01:38the heart rate is going at a rate that it's far too high to be safe

01:57so get ready to share a shift some days you get up and you don't know what's going to happen

02:18it's monday morning at barnesley casualty and in the hub it's a busy start for sister christy
02:24and lead nurse steph you've got a 58 year old on route and you've witnessed a collapse and what's that
02:34ambulances are arriving thick and fast
02:3877 year old male less than five minutes for them yeah thank you
02:47cubicles are full we've got all these ambulances coming in as well yeah
02:52a new rule the ambulance crews must hand over patients within 45 minutes is creating extra
03:00pressure we've had seven ambulances just arrive in the past 40 minutes but we've only got the two
03:06cubicles to take hand over in there so manager antonia joins the floor to support the team
03:15we're running out of space htc is a full we've got at least 11 patients waiting for beds
03:20down here we've got wards that we're having to keep down here because there's no beds yet
03:26my role is to support the department itself the patients the staff
03:31i love the fast-paced environment here no day is ever the same i never know what i'm going to

03:35to work to i can sit down in the morning have a cup of tea and then it's straight up

04:01as the red phone keeps ringing paramedics are rushing in a woman with suspected sepsis
04:08we'll see what her arms look like when she comes in dr underwood will be in charge of her care
04:16so this is the most severe form of infection that you can get your body is completely overwhelmed
04:25by the infection it needs a really high heart rate and basically all the different organs start to
04:33not work as well heart rate is going at a rate that it's far too high to be safe
04:46the blood pressure means that she's quite unstable so we've just got to try and get on top of
04:50things quite quickly
05:0467 year old diane was rushed in from her gp surgery one two three well done

05:34no nothing like this okay at the moment your heart's going kind of much faster than we would expect okay
05:42so it sounds to me like from the gp that you had a cough
05:47have you been coughing up phlegm yeah has that been for a couple of days yeah yeah and then you
05:51coughed up some blood today as well yeah and you're in feverish you said you felt sweaty yeah and
05:59is that been for a couple of days as well yeah okay yeah i will walk through oh really i'm
06:08just gonna
06:09have a quick listen to your chest if that's all right
06:17i'm going to get an x-ray in your chest so i can hear some crackles kind of on both
06:23sides
06:26sometimes we get a diagnosis it's really easy but actually with most patients it's about
06:31rolling in or out the most serious things and if you're doing that and you're treating that
06:40then you're making the correct start for the patient we'll get that x-ray we'll get the blood sent off
06:51we've got the fluids going in we'll get start some antibiotics and once we're giving you that
06:56treatment i'm hoping your heart rate will start to settle dr underwood must work fast sepsis is
07:06really serious we know that it unfortunately can lead to death
07:13it's something that's something that we aim to identify within an hour of a patient arriving

07:52in resus diane has suspected sepsis can you point to me exactly where that pain is
08:00dr underwood thinks the sepsis could be the result of a chest infection

08:48when she's got what we call kind of haziness on both sides definitely worse on the right side which
08:54is where i could hear the the crackles on her chest here and here so usually all the lung looks

09:05to me it looks like you you've probably got a bit of the chest infection
09:08just want to see what your blood pressure is doing
09:15her blood pressure's dropped a little bit more so i want to give her more fluid it's likely she's pretty
09:20dehydrated that happens with an infection so we're just going to give her a shot of fluid quite
09:27quickly in the antibiotics and then just some more background fluid
09:30it unless diane's body starts responding to treatment her condition could become critical
09:46we're not close aren't we we're cold in the hub the pressure is mounting have we got any spare cubes

09:55we're always going to go with four more ambulances arriving operational support manager antonia is

10:13it is quite stressful but i think we're all here to help each other out it's nice
10:18to be part of a team and have the people there around you we need to all work together
10:23to have that joint approach to everything is really imperative and needy
10:29we're just very very busy waiting for bed so all the cubicles are blocked now

