01:21there's the world's biggest crew in here for triage have you seen that there's 19 waiting
01:24have you seen all this advanced clinical practitioner chantelle sean open your eyes
01:33you okay sean
01:38and does that hurt there a bit a bit and advanced clinical practitioner jemma
01:43without boxing because i do that you don't box with your feet do you
01:49you've been in before been in a lot of times a repeat offender then yeah so get ready to share
01:56a shift it's a 24-hour service seven days a week with the team at barnesley casualty
02:08it's seven a.m and at barnesley casualty sister jane is running the hub
02:14tracy it's only jane i need a female surgical bedlock
02:20i'm around down here she's taking over from a very busy night shift
02:27we just need to get them out so that we can get some space
02:32we're always taking patients in by ambulance or walking or the police bring them in
02:40and it's difficult because the department becomes gridlocked
02:46and unfortunately we're reliant on other areas helping us out to clear the department
02:55a and e has quite a reputation of just speed and pace a lot of people can be frightened by
03:03it
03:04it's a daunting place to come and do a shift there's coming through two ambulances waiting to come in
03:10right have they said that they're keeping you in right i'm going to send you down surgical s deck
03:17just to get you out of here all right love hey up it's only jane down in a and a
03:22this lady
03:23and trace is aware with us she needs to come down to you all right darling all right love thank
03:28you bye
03:29right she can get to us sister jane moves one patient on but as one leaves another arrives
04:06this gentleman had a stroke at the end of september and since then
04:10she's been having non-athletic seizures advanced clinical practitioner chantelle will take charge
04:16of sean's care can you see me sean sean open your eyes for me open your eyes hello sean do
04:27you know
04:27where you are no you're in bansley any department have you got any pain anywhere
04:35yeah this one did you fall onto your arm
04:41you don't know have you had chest pain today whereabouts in your chest
04:56i need to get these answers from sean quickly but it's quite difficult with his condition
05:03sean was found on the floor by his wife sam did he lose consciousness and when i came down he
05:09was
05:09speaking to me i'm really glad his wife's here so we can get these answers and then i can get
05:14to the
05:14bottom of what's going off and did you not hear a crash or anything or nothing just i just looked
05:19as fast as you know and said help help like okay am i in a great so i just found
05:25downstairs
05:26he was on the floor yeah and then he were mourning his right arm were hurting him was like
05:31it's really hurting it hurts and then all of a sudden they were like his fingers have gone numb
05:35on his right side on his right side but since stroke it's his right side that it's been affected
05:41it's been affected yeah how many seizures did you often have like every day and he's ever about
05:50eight to ten a day eight to ten a day does it get like a warning sign or anything yeah
05:54normal his face twitches and it'll start going like this for his head his seizures are becoming
06:01more frequent and she's quite concerned so that's the reason she brought him to us we'll take an ecg
06:07pop a cannula into some blood and then we'll see what we find is that okay does it hurt there
06:15no what about there it hurts there is that where the pain's been coming from
06:20what about over here yeah it's tender there sean is flipping it in and out of consciousness
06:26but he is also complaining of chest pain so my job um at that moment in time is to figure
06:32out if
06:33this chest pain is contributing to his conscious level as well so it's quite important for me to
06:38establish that pain and where it's coming from mouse twitching i think i might have a seizure
06:45yeah sean sean sean sean sean open your eyes john open your eyes
07:07in
07:08your eyes sweetheart in resource Sean is having multiple seizures Sean open your
07:20eyes okay Sean advanced clinical practitioner
07:25Chantelle needs to determine the course Sean open your eyes for me Sean Sean
07:33open your eyes hello are you feeling not good how do you feel Sean tell me how do
07:43you feel oh you feel terrible does he normally talk after his seizures yeah no
07:50little straight out of him and he's just normal okay wife Sam has been Sean's main
07:57carer since his seizures started following a recent stroke is it only you'll have
08:06one about five seconds I'll stop then I'll go back into one and then it does that
08:12for about six or eight times and then they'll come around and they'll get on
08:16yeah but it's just continuing is he more prone to them when he's unwell when he
08:22gets hot when he gets hot I've noticed when he gets hot yes it's similar Sean Sean Sean Sean
