbing here, okay?
35:43When I numb, I usually try to squeeze and like wiggle it a little bit.
35:47Mm-hmm.
35:47It's the pressure change.
35:48If you inject something really quickly, that's more uncomfortable.
35:51Now I really want to show Dr. Tolson a real derm lesson.
35:55As you're a resident and you're learning about it, you know what an NF is,
35:58but you don't know what it looks like when you actually excise it.
36:01That's how you transfer to a practicing dermatologist.
36:05You actually see what things look like in real life.
36:07I'm going to like score the edges a little bit,
36:09because that actually gives you the plane in which you want to be in.
36:13So instead of lifting it up, like squeezing it like this,
36:17because that will undoubtedly leave it indent.
36:19Mm-hmm.
36:20I'm going to just take across and go straight.
36:22Like if I make this taut, I can make it go straight.
36:25For the NF, Dr. Leeds scores around the edges.
36:28I have never seen anyone do that before.
36:31It usually is only cut off with the scissors.
36:34So it's a really elegant way to avoid divots.
36:38But the hard part is the center.
36:40Once it pooches out, it kind of like, um, herniates out.
36:43Mm-hmm.
36:46It's like a little button.
36:48Baby's gone.
36:49Baby's gone?
36:50Yes.
36:51You can see some of the fat enucleating out.
36:53See, that's like the neurofibroma, I do think, you know?
36:59And see how I hold it? I don't lift it up.
37:01See, that's underneath.
37:02Yeah.
37:02But we're going to leave that here.
37:05Baby's off, and I'm glad to say we did not create any divots.
37:08So now we really want to get rid of mama.
37:10I know that Dr. Tolson is smart, and she's really paying attention.
37:13So I'm actually going to let her remove the skin tag and take the lead,
37:16with me standing by, of course.
37:18I'm going to start it, and then you can finish it,
37:21because we're going to keep it as flush as possible.
37:26Dr. Lee just hands me the scissors,
37:28and I want to show Dr. Lee that I can rise to the occasion.
37:32Keep it against the body here, right? Yeah.
37:35See, when we pull up, yeah, that's why you want to keep it exactly.
37:38Being very meticulous and precise about the last little bit.

39:42and thrilled to be cheering on her and Yankees in total comfort.
39:46Now that she's gotten that burden off her back,
39:49she's now able to focus completely on her kidney,
39:51and getting herself healthy.

40:07It has been two months since Dr. Lee removed the bump over my right eye,
40:12and I feel like a brand new person.
40:17The scar is small.
40:18I can't even tell where it is sometimes.
40:21That bump was something that I had to look at every single day,
40:24and it's such a great feeling to not have to feel like that anymore.
40:29Javar is my person, and I feel so safe with him.
40:33Javar and I have made a new wedding date.