10:38we've got a lot of ambulances coming not much space

11:37practitioner maddie what's happened well i was taking some i knew they'd just been
11:43back recycling coming this morning instead of just taking a couple of things i took four
11:48up and my slippers sent to stick on the step and it threw me that way

15:39pain in different ways sometimes it's hard for them to verbalize the pain so that's the one thing

15:49what if we can put some weight on is that okay so if you put your foot weight on your

16:40worth getting an x-ray just to rule out a fracture first and then make a plan from there okay

17:01how's that pain now coming and going pardon coming and going coming and going okay we'll keep an eye
17:08because we can give you some morphine if not okay over in recess dr underwood is battling to get diane's
17:14symptoms under control i'm given affluence and then antibiotics as well uh but both those things
17:22just haven't done the job it's a really common cause of death in this country yeah sepsis does kill
17:30so at this point i need to go and get a senior to get some advice about what to do
17:36next because i've
17:37given a fair amount of treatment and it hasn't improved things
17:45dr underwood refers to consultant dr rob jones
17:51my concern is we've given romance antibiotics we're only probably on our second liter at the moment
17:58but systolic is just still hovering below 90. a rate of this yeah 150 bang on which fits with flutter
18:06yeah shouldn't cause hypertension to that degree no she's already had as much fluid as as you would
18:13yeah yeah thanks so much yeah diane's gone into something called septic shock where
18:21your body has become so overwhelmed that actually it can't even maintain its own blood pressure she's
18:28at risk of organ damage but the critical care team are specialist doctors that look after people with
18:35conditions like yours where the infection is having a really big impact on your body
18:40and so we're going to give them a call
18:43this is charlotte i'm one of the emergency doctors is this the itu registrar because it's such a
18:51serious condition people have to be monitored much more regularly than they would be on a ward
19:19and a further three days on a ward
19:27she's now back home and has made a full recovery
19:39come through the normal hub let the ambulance nurse know when you arrive and i'll try and sort
19:45with more patients on their way there's no let up in the hub for sister christy
19:51and the team yeah no problem love thank you bye
19:57so how many cubes are blocked for you
20:02nine cubicles we've had a really busy day with both walk-ins and ambulances at the moment we've got
20:08seven ambulances arrived at the same time so we've got to make decisions on what can be done to get
20:13these patients where they need to be and make sure everybody's safe so they're doing everything they can
20:17to get the patients out to free space up down here so we've got one to watch 20. 15's gonna
20:24go now
20:25so we're just going to take them out the cubicle to take hand over this one and then have a
20:28slow walk
20:29up because part of the just waiting for a next stop one cubicle is cleared
20:37but yet more ambulances are on their way so if they're the one inbound and another one expected
20:54paramedics are bringing in a 73 year old man with atrial fibrillation an irregular heartbeat
21:09it's good when i was a little earlier yeah brian has just had open heart surgery and was only discharged
21:18from hospital yesterday well it's good when i was being let out in all of them last night
21:28a really good movie sure can't keep away do you like us that much
21:35are you feeling sick at all no the only thing i can feel at the moment is my heart is
21:44beating a bit
21:45a bit of pain brian's irregular heartbeat brian's irregular heartbeat puts him at risk of having a stroke
21:52right can i just have a a little look at you um so tell me what what's happened this young