08:31Sean open your eyes for me open your eyes
08:35open your eyes hello Sean how are you feeling you're not not feeling good there's
08:45many forms of seizures Sean these seizures can you just stop them normally like that
08:51you can stop the seizure and they'll come again and then it comes again epileptic seizures and then
08:57there's none epileptic and there's a whole host of other type of seizures so you can
09:01control these seizures not half that time but do you think you can do it now you try and do
09:09it
09:09are you alert and aware when they're happening so what's really important is establishing what's
09:14normal for Sean and what's abnormal when you're like talking to him it'd be all right as soon as
09:20you walked out the room it set off and then a nurse came back in and he talked to her
09:26and then she'd go
09:27off and he'd start again it's like if somebody keeps in talking or whatever it seems to be all right
09:33so yeah which is not classic it's not a classical seizure yeah because I won't be able to bring
09:40him out of it and he won't be able to talk to me straight away the increase in Sean's seizures
09:45could be the result of an infection so this lung sounds quite crackly and quiet so I'm going to get
09:52an x-ray of his chest you said he's had a cough how long for about four days they're going
09:58to come
09:58and do you an x-ray on your chest soon okay you need to you need to be a little
10:03bit alert and
10:04sat up for it okay so I can see if there's any infection all right you have to sit here
10:11and
10:11stroke his arm so you can keep him calm and then he didn't have a seizure that's your job Sam
10:17you're
10:19doing it as well you're a good spot there you go nice and calm but it's been in and out
10:27that many
10:28times it's like it's the norm now but this has been the worst he had a stroke about eight weeks
10:35ago two
10:36weeks in and he started with seizures he's got to go for like load electrodes on his head to see
10:44what's happening so we are going through tests to find out exactly what kind of seizures they are it
10:52just feels useless now because they were always on the goal he never stopped around doing some gardening
10:59building and now it it can't go along oh yeah you're all right hello Sean my name's Rachel got
11:15Lewis here as well we've come to do an extra on your chest love all right radiology have come to
11:20do
11:20x-rays right Sean we're gonna see me forward and pop the board behind you back all right love if
11:27the
11:27x-ray reveals a chest infection Sean will need urgent treatment over in the pediatric department I've got
11:47these things to put up for the children and they're gonna be like little stick puppets there's a little
11:52road out here so that they can use them to drive along the road there's like a little activity to
11:56do while they wait and see the doctor play leader Olivia is creating an interactive display about road
12:03safety so this is gonna be like the crossroads for Barnsley Hospital there's gonna be like a little
12:09lollipop man that's gonna stick here and then when the cars kind of come across they're gonna let the
12:14kids go fast I love my job every day is so fun they're just amazing the kids that come here
12:21this
12:21they're so lovely and so friendly right so we're going with that lady come on sweetheart one girl in
12:32paediatrics today is eight-year-old Harriet who has a suspected broken toe Harriet my name's Gemma all right
12:41walking down yeah come this way then treating casualties younger patients today is advanced
12:48clinical practitioner Gemma okay yeah movie brought with your mom and brother if you'll pop up on the
13:01bed there you go perfect so my name is Gemma what's your name mom Emma she's jumped off settee and
13:08she's
13:09banged a foot on the footstool it was sticking right out was all red and she was crying if that
13:16was my toy it just went boom and then like it hit it like there but it also hurt here
13:22and then it
13:22just went like that and it was like that the whole time and when it was sticking out did you
13:29do anything
13:29I didn't I didn't touch it and I'll try to move it or anything yeah no little bit swollen that
13:35into
13:35sweet time can I have a little look at you is that okay that hurt there sorry and just that
13:43hurt
13:43there a bit a bit okay compare it to the other side so it is sticking out so normally I
13:54would say
13:54would just strap it yeah that you've said it was sticking out that way and it is a little bit
13:59what
14:01I call angulated I think we might just need to make sure it's not like a fracture type dislocation okay
14:09so I think we will get an x-ray so give me two minutes okay and then we'll get you
14:13around okay
14:17thanks you're right yeah you've never really had any injuries before have you I've never broken a