22:02consultant dr liz needs to quickly find the cause
22:06what's happened this morning what's changed i can feel
22:12um that something is bumping my heart yeah side to side yeah right and it feels like it's knocking
22:20me warm they knew that brian has had surgery in the past for cancer of the esophagus
22:29so obviously your esophagus out 10 years ago so if it's not a crude question what do you have
22:35to deliver the esophagus have they lengthened your stomach or um they've reshaped my stomach then made
22:42it into a tube yeah and then brought it back up here so we'll keep an eye on you and
22:50then we'll decide
22:51what we're going to do i'm going to get a chest x-ray because i think we need to see
22:56what's going on in
22:56your chest if that's all right yeah yeah okay you just sit forward for me is that okay with the
23:06cause
23:06of brian's irregular heartbeat unknown dr liz must work quickly to make a diagnosis
23:28okay 15 17 is going six is going that's in fit to sit in the hub the new 45 minute
23:37policy
23:37means manager antonia is working against the clock to find spaces for patients who've arrived by ambulance
23:4645 minutes the crews have to hand over to our nursing staff and we're getting really tight at
23:53the moment just because we've got no space to hand over these patients and we've got one at 27 minutes
23:58at the minute so they're doing everything they can to prevent the crews having to leave that patient
24:14with a cubicle cleared for one new patient the team now need to find space for the rest
24:30so how are we doing brian you all right um i felt a lot worse earlier so at least we're
24:36heading in
24:36the right direction yeah in recess brian has an irregular heartbeat following open heart surgery
24:44the doctor just asked me just to do a quick x-ray of your chest is that all right sure
24:50oh that's not really comfortable i'm sure it is i've got that nugget
24:59right brian just breathe in for me hold it there and breathe normally
25:08there we go rest back hopefully a bit more comfortable than that board yes
25:12it doesn't actually look that different from the last one dr liz checks brian's x-ray apart from
25:24you've got some extra bits of wire in your chest yeah just what you need yeah his irregular heartbeat
25:32could be the result of an infection
25:36to me it does look like there is a little bit of some fluffiness down at the right side which
25:43could
25:43represent infection so i would be inclined just to give you an antibiotic because you've just been
25:52in hospital um and unfortunately hospitals breathe bugs and it's quite easy to pick up some sort of
25:59chest infection if the antibiotics don't stabilize brian's heartbeat his life remains at risk

26:36i'm just going to give her skin tear quickly and see if i can pull the skin back over
26:42nurse amelia is treating pat who sustained multiple injuries when she fell taking out her

30:12accident at work took some skin off front of my thumb drilled into it no not quite piece of
30:18metals cut it right okie dokes a lot of the injuries that come into any are work related injuries

30:28right then have a seat on there i'll get some gear and we'll have a look it is hopefully reassuring

30:36the patient if you're friendly and chatty we were saying there's been three cars on the roofs this week

30:46then they feel at ease before you start doing an intervention with them that's potentially going
30:52to be painful what's going on there then they're driving around corners too fast it's got to be
30:57speeding can't really be too bad a weather can it's not it's not been raining this week
31:04like a shedded piece of skin aren't you i'm not squeamish in the slightest i like wounds and the bigger
31:13the better the issue with that is it's like you've cheese gratered that piece of skin
31:20but it's quite dirty is that the cleanliness of your hands or will it come cleaner it will come
31:26cleaner yeah if you go to the sink and give it as good a clean as you can then i'll
31:32try to
31:33stir and strip it back across it so that we can try and let it recover you don't want to
31:37cut it off
31:38no i don't want to cut it off because it leaves that area prone to the infection but it might
31:44drop
31:45off when the undersurface of the wound is healed all right so give it a good wash in there with
31:50soap
31:50and water and hopefully we can get it cleaner than that
31:58mechanics have always got ingrained hands haven't you absolutely do you want me to get you some sugar
32:05do you normally wash it with sugar you get some tricks of the trade this sugar and washing up liquid
32:18are you feeling funny no but it's soft oh don't uh wash underneath it just wash on top thank you
32:30yeah it's all some people use even cold water it doesn't dissolve so it stays as a grain which gets
32:36in the skin and uh it gets all dirt out that's good is it nice good as new look yeah

32:53steady strip that little bit of skin back on it's more of a protection thing but we don't want it

32:58be dirty and you can bend your thumb all right everything's all right just keep it on for a couple

33:12days then you can take this off and change it to just a waterproof dressing which i'll give you a
33:16few spares so that's done and i'll just get you some spare dressings and then you can be on your
33:23way

34:48i was really looking forward to my first day of freedom today only to find them back where i started

35:11brian spent over a week in hospital but is now enjoying being in his own home
35:31in radiology nine-year-old archie is having his ankle x-rayed by radiographer rachel