14:24bone do you follow me and I'll show you where to go you walk around if Harriet's x-ray reveals
14:33that
14:33her toe is dislocated it will need to be manipulated back into position 33 have got no beds at the
14:53moment and I've got this neck of femur who they do know about and there's potentially another neck of
14:58femur that's coming in the hub the pressure is not letting up for sister Jane casualty is still
15:04jam-packed from a busy night shift it's looking busy at the moment there's 63 patients in the
15:12departments at the minute for 11 o'clock in the morning it's not great some of these patients have
15:18been here 14 15 16 hours so it's not really very good we're not a ward and we need to
15:24move patients out
15:25to get more patients in it's just a vicious circle really it's just never-ending you still have a duty
15:33of care towards those patients to ensure that they're safe they've come to a hospital to be
15:38treated for their illness and they need to be safe and sometimes it's just difficult we've been in there
15:44all morning nobody wants them in here for 12 hours because they're on trolleys but everybody's doing
15:52the best hi Shane I love a patient with a history of heart attacks has arrived 79 year old Tommy
16:06is
16:07taken straight to the ambulance bay so granddaughter's phone today because he's got this normally chest
16:14pain okay fine granddaughter Lauren called the ambulance you had a bit of a chest pain yeah yeah
16:22I've been chest pain again I've not been very good for a long time Tommy is all too familiar with
16:30Barnsley casualty I've been with a few times you had a gash on your back of your head didn't you
16:35you fell
16:35is that when I fell down the stairs yeah and you cut your head up and didn't you yeah yeah
16:40so you
16:40had to have it cloned great good job that was good job in there that was good hey Tommy my
16:48name is
16:48Jess am I okay to have look to see if I can get a cannula and to take a test
16:52yeah yeah and what are you allergic to
16:55I'm allergic to I used to be allergic to the song that's fun I was only in here but week
17:05and a half ago I
17:05think no two weeks yeah I was just gonna go and get you shopping for you before you decided to
17:13feel
17:13well again I've been staying with him for a few days we take it in turns because he's on his
17:19own now since
17:19he lost his wife my mama since then he's gone down I know so he's got a lot worse so
17:29he's got 21
17:33grandchildren and 16 great-grandchildren so we all take it in turns don't we granddad oh come on
17:46we got to see doctor now okay thank you Tommy Tommy must wait for the blood test results to find
17:56out
17:57if he's had a heart attack if he has he will need to stay in for observations so Ellie's coming
18:09in at
18:09two and then they've got a message on for any band threes they have sent me somebody down from the
18:13ward but only this morning in the hub sister Jane not only has a jam-packed casualty she's now two
18:20nurses down we've had some sick calls today when stephen are tired they become susceptible to catching
18:29illnesses so we all struggle really pretty rubbish we're not going to close the department for no staff
18:43it's a 24-hour service seven days a week so you just have to plow on
18:59in resus Sean is still having seizures advanced clinical practitioner Chantelle has the results of
19:06his chest x-ray his infection markers are up a little bit yeah but there's no pneumonia on his
19:12chest x-ray which is good so I'm going to give him some antibiotics to clear that chest infection
19:17so I have told the doctors upstairs about him yeah and they're happy for him to go upstairs when there's
19:22a
19:22bed yeah do you think the infections cause sometimes yeah sometimes if he's been a bit unwell and a bit
19:29feverish can upset him a little bit and hopefully antibiotics yeah yeah yeah and if he's okay tomorrow
19:36they might convert them to oral tablets and then he can take them at home yeah right I'll leave you
19:42with
19:42him okay hopefully good get the antibiotics in and hopefully get him back home and get back to
19:51well normal as possible Sean spent the night on the ward and was released the next day he's still
20:01undergoing investigation for his seizures there is no let up for advanced clinical practitioner
20:13chantelle as one patient leaves another arrives paramedics are bringing to resus a 94 year old woman
20:24with a high heart rate and dangerously low oxygen levels
20:41right we're going over onto here okay so a little ride are you ready ready steady slide oh you're light
20:50as a feather paramedic marcus hands over her care all right margaret what any other medical history so
20:58alzheimer's and osteoporosis as well how are you feeling at the minute it's fine you feel a little
21:07bit frightened our priority in emergency medicine is to find out the emergency but it's really important
21:13to try and make a patient regardless of age not be scared or frightened especially when they're confused as
21:18well so margaret do you know where you are at the minute yeah i think i've been before but this
21:25is
21:25this area that sets me and i have been before i do like to give comfort with touch do you
21:37want me to
21:37remind you yeah so you're in barnesley hospital at the minute yeah can you remember now don't worry you're
21:45fine and you're in the ane department you don't feel frightened at the minute oh i'm not right good
21:52were you feeling you're feeling did you and you had your two little boys over there yeah are they
21:57still over there rather did they come back you know i don't even know where they are all the best
22:08all
22:08right so your son's gonna come up okay so your son's coming up and he's gonna come and see you
22:14all
22:14right take care bye-bye see your son's around not in australia
22:24can i have a listen to your heart and your lungs is that okay and can i take some blood
22:30tests as well
22:30yeah patient reassured the next job is to find out what's causing margaret's dangerously low oxygen
22:45levels
22:55there's the world's biggest crew in here for triage have you seen that there's 19 waiting have you
22:59seen all this in the hub sister jane is running a casualty that's full of patience and short on staff
23:09kelly kelly kelly there is just nobody else that i can send down there i don't know what to do
23:14it's walk-ins there's 19 waiting to be seen with an hour and 15 minute wait
23:20and it just makes it bad for this afternoon because they're going to continue to come in
23:25it makes me want to go home
23:29we all rely on a certain number of nurses to get work and jobs done
23:33and obviously if you haven't got that nurse there if you want down then it falls on another nurse
23:38or two nurses to share that responsibility so it does make it difficult
23:45tom in rapid or lisa in resource take your pick but let me know you can do rapid with me
23:51have you seen
23:51how many there is to assess walk-ins oh you are kidding me
24:08you okay yeah right sweetheart can i have this arm just to take some blood
24:15in resource advanced clinical practitioner chantelle needs to find the cause of margaret's dangerously
24:22low oxygen levels
24:28they're not very good oh let's see what i can do
24:34i'm quite good you know i'll not be good now i've said that
24:37i'll never imagine you're a good girl i can see you're a good girl
24:42oh you can see i'm a good girl
24:45i can't see i'm no good girl
24:51right are we ready yeah oh wow oh it's in don't worry
25:00there we go there's no needling now oh it's all done oh it's just i'm getting the blood out
25:08not bad at all not bad are you sure are you sure i thought i hurt you then
25:20i promised you a nice cup of tea didn't i after this as your little treat
25:27in sensitive emotional situations i still like to be a little bit light-hearted
25:33and um make the patient feel more comfortable do you want anything to eat i hope we have a biscuit
25:42do you want a biscuit we've got some biscuits i try and be as human and normal as possible because
25:48that's what i would want as a patient i'd want someone to be just really normal and nice and light
25:53-hearted with me
25:55i'll see what i can find i'll rummage around
26:00i'm sure you'll find one i will margaret's sons join her as she heads for an x-ray
26:13have a seat we'll go with your shop real thank you
26:17not yet they're gonna tell us when they're ready
26:22also in radiology eight-year-old harriet is waiting for an x-ray to find out if she has dislocated
26:28her little toe i want to go i don't well i think you'll be going no matter what i might
26:37not be yeah
26:38that's why i wish i was you is it because i get all the sympathy from you
26:49what's your toe
26:55thank you
26:56wait do we go back where we were
26:57no where we came from
27:00yeah
27:02advanced clinical practitioner jemma looks at the x-ray
27:07i think she's definitely drafted it if you look at these ones we look for nice smooth
27:11smooth lines and on this one she's got a little bit of a step there but she's also got a
27:17bit of a bend
27:19here so that's what we're looking for it's not nice and smooth so it looks like she yeah she's fractured
27:23it
27:25all right have a chat with her
27:31hello hi so just had a little look at your x-ray looks like you have broken it okay but
27:39it's not
27:39out of line so we don't it's not what we call dislocated so what we need to do is just
27:44strap it
27:45together what about like pe and can she do that i don't want to do it i don't want to
27:50do it you
27:50don't want to do p8 no because i think we're doing like running i would um avoid it for a
27:56few weeks
27:57while she's um healing and then there's one other question would i have boxing because i do that boxing
28:04you don't box with your feet do you no as long as you're not in pain then you should be
28:10fine okay
28:11yeah yeah any more questions no thank you very much thank you
28:27also on shift is volunteer jane who's heard there's a familiar face in cubicle 10
28:35you've been in before a repeat offender then yeah now when we say a repeat offender we don't
28:45necessarily mean it in a bad way uh it it's a night actually sometimes it's nice to keep seeing them
28:51and tommy was one of them they were it were a nice repeat offender repeat offender tommy is here with
28:57his granddaughter lauren after suffering chest pains yeah he always causes trouble that's what it is
29:02does it you always remember them that caused trouble yorkshire men i'd say nice to see you but
29:08it's never nice to see you back in here is it but then again that's it yeah it's like it's
29:15meet and
29:16greeting it's like these airports about to start and charge now didn't we every time you come through
29:19do it see you later look anyway thank you dr hong tett has come to assess tommy hello thomas
29:32i just understand that you came in with the chest pain yeah what was the character of the pain is
29:37that
29:38sharp or it was a sharp pain because you bent already yeah are you in pain now no no
29:46any surgery to the heart or something he's got about 16 stents in his heart because he's had
29:51quite a few heart attacks i'm sorry to hear that i was only here a week ago i'll review your
29:56blood
29:56test and then i'll let you know the plan okay it is good to know that you're pain free now
30:01thank you
30:02the blood test will show if tommy has had another heart attack
30:10while he waits for the verdict 94 year old margaret is on her way back from radiology
30:24advanced clinical practitioner chantelle assesses margaret's x-ray
30:29she's got a lovely big pneumonia sat there that's what's causing it to be quite unwell
30:37hello are you okay how are you feeling you look tired i'm tired you look tired
30:48you've got a big infection down there what's that well what's that mean yeah it means we need

34:02to take painkillers or anything when you got home off no i've just took some thingamajigs
34:12it's just stuff from him right let's have you on this couch then
34:21so any pain in your hips
34:26that there where you were just pressing it then yeah there in here yeah i can feel it there
34:33sorry in there can you lift up and touch my arm straight as you can that's as far as okay
34:39and back
34:39down so you're definitely sorry in here yeah that's so yeah what about here it's all there
34:47so i think your hips all right it's your femur isn't it yeah we'll get a quick extra of your
34:52femur and
34:53then it might be that we have to get a doctor to have a look at you just to be
34:55sure so you'll get
34:56yourself dressed and then we'll take it from there is that okay follow these blue dots to x-ray
35:05all right flowers thank you problem if you're looking at 15 mile an hour and then just comes
35:18to a sudden stop that's quite a big impact on that area he's weight bearing he's walking up down
35:24so i think it's unlikely that he has got anything fractured there however we have had people that
35:29have walked him before with broken femurs if the x-ray shows his femur is broken
35:35caden won't be getting back on his bike anytime soon
35:53so your patient has just come to you from high dependency two they've pulled his bed so he can
35:59go up to the ward now lovely thanks charlotte do you need a porter you're on the ball with casualty
36:10full and her staff working non-stop sister jane brings out the emergency medication
36:23kimberly they've brought your chocolates ready yeah i'm putting them in draw for everyone
36:34kenny's sugar hit is good we have our sweet drawer which is generally always full and if it's not
36:40then uh there's words to be had the specialities come down and take our suites and that's not fair is
36:47it
36:53we are renowned in the hospital for the sweet drawer
37:05in cubicle 10 tommy is hoping to go home but dr tet has the results of his blood test
37:14i would like to keep in amu admission water why because he's got the chest pain and the first
37:25heart attack mother is raised i'm just worried that the heart attack is back when you get back home
37:33and then you need to come back okay i think it is best recommended to keep you
37:38we've got to stay in granddad he's said this morning it was just a pain but obviously it won't
37:43they're thinking it might be it might have been an attack
37:47so so his granddad here weren't even going to come in
37:51i didn't know what it was i just had a bit of pain i've had pain before
37:55but we used to it because he's had that many heart attacks
37:59he's got 16 cents in his heart because he's had that many heart attacks so
38:06don't let no help bother you bless you he doesn't look as though he's just had an
38:10heart attack does he
38:16i want to get back home you're going to stay aren't you granddad yeah i'm just hoping to snuff along
38:24tommy spent the night in hospital and he's now back at home being cared for by his family
38:33in the minor injury unit 17 year old caden is waiting for the results of his leg x-ray after
38:40a
38:40collision with a tree look at them this is the race uh yesterday that we're racing i've came a lot
38:50faster
38:52i was just going too down there too fast
38:54i was up to him yesterday so it's trying to go give me an eye to tap and he fell
38:59off three times
39:00yeah it's just it's just part of the sport really having fun
39:11consultant dr andrew snell has caden's results we do see a lot of sports related injuries ones that are
39:18the highest risk are certainly speed is related i've had a look at the x-ray no fracture on the
39:26x-rays but it's nice and clean all the way down down to the knee and this is from side
39:31on and there's
39:32your knee itself so that's good news but of course it's not only the bones that are you know at
39:37risk so
39:38um we need to have a quick look at the general function if you want to pop up on the
39:42bed
39:46how long have we been racing for i started this year oh did you yeah but you are you serious
39:52stuff
39:52or is it serious is it yeah very good well that's getting into it all now he did start with
39:57bmx but
39:58he's starting to get into some of the more stunts and wants to do the downhill stuff i think historically
40:03we haven't been necessarily as good as we should be at encouraging physical activity so when we do see
40:10cases of of sports and exercise related injury i think it's really important we congratulate them
40:17for being active lift this leg straight off the bed that's as far as i can get it where's the
40:21pain
40:22there all in that i'm using bending the knee lift it as high as you can right so with the
40:27knee bent
40:27it's all right yeah a bit tight but not as hard as with the knee straight okay back down and
40:35no
40:35numbness down the leg at all no feels normal both sides pull those back on and then you can get
40:40your
40:40shoes back on when's your next race october okay and what do you do for work build motor arms oh
40:48good
40:51brilliant that's great busy lads i don't think there's anything we need to do maybe for the first 48
40:59hours a bit of frequent icing will help just bring the swelling down so all in order very much all
41:07right nice to meet you both some good luck with it all including the motor homes yeah back to work
41:13tomorrow yeah yeah there you go you've been told yeah all right take care bye-bye
41:22kaden was absolutely right to be concerned and we were concerned but thankfully it looks like this
41:27time it's going to be what we call kind of soft tissue injury that will time and his body will
41:31do
41:31all the healing as things heal and he feels ready and safe to ride there's there's no reason i'm very
41:36keen for him to get back to riding very good news big sigh of relief yeah can go out back
41:43out on bike now
41:44see you later after a few weeks of taking it easy kaden is back doing what he loves best
41:51and is trying hard to avoid the trees hello is it harriet in paediatrics charge nurse johnny is treating
42:08eight-year-old harriet did it hurt yeah yeah but it did i cried and then i told mommy and
42:15then
42:16she said that she'd um come and then she went into a meeting for half an hour oh so she
42:23weren't even
42:23your first priority yeah oh god mama here all right there you go all right any problems just come back
42:33bill thank you so much thank you you're welcome bye bye bye bye
42:40i'm feeling a little bit guilty that um it's actually broken and i left her for half an hour
42:46um but yeah everyone's been really nice it's been really quick um so yeah happy that she's sorted
42:57go on then wait for this one yeah
43:03is daddy gonna pick us up no i don't think so wait what are we gonna do then get the
43:08bus
43:08thank you you too

43:44in the hub sister jane is handing over to sister kim
43:50let me bring my staff in up that one's gone to the ward there's a patient in cubicle seven
43:55who's a neck of femur she's not had a block because she's too restless and there is no bed for
44:00her
44:00tracy is aware of it and she's trying to sort it out to be in charge of the department you
44:05do have to
44:06have a certain flair about you you do have quite a lot of responsibility and obviously not everybody
44:11does want that can you do me a transfer i'll never ask you for anything else again in my life
44:21but i think every person should be given the respect that they deserve
44:26every team member that i work with work hard and to the best of their ability for what's best and
44:31safe
44:31for the patient that's it from me all right all right all right jane's shift may be over but the
44:40work of
44:40the casualty team continues
